⋄ Background Although China lacks a national dialysis registry, several local systems (such as the Shanghai Dialysis Registry, established in 1996) do exist. The present report provides information from the registry concerning the development of peritoneal dialysis (PD) in Shanghai. ⋄ Methods Data from the Shanghai Dialysis Registry collected between 1999 and 2006 were used in the analysis. All 58 dialysis centers in Shanghai, of which 30 have a PD program, entered data into the registry. ⋄ Results Point prevalence of PD on 31 December 2006 was 1204 patients, as compared with 459 patients in 1999. In 2006, 705 patients commenced PD, giving a treatment rate of 51.5 per million population. The percentage utilization of PD among all dialysis modalities exceeded 20% in 2006, as compared with 16% in 1999. The main cause of end-stage renal failure (ESRF) was chronic glomerulonephritis, affecting 40% of all patients. However, the incidence of diabetic nephropathy increased to 30.5% from 15.3% between 1999 and 2006, and represented the second major cause of ESRF in 2006. All PD patients were dialyzed with standard glucose-based solutions only. The annual death rate among PD patients decreased to 10.3% in 2006 from 16.1% in 1999. Infection was the leading cause of death and drop-out, although the peritonitis rate decreased (to 1 episode in 48.8 patient–months in 2006 from 1 episode in 36.6 patient–months in 1999). Dialysis adequacy, hepatitis infection control, and anemia control improved over time. ⋄ Conclusions The PD population in Shanghai has continuously grown and improved in clinical outcome over the 8 years from 1999 to 2006.

e11607 Background: To present the results of clinical practices for operable breast cancer (BC) in the developed areas of China during the past decade. Methods: Four academic levels of radiotherapy (RT) departments, located in the Southeast coast of China, were selected. A pattern of care study, about women with operated BC and post-operative RT during 1999 and 2006, was conducted. The recruited samples were evaluated for their medical files completeness at first. For qualified documentations, data collected included patient characteristics, clinical and pathologic features, and treatment course received. Frequencies and corresponding percentages were calculated and analyzed for comparisons between 1999 and 2006. Results: 97 in 1999 and 271 in 2006 were identified as qualified files. Patient's diagnosis median age were 48 (1996) and 46 (2006) years. The clinical staging with I, II, and III (UICC 1997) were in 11%, 69%, 13% of 1999 patients and 15%, 74%, 7% of the 2006. The pathological characteristics were clearly defined in 78% (1999) and 84% (2006), but hormonal receptors and Her2neu were performed in 59% and 0% in the 1999 vs. 73% (p < 0.05) and 1% in 2006 survey. As type and extent of surgery and RT, the percentage of conservative treatment was increased from 4% in 1999 to 12% in 2006 (p< 0.05), postmastectomy RT was done for all the others. The time interval from operation to initiation of RT were longer in 2006, the mean was delayed from 23.8 to 43.4 days. The axillary operation pattern including lymph node dissection, level I/II dissection, and sentinel lymph node biopsy (SLNB), were documented in 61%, 24%, and 8% of patients in 2006. For patients of 1999, no SLNB was performed and 16% with level I/II operation (p< 0.05). Systemic chemotherapy (CT) and/or hormonal therapy were used in 74% (1999) and 98% (2006) of patients respectively (p< 0.05). Anthracyclines and paclitaxel-based CT regimens use significantly increased between 1999 and 2006 (31% vs. 52% and 12% vs. 36%). Conclusions: The practice patterns established in this study provide a baseline data. The present results demonstrate a low level of compliance with Western guidelines of clinical practices with current BC standards and continued improvement in many categories. Establishment and widespread use of international guidelines in China are desirable. No significant financial relationships to disclose.

Abstract:
This report describes excavations at Basing Grange, Basing House, Hampshire, between 1999 and 2006. It embraces the 'Time Team' investigations in Grange Field, adjacent to the Great Barn, which were superseded and amplified by the work of the Basingstoke Archaeological & Historical Society, supervised by David Allen. This revealed the foundations of a 'hunting lodge' or mansion built in the 1670s and demolished, and effectively 'lost', in the mid-18th century. Beneath this residence were the remains of agricultural buildings, earlier than and contemporary with the nearby Great Barn, which were destroyed during the English Civil War. The report contains a detailed appraisal of the pottery, glass and clay tobacco pipes from the site and draws attention to the remarkable window leads that provide a clue to the mansion's date of construction. It also explores a probable link with what was taking place on the Basing House site in the late 17th and early 18th century.

