Protein phosphorylation dynamically integrates environmental and intracellular information to control virtually all biological processes. Challenges in determining the upstream regulation and downstream effects of phosphorylation obscures our functional understanding of cellular communication. In this thesis, I developed approaches to contextualise and prioritise phosphorylation events, focusing on the dynamic regulation of skeletal muscle metabolism by insulin and exercise. I measured phosphoproteomes of an in vitro panel of exercise-like stimuli to dissect upstream influences on exercise signalling. My integrative analysis with acute exercise phosphoproteomes revealed that emergent outcomes like protein secretion require concurrent kinase activation by distinct stimuli. To address the challenge of prioritising phosphosites with downstream functions most crucial to a biological phenomenon, I developed personalised phosphoproteomics. This experimental and computational framework identifies functionally enriched signalling by utilising human biological variance. To employ this method, I performed two independent phosphoproteomics studies investigating how exercise potentiates insulin sensitivity and how insulin resistance impacts metabolism in human skeletal muscle. My approach identified signalling on both known and previously unidentified sites on proteins intimately involved in glucose metabolism. This included a new co-operative relationship between mTOR and AMPK, for which I found a role in metabolic regulation. Collectively, these studies delineated skeletal muscle signalling responses and established personalised phosphoproteomics as a general approach to investigate the signal transduction underlying complex biology.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most prevalent chronic liver disease globally. Exercise is essential for MASLD management; however, many MASLD patients lack opportunities to exercise and lead active lives. This thesis project: i) evaluated exercise-related barriers and determined patient awareness of exercise's necessity for MASLD management (Study 1); ii) assessed patient attitudes toward exercise and awareness of MASLD through a qualitative analysis of interviews with MASLD patients (Study 2); iii) presented a case study of a 65-year-old female who improved her metabolic health through lifestyle changes, emphasizing exercise and diet (Study 3); and iv) evaluated liver function, metabolic indicators, and health-related quality of life in people with MASLD following an 8-week online, body-weight eccentric exercise intervention. Study 1: A survey on exercise habits, barriers to adherence, and awareness of exercise's effect on MASLD was sent to eighty-one patients at a hepatology outpatient clinic. Results indicated that 73% did not meet the exercise recommendation of 150 minutes of moderate exercise per week, and 54% were unaware that exercise could help with MASLD. Reported barriers included poor physical and mental well-being (57%), limited time (43%), apathy (31%), and fatigue from exercising (24%). These results point to specific barriers requiring intervention and education and suggest future exercise program settings that account for time limitations, fatigue, and attitudes toward exercise. Study 2: A thematic analysis of interviews with 18 MASLD patients revealed high awareness of the condition but low awareness of treatment via exercise. Patients expressed a preference for low-intensity, low-accessibility activities like walking, bodyweight, and resistance training. This study highlights the necessity of matching exercise options to patient preferences, ensuring feasibility for long-term adherence. Study 3: A qualitative single-patient case study assessed obstacles and facilitators to sustained lifestyle changes, demonstrating how holistic, inclusive lifestyle adjustments can impact MASLD. This intervention showed that with adequate instruction and support, the patient could make meaningful changes, reflecting how a customized approach can empower patients to manage symptoms effectively. Study 4: Sixteen participants were divided into an exercise group (n=9) and a non-exercise control group (n=7). The exercise group completed a supervised, progressive web-based body-weight eccentric exercise program for eight weeks. Participants in the exercise group showed significant improvements in hepatic steatosis severity (13.2±13.8%), waist circumference (4.4±13.5%), hip circumference (2.6±11.1%), and gamma-glutamyl transferase levels (23.9±30.0%). They also demonstrated enhanced (p < 0.05) physical fitness, including improvements in the sit-to-stand test (24.3±33.2%), 3-metre timed up and go test (-15.6±16.4%), 2-minute step test (37.5±20.5%), and single-leg balance test (25.1±37.6%). These findings suggest that home-based, web-delivered eccentric resistance exercises are an effective intervention for improving liver and metabolic health in patients with MASLD. Overall, the findings from these four studies contribute to a deeper understanding of the barriers, perspectives, and effective interventions for exercise in MASLD patients. The research underscores the need for greater awareness of exercise's role in MASLD management and the importance of addressing patient-specific barriers through tailored interventions. The novel web-based bodyweight eccentric exercise program shows significant potential for improving liver health and physical fitness in this population, addressing key obstacles like time constraints, fatigue, and accessibility. Future interventions should focus on increasing patient education and providing flexible, personalized exercise options to promote long-term adherence and better health outcomes in MASLD management.

