Анотація:
Pharmacists have had long-standing roles in public health, and the COVID-19 pandemic has broadened and accentuated their efforts in this area. Many pharmacists may be interested to expand pharmacy services to further support public health. While not intending to be exhaustive, this paper suggests potential areas for increased engagement and provides ideas for pharmacists who want develop and implement new initiatives to optimize the health of their patients and communities. The core functions of public health and the natural history of disease are presented as models to identify opportunities for pharmacists’ interventions. A three-step framework with practical strategies and helpful resources is proposed to identify and operationalize new services. Finally, the pharmacist’s role in clinical-community linkages is presented. It is hoped that this paper will stimulate additional ideas and actions in the pharmacy community to support public health.

While contraception prescribing by pharmacists has seen rapid growth in recent years, pharmacist-provided services that can impact maternal health encompass more than just contraception. Each phase of maternal health—preconception, pregnancy, and post-pregnancy—has unique needs, and pharmacists are well equipped to provide services to meet those needs and are more accessible than other healthcare providers. While pharmacist-provided maternal health services may lead to significant savings to the healthcare system, additional research to more fully capture the value of pharmacist-provided maternal health services is needed. Robust implementation of a pharmacist-provided maternal health services program will require partnerships between providers, payers, and pharmacists. Infant and maternal mortality, preterm birth, and unintended pregnancies are significant public health issues, and pharmacists should be seen as a capable workforce who can provide needed maternal health care and serve as a gateway into the healthcare system for those capable of pregnancy.

Roles of PhD supervisors are diverse, ranging from providing in-depth discipline-specific Public Health knowledge and technical (e.g., methodological) support to the students, encouraging them towards publications or conference presentations, offering pastoral support for student wellbeing, and finally preparing them to defend their thesis by conducting a mock viva. Effective supervision plays a vital role in a PhD journey reflecting on the quality of the PhD work, positive PhD experience, and supervisor-student relationship. While some student-supervisors team may encounter conflicting and challenging relationships, many relationships between PhD supervisor(s) and students progress into mentorship through joint publications and grant applications, career advice, and support establishing wider collaborative networks. Drawing from the wider experiences of the authors, this article highlights the responsibilities, opportunities, and sometimes the challenging nature of being a PhD supervisor. This reflection will inform good practices for PhD supervisors in countries including Nepal, where the numbers of PhD students in the field of Public Health is steadily increasing.

Local government public health offers an environment rich with opportunities for applying the principles of health psychology to local health challenges, conducting impactful research, and training others in the use of behavioural techniques to support behaviour change among local populations. Indeed, the authors were inspired to begin their own health psychology journeys from their work in local authorities and have worked in public health roles as qualified health psychologists since. The shift towards applying principles from behavioural and social sciences across public health practice supports the need for more health psychologists to work within public health settings. The authors reflect on some of the challenges arising from working in such multidisciplinary settings and offer some ideas going forward.

Health care providers routinely advise cancer patients to involve support persons in oncology care to fulfill critical support roles. This qualitative descriptive study explored alignment of triadic perceptions of support person involvement in oncology treatment visits and cancer-related care from the perspectives of patients with gynecologic cancer ( n = 18), regular visit-attending support people ( n = 16), and health care providers ( n = 10), including oncologists, nurses, and medical assistants. Semi-structured interviews ( N = 44) captured perceptions of facilitation and interference of support persons' roles within and outside appointments with oncology providers. Thematic analyses revealed alignment and divergence regarding support persons' instrumental, informational, and emotional support behaviors. Perspectives aligned regarding what support functions companions provide. However, patients and support persons emphasized the significance of instrumental followed by informational and emotional support, whereas oncology providers highlighted informational, followed by emotional and instrumental support. Discussion provides insight into each role’s perspective in the triad.

After the March 2011 Fukushima Daiichi Nuclear Power Plant accident, overseas experts and representatives of international organizations visited Japan to provide advice, technical support, and resources. Several international meetings on radiological protection and health issues have since been held in Fukushima to provide further advice. The content discussed has changed alongside local developments in health-related issues from radiation health effects and radiological protection to risk communication and psychological, public health, and social issues. The support of international organizations and experts has been valuable in implementing public health and support programs in Fukushima. The Fukushima accident showed that after a nuclear accident, authorities need to balance the risks of radiation with other health effects and develop programs to mitigate the overall effects on health (whole-health management), but there was little evidence of the importance of this at the time. Future research should examine international collaboration to assess this.

Summary This two-phase, qualitative study aims to obtain an overview of stakeholders in the network of people with intellectual disabilities (ID) and their perceived facilitating and hindering factors, expectations, and perceived roles and responsibilities with regard to health promotion. In phase 1, four workshops were conducted to provide insight into involved stakeholders. In phase 2, 29 semi-structured interviews were conducted with stakeholders regarding their views on health promotion. Data were analysed using stakeholder matrices and a combination of domain and thematic analysis. Daily caregivers were identified as the most important and influential stakeholders. Interviewed stakeholders perceived barriers to a healthy lifestyle as relating mainly to the person with ID and, although they stated that people with ID need support to be able to live healthily, there was ambiguity about roles and responsibilities for providing this support. Daily caregivers are not properly facilitated to support a healthy lifestyle. Stakeholders expressed the need for a culture change towards a greater health promotion ethos in care for people with ID. A facilitating context is needed in which the social network supports autonomy and offers opportunities to adapt to physical, social and emotional challenges. Stakeholders see the importance of, and are willing to support, healthy behaviour. They are hindered by a lack of a shared vision and united system in which all stakeholders know their roles and responsibilities. Promotion of a healthy lifestyle should be part of every service provider employee’s job and propagated throughout the organization as part of its mission and vision.