ABSTRACT Background Calf circumference (CC) is used in geriatric studies as a simple and practical skeletal muscle (SM) marker for diagnosing low SM and sarcopenia. Currently applied CC cutoff points were developed in samples including older participants; values representative of the full adult lifespan are lacking. Objectives We aimed to develop CC cutoff points and to identify relevant confounding factors from the large and diverse NHANES 1999–2006 population sample. Methods Demographic, anthropometric, and imaging data (DXA, appendicular lean mass) from the adult (age ≥18 y) NHANES sample were partitioned into subgroups according to sex, age, ethnicity, and race. Adults aged 18–39 y and BMI (in kg/m2) 18.5–24.9 were set as a reference population; CC cutoff points were derived at 1 and 2 SDs below the mean. Results The sample included 17,789 participants, 51.3% males and 48.7% females, with respective ages (mean ± SD) of 43.3 ± 16.1 y and 45.5 ± 16.9 y. CC was strongly correlated with appendicular lean mass, r = 0.84 and 0.86 for males and females (both P &lt; 0.001), respectively. Significant differences in mean CC were present across sex, ethnic, self-reported race, and BMI groups. Adjusting CC for adiposity using BMI revealed a decrease in CC beginning after the second decade in males and third decade in females. Rounded CC cutoff values for moderately and severely low CC were 34 cm and 32 cm (males), and 33 cm and 31 cm (females), respectively. Our findings support the use of BMI-adjusted CC values for participants outside the normal-weight BMI range (18–24.9). Conclusions This study defined CC values in a diverse population sample along with a BMI-adjustment approach that helps to remove the confounding effects of adiposity and thereby improves CC as a useful clinical estimate of SM mass.

AbstractObjective:The appropriate activation and effective utilization of air-medical transport (AMT) services is an important skill for emergency medicine physicians in the United States.Previous studies have demonstrated variability with regards to emergency medical services (EMS) experience during residency training. This study was designed to evaluate the nature and extent of AMT training of the emergency medicine residency programs in the United States.Methods:An identity-unlinked survey of the program directors of all Accreditation Committee for Graduate Medical Education (ACGME) approved emergency medicine residency programs was conducted.The survey focused on EMS and AMT resident training opportunities and was conducted in two phases (1999 and 2006) using near-identical methodologies.Results:Response rates of 82% and 84% were achieved in 1999 and 2006, respectively. Percentages of programs offering AMT experiences were similar between the two study phases (76% in 1999 and 65% in 2006). The roles of residents during AMT experiences ranged widely between observer-only, active team member, and medical director/team leader in both 1999 and 2006. Compared to those in 1999, programs in 2006 demonstrated a greater frequency of EMS rotations being provided earlier, by year of training during emergency medicine residency. Residencies located in non-metropolitan centers only were slightly more likely to offer AMT training than were those in metropolitan locations.Conclusions:A majority of emergency medicine residency programs offer AMT experience that includes both scene responses and inter-facility transports. The role of residents during AMT training varies widely, as does the timing of their experiences during residency. The geographical locations of programs do not appear to impact the availability of AMT training.

Remote sensing had provided the approach to monitoring LUCC and the environment response. The paper focus on monitoring LUCC and quantitative analysis of the thermal environment response. Based on the multi-temporal LandsatETM remote sensing images in 1999 and 2006, the earth surface brightness tempreture had been obtained by retrieving from LandsatTM6. The regional land use change and its spatio-temporal thermal environment evolution were analyzed with ENVI4.7 and Arc GIS 9.2. The research results showed that the land use changed evidently from 1999 to 2006. The regional urban expanding and the spatio-temporal thermal environment evolution was consistent from 1999 to 2006. By comparing the contrbution from the different land use types to the urban thermal environment effect, it was discovered that forest land was the largest contributor for the low area. The proportion of forest land decreased in the low temperature areas, and the proportion of urban land increased in high temperature area from 1999 to 2006. The urban heat island effect was obvious. The nakes arable land resulted the high temperature areas. The forest land and the agricultural land with the crop was helpful for the lower temperature areas.