The purpose of this study was to develop a personalized step test and a valid regression model that used non-exercise data and data collected during the step test to estimate VO2max in males and females 18 to 30 years of age. All participants (N= 80) successfully completed a step test with the starting step rate and step height being determined by the self-reported perceived functional ability (PFA) score and participant's height, respectively. All participants completed a maximal graded exercise test (GXT) to measure VO2max. Multiple linear regression analysis yielded the following equation (R = 0.90, SEE = 3.43 mL/kg/min): 45.938 + 9.253(G) - 0.140(KG) + 0.670(PFA) + 0.429(FSR) - 0.149(45sRHR) to predict VO2max (mL/kg/min) where: G is gender (0=female;1=male), KG is body mass in kg, PFA is the sum of the two PFA questions, FSR is the final step rate (step-ups/min), and 45sRHR is the recovery heart rate 45 seconds following the conclusion of the step test. Each independent variable was significant (p < 0.05) in predicting VO2max and the resulting regression equation accounted for roughly 83% (R2=0.8281) of the shared variance of measured VO2max. Based on the standardized B-weights, gender (0.606) explained the largest proportion of variance in VO2max values followed by PFA (0.315), body mass (-0.256), FSR (-0.248), and the 45sRHR (-0.238). The cross validation statistics (RPRESS = 0.88, SEEPRESS = 3.57 (mL/kg/min-1) show minimal shrinkage in the accuracy of the regression model. This study presents a relatively accurate model to predict VO2max from a submaximal step test that is convenient, easy to administer, and individualized.

Nowadays, the problem of physical inactivity is one of the major global issues and it is considered by the WHO and the UN as the leading cause of mortality due to non-communicable diseases. At the same time, although the physical exercise has proven to be the best way to fight against these sicknesses, most of the consumers evoke a series of barriers against the adoption of regular exercise. The purpose of this thesis is to propose alternative approaches based on the concept of perceived value in marketing which has proven to be a good tool to motivate and retain consumers in the frame of other types of goods and services. We focused on the sources of consumer perceived value in personalized marketing, community marketing and experiential marketing, the aspects of which were integrated in a combinatorial approach, the aim of this method being to propose the concept of the exercise composite variable as a trade-off between costs and benefits. We confirmed that the perceived values of the different composites of the practice of exercise combined to form the overall perceived value of the exercise. In addition, we also confirmed that this overall perceived value of the exercise ultimately influenced the consumer’s intent to practice regular exercise and to recommend it.The results of this research allowed us to suggest that the offer of exercise should be made as à la carte menu in order to facilitate consumers’ decision-making process and so to motivate and to retain them

