ObjectivesWe investigated medical staff interpretations and descriptions of internal communication quality in elite football teams to determine whether internal communication was correlated with injuries and/or player availability at training and matches.MethodsMedical staff from 36 elite football clubs across 17 European countries produced 77 reports at four postseason meetings to provide their perceptions of internal communications in their teams. They also recorded data on individual players’ exposure to football and time-loss injuries.ResultsThe injury burden and incidence of severe injuries were significantly higher in teams with low quality of communication between the head coach/manager and the medical team (scores of 1–2 on a 5-point Likert scale) compared with teams with moderate or high-quality scores (scores of 3–5; p=0.008 for both). Teams with low scores had 4%–5% lower training attendance (76% vs 83%, p=0.001) and less availability at matches (82% vs 88%, p=0.004) compared with teams with moderate or high communication quality scores.ConclusionsThe quality of internal communication within a team was correlated with injury rates, training attendance and match availability.

Анотація:
All high schools should implement exertional heat illness (EHI) safety strategies. We determined if there were differences in the implementation of EHI safety strategies between schools with and without additional paid athletic trainers (ATs) or a team physician present at preseason football practices. High schools with multiple ATs or a team physician implemented more EHI prevention and management strategies than schools with only a single AT, including training staff in EHI recognition and treatment and having an emergency action plan. However, schools with a paid team physician were more likely to have double practices in the first week of football practice. Schools with additional medical personnel at football preseason practices were more likely to implement EHI safety strategies.

This study aimed to understand coaches’ approaches to including children with disabilities in a community-based Australian rules football program for children. Football coaches for the program, called Auskick, completed an online survey providing qualitative and quantitative information about their experiences of inclusive coaching. Coaches (N = 130) completed the survey over 2016/2017. The average years of coaching experience was 3.3 (range 0–19 years). While 79% of coaches had experienced a child with a disability attending their football centre, only 31% of coaches (56% of paid coaches and 27% of volunteer coaches) had completed disability training. Autism, attention deficit hyperactivity disorder and intellectual disability were the most common types of developmental disabilities, and asthma, vision and hearing problems were the most common physical disabilities of children attending the coaches’ centres. Eighty-nine coaches provided examples of inclusive approaches. The most common was having an inclusive attitude, asking the parents for help, making adaptations to suit the child, giving extra time, getting additional help and staff training. Community football coaches frequently work with children who have a broad range of developmental and physical disabilities. Coach disability training is needed to support children with disabilities attending these types of sporting programs in the community.

BackgroundDo coaches’ leadership styles affect injury rates and the availability of players in professional football? Certain types of leadership behaviour may cause stress and have a negative impact on players’ health and well-being.AimTo investigate the transformational leadership styles of head coaches in elite men’s football and to evaluate the correlation between leadership styles, injury rates and players’ availability.MethodsMedical staff from 36 elite football clubs in 17 European countries produced 77 reports at four postseason meetings with a view to assessing their perception of the type of leadership exhibited by the head coaches of their respective teams using the Global Transformational Leadership scale. At the same time, they also recorded details of individual players’ exposure to football and time-loss injuries.ResultsThere was a negative correlation between the overall level of transformational leadership and the incidence of severe injuries (rho=−0.248; n=77; p=0.030); high levels of transformational leadership were associated with smaller numbers of severe injuries. Global Transformational Leadership only explained 6% of variation in the incidence of severe injuries (r2=0.062). The incidence of severe injuries was lower at clubs where coaches communicated a clear and positive vision, supported staff members and gave players encouragement and recognition. Players’ attendance rates at training were higher in teams where coaches gave encouragement and recognition to staff members, encouraged innovative thinking, fostered trust and cooperation and acted as role models.ConclusionsThere is an association between injury rates and players’ availability and the leadership style of the head coach.

Collegiate American football has a high rate of injury. The Lower Quarter Y-Balance Test (YBT-LQ), a dynamic assessment of lower extremity strength, mobility, and balance, has been purported to identify athletes at risk for injury in different sports including football. Previous studies examining the association between YBT-LQ and injury have reported varied findings; therefore, the purpose of this study was to assess if preseason YBT-LQ performance predicted whether football players would sustain a non-contact lower extremity or low back (lower quarter (LQ)) injury during the season. Fifty-nine male collegiate American football players (age 20.8 ± 1.3 y, height 1.8 ± 0.1 m, body mass 94.6 ± 14.2 kg) completed a survey of training and injury history and had their YBT-LQ performance assessed at the start of the season. Athletic training staff tracked the occurrence of non-contact LQ injuries during the season. There were no significant relationships found between preseason YBT-LQ values and incidence of non-contact LQ injury in this population of collegiate American football players. This study is consistent with recent reports that have not found a significant association between preseason YBT-LQ values and LQ injury. These results suggest that, in isolation, the YBT-LQ may have limited utility as a screening test for non-contact injury in collegiate football players.