Background The uptake and utilisation of allied health assistants as professional support staff has been variable across disciplines and jurisdictions. Although they are potentially very important in the current health workforce context, there is little agreement on their roles or the most suitable methods to define these roles. Method Based on a review of literature, existing role descriptions and focus groups, a Delphi survey process was undertaken. This process comprising three rounds of discussion and clarification via email, with between 107 and 188 participants, was undertaken to define and establish consensus on allied health assistant roles at three levels. Results Three cycles of editing, qualitative feedback and rating of agreement with statements resulted in substantial clarification of roles and a meaningful degree of consensus regarding the role and scope of such positions. High levels of agreement were not reached for more high-level or contested clinical tasks. Conclusions The Delphi process resulted in key tasks and roles being defined and contentious aspects clearly identified. The process facilitated engagement with workforce members most closely affected by these questions. It was a useful means of drawing together the opinions of the workforce and informing implementation trials to follow. What is known about the topic? Allied health assistants are important members of health teams. Current developments in health services necessitate considerable growth in these positions. The role and scope of practice of allied health assistants is poorly defined and varies between disciplines, settings and facilities, which threatens the establishment of these positions. What does this paper add? This study describes a methodology used to define the role and scope of practice of allied health support staff, which resulted in high levels of consensus and documentation of concerns regarding these positions. Tasks and roles have been defined at different allied health assistant position levels. What are the implications for practitioners? The definition of roles and establishment of scope of practice of emerging positions can be substantially advanced by well researched and widely consultative methods. For more advanced allied health assistant positions to be effectively implemented, tasks relating to treatment, leadership, documentation, assessment and team participation must be clearly elucidated and agreed.

<p><i>Groupwork in a criminal justice setting has traditionally taken the form of a group of offenders undertaking a programme of structured work facilitated by (usually two) professionals. Circles of Support and Accountability (COSA) represent a departure from this traditional model. The COSA model uses a group of volunteers to form a ‘Circle’ around an offender who is viewed as being very much part of the group. The approach is currently being used in the UK as part of the risk management process for convicted sex offenders who are experiencing social isolation, a factor that has been linked to the risk of re-offending. The goal of our research was to understand the COSA group process and this article presents a study of a relatively new and different approach to groupwork within criminal justice. We looked at the expectations and opinions of both core group members and volunteers to make some assessment of how this non- traditional form of groupwork operates. The research presented an almost entirely positive picture of COSA from the perspective of both the volunteers and the core group members. All Core members (and we acknowledge that the research focus is a small group of male offenders) felt that the group experience offered them support and a sense of belonging that had previously been missing in their lives.</i></p>

An urban, public sector Area Mental Health Program reviewed its own bilingual counsellor programas part of an Area-wide quality improvement project, and found that the counsellors' roles needed tobe better defined; that mainstream staff needed to have more access to their expertise as culturalconsultants; and that their function as an Area team, rather than as service based staff, needed to beencouraged. The bilingual counsellors decided to take up this challenge, and with the support of theArea Director of Mental Health, worked together to redefine their roles.

Public Health requires a trans-disciplinary approach to tackle the breadth and depth of the issues it faces. Public health decisions are reached through the compilation of multiple data sources and their thoughtful synthesis. The complexity and importance of these decisions necessitates a variety of approaches, with simulations increasingly being relied upon. This dissertation describes several research efforts that demonstrate the utility of highly detailed simulations in public health decision-making. Simulations are frequently used to represent dynamic processes and to synthesize data to predict future outcomes, which can be used in cost-benefit and course of action analyses. The threat of pandemic influenza and its subsequent arrival prompted many simulation-based studies. This dissertation details several such studies conducted at the federal policy level. Their use for planning and the rapid response to the unfolding crisis demonstrates the integration of highly detailed simulations into the public health decision-making process. Most analytic methods developed by public health practitioners rely on historical data sources, but are intended to be broadly applicable. Oftentimes this data is limited or incomplete. This dissertation describes the use of highly detailed simulations to evaluate the performance of outbreak detection algorithms. By creating methods that generate realistic and configurable synthetic data, the reliance on these historical samples can be reduced, thus facilitating the development and improvement of methods for public health practice. The process of decision-making itself can significantly influence the decisions reached. Many fields use simulations to train and evaluate, however, public health has yet to fully adopt these approaches. This dissertation details the construction of highly detailed synthetic data that was used to build an interactive environment designed to evaluate the decision-making processes for pertussis control. The realistic data sets provide sufficient face validity to experienced public health practitioners, creating a natural and effective medium for training and evaluation purposes. Advances in high-performance computing, information sciences, computer science, and epidemiology are enabling increasing innovation in the application of simulations. This dissertation illustrates several applications of simulations to relevant public health practices and strongly argues that highly detailed simulations have an essential role to play in Public Health decision-making.