Background and Purpose: To investigate the effect of an individualized exercise program versus a generic group-based exercise program on balance, gait, and functional performance of older adults categorized as having mild balance dysfunction and living in residential care facilities. Methods: Single blind randomized control design. One hundred-twenty residents fulfilled screening criteria for mild balance dysfunction based on the BioSwayTM balance and the Multi-Directional Reach Test (MDRT) primary outcome measures. Secondary assessment was completed using the Modified Physical Performance Test (PPT), hand-held dynamometer (lower-limb muscle strength testing), and gait speed analysis. Sixty subjects received individualized treatment from physical therapists (8 weeks). Another sixty subjects received generic group-based exercises (8 weeks). All outcome measures were collected at baseline and post-intervention (ninth week); and BioSwayTM and PPT measures at follow-up (thirteenth week) for the individualized group. Results: Individualized group (n=60) showed significant improvement compared to the group-based group (n=60) on the two BioSwayTM scores (limits of stability, p < .001; and postural stability, p = .016), the MDRT scores (forward reach, p < .001; backward reach, p = .007; right lateral reach, p < .001; and left lateral reach p < .001), the strength scores (hip flexors, p = .010; knee extensors, p = .002; hip abductors, p = .009; and ankle dorsiflexors, p = .025), the PPT outcomes (p < .001), and the gait scores (p = .012). Effect sizes ranged from small to large, with the largest sizes for limits of stability and MDRT. There were no significant differences between groups for the mCTSIB (p = .538). However, 96.7% of subjects in the individualized group scored within one SD of the reference mean, relative to 75% in the group-based group. At follow-up, the individualized group showed significant differences over time with medium to large effect sizes on the PPT (p < .001), limits of stability (p < .001), postural stability (p < .001), and mCTSIB (p = .005) measures. Post-hoc analysis revealed retention of gains for all measures at follow-up, except the mCTSIB. Conclusion: The individualized group showed significant improvements in the areas of balance, strength, mobility, and functional outcomes.

According to projections by the World Health Organization (WHO), it is estimated that by 2050, more than 22% of the global population will be aged 60 years or older. This demographic shift is accompanied by a growing prevalence of chronic diseases. In France, one in four individuals currently suffers from a chronic condition, a proportion that rises to three out of four among those aged 65 and above. These diseases significantly contribute to mortality, accounting for 74% of total deaths in 2019. Fortunately, many of these conditions can be prevented or delayed by adhering to the physical activity recommendations provided by the WHO. Extensive research has demonstrated the profound impact of physical activity on prolonging lifespan and enhancing overall quality of life across populations. With this perspective in mind, the research objectives of this study lie within the realm of Prognosis Health Management (PHM) and prescription sports, with a specific emphasis on the healthcare trajectory of patients with chronic conditions. However, the precise methodology for control, particularly the feedback mechanism pertaining to individuals' behavior during physical activity, remains to be delineated. To tackle this challenge, the project proposes the implementation of a connected medical device, specifically an electric-assisted bicycle integrated with embedded artificial intelligence. This combination aims to collect data and contribute to the prevention of chronic pathologies for individuals. The primary scientific endeavor entails designing and integrating an innovative and adaptable hardware and software architecture, incorporating state-of-the-art sensors that utilize flexible technology. This framework facilitates the monitoring of both occasional and experienced cyclists during their practice, enabling intelligent control of the level of electrical assistance and the implementation of medical instructions based on prior medical assessments

This thesis studies the effects of import competition from China and Eastern Europe on the health and fertility decisions of German individuals working in manufacturing. Individuals are matched with separate measures of exposure to competition from China and Eastern Europe, respectively. To isolate exogenous supply shocks from the origin, instrumental variables for competition from each of China and Eastern Europe are constructed. Results in Chapter 1 suggest that higher import competition worsens individual health via job displacement, wage decline, shortened employment duration, increased reliance on welfare and less future orientation, with Chinese import competition affecting individuals twice as much. Health declines as individuals increase their visits to the doctor, exercise less frequently and have a higher probability of developing chronic illness. Also, there is some evidence that individuals do not tend to become disabled but may be slowly pushed into chronic illness. Findings in Chapter 2 show that import competition negatively affects the individual’s probability of having children via reduced earnings, lower satisfaction with personal income and shortened employment duration. The chapter then investigates effects of import exposure by gender. Results show that male and female fertility choices differ upon rising import competition. Higher import exposure lowers female earnings and job autonomy, which in turn generates a lower opportunity cost of work, to the point where having children would become a more rewarding alternative for female workers. By contrast, increased import exposure negatively affects male workers’ fertility through reduced earnings and employment duration.

