Abstract:
Under the assumption of good isospin symmetry, Tz = ±3/2 → ±1/2 Gamow-Teller (GT) transitions in a mass A isobar are analogous, where Tz is the z component of isospin T defined by 1/2(N - Z). We studied the Tz = +3/2 → +1/2 GT transitions by using the 37 Cl (3 He ,t)37 Ar and 41 K (3 He ,t)41 Ca reactions at E beam = 140 MeV / nucleon . The GT transition strengths in 37 Ar and 41 Ca were obtained up to the excitation energy (Ex) of 14.2 MeV and 10.4 MeV with energy resolutions of 30 and 35 keV, respectively. The obtained GT strengths were compared with those of the Tz = -2/3 → -1/2 mirror transitions, measured by the β-decay of 37 Ca and 41 Ti , respectively. It was found that the overall distributions of the mirror transitions were similar in both A = 37 and A = 41 systems. In A = 37 the differences seen at lower excitation energies are suggested to be the effect of the tensor interaction in the charge-exchange reaction. The differences seen at higher excitation energies are suggested to be the effect of the Coulomb interaction that can break the mirror symmetry of the strength distributions.

Abstract:
To analyze the effect of hyperthermia on the vascular response, the isometric response of isolated rabbit femoral artery segments was recorded at 37°C and hyperthermia (41 and 44°C). Contraction to potassium (5 × 10−3-5 × 10−2 M) was significantly greater at 41 and 44 than at 37°C and increased by inhibition of nitric oxide (NO) synthesis with N ω-nitro-l-arginine (l-NNA; 10−4 M) or endothelium removal at 37°C but not at 41 or 44°C. Norepinephrine (10−9-10−4M) produced a concentration-dependent contraction greater at 41 or 44 than at 37°C and not modified by endothelium removal orl-NNA at either temperature. Phenylephrine (10−9-10−4M) produced a contraction increased by warming to 44°C but not to 41°C. The specific α2-adrenoceptor agonist BHT-920 produced a weak contraction, reduced by the α1-adrenoceptor antagonist prazosin (10−6 M) and increased at 44°C but not at 41°C. The concentration-dependent contraction to endothelin-1 (ET-1; 10−11-10−7M) was increased by warming to 41 and 44°C and by endothelium removal or l-NNA at 37°C but not at 41 or 44°C. Response to ET-1 was reduced by endothelin ETA-receptor antagonist BQ-123 (10−5 M) and ETB-receptor antagonist BQ-788 (10−5 M). In arteries precontracted with ET-1 (10−8-3 × 10−8 M), relaxation to sodium nitroprusside (10−8-10−4M) was increased at 41 and 44°C vs. at 37°C, but that of ACh (10−8-10−4M) or adenosine (10−8-10−4M) was not different at all temperatures studied. Relaxation to ACh, but not adenosine, was reduced similarly byl-NNA at all temperatures studied. These results suggest hyperthermia in muscular arteries may inhibit production of, and increase dilatation to, NO, resulting in unchanged relaxation to ACh and increased constriction to KCl and ET-1, and may increase constriction to stimulation of α1-adrenoceptors by NO-independent mechanisms.

Abstract Objective This study aimed to evaluate platelet counts at delivery in uncomplicated pregnancies between 37 and 41 weeks of gestation. Study Design Platelet counts in women 16 to 45 years of age from August 1, 2011, through May 15, 2018, with a singleton pregnancy that delivered from 370/7 to 416/7 weeks of gestation. Women with pregnancy-related complications, preexisting disorders, or on medications that could affect platelet counts were excluded. Results A total of 18,526 women had uncomplicated pregnancies with mean platelet count from 370/7 to 416/7 weeks of gestation of 220 × 109/L. The lower limit 95th percentile (2.5% quantile) was 120 × 109/L. Platelet counts decreased weekly from 37 to 41 weeks of gestation, becoming significant at 39 weeks of gestation and beyond, compared with 37 weeks (p < 0.01). Mean platelet counts: 225 × 109/L at 37 weeks, 223 × 109/L at 38 weeks, 219 × 109/L at 39 weeks, 218 × 109/L at 40 weeks, and 216 × 109/L at 41 weeks of gestation. Platelet counts of less than 150 × 109/L occurred in 9.7%. Conclusion Platelet counts in uncomplicated pregnancies decrease weekly from 37 to 41 weeks of gestation. This will be important to consider when serial platelet values are monitored for other indications.