Latina breast cancer survivors, especially those who have undergone chemotherapy, may face major disruptions to their diet and eating habits; and a major concern is their ability to function in daily life roles throughout survivorship. Food and nutrition are suggested to be significant factors that can influence the process of cancer development, yet little research has been done to understand the nutritional needs, roles, and expectations among Latina breast cancer survivors. The purpose of this study was to understand Latina breast cancer survivors’ perceptions of the role of diet and nutrition in helping them cope with the effect of chemotherapy treatment. This study was guided by the Transactional Model of Stress and Coping and employed a qualitative research design. A purposive sample of Latina breast cancer survivors (n=32), caregivers identified by the survivors (n=8), and key informants (n=6) were interviewed in their language of preference (Spanish or English) using a semi-structured, open-ended interview guide that explored how coping with a cancer diagnosis influences cancer survivors and caregivers’ nutritional needs and practices. The data were analyzed using an inductive approach to develop an a priori codebook. Thematic analysis was performed using MAXQDA, and emergent codes were added as appropriate. The results yielded that Latina breast cancer survivors emphasized the value and meaning of nutrition; this was accomplished by increasing their nutritional awareness to restore health, improve the body’s immunity, and regain/maintain agency and control of their food consumption. Food provided by friends and family, assistance from nutritional organizations, doctors’ recommendations, and negative chemotherapy side effects influenced Latina breast cancer survivors and caregivers’ decisions about diet and nutrition during and after treatment. Negative chemotherapy side effects and positive support from husbands, family members, and friends affected the role of Latina women as the ‘nutritional gatekeepers’ in the family, since breast cancer diagnosis and treatment disrupt or limit the role of Latinas within the household unit. Findings from the interviews with Latina breast cancer survivors, caregivers, and key informants indicated that cultural expectations among this population shape the kinds of support and assistance that are provided by family members. Lastly, this research study revealed unmet information needs: Latina breast cancer survivors relied on organizational and online nutrition-related resources but identified a gap in culturally and linguistically tailored information related to diet and nutrition following diagnosis. The study concluded that Latina breast cancer survivors and caregivers mentioned no routine dietary assessment for cancer survivors after beginning chemotherapy and during treatment. Additionally, caregivers specified needing instrumental support to better meet the nutritional needs of Latina breast cancer survivors. This study added to our understanding of Latina breast cancer survivors’ unmet needs that could contribute to the creation of future nutrition education programs and interventions most appropriate during cancer survivorship for diagnosed individuals and their caregivers.

ABSTRACT Introduction: The present study examined whether social support served as a mediator (i.e., an apparent causal mechanism) for the relationship between mental illness (MI) and drug and/or alcohol abuse (SA). Objective: This study’s objective was to determine the role of social support as a potential mediator in the relationship between mental illness and drug and/or alcohol abuse. Methods: We utilized data from a randomized controlled trial (RCT), conducted on individuals at risk for HIV in Philadelphia jails by following 600 study participants coming out of jails. Out of these 600 individuals, data was selected for individuals with mental illness and drug and/or alcohol abuse. In the parent study, these individuals were identified by asking questions about their mental illness, and drug and/or alcohol abuse problems. They also answered Norbeck Social Support Questionnaire about social support. They provided demographic data on their age, race, gender, religion, marital status and education level, which was collected as part of a face-to-face demographic interview conducted during the baseline assessment. Results: Data was analyzed using negative binomial regression method to test for mediation effect. Results indicated that social support mediated the relationship between mental illness and drug abuse. We ran two generalized linear and one general linear regression models. In the first model, we looked at the total effect of mental illness on drug abuse (c); we found that the incidence rate for drug abuse (SA) would be expected to increase by a factor of 1.387, (IRR,1.387; CI,1.270-1.515; P =0.000) for every one-unit increase in mental illness (M1), while holding all other variables in the model constant. In the second model we adjusted our mediator, social support (SS) and looked at the direct effect of mental illness on drug abuse (c’). We interpreted that for every one-unit increase in mental illness, the incidence rate for drug abuse would be expected to increase by a factor of 2.717, (IRR,2.717; 95% CI,1.629 - 4.532; P = .000). For every one-unit increase in social support, the incidence rate for drug abuse is expected to decrease by a factor of .498, (IRR,.498; 95% CI,.443 - .560; P = .000). In the third model we found, for every one-unit increase in mental illness, there is an associated increase by 2.495 units in social support, P= .000. There was an inconsistent mediation in our model. Effect size for mediation by Percent mediation (Pm) method was found to be 0.64, it is the proportion of the effect that is mediated by our mediator social support. Sobel test showed the significance of mediation with a test statistic of 4.8282 at a significance level of 0.000. Conclusion: Our data supported an alternative theory of inconsistent mediation. We found that social support mediates the relationship between mental illness and drug abuse, where positive social support has a stimulator effect on mental illness and a suppressor effect on drug abuse. Mental illness may have direct unfavorable effects on the drug abuse (outcome) and positive social network has beneficial effects on this outcome. We conclude that positive support allows betterment of mental health of patients and prevents involvement in drug abuse. Further, there is a need to consider both the positive and negative effects of social support while keeping in mind these associations may differ among sociodemographic groups.

Attachment theory posits that insecure attachment orientations reflect activation of the attachment behavioural system, and therefore deactivation of its complementary counterpart the exploration system, which is associated with feeling less energized. We hypothesized that less perceived social support and higher fatigue would prospectively explain the link between insecure attachment and well-being in emerging adulthood (ages 18–25), a period in which exploration and social relationships are critical. Participants aged 18–25 completed surveys initially and two weeks later (N=153). Temporal multiple mediation revealed that, at T1, both forms of insecure attachment (anxious and avoidant) were associated with lower T2 well-being, with less perceived social support and higher levels of fatigue each uniquely explaining these associations, after controlling for the effects of each form of attachment on the other. Our findings suggest that deactivation of the exploration system and information processing biases regarding the availability and trustworthiness of others may compromise well-being for emerging adults with an insecure attachment orientation.

