Abstract:
This research aims to determine the effect of team cohesion on work engagement with the mediating role of psychological empowerment. JD-R model is used to explain the phenomenon. We learn about how people who are engaged in ongoing working in team give an impact on people work engagement. Based on quantitative research from 557 participants from Indonesian military forces that were temporarily placed to carry out peace missions on conflict countries. By using Process to analyze data, we found that psychological empowerment partially mediate the relationship of team cohesion on work engagement (path a: Coeff = .29, SE = .04, p < .01, 95%CL .22 to .37; path b: Coeff = .23, SE = .03, p < .01, 95%CL .17 to .30; path c’, Coeff = .10, SE = .03, p < .01, 95%CL .04 to .17).

Abstract:
299 Background: Administrative claims data are widely used to identify treatment patterns. However, as new drugs are brought to clinical practice, treatment regimens become more complicated for cancer patients receiving both chemotherapy and biologic agents. We evaluated the influence of using different treatment length algorithms on identification of treatment patterns for patients with metastatic colorectal cancer (mCRC). Methods: We identified patients aged 18 or older with at least one primary CRC diagnosis between 01/01/2008 and 03/31/2012 in the MarketScan databases. mCRC patients were included if claims indicated the presence of distant metastases (ICD-9-CM codes: 196.x, 197.0 to 197.4, 197.6 to 197.8, 198.x, and 199.0). All treatment claims were extracted and patients were followed for more than 6 months, until end of enrollment or study end date (03/01/2013). We identified all chemotherapy and/or biologic drugs given to a patient during 30, 60, or 90 days after initiation of treatment and compared differences in type and number of treatment lines (TLs). Results: A total of 2,046 mCRC patients received at least one TL. Using a 30-day cutoff period, 30% patients received 1 TL; 30% received 2 TLs; and 40% received ≥3 TLs. Results changed after using a 60-day cutoff period: 39% patients with 1 TL, 33% with 2 TLs, and 28% with ≥3 TLs. If using 90 days as a cutoff period, 44% received 1 TL; 35% received 2 TLs; and 22% received ≥3 TLs. For all cutoff periods, the most common first TL prescribed were FOLFOX +/- bevacizumab and 5-FU/LV. For patients receiving a second TL, FOLFOX was most frequently prescribed if using a 30-day or 60-day cutoff, while FOLFIRI/bevacizumab had a higher prescription rate with a 90-day cutoff. Regimens varied in third and fourth-line therapy using different cutoff periods and FOLFIRI +/- bevacizumab was more frequently used than other regimens. Conclusions: The cutoff period did not significantly influence treatment regimens detected for first and second TL. Using a 30-day cutoff period resulted in 30% patients with one TL and 40% with three or more TLs, while with a 90-day cutoff, 44% patients received one TL and only 22% received three or more TLs.

Abstract:
Abstract Introduction Approximately 40%-60% of menopausal women suffer from sleep disturbances, commonly linked to vasomotor symptoms of hot flashes and night sweats. This study aims to evaluate sleep patterns and quality in peri- and post-menopausal women using actigraphy. Methods Thirty-nine participants (45 to 65y, BMI 25.62±3.02 kg/m², Insomnia Severity Index (ISI) score 10.98±4.10) experiencing menopausal symptoms underwent actigraphy recordings of sleep onset/offset, duration, and mid-sleep times over at least three consecutive nights with 357 nights of data analysed. Results The vasomotor subscales (hot flash and night sweat, Greene Climacterics questionnaire) yielded a score of 2.43±1.15 (compared to general population: 1.79±1.12 and menopause clinic: 4.41±1.70). Sleep onset times ranged from 19:06-03:16 with the most common time at 22:00-22:59 (31.1%) followed by 23:00-23:59 (27.5%). Sleep offset times ranged from 03:39-11:28 with the most common time at 06:00-06:59 (34.5%). The average sleep duration ranged from 231 to 819 minutes with the most common duration 470-529 min (33.6%) with 43.1% of nights with <8h of sleep. The mid-sleep times ranged from 23:50-05:19 with the most common mid-sleep time at 02:30-02:59(19%), followed by 02:00-02:29 (17.9%) and 03:00-03:29 (17.4%). Discussion In consideration of demographic factors, moderate vasomotor symptoms and some irregularities in sleep-wake timing, the menopausal women in this study largely have adequate sleep duration of around eight or more hours. It is likely that their symptoms were not as severe as the clinic sample. However, it is not clear if better sleep in this group is linked to increased awareness of sleep as a health behaviour.

