Abstract:
The notion of gluing of abelian categories was introduced in a paper by Kazhdan and Laumon in 1988 and studied further by Polishchuk. We observe that this notion is a particular case of a general categorical construction.We then apply this general notion to the study of the ring of global differential operators $\mathcal{D}$ on the basic affine space $G/U$ (here $G$ is a semi-simple simply connected algebraic group over $\mathbb{C}$ and $U\subset G$ is a maximal unipotent subgroup).We show that the category of $\mathcal{D}$-modules is glued from $|W|$ copies of the category of $D$-modules on $G/U$ where $W$ is the Weyl group, and the Fourier transform is used to define the gluing data. Indonesia bersama seluruh negara berkembang berupaya mencapai kesepakatan Millenium Development Goals (MDGs) dengan salah satu sasaran menurunkan angka kematian neonatal dari 20 per 1.000 kelahiran hidup menjadi 15 per 1.000 kelahiran hidup. Penelitian ini bertujuan mengetahui hubungan pelayanan kesehatan ibu dengan kematian neonatal di Kabupaten Lampung Timur tahun 2011. Penelitian dengan desain studi kasus kontrol ini mengamati kasus ibu yang mengalami kematian neonatal dan kontrol ibu yang tidak mengalami kematian neonatal. Analisis multivariat menemukan pelayanan antenatal dan pertolongan persalinan berhubungan secara signifikan dengan kematian neonatal, setelah mengendalikan variabel umur ibu dan riwayat kehamilan (OR = 16,32; nilai p = 0,000); dan (OR = 18,36; nilai p = 0,31). Bayi yang dilahirkan dari Ibu dengan pelayanan antenatal tidak lengkap berisiko mengalami kematian neonatal 16,32 dan 18,36 kali lebih besar daripada bayi yang dilahirkan. Ibu dengan pelayanan antenatal lengkap dan penolong persalinan profesional. Tidak ada hubungan penolong persalinan dengan kematian neonatal, setelah mengontrol variabel pelayanan antenatal, umur ibu, riwayat kehamilan, riwayat penyakit, dan riwayat persalinan. Disarankan meningkatkan kualitas pelayanan antenatal dengan memerhatikan faktor umur ibu dan riwayat persalinan, mengembangkan kegiatan audit maternal perinatal serta meningkatkan keterampilan petugas penolong persalinan.All developing countries including Indonesia seek to reach agreement the Millennium Development Goals (MDG’s). It is objectives include reducing neonatal mortality by 25 percent from 20 per 1,000 live birth to 15 per 1,000 live births. This study aimed to determine the relationship of maternal health services with neonatal mortality in East Lampung District in 2011. This study used case control design to compare between the groups of mother whom have neonatal deaths (cases) and neonatal life (control) in East Lampung District in 2011. The result on antenatal care variables found that antenatal care and birth attendant had significant correlation with neonatal death, after controlling age and pregnancy history variable (p value = 0.000, OR = 16.32; p value = 0.31, OR = 18.36). The babies from mothers who did not get completed prenatal care risk of 16.32 times have neonatal death than babies born from mothers who received completed maternal care. There was no association between neonatal mortality and birth attendant, after controling variables of antenatal care, maternal age, pregnancy history, medical history and chilbirth history. Based on this study, it is suggested to increase activity of maternal perinatal audit, improve the quality of antenatal care, maternal delivery, and develop other support activities to prevent neonatal mortality in East Lampung District.

Приведены результаты исследований по влиянию систем рециркуляции и перепуска выпускных газов на экологические и экономические показатели работы судового дизеля 16V32 фирмы Wartsila-Sulzer. Предложено использование комплексной системы управления выпускными газами, сочетающей в себе названные системы. Экспериментально подтверждено, что комплексное управление выпускными газами дизеля 16V32 обеспечивает снижение эмиссии оксидов азота на 31,27-38,23 % при увеличении удельного эффективного расхода топливана 2,68-1,11 %.

