Abstract:
In order to expand hepatitis C virus (HCV) screening, a change in the diagnostic paradigm is warranted to improve accessibility and decrease costs, such as utilizing dried blood spot (DBS) collection. In our study, blood from 68 patients with chronic HCV infection was spotted onto DBS cards and stored at the following temperatures for one week: −80 °C, 4 °C, 21 °C, 37 °C, and alternating 37 °C and 4 °C; to assess whether temperature change during transportation would affect sensitivity. Sample was eluted from the DBS cards and tested for HCV antibodies (HCV-Ab) and HCV core antigen (core-Ag). HCV-Abs were detected from 68/68 DBS samples at −80 °C, 4 °C, 21 °C, and 67/68 at 37 °C and alternating 37 °C and 4 °C. Sensitivity of core-Ag was as follows: 94% (−80 °C), 94% (4 °C), 91% (21 °C), 93% (37 °C), and 93% (37 °C/4 °C). Not only did temperature not greatly affect sensitivity, but sensitivities are higher than previously reported, and support the use of this assay as an alternative to HCV RNA. We then completed a head-to-head comparison (n = 49) of venous versus capillary samples, and one versus two DBS. No difference in core-Ag sensitivity was observed by sample type, but there was an improvement when using two spots. We conclude that HCV-Abs and core-Ag testing from DBS cards has high diagnostic accuracy and could be considered as an alternative to HCV RNA in certain settings.

Abstract:
95 Background: There is little evidence that chemotherapy (CT) impacts on outcome of patients with MSEC. We designed an ongoing randomized phase 2 trial to detect a progression-free survival benefit of CT continuation over CT discontinuation in disease-controlled and ECOG≤2 patients at 6 weeks after an initial CT treatment. The aim of the present study was to identify predictors of disease control at 6 weeks (DC6wkx) in MSEC patients receiving platinum-based CTs as first-line treatment for metastatic disease. Methods: Among 68/70 evaluable patients included between 1/2011 and 7/2013 who received at least 1 CT cycle, 58 were evaluable for disease assessment at 6 weeks. Ten patients were not evaluable (early death: 4, patient’s decision: 2, concomitant disease 1, early progressive disease 1, other reasons: 2). Baseline demographic, clinical, biological, and tumor characteristics were tested for prediction of DC6wkx. Significant variables for DC6wkx were identified with the chi-squared test and logistic regression. Results: Baseline patients characteristics were as follows: median age: 61.5yo; male: 57/68; ECOG 0/1/2: 13/42/13; metachronous/synchronous MSEC: 38/30; number of metastatic sites 1/2/>2: 35/20/13; metastatic location: lung 36, liver 23, bone 11, nodes 37, other 11; prior exposure to CT: 37/68; time from previous CT exposure: ≤ 6m 6/37, 6-12m 14/37, > 12m 17/37; gr>2 dysphagia (Atkinson) 19/67; BMI<18.5kg/m²: 13/68. Current CTs were FU-CDDPq3w 2/68, LV5FU2-CDDPq2w 15/68, FOLFOX 51/68, and patients received the following number of cycles 1/2/>2: 5/7/54. DC6wkx rate was 65.7%, with 16/68 PR (23.5%) and 28/68 SD (42.2%). Albumin (p<0.01), BMI (p<0.02), bone metastases (p<0.005), gender (p<0.047) and ECOG status (p<0.05) were predictive of DC6wkx. Normal or overweight BMI, grade 0 albumin, and no bone metastases, were predictive of DC6wkx in multivariate analysis. Conclusions: DC at 6 wks was 65.7% in MSEC receiving platinum-based CTs as first line treatment for metastatic disease. Normal or overweight BMI, normal albumin, and the absence of bone metastasis were significant predictors of DC6wkx in this prospective phase 2 trial. Clinical trial information: NCT01248299.

Abstract John 2:17 quotes Ps 68:10: “Zeal for your house will consume me.” Interpreters disagree about whether consume portrays Jesus’s zeal overwhelming him during the temple incident or leading to his death. They also disagree about whether John alludes metaleptically to the whole psalm, especially the rebuilding of Jerusalem in Ps 68:36–37. This article argues that consume portrays Jesus’s death. It substantiates that John alludes to the whole psalm, not only the rebuilding of Jerusalem in 68:36–37, but also the table becoming a trap and the pouring out of wrath in 68:23, 25. These echoes suggest that Jesus embodies the judgment of God in the temple incident, the suffering of the psalmist in his death, and the restoration of Jerusalem in his resurrection. The story from the Psalter is thus reconfigured in the temple incident: God rebuilds the forsaken city by identifying with Israel’s exile in the crucified body of Jesus.

