Metformin is an oral antidiabetic largely prescribed in the treatment of type II diabetes. Overdose is associated with life-threatening lactic acidosis. We report the case of the highest metformin concentration ever described secondary to a voluntary suicidal intake. The patient developed a severe lactic acidosis and hemodynamic shock successfully treated with high-flow hemofiltration. Time to start extrarenal epuration is capital to avoid poor evolution.

Idiopathic hyperammonemia is a rare but potentially fatal complication occurring in patients with acute leukemia or bone marrow transplantation. The role of some specific anticancer drugs may be discussed, but the etiology of hyperammonemia is often multifactorial. We report the case of a 40-year-old woman who developed fatal idiopathic hyperammonemia two weeks after induction chemotherapy with idarubicin-aracytine for acute myeloid leukemia. Despite intensive care management and extrarenal epuration, the patient was declared brain dead two days after hyperammonemia onset.

Kunstschilder Prosper De Troyer (1880-1961) behoorde tot de Vlaamse potentiële avant-garde, die zich bewoog in de rand van het activisme. Tijdens de twintiger jaren evolueerde hij in figuratieve, expressionistische zin, waarbij hij steeds sterker tendeerde naar een beeldentaal die kon gelden als de Identitätsnachweis van het Vlaamse volk. Dat beeld was overigens in overeenstemming met de smaak (en de doelstellingen) van de Nieuwe Orde; De Troyer ging uiteindelijk de weg van de culturele collaboratie en kwam na de Tweede Wereldoorlog in aanraking met de repressie en de epuratie.