The present thesis was conceptualised and conducted against the backdrop of rapidly emerging research that challenges the conventional depiction of OCBs as positive extra-role behaviours that produce beneficial outcomes to both individuals and organisations. The thesis contends that OCBs may be either beneficial or detrimental to individual performers depending on their perceptions of the psychosocial work environment – i.e. perceived job control and perceived organisational support. Both perceived job control and organisational support have been researched in existing theoretical models and prior research which depict and assess these variables as key moderators in the relationship between work stressors and job strain. Hence, the present thesis hypothesised that both job control and support will moderate the effects of Time 1 OCBs (peer reports of OCB-I and OCB-O) on various individual-level outcomes of job satisfaction, organisational commitment, role ambiguity, role overload, work-family conflict, physical exhaustion and work-related depression measured at Time 2, based on a two-wave longitudinal panel methodological design. This newly proposed moderation model was tested across three interrelated Studies (Study 1, Study 2, and Study 3) in which the first two studies were cross-sectional based on Time 1 and Time 2 data, respectively, and the final study provided a longitudinal version of the same analyses. A direct effects model (where the effects of Time 1 OCBs on the Time 2 outcomes were assessed) and a mediation model (in which role stressors were modelled as mediators between OCBs and job attitudes and health) were also examined, alongside the proposed moderation model. In Study 1, based on data from 562 employees in Barbados captured at the first wave, structural equation modelling (SEM) analyses revealed that the direct effects model emerged superior to the mediation and moderation models. There were no significant interaction effects of control and support on any of the outcomes in Study 1. In Study 2, based on data from 427 employees (an attrition rate of 24%) captured at the second wave, the SEM analyses revealed that both mediation and moderation models emerged as the superior models. In the moderation model, both control and support emerged as significant moderators in several relationships between OCBs and the outcome variables. Finally in Study 3, the longitudinal SEM analyses revealed that the ‘normal causation’ direct effects model emerged superior to the reverse and reciprocal causation models as well as the mediation models. The moderation model also emerged as a superior model in which both control and support moderated several relationships between Time 1 OCBs and Time 2 outcome variables. Overall, the present thesis provided some support for the proposed moderation model and is consistent with key assumptions underlying existing theoretical models and findings of prior research on the stressor-strain relationship. The findings reinforced the role of personal job resources such as job autonomy and organisational support as critical factors that can buffer the potentially negative effects of OCBs for individual performers. Theoretical and practical implications, future research recommendations, and study limitations have been discussed in the final chapter of the thesis.

In this study, autism parent-led support groups and the contribution these groups had in creating parental advocacy were studied. The primary purpose of the study was to determine if autism parent-led support groups promoted advocacy skills of parents of children with autism. The secondary purpose of the study was to evaluate the level and extent of support provided by parent-led support groups. The qualitative data collection was done through individual interviews of parents of children with autism whose children were of any age. Through data analysis 6 major themes were identified: advocacy, emotional support, difficulty with funding, informational support, instrumental support, and appraisal support. Parents indicated that parent-led support groups assisted them in acquiring advocacy skills and provided good emotional support, informational support, and appraisal support. This research was important because parents of children with autism have greater problems with treatments than parents of children with other chronic developmental disorders, and many parents felt they needed to obtain advocacy skills in order to find the best interventions for their child. The social change implication of this study is that people can better understand the impact that parent-led support groups can have in facilitating advocacy among parents who participate in the groups. This study also brings to light the support these groups provide, and how this support impacts their lives.

This three-paper dissertation examined the association between social and economic factors and HIV risk behavior among adolescents and young adults in Cape Town, South Africa. Studies in South Africa have demonstrated an important association between receiving financial or material resources and sexual risk behaviors. Most research has focused on economic exchanges that occur as part of transactional sex, and not economic support in main partnerships—which may not be transactional. Furthermore, little research has focused on men. For the first paper, cross-sectional data from Black African and Coloured substance-using women (N = 671) were analyzed to examine the association between the frequency of economic support and unprotected sex in main partnerships. Negative binomial and multinomial logistic regression analyses were conducted to model the number of unprotected sex acts and the consistency of condom use, respectively. Results indicated that receiving daily or almost daily support was positively associated with the number of unprotected sex acts; an association was not observed with consistency of condom use. For the second paper, binomial logistic regression was used to examine whether receiving economic support from a main partner was associated with unprotected last sex, and to test whether gender modified this association among young Black African and Coloured women and men (N = 1,456). Economic support from a main partner was not associated with sexual risk; however, receiving economic support from someone in the household other than a main partner was protective for women. Employment was protective for men and women. The third paper used discrete-time hazard analyses to examine whether employment and schooling were associated with sexual initiation among Black African, Coloured, and White adolescents (N = 1,326). Results indicated that although employment was not associated with sexual initiation, working while in school was associated with greater odds of sexual initiation. These papers provide a better understanding of the social and economic factors that may impact sexual risk behavior. The association between employment and risk behavior is complex, and how sexual risk is defined and measured matters. Findings also suggest the importance of assessing multiple sources of economic support, including frequency of support.