Abstract:
ABSTRACT Objective: To investigate the potential relationship between the human leukocyte antigen (HLA) type (class I and II) and the response to several disease-modifying therapies (DMTs) in patients with multiple sclerosis (MS). Methods: We analyzed clinical data of 87 patients with MS at the beginning and end of each type of DMT including the disease duration, Expanded Disability Status Scale and Multiple Sclerosis Severity Score (MSSS). Genotyping of HLA-DRB1, HLA-DPB1, HLA-DQB1, HLA-A, HLA-B and HLA-C alleles were identified using high-resolution techniques. Statistical correlation between the HLA type and response to DMTs was done using the initial and final MSSS. Results: Statistical relationships (p < 0.05) were found for only 15 of 245 alleles tested. There was a reduction in the MSSS for patients treated with corticosteroids (DRB1*15:01, DPB1*04:01, DQB1*02:01 and DQB1*03:01), azathioprine (DRB1*03:01, DPB1*04:01, DQB1*03:02, DQB1*06:02, HLA-C*07:02), interferon β-1a 22 mcg (DRB1*11:04, DQB1*03:01 and DQB1*03:02), interferon β-1a 30 mcg (DPB1*02:01, HLA-C*05:01) and interferon β-1b (DQB1*02:01). Conclusion: These findings suggest a few relationships between the HLA and response to DMTs in the disability for some types of HLA class I and II alleles in a specific subset of MS patients.

Abstract:
Horne-Austberga, IPAQ,
 MCTQ и PSQI.

Abstract:

Abstract:
This article brings to the fore the need to approach current education from the perspective of cultural diversity, taking into account its religious-moral dimension. It can be seen that the religiousmoral dimension of education leads to the acceptance of cultural diversity, as a norm of conduct, this being the ideal towards which any society tends, at all times. All the more the reference is made to the approach to diversity with flexibility and tolerance in the last 30 years, from the perspective of sociocultural, linguistic, psychological or physical differences between students. The school, as an educational and cultural institution, aims to offer equal opportunities to everyone, the opportunity to learn at their own pace and adapted expression of individual personality traits. From a global perspective, cultural diversity implies the application of the principle of normalization. This ensures the access of all people to current living conditions, regardless of ethnicity, race, religion, culture, social group, etc. Each student, as a partner in the act of teaching-learning-evaluation, comes with his own experience, has his own personality, which we find in collaborative relationships, learning style, interest in existential issues and, necessarily, in the cultural context to which he belongs.

The nappe flow regime is divided into three sub-types, characterized by the formation or absence of hydraulic jumps on the bed of the stairs. The skimming flow regime is sub-divided considering the geometry of the steps and the flow conditions that lead to different configurations of the flow fields near the steps. For this, we have made an experimental approach in the laboratory of civil engineering at the University of Laghouat (Algeria), in three (03) models: model A (4cm x 7.5cm 4cmx) and model B (8cm x 7.5cm 8cmx), and the third model C size (12cm x 12 cm x 7.5 cm) developed "Plexiglas." Three slopes were studied in this experiment:  = 20 °, 30 ° and  =  = 45 ° for the three models, with flow rates ranging from q = 0.96 l / s / ml up to q = 33.02 l / s / ml.The purpose of this experimental study is to show the evolution of the velocity profiles in the cross sections of the flows, and pressure variations in the three studied models. The analysis of our results led us to conclude that, The result obtained in the model B and C where the flow is in nappe flow; the pressure field increases in horizontally steps and decreases in vertically steps; this is due to the effect of the impact of the flow on the step; and against the pressure in step is below atmospheric pressure (vacuum). Furthermore, the speeds increase significantly with increasing rate of flow and accelerated more than one moves downstream; In the last, for the skimming flow, longitudinal flow velocity distribution is nearly constant which is shown in the uniform area flow.

