Abstract:
Waste management is one of the most challenging problems. This study investigates the influence of household attributes on waste minimisation behaviour in Shah Alam City, Malaysia. Through a household survey, 300 respondents were randomly chosen. Findings from the study revealed that respondents behaviour has statistically significant associations with 4 socio-demographic, namely gender [F(298) = 7.33, p = 0.00]; race [F(2, 297) = 3.22, p = 0.04]; marital status [F(2, 297) = 3.51, p = 0.03]; home ownership [F(2, 295) = 7.57, p = 0.00]. The results of this analysis will facilitate the implementation of policies for sustainable solid waste minimisation.

Abstract:
Serine hydroxymethyltransferase belongs to the α class of pyridoxal-5´-phosphate enzymes along with aspartate aminotransferase. Recent reports on the three-dimensional structure of human liver cytosolic serine hydroxymethyltransferase had suggested a high degree of similarity between the active-site geometries of the two enzymes. A comparison of the sequences of serine hydroxymethyltransferases revealed the presence of several highly conserved residues, including Pro-297. This residue is equivalent to residue Arg-292 of aspartate aminotransferase, which binds the γ-carboxy group of aspartate. In an attempt to change the reaction specificity of the hydroxymethyltransferase to that of an aminotransferase and to assign a possible reason for the conserved nature of Pro-297, it was mutated to Arg. The mutation decreased the hydroxymethyltransferase activity significantly (by 85–90%) and abolished the ability to catalyse alternative reactions, without alteration in the oligomeric structure, pyridoxal 5´-phosphate content or substrate binding. However, the concentration of the quinonoid intermediate and the extent of proton exchange was decreased considerably (by approx. 85%) corresponding to the decrease in catalytic activity. Interestingly, mutant Pro-297 Arg was unable to perform the transamination reaction with l-aspartate. All these results suggest that although Pro-297 is indirectly involved in catalysis, it might not have any role in imparting substrate specificity, unlike the similarly positioned Arg-292 in aspartate aminotransferase.

Abstract Abstract 3301 Moesin is a member of the protein 4.1 superfamily and shares structure homology with ezrin and radixin. The phosphorylation of moesin increases the aggregation of Fas, activation of capases, and triggers the apoptotic pathways through the Rho-ROCK pathway. The moesin protein is highly expressed in human platelets and phosphorylation of threonine558 is associated with the activation of platelets. Autoantibodies against moesin have not been evaluated in patients with ITP. Thus, we designed and expressed three polypeptides of moesin: N1-297 terminal, α 298–470 helix domain and C471-577 terminal for ELISA assays. Serum samples from patients with ITP (n=148), patients with non-immune thrombocytopenia (n=32), and other patients with hematologic diseases (n=102) as well as gender-matched healthy control subjects (n=50) were evaluated. All ITP patients clinically met an eligibility of ITP minimal standardized criteria from an international working group.1 The titers of moesin autoanitbodies were significantly elevated in the sera from patients with ITP compared with healthy subjects (Mean of autoantibodies titers = N1-297 0.515/0.155; α298–470 0.252/0.159; C471-577 0.430/0.144, P < 0.01). The levels of moesin autoantibodies against N1-297 and C471-577 in ITP patients were also markedly higher than in patients with non-immune thrombocytopenia (Mean of autoantibodies titers = N-1-297 0.515/0.168; C471-577 0.430/0.103, P < 0.05). However, the titer of autoantibodies against moesin ? 298–470 helix domains was similar in ITP, non-immune thrombocytopenia and in patients with other hematologic diseases. When an autoantibody cut-off value of ±2D in normal control subjects (n=50) was assigned, the serum levels of moesin autoantibodies in ITP patients were found to be elevated in 70% (103/148) positive for N-1-297, 44% (65/148) positive for C471-577 and only 6.1% (9/148) positive for α 298–470. Moreover, patients with non-immune thrombocytopenia (n=9), anaphylactic purpura (n=11) and myelodysplasticsyndrome, MDS (n=12) were all negative for moesin autoantibodies. In other groups of patients with hematologic diseases, multiple myeloma (n=15), pure red cell aplasia (n=2), and autoimmune hemolytic anemia (n=18), only autoimmune hemolytic anemia demonstrated 4.5% (3/18) positive for N-1-297 and all were negative for C471-577. Notably, severe ITP patients with iron deficiency anemia had high serum moesin autoantibodies against N-1-297 and C471-577. We also found 15.6% (6/38) positive moesin autoantibodies against C471-577 in ITP patients with systemic lupus erythematosus (SLE). To validate the ELISA results, we examined the specificity of moesin autoantibodies in ITP by Western blot analysis. Three polypeptides of moesin were run on SDS-PAGE and transferred onto nitrocellulose membrane. The immunoblotting showed autoantibodies against ITP that specifically recognized the moesin C471-577 or N1-297 polypeptide. In conclusion, this is the first study to show the elevation of serum moesin autoantibodies in patients with ITP. We propose that moesin autoantibodies specific for C471-577/N1-297 may be specific serum biomarkers for clinical diagnosis and differentiation of ITP from non-immune thrombocytopenia and other hematologic diseases. Age-standardised rates were calculated for first admissions to hospital in Scotland with ICD-9 diagnoses of schizophrenia, affective psychoses, paranoid psychoses, reactive psychoses and depressive neuroses (ICD-9 295, 296, 297, 298 and 300.4) for the period 1969–88. First-admission rates for schizophrenia declined by an average of 3.3% per year in males and 4.4% per year in females over the period. The first-admission rate in males in 1988 was 8.4/100 000 (57% of 1969 rate) and in females was 4.8/100 000 (43% of 1969 rate). Rates for depressive neuroses, affective psychoses, reactive psychoses and combined psychoses also fell. Rates for mania rose, as did those for paranoid states in males. The decrease in first-admission rates is likely to reflect a true decrease in the incidence of schizophrenia over the period. The decline was unlikely to be accounted for by diagnostic change because there was no reciprocal increase in any other diagnosis sufficient to account for the change, and the rates for combined psychoses also decreased. There was evidence that rates for schizophrenia declined to a greater extent in younger age groups, especially in females. This could imply the presence of a birth cohort effect.