ADHD is one of the most common behavioral disorders diagnosed in children. These children have difficulties regarding the regulation of emotions, maintaining attention and impulse control, all of which influence family and social life. The aim of this study was to describe and explore the everyday life of families with a child with ADHD and public health nurses’ role in relation to these families. The parents were contending with- and adapting to the parental role and social network. The family attempted to safeguard a functioning family in managing their everyday life, tuning themselves in on the child’s shifting moods, using strict boundaries and developing special skills. The family fought for acceptance and inclusion when interacting with their social network and professionals. Parents with ADHD and families with non-medicated children reported more problems in family functioning. Characteristics in parents and the child with ADHD, as well as support from the social network and community health services, all influenced family functioning. The PHNs described their role as both a peripheral and collaborating partner, asking for guidelines and multidisciplinary collaboration. The public health nurse is in a unique position to support and supervise these families.
Aim: The overall aim of this thesis was to describe and explore everyday life in families with a child with ADHD and public health nurses’ role in relation to these families. Methods: An explorative and descriptive design with qualitative and quantitative methods was used. In Study I, data was collected with individual interviews with nine parents, and analyzed using phenomenology. In Study II, the data was collected with individual interviews with 17 family members, and analyzed with phenomenography. In Study III, data was collected with a questionnaire responded by 265 parents, and analyzed with statistics. In Study IV, data was collected with group- and individual interviews with 19 public health nurses, and analyzed with phenomenography.                                                                                                  Main findings: The families’ everyday life was influenced by living in unpredictability, though they were striving for predictability. The experience of being a parent was described as contending and adapting every day, like windsurfing in unpredictable waters (I). The family tried to safeguard a functioning family in managing their everyday life and developing special skills, within the family and the society. They fought for acceptance and inclusion in relation to the social network and professionals (II). Parents’ sense of coherence, children’s behavior, support from social networks and community health services had all an impact on family functioning (III). The PHNs described their role as both a peripheral and a collaborating partner and they asked for guidelines and multidisciplinary collaboration (IV). Conclusions: Everyday life in families with ADHD is both demanding and giving. Acceptance and support from the social network and supervision from the professionals are essential. The public health nurse is in a unique position to support and supervise these families.

Abstract This chapter analyses the challenges and the adequacy of existing frameworks to provide a strong foundation to support global responses to antimicrobial resistance. Calls for global responses are indicative of a growing global commitment to seeking practical means of tackling the growing problem of antimicrobial resistance. While antimicrobial resistance is often conceptualised as an emergency, the application of the International Health Regulations, designed to govern responses to public health emergencies of international concern, remains unclear. Furthermore, there may be challenges for countries in developing and resourcing national approaches to address antimicrobial resistance. Clarity and agreement around definitions of key concepts related to antimicrobial resistance will also be essential to antibiotic stewardship and development of policy in this area. Finally, improvements to health systems as a result of the Sustainable Development Goals may help to support improvements in public health and may play a role in global strategies to address antimicrobial resistance.

AbstractIncluding the voice of the father in research related to fathering is essential. This chapter describes efforts to establish a new public health surveillance for fathers in the United States. The ultimate goal of this research is to study fatherhood to support healthy child development by collecting valuable information on father health and experiences in the perinatal period. Understanding fathers is key to learning about and improving family health, since fathers impact mothers and babies’ health. Studying the health of fathers during the transition to fatherhood can also provide a window into the overall health status of men, expanding the field of public health to include more specifics on the health of fathers and their impact on families. Currently surveillance systems are lacking in including the fathers’ perspective in measurement of paternal health and the transition to fatherhood. To address these issues we utilized a multi-pronged approach to inform development of a public health surveillance system for fathers: (1) review of the current literature to identify gaps in knowledge on the role of fatherhood in male and family health and identify current national-level surveillance data on fathers; (2) assessed feasibility of identifying participants to implement a surveillance system for fathers; (3) conducted formative research to develop methodology; and (4) piloted a public health surveillance system called the Pregnancy Risk Assessment Monitoring System for Dads or “PRAMS for Dads.”