Context The “Appropriate Medical Coverage for Intercollegiate Athletics” (AMCIA) document was created to support assessment and calculation of athletic training personnel requirements. However, little is known regarding disparities between current and recommended staffing practices. Objective To identify the staffing and employment characteristics of athletic health care services at Football Bowl Subdivision-level institutions. Design Cross-sectional study. Setting Web-based survey. Patients or Other Participants Head athletic trainers and athletic training staff members who were knowledgeable about budget and staff. Main Outcome Measure(s) The survey, Assessment of Staffing Levels at National Collegiate Athletic Association Football Bowl Subdivision-Level Institutions, was used to evaluate personal, university, and staff demographics; staffing and employment topics; and AMCIA variables and use. Results The survey was accessed and partially completed by 104 individuals (response rate = 84.6%). A total of 79 athletic trainers (response rate = 76%) completed the entire survey. One-third of the respondents (34.2%, n = 26) met the recommended number of full-time equivalents (FTEs) for football, two-thirds of the respondents (65.7%, n = 50) failed to meet the recommendation, and 26.2% (n = 27) were missing data needed for FTE calculation. Among those who did not meet the recommended FTEs (n = 50), 38.0% (n = 19) were within 1 FTE of being compliant, 26.0% (n = 13) were within 2 FTEs, and 24.0% (n = 12) were within 3 FTEs. About one-third of respondents (35.9%, n = 37) reported not using the AMCIA, citing lack of funding (29.7%, n = 11), lack of administrative support (21.6%, n = 8), and other reasons (37.8%, n = 14). Conclusions The majority of institutions that used the AMCIA were able to provide justification for staffing. For most of the institutions that failed to meet their recommendation, adding 1–3 FTE athletic trainers for football would change their compliance status. A uniform definition of the term FTE within collegiate athletics is needed to allow for structured assessment and allocation of staffing and workloads.

AimTo assess the emergency response planning and prevention strategies for sudden cardiac arrest (SCA) across a wide range of professional football clubs in England.MethodsA written survey was sent to all professional clubs in the English football league, namely the Premiership, Championship, League 1 and League 2. Outcomes included: (1) number of clubs performing cardiac screening and frequency of screening; (2) emergency planning and documentation; (3) automated external defibrillator (AED) training and availability; and (4) provision of emergency services at sporting venues.Results79 clubs (86%) responded to the survey. 100% clubs participated in cardiac screening. All clubs had AEDs available on match days and during training sessions. 100% Premiership clubs provided AED training to designated staff. In contrast, 30% of lower division clubs with AEDs available did not provide formal training. Most clubs (n=66; 83%) reported the existence of an emergency action plan for SCA but formal documentation was variable. All clubs in the Premiership and League 1 provided an ambulance equipped for medical emergencies on match days compared with 75% of clubs in the Championship and 66% in League 2.ConclusionsThe majority of football clubs in England have satisfactory prevention strategies and emergency response planning in line with European recommendations. Additional improvements such as increasing awareness of European guidelines for emergency planning, AED training and mentorship with financial support to lower division clubs are necessary to further enhance cardiovascular safety of athletes and spectators and close the gap between the highest and lower divisions.

Objectives:To investigate the relationship between the proportion of preseason training sessions completed and load and injury during the ensuing Australian Football League season.Design:Single-cohort, observational study.Methods:Forty-six elite male Australian football players from 1 club participated. Players were divided into 3 equal groups based on the amount of preseason training completed (high [HTL], >85% sessions completed; medium [MTL], 50–85% sessions completed; and low [LTL], <50% sessions completed). Global positioning system (GPS) technology was used to record training and game loads, with all injuries recorded and classified by club medical staff. Differences between groups were analyzed using a 2-way (group × training/competition phase) repeated-measures ANOVA, along with magnitude-based inferences. Injury incidence was expressed as injuries per 1000 h.Results:The HTL and MTL groups completed a greater proportion of in-season training sessions (81.1% and 74.2%) and matches (76.7% and 76.1%) than the LTL (56.9% and 52.7%) group. Total distance and player load were significantly greater during the first half of the in-season period for the HTL (P = .03, ES = 0.88) and MTL (P = .02, ES = 0.93) groups than the LTL group. The relative risk of injury for the LTL group (26.8/1000 h) was 1.9 times greater than that for the HTL group (14.2/1000 h) (χ2 = 3.48, df = 2, P = .17).Conclusions:Completing a greater proportion of preseason training resulted in higher training loads and greater participation in training and competition during the competitive phase of the season.