Abstract Four monoclonal antibodies, SBU.II 28-1, 37-68, 38-27, and 42-20, each recognizing a distinct, non-overlapping subset of sheep class II molecules, were used to purify class II molecules from a single sheep. Four class II alpha subunits designated 28-1 alpha, 37-68 alpha, 42-20 alpha, and 38-27 alpha and five class II beta subunits designated 28-1 beta, 37-68 beta 1, 37-68 beta 2, 42-20 beta, and 38-27 beta were compared by N-terminal sequence analyses. Two distinct alpha subunits were identified; the 28-1 alpha, 37-68 alpha, and 42-20 alpha subunits all had identical N-terminal amino acids sequences, which exhibited about 75% homology with HLA-DR alpha and mouse E alpha polypeptides. In contrast, the 38-27 alpha sequence exhibited about 80% sequence homology with HLA-DQ alpha and mouse A alpha polypeptides. In general, sheep beta subunits displayed insufficient sequence homology to enable correlation with human beta-chain sequences; however, the 38-27 beta-chain sequence showed homology with the HLA-DQ beta sequence. The conserved sequence surrounding the site for N-linked glycosylation within human/mouse beta polypeptides (residues 19 to 21) was not present in sheep beta sequences and in contrast with the beta-chains of mouse and man, sheep beta polypeptides contained between 1 and 3 positionally variable cysteine residues (residues 13 to 15 inclusive). Individual sheep beta subunits exhibited extensive sequence heterogeneity and each consisted of a unique population of beta polypeptide species. At least 16 different beta polypeptide sequences were identified from a single sheep and the existence of no fewer than nine non-allelic beta genes was inferred from the sequence data. We have previously provided evidence suggesting that the sheep has multiple major histocompatibility complex class II alpha and beta genes related to those of all three HLA-D subregions. The present results suggest that a number of these genes encode HLA-DQ-like heterodimers and that a sheep DR-like alpha gene product is shared with the products of a large and heterogeneous sheep beta gene family.

ABSTRACTOrientia tsutsugamushiis the etiological agent of scrub typhus, a mite-borne, febrile illness that occurs in the Asia-Pacific region. We conducted strain characterization ofO. tsutsugamushiisolates from chiggers obtained from rodents based the nucleotide sequence of the 56-kDa outer membrane protein gene. With the use of PCR, a total of 68 DNA sequences of 56-kDa antigen genes were amplified. Phylogenetic analysis revealed that there were at least six definable clusters among the 68 isolates: 37% Karp-related strains (25/68), 27% TA763 strains (18/68), 12% JG-related strains (8/68), 19% Kato-related strains (13/68), 4% divergent strains (3/68), and 1% representing a Gilliam prototype strain (1/68). Overall, theO. tsutsugamushigenotypes exhibited a high degree of diversity, similar to that seen in strains from the rest of the areas where scrub typhus is endemic. Moreover, the 56-kDa protein sequence similarity betweenO. tsutsugamushiisolates from mites and those from human patients (H. Y. Lu et al., Am. J. Trop. Med. Hyg. 83:658-663, 2010) were striking, thus highlighting potential risk factors for this emerging zoonotic disease.

Abstract:
This retrospective study aimed to assess the prevalence of ultrasonographic detected adrenomegaly in clinically ill cats, evaluating the final established diagnosis, describe adrenal ultrasound findings and if the adrenomegaly was suspected or incidental. Abdominal ultrasonography reports of cats presenting to a veterinary teaching hospital between October 2018 and February 2021 were retrospectively reviewed. Cats showing adrenomegaly (one or both glands having a dorsoventral axis >4.8 mm) were selected and medical records respectively evaluated. Nine-hundred and eighty-three ultrasonographical reports were selected, of which, 68 (7%) disclosed adrenomegaly. European/Domestic Short-Hair (62/68; 91%) male (44/68; 65%) castrated (35/44; 80%) cats were overrepresented. Adrenomegaly was an incidental finding in 62/68 (91%) cats while in 6/68 (9%) it was identified in the context of investigating a potential adrenal disease. Concerning established diagnosis, chronic kidney disease was overrepresented (25/68; 37%), followed by endocrinopathies (20/68; 29%). Adrenomegaly was bilateral in 53% (36/68) of cases. In unilateral cases (32/68; 47%), it was more prevalent on the left side (23/32; 72%), with a normal-sized contralateral adrenal gland. Left adrenal demonstrated a larger size and a tendency to oval shape. This study assesses the prevalence of adrenomegaly in clinically ill cats, reinforcing it can be an incidental ultrasound finding.