Studies have demonstrated that heat shock is associated with alteration in energy metabolism. In this study, we investigated the effect of heat shock on gene expression and secretion of adiponectin and leptin, and gene expression of Hspa2 and Pparγ in 3T3-L1 adipocytes. Compared with 37 °C, adiponectin mRNA was higher at 39 °C, and lower at 41 °C. Leptin mRNA was higher when adipocytes were exposed to 41 °C compared with 37 and 39 °C. Secretion of adiponectin increased at 39 °C, and when cells were exposed to 41 °C it was not detectable. Leptin secretion increased significantly at 41 °C, compared with 37 and 39 °C. Hspa2 mRNA was increased at 39 °C, and the highest level was reached at 41 °C. Pparγ mRNA exhibited a substantial increase in a temperature-dependent manner. The study provides the first evidence of a possible direct effect of heat shock on adiponectin and leptin gene expression and secretion, and demonstrates that the expression of the two adipokines is differentially regulated at the temperatures tested.

Abstract:
Objectives. Delivery of warmed, humidified air via nasal high flow therapy could potentially reduce replication of temperature-sensitive viruses in the upper respiratory tract. This study investigates whether nasal high flow therapy is well tolerated by healthy adults at 37°C and 41°C.Methods. In this randomised, double-blind, controlled crossover pilot trial, nasal high flow therapy was used to deliver humidified air at 35 L/min, at either 37°C or 41°C, for three one-hour sessions of use over one day. The alternative was delivered at least 14 days later. Ten healthy, nonsmoking adults were asked, via questionnaire after each day’s use, whether they would use nasal high flow therapy while being unwell with a cold or flu if it was demonstrated to improve symptoms.Results. All participants completed both interventions. Eighty percent responded “yes” to future use of nasal high flow therapy, for both 37°C and 41°C. There was no significant change from baseline in saccharin times following either intervention or in the following morning.Conclusions. Delivering humidified air via nasal high flow therapy at both 37°C and 41°C is well tolerated by healthy adults. This supports investigation into the potential use of nasal high flow therapy as treatment in viral upper respiratory tract infections.Trial Registration. This trial is registered withACTRN12614000183684(tolerability study of nasal delivery of humidified & warmed air).

Abstract:
This study explores the lived experience of parents of male patients who have been detained under a hospital order with restrictions (section 37/41 Mental Health Act 1983), subsequently conditionally discharged into the community, and then recalled under the restriction order back to hospital. While much research has focused on the impact of having a relative with a diagnosis of mental illness, there has been a dearth of research into the impact of having a relative detained in a medium secure forensic unit. As yet, no research has been conducted into the experience of families where their relative has been discharged from a medium secure forensic unit, returning to the community after a long period of detention. Neither has there been any exploration into what parents experience if their son is subsequently recalled under the restriction order back to hospital. Interpretative phenomenological analysis was used as a way of exploring this experience for six parents of five sons who had been conditionally discharged and subsequently recalled to medium secure forensic psychiatry units in the South-East. The analysis yielded themes which related to the personal and emotional impact that parents experienced in terms of a cyclical pattern of hope when their son was discharged, followed by loss and fear, accompanied by a sense of powerlessness when his mental state deteriorated, and relief together with disappointment at recall. There was a reappraisal of the expectations they had of their son which enabled them to regain hope for him. A further theme of ii responsibility highlighted the responsibility that parents assumed for their son, doing what they can for him whether he was in hospital or the community. Although they desired him to progress and be autonomous, parents recognised that their son may never be fully independent and may always require their support. They hoped that when he was conditionally discharged they would be able to share this responsibility with the supervising team. However, it seemed that when their son‟s mental state deteriorated, parents struggled to enlist the help of mental health services and were uncertain as to who held responsibility for him. Feeling isolated and unsupported at this time, they also felt blamed by services, reciprocating by blaming services. Not only unable to get what they perceived as an appropriate level of help for their son from services while he was conditionally discharged, parents described trying to access a system that seemed impenetrable, and responding only when „something had happened‟. When he was finally recalled, they experienced the system as having engulfed their son, and recognised how difficult it would be to get him out. Throughout the whole process they felt that their own expertise as parents was marginalised, and they described not feeling part of the team despite Governmental stress on carers being recognised as expert partners in care by services. A number of limitations of the study are discussed, together with the researcher‟s reflexive account and implications for practice. The study offers a deeper understanding of what parents in this situation may experience, and offers a broader perspective for those who work with them.