Celem artykułu jest próba odpowiedzi na pytanie, w jakim stopniu leksyka swoista Nowego Testamentu Biblii gdańskiej (1632) w przekładzie Daniela Mikołajewskiego wywodzi się z Nowego Testamentu w przekładzie Jakuba Wujka (1593). Leksemy swoiste Nowego Testamentu Biblii gdańskiej (1632) to te autosemantyczne hasła leksykonu tego tekstu, które nie wystąpiły w leksykonach trzech zestawianych z przekładem gdańskim z 1632 roku wcześniejszych renesansowych przekładów Nowego Testamentu, a mianowicie w przekładach kręgu ewangelickiego – w Nowym Testamencie Biblii brzeskiej (1563), w Nowym Testamencie gdańskim (1606) oraz w jezuickim Nowym Testamencie Biblii Jakuba Wujka (1599). Analiza wykazała, że w zakresie leksyki swoistej ewangelickiego Nowego Testamentu Biblii gdańskiej z 1632 ewentualna zależność filiacyjna tego przekładu od katolickiego Nowego Testamentu Jakuba Wujka z 1593 roku choć jest niewielka, to empirycznie prawdopodobna.

Noise generated during elevator operation in high-rise residential buildings has a significant impact on the healthy and sustainable building environment of residents. However, there is still no clear standard for the measurement method. There are other international standards other than ISO 16032 on how to measurement elevator noise, but this standard relates to general construction equipment. In particular, it is not suitable as a method to measure elevator noise because it was proposed without considering the situation of modern high-rise residential buildings. In this study, by reflecting the characteristics of the elevator as a noise source and the structural characteristics of modern high-rise residential buildings, improvement measures were proposed for the elevator noise measurement conditions in ISO 16032, which are cited as their own standards in many countries including Korea. As an improvement plan, it contains information on elevator operating conditions, measurement frequency bands, evaluation unit, and measurement locations. As a result of applying these four improvement measures, it was verified that the elevator noise of modern high-rise residential buildings can be appropriately measured, unlike the existing partial elevator noise measurement method in ISO 16032.

Background HIV testing guidelines provided by the Centers for Disease Control and Prevention (CDC) are continually changing to reflect advancements in new testing technology. Evaluation of existing and new point-of-care (POC) HIV tests is crucial to inform testing guidelines and provide information to clinicians and other HIV test providers. Characterizing the performance of POC HIV tests using unprocessed specimens can provide estimates for the window period of detection, or the time from HIV acquisition to test positivity, which allows clinicians and other HIV providers to select the appropriate POC HIV tests for persons who may be recently infected with HIV. Objective This paper describes the protocols and procedures used to evaluate the performance of the newest POC tests and determine their sensitivity during early HIV infection. Methods Project DETECT is a CDC-funded study that is evaluating POC HIV test performance. Part 1 is a cross-sectional, retrospective study comparing behavioral characteristics and HIV prevalence of the overall population of the Public Health–Seattle & King County (PHSKC) Sexually Transmitted Disease (STD) Clinic to Project DETECT participants enrolled in part 2. Part 2 is a cross-sectional, prospective study evaluating POC HIV tests in real time using unprocessed whole blood and oral fluid specimens. A POC nucleic acid test (NAT) was added to the panel of HIV tests in June 2018. Part 3 is a longitudinal, prospective study evaluating seroconversion sensitivity of POC HIV tests through serial follow-up testing. For comparison, HIV-1 RNA and HIV-1/HIV-2 antigen/antibody tests are also performed for participants enrolled in part 2 or 3. A behavioral survey that collects information about demographics, history of HIV testing, STD history, symptoms of acute HIV infection, substance use, sexual behaviors in the aggregate and with recent partners, and use of pre-exposure prophylaxis and antiretroviral therapy is completed at each part 2 or 3 visit. Results Between September 2015 and March 2019, there were 14,990 Project DETECT–eligible visits (part 1) to the PHSKC STD Clinic resulting in 1819 part 2 Project DETECT study visits. The longitudinal study within Project DETECT (part 3) enrolled 27 participants with discordant POC test results from their part 2 visit, and 10 (37%) were followed until they had fully seroconverted with concordant positive POC test results. Behavioral survey data and HIV test results, sensitivity, and specificity will be presented elsewhere. Conclusions Studies such as Project DETECT are critical for evaluating POC HIV test devices as well as describing characteristics of persons at risk for HIV acquisition in the United States. The Journal of Medical Internet Research (JMIR) was an early pioneer of open access online publishing, and two decades later, some readers and authors may have forgotten the challenges of previous scientific publishing models. This commentary summarizes the many advantages of open access publishing for each of the main stakeholders in scientific publishing and reminds us that, like every innovation, there are disadvantages that we need to guard against, such as the problem of fraudulent journals. This paper then reviews the potential impact of some current initiatives, such as Plan S and JMIRx, concluding with some suggestions to help new open-access publishers ensure that the advantages of open access publishing outweigh the challenges.

