We read the article by Alain and Zipf who reported a 4-year-old boy with elevated thyroxine-binding globulin (TBG) level, elevated serum concentrations of both tri-iodothyronine (T3) and thyroxine (T4), short stature, delayed bone age, and an exaggerated thyroid-stimulating hormone (TSH) response to thyrotropin-releasing hormone stimulation. The child showed a marked improvement in growth with T3 therapy. In searching through the literature, we have come across a few more examples of TBG excess associated with hypothyroidism.