Actual playing time is an important problem in the progress of a football game, with implications in both the scientific guidance of the training process and the achievement of superior results. The issue addressed in this research is useful, because it has been noticed that, in the official games played by seniors, a lot of time is wasted on:Ÿ fixed moments (phases) – throw-in, corner kick, direct and indirect free kicks, referee’s ball, kick-off;Ÿ substituting players;Ÿ player injuries during games;Ÿ solving unexpected situations – the entry of a supporter on the field, the existence of several balls on the field, as well as the discussions between referees and technical staff members. One of the important features of current football, the game dynamics, is also highlighted by a significant number of technical and tactical actions. In this regard, actual playing time must be as long as possible to the detriment of breaks and fragmentations occurred during games. In the context of major competitions – European cups, European Championship and World Championship, there is a visible trend to increase the actual playing time, which can be achieved by using, in the training of football players, some rationalized and standardized methods and means with increased efficiency.

Objective: To describe a case of exertional rhabdomyolysis in a collegiate American football player after preventive cold-water immersion. Background: A healthy man (19 years old) participated in full-contact football practice followed by conditioning (2.5 hours). After practice, he entered a coach-mandated post-practice cold-water immersion and had no signs of heat illness before developing leg cramps, for which he presented to the athletic training staff. After 10 minutes of repeated stretching, massage, and replacement of electrolyte-filled fluids, he was transported to the emergency room. Laboratory tests indicated a creatine kinase (CK) level of 2545 IU/L (normal range, 45–260 IU/L), CK-myoglobin fraction of 8.5 ng/mL (normal &lt; 6.7 ng/mL), and CK-myoglobin relative index of 30% (normal range, 25%– 30%). Myoglobin was measured at 499 ng/mL (normal = 80 ng/mL). The attending physician treated the athlete with intravenous fluids. Differential Diagnosis: Exercise-associated muscle cramps, dehydration, exertional rhabdomyolysis. Treatment: The patient was treated with rest and rehydration. One week after the incident, he began biking and swimming. Eighteen days later, the patient continued to demonstrate elevated CK levels (527 IU/L) but described no other symptoms and was allowed to return to football practice as tolerated. Two months after the incident, his CK level remained high (1900 IU/L). Uniqueness: The athlete demonstrated no signs of heat illness upon entering the cold-water immersion but experienced severe leg cramping after immersion, resulting in a diagnosis of exertional rhabdomyolysis. Previously described cases have not linked cold-water immersion with the pathogenesis of rhabdomyolysis. Conclusions: In this football player, CK levels appeared to be a poor indicator of rhabdomyolysis. Our patient demonstrated no other signs of the illness weeks after the incident, yet his elevated CK levels persisted. Cold-water immersion immediately after exercise should be monitored by the athletic training staff and may not be appropriate to prevent muscle damage, given the lack of supporting evidence.

Personal Informatics (PI) are information systems that allow people to process activities with the usage of information technology, aiming to produce informational products (data) either for themselves or for others. Technologies that enable PI are becoming increasingly popular, assisting people in collecting personally relevant information about their body and their behaviour. In sports industry nowadays, a great variety of PI wearable tools offer support to athletes and training staff to improve their performance. An example of such tool is the Electronic Performance and Tracking Systems (EPTS), which are a combination of hardware and software that facilitates the collection, storage, analysis and management of professional athletes’ fitness and health data. Although significant and broadly used, EPTS have not yet received much attention from researchers and, thus, understudied. Therefore, the master’s thesis explores the perceptions of professional football athletes and training staff regarding the use of EPTS in their everyday training and work. Furthermore, the master’s thesis research explores the benefits and challenges that professional football athletes and training staff experience when using EPTS in their everyday training and work. The master’s thesis study adopts the interpretive paradigm and qualitative ethnographic approach. The research data was collectedthrough direct observations in the field and semi-structured interviews from Greek professional football athletes and Greek training staff that use wearable EPTS in their everyday training and work and was analysed thematically. A theoretical framework, which is built upon relevant literature from the informatics field and along with the theory of sensemaking, is used to understand, interpret and discuss the research findings. The research outcome of the master’s thesis shows that communication is at the core of EPTS enabling football players and training staff to improve individual and team performance. Organizing of every day starts and ends with EPTS analysis and evaluation and better organized and daily evaluated football methodology appears as key benefit for the club. Coaches and trainers are now more data driven and accurate and analysts and trainers that conduct analysisof the data provided by EPTS are new members of the training staff. Evidence provided by EPTS build trust between staff and players and in the training staff. Visualization tools for presenting insights need to be further improved with the addition of in-field monitors and 3D presentations. Furthermore, it is important for training staff members to have ethical and consistent strategy on how data derived from EPTS are used on how data are communicated.  The research complements previous research on personal informatics and adjusts them to elite team sport context and adds to the theory of sensemaking regarding how users make sense of PI tools that are related with their everyday routines at work. In addition, it offers football training staff members a model for efficient use of EPTS technology into the everyday football practices and a model of sustainable use aiming the overall improvement of team performance.