Abstract:
Whilst risk assessments have come to assume an increased level of importance in mental health policy and practice in England and Wales since the 1990s, there has been relatively little focus on the way in which service users themselves experience such practices. This thesis examines the views of offenders subject to Ministry of Justice restrictions under section 41 of the Mental Health Act 1983 towards their own offending, risk assessments and supervision. A primarily qualitative methodology is used to examine the views of 19 service users. Using theories of identity, the thesis examines the way that research participants explained and justified offending behaviour. It is argued that mentally disordered offenders used illness as a form of mitigation, utilised ‘techniques of neutralisation’ or employed a combination of these approaches. The thesis moves on to use governmentality theory as a means to analyse participants’ awareness and views toward risk assessment practices and their conditions of discharge. Although participants believed that assessments were seen as important by mental health professionals, they were often unaware of the content of such assessments. Research participants usually had little or no knowledge of their conditions of discharge. Service user perceptions of the supervision process are then examined with reference to theories of social control. It is argued that service users held a range of views towards the supervision order, seeing it variously as a means of identifying and supporting them as mentally ill individuals; as a means of establishing internal controls or as a negative means of labelling socially stigmatised behaviours. The thesis concludes through considering the ways in which social workers might consider the use of risk assessment practices through an ethical framework. It I argued that social workers should seeks to bridge Kantian and utilitarian perspectives through a consideration of service user autonomy.

In der vorliegenden Arbeit wird der Entwurf von Funkempfängern für digitale Modulation betrachtet. Dabei liegt der Schwerpunkt der Betrachtungen beim Finden einer geeigneten Empfängerarchitektur, der Spezifikation der einzusetzenden Bausteine, der Auswahl einer geeigneten Arbeitsfrequenz und eines passenden Modulationsverfahrens sowie der Festlegung einer dem Einsatzzweck angepaßten Datenrate. Ein wesentlicher Punkt ist die der Empfangsaufgabe angepaßte gewichtete Bekämpfung der auftretenden Störbeiträge. Dafür werden zunächst Grundlagen, wie wichtige Modulationsverfahren, Empfängerarchitekturen sowie Bestimmungsgleichungen für Parameter eingeführt. Darauf aufbauend wird eine allgemeingültige Methode zum Entwurf von Funkempfängern für digitale Modulation entwickelt. Diese Methode kann stets angewendet werden, wenn ein Empfänger für digitale Modulation entworfen werden soll. Wesentliche zu berücksichtigende Einsatzparameter und Randbedingungen werden an passender Stelle in den Entwurf einbezogen und eine entsprechend angepaßte Lösung wird weiterverwendet. Die Entwurfsmethode eignet sich sowohl für den freien Empfängerentwurf als auch für Entwürfe, die sich an der Einhaltung von Standards orientieren müssen. Der Kerngedanke besteht darin, das alle Störeinflüsse einen gleichgroßen Beitrag zur Signalverfälschung liefern sollen. Um dies zu erreichen, werden potentielle Störeinflüsse analysiert und entsprechend ihres Einflusses gewichtet zur weiteren Bestimmung der notwendigen Eigenschaften der Empfängerbaugruppen herangezogen. In der vorliegenden Arbeit wird hauptsächlich der allgemeine Anwendungsfall des freien nicht standardbezogenen Empfängerentwurfes betrachtet. Es wird aufgezeigt, wie durch die Analyse und entsprechende Berücksichtigung einer variablen Anzahl von Störeinflüssen die Eigenschaften der einzelnen Empfängerkomponenten jeweils bestmöglich eingestellt werden können. Diese Methode wird anschließend anhand von verschiedenartigen Beispielen weiter illustriert und ihre erfolgreiche Anwendbarkeit veranschaulicht. Anhand einer Mischerschaltung wird beispielhaft auf grundlegende Ansätze zur Verbesserung der Linearitätseigenschaften eingegangen. Dabei werden spezifische Unterschiede zwischen einer Superhet- bzw. einer Direktmischarchitektur hervorgehoben. Weiterhin wird eine Methode zur Wiederverwendung (Re-Use) von analogen Schaltungen oder ähnlichen Objekten vorgeschlagen. Dabei werden die Schaltungen anhand einer Vorgabe bewertet, und es wird eine Rangfolge aufgestellt. Damit ist gewährleistet, daß die der Vorgabe am besten entsprechende Schaltung gefunden werden kann, auch wenn eine 100%ige Übereinstimmung nicht gegeben ist. Die hierfür notwendigen Bewertungsalgorithmen werden ausführlich dargestellt und ihr jeweiliger Einsatz wird erläutert. Abschließend erfolgt eine überblicksweise Darstellung von Simulationswerkzeugen und -verfahren, die für den Empfängerentwurf bedeutsam sind. Dabei wird hauptsächlich auf die Verbindung von System- und Schaltungsentwurf eingegangen, und es werden bestehende Probleme dargelegt sowie vorhandene Lösungsmöglichkeiten aufgezeigt. Einige im Zusammenhang mit der Simulation interessante Modellierungsgesichtspunkte werden ebenfalls dargestellt.