This report presents a personalized exercise training chatbot for individual users based on data collected from the Internet of Things (IoT), particularly wearable fitness devices. The chatbot is designed with our goal of motivating users to exercise more by discussing exercise statistics with the user, such as whether their daily steps have increased, decreased, or remained steady. In this work I first survey a few examples of how increased interest in fitness and the promotion of healthy lifestyles is driving demand for personalized artificial intelligence, wear- able computing, and ubiquitous computing applications. Next, I describe the design of a data-driven ”personal trainer” chatbot. I then develop a prototype persuasion system based on interactive dialogs delivered via a front-end application, that collects data from wearable equipment using back-end data loggers that I instrumented as a mobile application. Finally, I describe the process of deploying and demonstrating this prototype along with technical challenges and early findings. The overall system consists of (1) the back-end Coach agent, an Android application that collects data from all wearable instruments, and (2) the front-end Me agent, which initiates and continues conversations with the user using notifications that are in turn based on data from the Coach agent. This data-driven ensemble reminds the user to exercise and also gives the user a chance to provide feedback via human/agent interactive dialogs. In this project, I used only one wearable device, the MI Band 2, and get real-time steps and weekly step aggregates from it. The human/agent dialogues are deployed via the Slack groupware platform. Google Sheets is used as a web service for updating and exchanging data.

Regular sleep could restore personal physical exertion, and reorganize the human cognition and memory. People treat sleep-related issues seriously in modern society. Home-based sleep recording are more convenient and cheaper than sleep disorder diagnoses in the hospital. Also, daytime activities will affect sleep quality. This thesis focuses on home-based sleep and activity recording. Exercise is an important factor that affects sleep. Proper exercise could improve sleep quality. Improper exercise may cause negative impact in sleep. This research aims at following problems: (1) the interactions between sleep and daytime exercise are complicated; (2) most wearable devices could not provide actuate sleep recording data; (3) personal health data exchange and integration. This theses proposed a cross-device sleep efficiency evaluation mechanism, and implemented an exercise prescription service. The exercise prescription service is based on a sleep diary testbed - HSS (Health Sleep Space). This research extends HSS with wearable devices and daytime exercise diary. Users realize their sleep quality easily from the Sleep Quotient (SQ). The exercise prescription provides exercise suggestion to user. Following the suggestion, user could improve their sleep efficiency.

This dissertation focuses on the topic of using digital approaches in cognitive rehabilitation. The interest in this subject derives from the assumption that current digital rehabilitation platforms are not customizable according to the individuality and needs of the patient. For this reason, the main goal of this work is contributing to fill this gap by proposing a platform capable of producing personalized cognitive exercises to foster a more effective rehabilitation process. The work methodology can be divided into two fundamental parts: a prior study based on multidisciplinary literature seeking to identify factors influencing cognitive rehabilitation and which types of treatment are most effective, as well as a review of existing digital approaches for preparing digital exercises meant to help cognitive rehabilitation, followed by a practical component related to the proposal and prototyping of a new platform. In this phase a Human-Centered Design approach was used with the collaboration of experts of the Centro de Medicina de Reabilitao da Regio Centro - Rovisco Pais for which the platform was specifically created.