Objective: We studied posterior pituitary function in 27 patients with Sheehan’s syndrome and 14 controls. Design: All patients were investigated by water deprivation test and 26 of them by 5% hypertonic saline infusion test. None of the patients had symptoms of diabetes insipidus and all patients were on adequate glucocorticoid and thyroid hormone replacement therapy before testing. Results: According to dehydration test, 8 (29.6%) patients had partial diabetes insipidus (PDI group) and 19 (70.3%) had normal response (non-DI group). During the 5% hypertonic saline infusion test, the maximal plasma osmolality was higher in PDI (305 ± 4.3) and non-DI (308 ± 1.7) groups when compared with controls (298 ± 1.7 mOsm/kg; P < 0.005), but the maximal urine osmolality was lower in PDI group (565 ± 37) than in non-DI (708 ± 45) and control (683 ± 17 mOsm/kg) groups (P < 0.05). The osmotic threshold for thirst perception was higher in PDI (296 ± 4.3) and non-DI (298 ± 1.4) groups when compared with control group (287 ± 1.5 mOsm/kg) (P < 0.005). Basal plasma osmolalities were also higher in PDI (294 ± 1.0) and non-DI (297 ± 1.1) groups than in controls (288 ± 1.2 mOsm/kg; P < 0.001). Conclusions: Our findings demonstrated that patients with Sheehan’s syndrome have an impairment of neurohypophyseal function. The thirst center may be affected by ischemic damage and the osmotic threshold for the onset of thirst in patients with Sheehan’s syndrome is increased.

Abstract:
Abstract Background Lactobacillus are low virulence commensal organisms which are commonly found in the human oral cavity, gastrointestinal and genitourinary tracts. Although Lactobacillus bacteremia (LB) is rare, evidence aggregating from case reports has implicated LB in several medical conditions. As such, there is reason to suggest that the presence of these organisms in blood cultures may not be due to spurious contamination, but rather, indicative of clinically meaningful events capable of inducing serious illnesses. The purpose of this study is to characterize the risk factors, clinical significance and outcomes of patients with LB. Methods We retrospectively reviewed the medical records of patients presenting to a large urban teaching hospital between January 1, 2017 and December 31, 2018, who were found to have LB. Identified individuals were grouped into two mutually exclusive case categories: true LB cases or non-true cases (i.e., contamination). Individuals with ≥1 positive blood and were started on appropriate antibiotics were considered true cases. Those with positive cultures not started on appropriate antibiotics were considered contaminants. Results A total of 14 patients were identified during our study period, with majority considered true LB cases [71.4%; n = 10]. These 14 individuals were mostly males [64.2%; n = 9] and reported no use of Lactobacilli probiotics [78.6%; n = 11] or antacids [57.1%; n = 8]. On average, true LB cases were older (mean [SD]): 80.1 [±10.9]vs. 54.0 [±19.1] years) and required longer hospitalization (38.5 [(±27.6] vs. 8.0 [(±6.2] days) compared to non-LB cases, respectively. Among the 10 true LB cases, the suspected source of infection included gastrointestinal system [50%; n = 5], infective endocarditis [10%; n = 1], genitourinary system [10%; n = 1]; and could not be determined in 3 [30%] cases. Concurrent infection with candida and gastrointestinal microbes were noted in four (40%) of the true LB cases, respectively. Overall, five deaths were observed, with 4 [80%] occurring in true LB cases and one in a non-LB case. Conclusion LB should not be dismissed as contaminants particularly in at-risk patients for LB, such as the elderly or immunocompromised individuals. Disclosures All Authors: No reported disclosures

With his well-known innovative intellectual skills, Syed Nawab Haider Naqvi has converted his several Presidential Addresses given at the Annual General Meetings of the Pakistan Society of Development Economists into a succinct and stimulating book. With its publication, Dr Naqvi has placed students of development economics in his debt for a lucid and systematic treatment of a subject which mainstream economists regard as a dying species, while the more knowledgeable ones, agreeing with Dr Naqvi, see it as very much alive though in need of revitalisation. Development economics, as a new paradigm or a distinct sub-discipline of economics, is only half-a-century old. The end of the Second World War saw the emergence of a new, independent but underdeveloped world with high expectations of quick prosperity. It was soon discovered, however, that mainstream economics was either neutral or unwilling to offer direct solutions to the serious problems that developing economies faced, particularly with regard to questions of alleviating widespread poverty and removing the abominably high income and wealth inequalities. It is against this background that development themes attracted the special attention of a large body of economists, who not only tried to analyse such themes theoretically with the help of the existing as well as new economic postulates but also offered vital remedies to solve the practical problems of under-development. In this way, development economics emerged as perhaps the best example of a synthesis between the positive and the normative aspects.