Background: Epilepsy is a risk factor for suicidal ideation (SI). SI still poorly investigated in caregivers of people with epilepsy (PWE). Aim: We aimed to analyze the prevalence and predictors of SI in adults PWE and caregivers. Methods: We analyzed 548 consecutive PWE (60% women; median age 41 [18-83]) and 191 caregivers (72% women; median age 47 [18-82]) followed at Outpatients’ epilepsy clinics from a tertiary center. We used “item nine” (item scores ≥1) of the BDI-II to determine the SI presence. The presence of symptoms of anxiety (with BAI) and depression (with BDI-II) was defined with scores ≥14 (for both PWE and caregivers). The presence of anti-seizure drugs (ASDs) adverse effects was defined with the “Liverpool Adverse Events Profile” score≥46. Epidemiological factors and anxiety were investigated as predictors of SI in caregivers and PWE (along with seizure frequency, epilepsy-type, and ASDs’ adverse events). Results: Depression was present in 41% of the PWE and 32% of the caregivers (p=0.04). Anxiety was observed in 37% of PWE and 33% of caregivers (p=0.32). The SI frequency was higher in PWE (19%) compared to the caregivers (11%; p=0.02). Logistic regression analyses were performed for both groups separately. For the PWE (model accounted for 17%-27%), the most significant predictors were anxiety (OR 4.4, p<0.001), presence of ASDs’ adverse effects (OR 2, p=0.021), recurrent seizures (OR 3, p<0.004), and younger age (OR 0.98, p<0.037). For the caregivers (model accounted for 18-37%), only anxiety (OR 43, p<0.001) predicted SI. Conclusion: Identifying SI predictors is equally necessary for PWE and caregivers.

Abstract:
Rendering of large 3D scenes with a convincing level of realism is a challenging computer graphics problem. One of the common approaches to solving this problem is to use different levels of details (LOD) for scene objects, depending on their distance from the observer. Using hierarchical levels of detail (HLOD), when levels of details are created not for each object individually, but for large groups of objects at once, is more effective for large scenes. However, this method faces great challenges when changes occur in the scene. This paper discusses a specific class of scenes with a deterministic nature of events and introduces a method for effective rendering of such scenes based on usage of so-called hierarchical dynamic levels of details (HDLOD). Algorithms for generating HDLOD and their use for visualization of the scenes are also described.

