Same-sex and gender-variant couples are similar to heterosexual/gender-traditional couples in many ways. However, lesbian, gay, and bisexual couples and couples with one or both transgender partners (LGBT couples) exist in a stigmatizing environment that provides no social structures for them. As a result, these couples face three types of challenges: minority stress, lack of social support, and role and relational ambiguity. The author reviews research on these three challenges and offers specific techniques to address them. A conceptual model creates a bridge between sociocultural challenges and the psychological-relational consequences, affording therapists an understanding of how to plan and implement effective interventions. In addition, seven basic premises provide parameters for work with LGBT couples.

The American Psychiatric Association’sPrinciples of Medical Ethicsemphasize competence, respect and up-to-date knowledge as a basis for appropriate professional behavior toward lesbian, gay, bisexual, and transgender (LGBT) patients. This chapter first reviews historical psychiatric attitudes towards LGBT patients that could be construed, at best, as patronizing and, at worst, overtly hostile. In modern clinical practice, as opposed to trying to “cure” homosexuality or “transsexualism,” LGBT patients are helped to live their lives according to their own natures and desires. This chapter outlines some common clinical questions raised by LGBT patients—what is known and not known about the origins of homosexuality and transgenderism, sexual orientation conversion efforts (SOCE), therapist self-disclosure, how therapists should address LGBT patients, and controversies surrounding treatment of transgender children—as well as ethical issues raised in these clinical encounters.

In a world that is responding to ever-changing ideas and expressions of gender, this book adds new insights on transgender children and the parents who support them. Drawing on in-depth interview data with more than fifty parents, the book examines parents’ shifting understandings of their children’s gender and how they come to help their children make sense of their identities and their bodies. Throughout these processes, the book shows that parents’ meaning-making and decision-making often challenge LGBT rights discourses, as well as queer political tenets, in unexpected ways. These dynamics surface in three key areas: (1) gender and sexuality, (2) the gender binary, and (3) the body. Throughout parents’ understandings, gender identity and sexual orientation do not always present as radically separate aspects of the self, but are more fluid and open to reconsideration, given new cultural contexts, opportunities, and phases of the life course. And despite increasing cultural visibility around nonbinary identities, “gender-expansive” child-rearing often looks, fundamentally, very binary and gender-stereotypical, per the children’s own assertions and expressions. Lastly, parents often utilize highly medicalized understandings of transgender embodiment, which nevertheless resonate with some children’s sensibilities. Altogether, these families depart from conventional understandings of gender, sexuality, and the binary, but in ways that prioritize child-centered shifts, meanings, and parenting models, not necessarily LGBTQ politics or paradigms. This marks new ground for understanding the mechanisms and parameters of the (trans)gender change afoot.

This chapter explores compañeros' experiences of stigmatization related to their nonconforming gender behavior. Collectively, the life stories show that childhood is the period in which most stigmatization is experienced—and perhaps causes the most lasting consequences. At this time in life, family and school are the most salient sources of stigma, as they are the main socializing agents. Some of these compañeros internalized such stigma, but most of them have been able to overcome the internalization in the course of their lives. Yet, they speak of depression, failed suicide attempts, and dislocation as consequences of the stigma they endured. The experienced stigma and its consequences are particularly severe for those activists who identify as drag queens, transexual, or transgender. Since they were children, their actions were perceived by others as flagrant transgressions of gender norms.

The volume explores the everyday security practices of various people in Kyrgyzstan that feel threatened on the grounds of their ethnic belonging, gender or sexual orientation. In doing so, it provides a bottom-up perspective of security and insecurity in Kyrgyzstan, which differs from more state-centric and elitist accounts on this subject. Case studies include the Uzbek and the Lyuli minorities in the city of Osh in southern Kyrgyzstan, young women in the capital city of Bishkek, ethnically mixed couples and the lesbian, gay, bisexual and transgender (LGBT) community. Each case applies ethnographic methods to follow individuals in their everyday lives and asks how they deal with the various insecurities they face. The volume studies security in cafes and restaurants, in kindergartens and schools, public transport, bazaars, taxis, virtual chat rooms and nightclubs. It argues that seemingly trivial aspects of everyday life, such as food and music, children's education or romantic first love, are important to gaining a more comprehensive picture of what security in Kyrgyzstan is all about. All contributions apply the analytical concept of securityscapes. The volume should be of relevance to scholars and students from social anthropology, security studies, gender studies and queer studies with an interest in Central Asia.

The Handbook of Evidence-Based Mental Health Practice with Sexual and Gender Minorities represents the first compendium of evidence-based approaches to sexual and gender minority (SGM)-affirmative mental health practice. In the past several years, clinical researchers have begun developing and adapting evidence-based mental health treatment approaches to be affirmative of SGM individuals’ unique mental health. Because these approaches draw on research documenting unique psychosocial processes underlying SGM individuals’ mental health as well as adapt existing evidence-based treatments to impact these processes, these treatments can be considered evidence-based. Because these approaches promote effective coping with stigma-related stress and are often developed with feedback from SGM community members, these treatments can also be considered SGM-affirmative. This handbook compiles these approaches, including evidence-based treatments for specific populations within the SGM community (e.g., youth, transgender individuals, same-sex couples, parents, and bisexuals), for specific mental health problems (e.g., anxiety, depression, substance abuse, trauma, eating disorders, sexual health), and using novel modalities (e.g., group therapy; acceptance-based, dialectical behavior therapy; attachment-based, transdiagnostic therapy). Each chapter includes conceptual background and practical guidance so that mental health practitioners, researchers, educators, and students can both understand how to implement each of these approaches and develop future tests of their efficacy and the efficacy of other SGM-affirmative approaches.