Abstract:
Introduction: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL), is a non-atherosclerotic, nonamyloid, hereditary cerebral disease of small vessels and capillaries caused by mutations in the NOTCH-3 gene located on chromosome 19. The presence of granular osmophilic material (GOM) deposition in the smooth muscle cells of vessel walls is the pathological hallmark of arteriopathy in CADASIL. GOM deposits in the basal lamina of smooth muscle of small vessels are pathognomonic for CADASIL. The presence of GOM in capillary blood vessels of the skin and muscle in biopsy and genetic studies (NOTCH-3 analysis) plays a key diagnostic role. Biopsy tests have high specificity (up to 100%) and low sensitivity (less than 50%). The NOTCH-3 test has been proposed as the primary diagnostic approach, allowing detection of 90% of affected individuals. CADASIL has an estimated prevalence between 2 and 5 in 100,000, and the phenotypic study demonstrates different clinical symptoms in the course of the disease within the same family. The average age of onset of clinical symptoms varies between 48.3 years in men and 52.2 years in women. Characteristic symptoms of migraine, stroke or TIA (transient ischemic attack), behavioral changes, early and progressive cognitive changes associated with leukoencephalopathy on imaging studies. The large association of symptoms often causes the diagnosis of CADASIL to be delayed. In this case, we had an association of gait ataxia within the framework of motor alterations, demonstrating the wide range of symptomatology of the pathology. This case report presents a familial course that started outside the most prevalent age group in the studies described and with an atypical presentation in an affected generation. Case reports: Case 01: woman, 68 years old, started progressively forgetting to perform household activities after the age of fifty, associated with primarily generalized myoclonic epileptic seizures, evolving rapidly within five years to walking apraxia with the use of a wheelchair and tonic-tonic epileptic seizures. bilateral clonic disorders, comprehension aphasia and bradypsychism. Relatives report previous migraine without chronic aura and REM (rapid eye movement sleep) sleep behavior disorder (RMSD). On neurological examination, severe ataxia with bilateral dysdiadochokinesia associated with bilateral hypometric index-index. Bilateral ROT 4+/4+ with bilateral Hoffman and Babinski signs. MMSS and MMII with FGM 4/5 proximal and distal. Case 02: woman, 36 years old, pastry chef, had episodes of forgetting about everyday activities of her work, progressive in the last three years (cake recipes, budget accounts, orders placed) associated with confusion for spatial location on the way home/ work, evolving to apraxia in writing letters and words and difficulty with calculations associated with monoparesis of the right lower limb for twelve months with progression to paresis of the lower limbs after six months and evolution to paresthesia of the upper limbs for three months. Associated with the condition, he has migraine without chronic aura and RMSD. The neurological examination showed Mini-Mental State Examination 22/30 (expected score of 29), list of animals in one minute: 09 animals; list of words starting with “F”: 03 words; clock test: 2/4; difficulty with calculations and digital agnosia with right/left apraxia; Upper limbs: eutrophic, FMG 4/5, bilateral distal; FMG 5/5 bilateral proximal; Lower limbs: eutrophic, bilateral FMG 4/5 distal and proximal with positive Mingazini; atypical gait with evidenced weakness in heel, toe and tandem gait; Bilateral dysdiadochokinesia with eumetric, slowed indexindex; ROT 4+/4+ in the right side with positive Hoffman and Babinski signs. MRI Brain (17/09/2020): extensive area of hypersignal on T2 and FLAIR (T2- weighted-Fluid-Attenuated Inversion Recovery) involving the periventricular white matter in all lobes without atrophic or expansive effect; Case 03: woman, 45 years old, started behavioral arrest epileptic seizures at the age of thirty-two, progressing to focal dysperceptive seizures with progression to bilateral tonic clonic seizures after eight months and multiple episodes of anterograde amnesia, presenting forgetfulness related to everyday work activities (exchanged worksheets , payments, calculation errors and budgets); associated with the condition presented migraine without chronic aura and RMSD. Genetic Test (04/06/2017): Heterozygous alteration in exon 8 of the NOTHC – 3 gene. Discussion: CADASIL presents a rare cause of cognitive decline and is often overlooked in diagnosis, except in cases of high clinical suspicion in a familial course. Access to imaging tests becomes fundamental for the diagnostic segment and the primordial genetic test for etiological elucidation and family planning, in the report described the family presentation with the same course of satellite symptoms (migraine without aura, and RMSD) associated with cognitive alteration with anticipation of age of onset are hallmarks of clinical thinking. Cases described in the literature show that the clinical symptomatology is not necessarily related to the level of brain injury observed in the imaging exam, which could be explained by personal factors and which exon is affected. The NOTCH-3 gene has 24 exons, in which the literature reports exon 4 as the most common mutation, followed by 3, 5 and 6, mainly in the Caucasian population. In an Asian population, the most affected exon is 4 and 11, which is also found in Italian descendants. The mutation in exon 8, described in the clinical case, is found in a population of Portuguese origin, being the second most common mutation in this nationality, behind the mutation in exon 4. Brain MRI studies have tried to elucidate the most affected brain regions, aiming to trace a line of evolution. Involvement of the temporal lobe, external capsule and corpus callosum are described as probable markers of CADASIL, and can be used as an aid in the diagnosis due to its specificity of 86% and sensitivity of 89%. Studies also show that the frontoparietal area has frequent findings of hyperintensity (100%), followed by the temporal lobe (83%), less frequently affecting the brainstem, occipital lobe and cerebellum. Despite the great advances in the specialized literature, the causes of the important cognitive dysfunction presented in the course of the evolution of CADASIL remain unclear. However, studies have suggested that the process of cognitive decline is more related to the loss of cortico-subcortical connections than to brain atrophy itself, with these disconnections resulting from repeated transient ischemic accidents. Science has been looking for ways to change the prognosis of CADASIL, recent studies in gene therapy and neurogenetics show the importance of thinking about this pathology as a genetic disease of great importance to change the prognosis of this pathology.

Abstract:

Abstract:

Abstract:
The median age at first marriage in the United States has increased steadily since the mid-20th century. In the mid-1950s, the median age was at a record low of just over 20 for women and 22 for men, but by 2020, the median age was 28 for women and 30 for men (see Figure 1). The median age at first marriage has increased similarly for both men and women. Consequently, the gender gap in the median age at first marriage has persisted, fluctuating between 1.6 and 2.7 years. This profile uses data from the 2019 American Community Survey (ACS), 1-year estimates to track the trends in women’s and men’s median ages at first marriage. The ACS is ideal because it provides the best annual data on marital status and demographic characteristics allowing for direct estimation of the median age at first marriage (Simmons & Dye, 2004). This is an update to our previous profiles on the topic for the years 2017 (FP-19-06), 2014 (FP-16-07), 2013 (FP-15-05), 2010 (FP-12-07), and 2008 (FP-09-03).