ABSTRACT Background: Young mothers go through the transitional stages of becoming parent and adult simultaneously. This poses a breastfeeding challenge that older mother does not encounter. The coverage of breastfeeding in young mothers was lower and the average duration of breastfeeding was shorter than that of adult mothers. The study aimed to review experience of breastfeeding in young mothers. Subjects and Method: A scoping review framework were adapted from Arksey and O’Malley method. It consists of five stages: 1) Identify research questions, 2) Identify relevant articles, 3) Select studies, 4) Comply data, 5) Summarize and report results. The inclusion criteria were original research, published from 2009-2019, and in English language. The exclusion criteria were randomized controlled trial study, reviewed articles, and reports or books. The quality of the article was assessed using Hawker’s Quality Assessment Tool and reported by PRISMA flowchart. Results: Eleven articles were selected from 562. The experience of breastfeeding in young mothers has been described in three mains thematic: (1) Mother decision to breastfeed were more self-centered, (2) The success of breastfeeding was supported by many factors, including feeling comfortable while breastfeeding, family and peer support, knowledge of breastfeeding benefits, and support from health workers, and (3) Obstacles during breastfeeding such as mothers feeling confined, community responses, worries about food consumption, have many roles, the assumption that babies are more satisfied with formula milk, physical discomfort, and inadequate response from health workers. Conclusion: Experience of breastfeeding in young mothers is influenced by three aspects: 1) Decision making to breastfeed, 2) Factors that support the implementation of breastfeeding, and 3) Constraints during breastfeeding. The World Bank (WB) was established in 1944 for the purpose of issuing long-term loans to governments for reconstruction and economic development following the Second World War (1). Over the time perspectives on development have changed dramatically. In particular, the WB’s focus began to shift to investments in health, energy, telecommunication, transport and infrastructure to earn more profit. In the field of health, the WB has focused on three areas, especially in developing countries: health, nutrition, and population. WB now has a more sophisticated view of well-being, living standards, and poverty and is committing more than 1 billion USD annually for new health projects. (2). The WB’s roles include financing, provision of information, surveillance, technical assistance and training and policy advice (3). The WB has achieved some gains in the fight against poverty. Reducing poverty focuses in part encompassing policies to promote equality but inequalities are still increasing all over the World, especially in the developing countries (4). However, the WB has faced a lot of critiques related to health. Some critiques related to health sector polices and say the bank's conditions on borrowing countries emphasize privatization and public sector contraction. This involved reducing government expenditures (in some cases for health) which have deleterious health effects (2). Other critiques related to the way of raising funds called a user charge for using public sector health services and point to evidence showing that user charges result in a decline in the uptake of services, especially among the people who are most socioeconomically deprived. The bank is also criticized for introducing DALYs to global health assessments. Critics point out that the introduction of DALYs was not based on sound methodology and that the underlying assumptions for their usefulness are weak (2, 5). Finally, the WB is also accused of bribing or conniving top government officials in the developing countries where it projects (6). There is a need for strengthening across the WB in several areas. A critical area is for the WB to strengthen its ability to work on multi-stakeholder solutions through engagement with the public sector, private sector, and citizens, and support primarily the public sector for health services. Similarly, about half of low-income countries are classified as fragile and conflict-affected, posing particular challenges. Furthermore, progress in fighting against poverty and sharing prosperity is accompanied by rising inequality in many countries. So, the WB should increase its efforts to address these issues (4). Introducing evidence into policy making is also a key issue to be strengthened for the future (5). It is also needed to strengthen the monitoring and evaluation methods in the countries.