Background Technology-assisted intervention and prevention strategies present opportunities for substance use–related research with emerging adults (EAs) and their peers. Emerging adulthood is a developmentally distinct period in which individuals between the ages of 18 and 29 years undergo unique emotional, cultural, developmental, and biological changes as they transition into adulthood. Crowdsourcing, or gathering feedback from a large group within web-based communities, offers researchers a unique and cost-effective way to obtain large amounts of information in a short period. Objective This paper presents market feedback obtained via Amazon’s Mechanical Turk from EAs (N=458) on the acceptability and utility of brief intervention scripts for a smartphone app currently under development. The mobile app, Harbor, teaches friends of EAs with substance use problems effective and supportive strategies for helping their friend make changes in their substance use behavior. Methods We examined feedback on the wording of the intervention scripts and estimated the market size of EAs who may use this app. Furthermore, we calculated correlations between script ratings and measures of personal risky drinking (ie, Alcohol Use Disorder Identification Test) and the participants’ use of confrontational, enabling, or supportive behaviors with an existing friend. Results Approximately half of our sample (208/458, 45.4%) indicated that they had a close friend for whom they had concerns about their substance use, suggesting a potentially high demand for an app such as Harbor. Initial findings suggest that peers who engage in less enabling behaviors with friends who have a substance use problem exhibited lower risky drinking behaviors overall (r206=−0.501; P<.001). Concerning acceptability, 98.0% (449/458) of the sample rated the scripts’ dialogue as either somewhat, moderately, or extremely realistic (mean 3.92, SD 0.48) on 5-point Likert scale items. Finally, 95.4% (437/454) of respondents indicated that the scripts would be at least slightly helpful for training peers to help their friends with substance use issues. Finally, individuals who were better able to identify enabling language in enabling scripts self-reported fewer enabling behaviors toward their friend’s substance use (r206=−0.236; P=.001). Conclusions There exists a demonstrated level of desirability and acceptability among EAs for a mobile app such as Harbor. EAs who wish to engage in more supportive behaviors with their friends who engage in substance use and who are amenable to assisting their friends with sobriety likely would use and benefit from this app.

Abstract:
Background Defining the transition from relapsing-remitting multiple sclerosis (RRMS) to secondary progressive multiple sclerosis (SPMS) can be challenging and delayed. A digital tool (MSProDiscuss) was developed to facilitate physician-patient discussion in evaluating early, subtle signs of multiple sclerosis (MS) disease progression representing this transition. Objective This study aimed to determine cut-off values and corresponding sensitivity and specificity for predefined scoring algorithms, with or without including Expanded Disability Status Scale (EDSS) scores, to differentiate between RRMS and SPMS patients and to evaluate psychometric properties. Methods Experienced neurologists completed the tool for patients with confirmed RRMS or SPMS and those suspected to be transitioning to SPMS. In addition to age and EDSS score, each patient’s current disease status (disease activity, symptoms, and its impacts on daily life) was collected while completing the draft tool. Receiver operating characteristic (ROC) curves determined optimal cut-off values (sensitivity and specificity) for the classification of RRMS and SPMS. Results Twenty neurologists completed the draft tool for 198 patients. Mean scores for patients with RRMS (n=89), transitioning to SPMS (n=47), and SPMS (n=62) were 38.1 (SD 12.5), 55.2 (SD 11.1), and 69.6 (SD 12.0), respectively (P<.001, each between-groups comparison). Area under the ROC curve (AUC) including and excluding EDSS were for RRMS (including) AUC 0.91, 95% CI 0.87-0.95, RRMS (excluding) AUC 0.88, 95% CI 0.84-0.93, SPMS (including) AUC 0.91, 95% CI 0.86-0.95, and SPMS (excluding) AUC 0.86, 95% CI 0.81-0.91. In the algorithm with EDSS, the optimal cut-off values were ≤51.6 for RRMS patients (sensitivity=0.83; specificity=0.82) and ≥58.9 for SPMS patients (sensitivity=0.82; specificity=0.84). The optimal cut-offs without EDSS were ≤46.3 and ≥57.8 and resulted in similar high sensitivity and specificity (0.76-0.86). The draft tool showed excellent interrater reliability (intraclass correlation coefficient=.95). Conclusions The MSProDiscuss tool differentiated RRMS patients from SPMS patients with high sensitivity and specificity. In clinical practice, it may be a useful tool to evaluate early, subtle signs of MS disease progression indicating the evolution of RRMS to SPMS. MSProDiscuss will help assess the current level of progression in an individual patient and facilitate a more informed physician-patient discussion.