Because the social structure began to change to ageing society in mid-21st century, the elderly populations increase which leading to the type of disease changing and changing the need and high cost in medicine. Therefore, long-term care for seniors and health care will be devoting more budgets and effort, the telecare is an important policy for government especially. The telecare promote the core which combines of medical services, care services, life and well-being of scientific and technological development, science and technology, in order to construct the comprehensive care services. Therefore, by leading in the information and communications technology, developing friendly user interface to drive by telecare, home care, personal health records, sports care, and medical electronic products and physiological monitoring of information support services. The services which provided in the exercise care system are the most abundant and complete in hospital. Through the complete equipment which be applied to the detection of heart and lung disease, exercise rehabilitation treatment and tracking of the establishment of the exercise prescription. However, the exercise care system which provided by hospital exist limit on the time and space, which cannot be applied in daily life. In order to achieve comprehensive exercise care, in many previous studies, fitness brands and medical equipment companies, the introduction of related products such as the pedometers, heart rate monitors, blood glucose, pulse oximeter and other telecare system. The products provide the user to carry at any time to break the limitations of time and space, and the precise physiological data to avoid the difference between subjective and objective assessment of the user's own physical condition in exercise, there are many studies which combine physiological information and the smartphone to design of telecare system. However, this functional information of products mostly use single physiological signal to provide a single service to users, so it is difficult to directly applied to the exercise care-related services. The purpose of this research is aimed to develop a personalized seamless exercise care system by integrating smart handheld devices which not are limited on the time and space, using the smartphone to integrate physiological signal and activity recognition to design personalized exercise care alert system, providing users to understand the sports safety with cardiopulmonary care. Using this study is to assess the feasibility of exercise care system.

This project aims to develop a conscious thinking about the dynamics that occurred during the Professional Internship, which took place in Fitness Academy of Lisbon’s University Stadium, in Avenida Prof Egas Moniz, under the supervision of the academy’s director and the collaboration of three colleagues, in the school year 2014/2015. The internship is inserted in the second year of the Master in Exercise and Health, in Faculdade Motricidade Humana (University of Lisbon), which goal is to teach the tools and methodologies that allow the promotion, conception and the implementation of exercising programmes. Ultimately, the main purpose is to improve a healthy condition and to prevent diseases, through the development of a document that justifies all the formation process which was done in the internship. This internship is the result of the pedagogic process, which includes the construction of knowledge, learning and skills’ development, under supervision. Therefore, this project allowed the approximation between the academic formation and the professional reality, articulating the theory into the practical professional role, also allowing the integration in the social reality. This is a fundamental component in the student’s formation. The initial part of the internship focused in the improvement and the evaluation of theoretical concepts and competences that were needed in order to develop the practical functions. After this, the work was more practical and the mail goals were: 1) to acquire competences to identify, to catalogue and to develop a critical analyse of the institution that provides services related to physical activity and health; 2) to acquire competences in order to prescript exercises and to do a personal follow-up, according to the client’s needs; 3) to evaluate the client’s physical condition and corporal composition; 4) follow-up the client’s performance, through the Evaluation and Counselling Department (Gabinete de Avaliação e Acompanhamento) do EUL; and, 5) personal follow-up of three clients of the Fitness Academy, evaluating (the corporal composition, cardiothoracic capacity, endurance, muscular flexibility and posture) and also advising specific training programmes, according to the clients’ needs and aspirations.

The current curricular internship report results as an integral and conclusive part of the master’s degree in Exercise and Health from the Faculty of Human Kinetics. The internship was held during the season of 2018/2019, taking place at the Complexo Desportivo da Ajuda (CDA), in the Ajuda University Campus, city of Lisbon. The work developed during the internship focused on the consolidation and improvement of knowledge, in the context of supervised practice (exercise room and group classes) to develop skills of a practicing professional and health. Physical inactivity is a major social problem. At the same time, the demand for gyms has been increasing to face physical inactivity, improve quality of life, or for aesthetic and personal reasons. Thus, this report aims at showing the importance of exercise-oriented practice and at emphasizing the role of the exercise instructor and personal trainer in increasing motivation and personal accomplishment. For the Personal Trainer to be able to assume such a role he needs to acquire prior knowledge, have the opportunity to apply this knowledge and then reflect on the effectiveness of his intervention. Therefore, the present report also portrays the work done throughout the internship as a contextual element contributing to the development of new learnings, skills and knowledge to complete a Master in Exercise and Health, from a conceptual and professional point of view.