ABSTARCT Background: Postpartum blues or baby blues is a transient state of increased emotional reactivity experienced by some postpartum mothers within 14 days after delivery. Family support, especially the husband’s role, is important to overcome the postpartum blues experience of mothers. This study aimed to determine the influence of husband’s support on health-seeking behavior in Madurese mothers with postpartum blues. Subjects and Method: This was a cross-sectional study conducted at Maron Community Health Center, Probolinggo, East Java, from April to June 2020. A total of 40 Madurese mothers with postpartum blues was selected for this study. Postpartum blues was measured by the Edinburgh Postnatal Depression Scale (EPDS). The dependent variable was health-seeking behavior. The independent variable was husband’s support. The data were collected using questionnaire. Correlation between health-seeking behavior and husband’s support was analyzed by Spearman rank correlation. Results: Health-seeking behavior was positively correlated with husband’s support (r= 0.95; p <0.001). Conclusion: Health-seeking behavior is positively correlated with husband’s support among women with postpartum blues. ABSTRACT Background: Impaired growth and development of children remains a serious problem globally. The role of the family, especially the support and participation of parents are the important factors in monitoring the growth and development of infants/ toddlers. This study aimed to determine the effect of family support on the growth and development of infants/ toddlers. Subjects and Method: This was a cross-sectional study conducted at Kragilan Village, Mojolaban, Sukoharjo, Central Java in November 2018. The study subjects were total of 115 mothers with children aged 1-60 month. Multistage cluster sampling technique was conducted. Dependent variable was growth and development of children. Independent variable was family support. The data for family support was collected by questionnaire. The data for growth of infants/ toddlers was collected by assessing growth chart (monitoring whether weight gain or loss from previous month), assessing length/height-for-age (normal height or stunted), head circumference measurement (macrocephalic/ microcephalic/ normocephalic), and assessing teething chart (erupts/ loss of teeth in accordance with age or not). The data for development of infants/ toddlers was collected by evaluating the gross motor, fine motor, language, and social skills based on child development pre-screening questionnaire. Results: Good family support increases growth (b= 0.895; p <0.001) and development (b= 0.337; p <0.001) of infants/ toddlers. This study aimed to examine the effect of activity level of the integrated health post on the community health workers performance on child growth and development health services Subjects and Method: A cross-sectional study was conducted at 25 posyandus in Karanganyar, Central Java, from August to September 2019. A sample of 200 cadres was selected by stratified random sampling. The dependent variable was cadre performance. The independent variables were education, employment, knowledge, attitude, training, tenure, posyandu facilities, social support, and village government support. The data were collected by questionnaire and analyzed by a multiple logistic regression run on Stata 13. Results: Posyandu cadre with good performance was 50.50%, had education ≥Senior high school was 63%, and working at home was 88%. Cadre performance on child growth and development health services increased with education ≥Senior high school (b= 1.27; 95% CI= 0.24 to 2.30; p= 0.015), working at home (b= 1.41; 95% CI= 0.39 to 2.42; p= 0.007), high knowledge (b= 1.53; 95% CI= 0.56 to 2.49; p= 0.002), positive attitude (b= 1.41; 95% CI= 0.33 to 2.50; p=0.011), had trained ≥2 times (b= 1.33; 95% CI= 0.37 to 2.29; p=0.007), tenure ≥10 years (b=1.21; 95% CI= 0.25 to 2.18; p= 0.014), good facilities (b= 1.57; 95% CI= 0.54 to 2.59; p= 0.003), strong social support (b= 1.28; 95% CI= 0.28 to 2.29; p= 0.013), and strong village government support (b=1.28; 95% CI= 0.26 to 2.31; p=0.014). Posyandu had strong contextual effect on cadre performance on child growth and development health services with intra-class correlation (ICC)= 27.55%. Conclusion: Cadre performance on child growth and development health services increases with high education, working at home, high knowledge, positive attitude, had trained ≥2 times, tenure ≥10 years, good facilities, strong social support, and strong village government support. Posyandu has strong contextual effect on cadre performance on child growth and development health services. Background: HIV still becomes major global public health problem in the world. People living with HIV AIDS can suffer opportunistic infection, for example Tuberculosis. The role of family and local support is important to prevent opportunistic infection. The study aimed to determine the association between family characteristics and local support (servant leadership) to find TB case in People Living with HIV AIDS. Subjects and Method: This was a case control study conducted in Kupang city, East Nusa Tenggara, in July 2020. A sample of 100 people consisted of 50 cases and 50 control was selected by purposive sampling. We measured family characteristics and local support (servant leadership (altruistic calling, emotional healing, wisdom, persuasive or motivation) and social modals (beliefs, norms, networks). Data was analyzed by using multivariate logistics regression. Results: The study subjects who did not work had 4.76 times odds of having a higher perception of local support for altruistic calling domain (AOR= 4.76; 95% CI= 1.09 to 20.91; p= 0.039). the study subjects who did not work (AOR= 4.64; 95% CI= 1.07 to 20.03; p= 0.040), worked as entrepreneurs (AOR= 4.69; 95% CI= 1.17 to 18.82; p= 0.029), had a junior high school education (AOR= 6.41; 95% CI= 1.33 to 30.94, p= 0.021), had a senior high school education (AOR= 3.47; 95% CI= 1.16 to 10.41; p= 0.0260) had higher local support for emotional healing domain. The unemployed study subjects had 5.04 times odds of having higher perceptions of local support for persuasive or motivation domain (AOR= 5.04; 95% CI= 1.07 to 23.64; p= 0.041). Conclusion: Family characteristics have significant relationship with perceptions of local support. The government needs to seek the role of family and local support in preventing opportunistic tuberculosis infection in people living with HIV AIDS. ABSTRACT Background: Health workers (cadre) play role in controlling non-communicable disease by promoting people to participate in any integrated health posts (posbindu) activities. This study aimed to examine factors affecting the performance of community health workers at the integrated non communicable disease health post in Karanganyar, Central Java. Results: Cadre performance increased with high motivation (OR= 6.15; 95% CI= 2.75 to 13.76; p<0.001), good leadership (OR= 2.24; 95% CI= 1.04 to 4.83; p= 0.039), strong social support (OR= 3.62; 95% CI= 1.68 to 7.80; p=0.001), good skill (OR= 5.56; 95% CI= 2.40 to 12.86; p<0.001), had trained (OR= 2.42; 95% CI= 0.91 to 6.42; p= 0.076), and tenure ≥1 year (OR= 5.56; 95% CI= 2.40 to 12.86; p<0.001). Conclusion: Cadre performance increases with high motivation, good leadership, strong social support, good skill, had trained, and tenure ≥1 year. ABSTRACT Background: Midwives performance play an important role in reducing maternal and neonatal morbidity and mortality. The purpose of this study was to analyze factors associated with midwives performance in Mojokerto, East Jva, Indonesia. Subjects and Method: A cross sectional study was carried out in Mojokerto, East Java, Indonesia. A sample of 200 midwives who worked in community health centers was selected randomly. The dependent variable was works performance. The independent variables were age, tenure, employment status, perceived financial compensation, social support, workload, and motivation. The data were collected by a questionnaire and analyzed by a multiple logistic regression run on Stata 13. Results: Work performance in midwives increased with age ≥42 years (OR= 9.2; 95% CI= 1.91 to 44.72; p= 0.006), tenure ≥18 years (OR= 4.5; 95% CI= 1.04 to 19.46; p= 0.044), high perceived financial compensation (OR= 10.65; 95% CI= 2.23 to 50.97; p= 0.003), strong social support (OR= 12.53; 95% CI= 2.59 to 60.70; p= 0.002), low workload (OR= 10.88; 95% CI= 2.41 to 49.12; p= 0.002), and strong motivation (OR= 13.52; 95% CI= 2.64 to 69.21; p= 0.002). Work performance decreased with non civil servants (OR= 0.071; 95% CI= 0.01 to 0.55; p= 0.011). Conclusion: Work performance in midwives increases with age ≥42 years, tenure ≥18 years, high perceived financial compensation, strong social support, low workload, and strong motivation. Rapid population growth, poor and uneven economic performance, and slow progress in health and education are interrelated phenomena. However, while there is strong support for public policies aimed at economic recovery and human resource development in the Philippines, there is still a lack of consensus on the need for public policy to moderate population growth and on the role of a government-sponsored family planning program in overall population and development activities. This paper examines alternative population projections and analyzes the contribution to future population growth of unwanted fertility, high desired family size, and population momentum. The aims are to highlight the multiple policy responses that are needed to moderate rapid population growth and to clarify a number of factors that have prevented the development of a consensus on Philippine population policy.