Abstract:
Objetivos Describir la población observada en una consulta de salud mental en una organización no gubernamental (ONG) que apoya a personas con trastornos por uso de sustancias en Lisboa, Portugal, y analizar la patología dual existente. Material y métodos Se realizó un estudio observacional retrospectivo de todos los pacientes observados en un período de 18 meses desde la creación de la consulta de salud mental. Toda la información recopilada se extrajo de forma anónima y se destruyó después del procesamiento. Resultados y conclusiones En los 18 meses desde la creación de la consulta de salud mental se observaron 76 pacientes, 66% hombres. La mayoría tenía entre 35 y 44 años (el promedio era de 41 años) y el 74% tenía nacionalidad portuguesa. El 29% de los pacientes eran indigentes u ocupaban casas abandonadas. Una parte de la población presentaba únicamente enfermedad mental o trastorno por adicción, pero la mayoría (66%) presentaba patología dual. Las patologías más frecuentes como diagnóstico primario fueron los Trastornos mentales y del comportamiento derivados del consumo de sustancias psicoactivas (37 %), en su mayoría por plurisustancias, y los Trastornos afectivos (37 %), seguidos de la Esquizofrenia, el trastorno esquizotípico y delirante ( 13%). La tasa de trastorno por uso de sustancias fue la siguiente: alcohol 49%; benzodiazepinas 24%; cannabinoides 50%; cocaína 43%; heroína 37%. El 21% de los pacientes están en un Programa de Mantenimiento con Metadona. También se comprueba que el 59% de los pacientes tienen patología médica importante (principalmente infecciosa o neurológica). Cada vez más, se hace imperativo reconocer los trastornos de adicción como fuente y consecuencia de la enfermedad mental, en un ciclo que se perpetúa y requiere un abordaje conjunto, fundamental para brindar la mejor atención en salud mental a quienes padecen patología dual.

Abstract In the present study, the characteristics of heat transfer are experimentally investigated on the falling film evaporation of alternative pure refrigerant HFO-1233zd(E) in a plate-fin heat exchanger having a vertical rectangular minichannels of 2.11 mm hydraulic diameter consisting of serrated-fins. The refrigerant liquid is supplied to the channel through 37 holes of a liquid distributor. The liquid flowing down vertically is heated electrically from the rear wall of the channel and evaporated. The experimental parameters are as follows: the mass velocities are varied 7.4∼55.1 [kg/(m2s)], the heat fluxes are varied 8.7∼50 [kW/m2] and the pressures are about 100 [kPa]. In the case of small mass velocity being smaller than 20 [kg/(m2s)], heat transfer coefficient decreases monotonously and gradually with increase of quality and take value of 5∼1[kW/m2K]. It is thought that the cause of lower value of heat transfer coefficient is occurring dry patch and aria enlargement of dry area in the downstream region. In the case of large mass velocity being larger than 41 [kg/(m2s)], the value of heat transfer coefficient becomes larger with increase of quality except for the case of mass velocity being 41 [kg/(m2s)] and heat flux being 50[kW/m2]. Improved heat transfer is to be thinkable that the liquid and vapor flow becomes dripping in the middle-stream region, and turns into mist flow with thin liquid film on the fin surface in the downstream region of quality being larger than 0.4.

Abstract:
Introduction: Little is known about the impact of enzyme-inducing antiseizure drugs (EI-ASD) on the reduction of Bone Mineral Density (BMD) in men with epilepsy (MWE). Objectives: To evaluate the BMD in MWE exposed to EI-ASDs (phenytoin, carbamazepine and phenobarbital) and its relationship with the duration of epilepsy. Methods: We evaluated BMD from 74 consecutive MWE (median age (range), 52.5 (25- 74) years) exposed to previous or current EI-ASDs, followed at UNICAMP-Brazil. Individuals were split into two groups (young-group, 31 individuals [25-49 years]; older group, 43 subjects, [50-74 years]). The BMD test evaluated t-score indexes from the femoral neck, whole femur and lumbar spine. Osteopenia was defined with t-score of - 1.0 to -2.4; osteoporosis, with T-scores lower than -2.5. Data were extracted from medical records. We analyzed data with SPSS22, performed chi-square tests for categorical variables and applied a partial correlation test (controlled for age) between BD scores and duration of epilepsy. Results: BMD was reduced in 49/74 men (66.2%). Both groups presented equivalent proportions of BMD abnormalities (p=0.087) (young-group [14/41 normal (45%), 12/31 osteopenia (39%), 5/31 osteoporosis (16%)]; older-group [11/43 normal (26%), 16/43 osteopenia (37%), 16/43 osteoporosis (37%)]. BMD did not correlate with the duration of disease or age of onset. Conclusion: BMD reduction is highly prevalent in MWE exposed to EI-ASD, including young individuals. Data suggest that exposure to EI-ASD may associate with early BMD reduction, which evolve to osteopenia and osteoporosis. BMD evaluation in MWE and appropriate treatment may be necessary to reduce fractures’ risk.