At national and aggregate levels, COVID-19 vaccination across G7 countries appears successful. To date, 79.4% of the total population of G7 countries have received a first dose, 72.9% a second, and 45.4% a booster shot (28th April 2022 data). In France, 80.6% of the total population has had a first dose, 78.2 % have had two doses, and 55.4% have had their booster jabs (28th of April 2022 data). In the UK, 79.3% of the total population has received one dose, 74.1% a second one, and 58.5% have received a booster. In Italy, 85.2% of the total population has had a first dose, 80.4% have had two doses, and 66.5% have had their booster jabs (28th of April 2022 data). These figures indicate enthusiasm across G7 countries for COVID-19 vaccines. Yet high overall vaccination rates at the national level, disguise significant in-country disparities. For example, by the end of 2021, less than 50% of residents of the Northern Districts of Marseille were vaccinated, compared with over 70% in wealthier neighbourhoods. In the Ealing borough of Northwest London, 70% of the eligible population has had a first dose – which is almost 10% percent below the national average (4th of April 2022 data). Disparities are also seen in other urban metropolises across the G7. This brief investigates these disparities through the lens of “vaccine (in)equity”, focusing on the role of local actors. It builds on ethnographic and qualitative research carried out in the Northern Districts of Marseille and ongoing research engagement around vaccine equity in Ealing (Northwest London), as well as qualitative research carried out in Italy among networks of healthcare providers, intercultural mediators, and civil society organizations that collaborated during the COVID-19 campaign in the Emilia Romagna region and in Rome. This brief is based on research conducted between October and December 2021 in Marseille and ongoing engagement in Ealing which started in May 2021. It identified how local governments, health actors, community groups and residents play key roles in shaping vaccine (in)equity. This brief was developed for SSHAP by Santiago Ripoll (IDS), Tabitha Hrynick (IDS), Ashley Ouvrier (LaSSA), Megan Schmidt-Sane (IDS), Federico Federici (UCL) and Elizabeth Storer (LSE). One of the roles of the ANE OR/TA Project in India was to participate in policy dialogues with national counterparts, in the public sector and among NGOs, about expanding contraceptive choices, and to provide technical assistance for facilitating changes in service-delivery procedures. The public sector provides five contraceptive methods through its 11,500 hospitals and primary health care facilities. NGOs, private physicians, and pharmacies have access to a broader range of brands. While India is one of the world's leaders in contraceptive research, in recent years products have come to market slowly. New technology is often embraced, however the cost of contraceptive options is not trivial in the Indian context. As stated in this report, the OR Project became formally involved in the effort to expand contraceptive choices in 1993 at the request of the USAID Mission in India. The Secretary of Family Welfare supported concerns for quality and choice as part of the preparation for the International Conference on Population and Development held in Cairo, September 1994. Similar recommendations were made during development of a draft national population policy.

The COVID-19 pandemic has created enormous challenges for national economies, livelihoods, and public services, including health systems. In January 2021, the World Health Organization proposed an international treaty on pandemics to strengthen the political commitment towards global pandemic preparedness, control, and response. The plan is to present a draft treaty to the World Health Assembly in May 2021. To inform the design of a support system for this treaty, we explored existing mechanisms for periodic reviews conducted either by peers or an external group as well as mechanisms for in-country investigations, conducted with or without country consent. Based on our review, we summarized key design principles requisite for review and investigation mechanisms and explain how these could be applied to pandemics preparedness, control, and response in global health. While there is no single global mechanism that could serve as a model in its own right, there is potential to combine aspects of existing mechanisms. A Universal Periodic Review design based on the model of human rights treaties with independent experts as the authorized monitoring body, if made obligatory, could support compliance with a new pandemic treaty. In terms of on-site investigations, the model by the Committee on Prevention of Torture could lend itself to treaty monitoring and outbreak investigations on short notice or unannounced. These mechanisms need to be put in place in accordance with several core interlinked design principles: compliance; accountability; independence; transparency and data sharing; speed; emphasis on capabilities; and incentives. The World Health Organization can incentivize and complement these efforts. It has an essential role in providing countries with technical support and tools to strengthen emergency preparedness and response capacities, including technical support for creating surveillance structures, integrating non-traditional data sources, creating data governance and data sharing standards, and conducting regular monitoring and assessment of preparedness and response capacities.

Childhood asthma is a significant public health problem in the United States. Barriers to effective asthma management in children include the need for caregivers to identify and manage diverse environmental triggers and promote appropriate use of preventive asthma medications. Although health care providers may introduce asthma treatments and care plans, many providers lack the time and capacity to educate caregivers about asthma in an ongoing, sustained manner. To help address these complexities of asthma care, many providers and caregivers rely on patient navigators (defined as persons who provide patients with a particular set of services and who address barriers to care) (Dohan & Schrag, 2005). Despite growing interest in their value for chronic disease management, researchers and providers know little about how or what benefits patient navigators can provide to caregivers in managing asthma in children. To explore this issue, we conducted a mixed-method evaluation involving focus groups and a survey with caregivers of children with moderate-to-severe asthma who were enrolled in the Merck Childhood Asthma Network Initiative (MCAN). Findings suggest that patient navigators may support children’s asthma management by providing individualized treatment plans and hands-on practice, improving caregivers’ understanding of environmental triggers and their mitigation, and giving clear, accessible instructions for proper medication management. Study results may help to clarify and further develop the role of patient navigators for the effective management of asthma in children.