Abstract:
379 Background: Somatostatin analogue functional imaging with Ga-68 Dotatate PET/CT has demonstrated superiority in lesion detection in patients with NETs. The effect of this novel imaging modality on US clinical practice and its usefulness in different types of NETs is not well described. We describe the impact of initial NETSPOT imaging on diagnosis and management in NET patients at a large urban medical center. Methods: Consecutive patients diagnosed with NETs and referred to our institution who received an initial Ga-68 Dotatate PET/CT between 07/2017-09/2018 were included. Imaging was reviewed and compared to prior available CT, MRI, and/or In-111 Pentetreotide scans. Results: Among 101 patients, 51/50 were female/male, tumor origins were gastroenteropancreatic (GEP) (75%), Unknown Primary (UP) (13%), lung (8%), thymic (2%), and other (2%). All tumors were histologically well/moderately differentiated and 37/51/3/10 were G1/G2/G3/Unknown, respectively. Initial imaging with Ga-68 Dotatate PET/CT revealed additional metastatic disease in 37 of 77(48%) patients with prior evidence of metastatic disease. Most common sites were distant lymph nodes (18), bone (15) and liver (9), peritoneal/pleural (4). A previously UP tumor was identified in 3 patients. No patients with metastatic lung carcinoids (6 atypical, 2 typical) or thymic NETs (2 atypical/G2) NETs had evidence of Ga-68 Dota PET/CT uptake above reference liver SUV levels. Results of imaging altered patient management as follows: 14 initiated systemic therapy due to documentation of progression, in 6 surgical therapy was altered, in 4 biopsy/other management was changed. In 11 patients with no tumor Ga-68 Dotatate uptake, decisions about use of PRRT and somatostatin analogues was altered. Conclusions: In this series, Ga-68 Dotatate PET/CT altered diagnosis and management in 35/101 NET patients. Among GEP and UP NETs, Dotatate imaging diagnosed primarily new nodal, bone, liver and pleural/peritoneal metastases missed by other imaging modalities. Results support the routine use of Ga-68 Dotatate PET/CT in the care of patients with advanced and early stage NETS.

Objetivo: determinar el comportamiento epidemiológico de las enfermedades más frecuentes, transmitidas por vectores en niños guatemaltecos entre los años 2015-2019. Materiales y métodos: estudio descriptivo, de corte transversal, retrospectivo, con datos del Sistema de Información Gerencial de Salud (SIGSA), del Ministerio de Salud Pública y Asistencia Social de Guatemala, (MSPAS). Se determinaron la distribución geográfica por departamentos, los rangos de edad con mayor número de casos y la enfermedad transmitida por vectores con mayor frecuencia en el país. Resultados: del total de casos reportados de enfermedades transmitidas por vectores, los niños de 5 a 9 años de edad representaron el 37% de las personas afectadas. El dengue fue la enfermedad más frecuente, 68% del total. Conclusiones: la mayoría de los casos de enfermedades transmitidas por vectores reportados del 2015-2019, corresponden a Dengue con un 68%. El rango de edad más afectado fue de 5 a 9 años, con un 37% de los casos reportados. En el departamento de Guatemala ocurrió la mayor proporción en tres enfermedades.

Dans cette thèse ont été analysées les conditions et le contexte de la progression mondiale du phénomène médiatique, d'une part par l'étude des médias d'information à vocation internationale, mais aussi par l'étude des enjeux financiers qui ont transformé l'industrie de la communication en un acteur très puissant de l'économie globale. Cette implication majeure de la chose médiatique dans le système international a donné lieu à l'émergence d'une véritable opinion publique mondiale devenue le théâtre d'entreprises d'influence destinées à orienter les esprits au moyen de l'élaboration de contenus adaptés à leur hétérogénéité. Ont également été étudiés dans cette étude les moyens d'action et de réaction des Etats face à un phénomène médiatique indissociable des relations internationales et faisant fi des représentations classiques du concept de souveraineté. Les Etats eux-mêmes en ont fait un outil de propagande vouée à la mise en valeur de leurs idéologies ou de leurs politiques. De même, une présence croissante des médias dans les conflits militaires, tant comme témoins qu'en tant qu'acteurs directs ou indirects de ces guerres, a été constatée. Les incidences géostratégiques relatives à cette utilisation internationale des médias sont-elles susceptibles d'engendrer l'impuissance des Etats face aux flux d'informations, d'autant qu'un certain nombre de contraintes juridiques, économiques et techniques pèsent dorénavant sur eux ? Cet affaiblissement apparent de l'Etat ne l'a pas laissé sans réactions, certaines étant particulièrement anti-démocratiques, d'autres plus constructives et favorables à la diffusion de la pensée humaine et au progrès des sociétés dans le cadre de la mondialisation
The context and the conditions under which the media phenomenon has grown so swiftly on a worldwide basis have been analyzed throughout this thesis. On one hand by the study of international information media, on the other by the study of the financial stakes which transformed the communication industry into a powerful actor of the global economy. This major implication of the media in the international system gave rise to the emergence of a worldwide public opinion being now targeted by operations of influence destined to orient hearts and minds with specific contents. Furthermore, States' actions and reactions facing a media phenomenon, now fully part of international relations, and regardless of classical concepts of sovereignty, have been studied in this work. States themselves used media as a propaganda tool to emphasize their ideologies and policies. Besides, the growing involvement of media in military conflicts, as witnesses or now as direct and indirect actors of these wars, has been established. As a result, is the geostrategic impact due to global use of media likely to generate a growing weakness of States facing relentless flows of information especially since legal, economical and technical aspects have favored this situation? This apparent weakening of States generated reactions, some of which are particularly anti-democratic, but others more constructive and willing to contribute in the diffusion of human thought and in the progress of human societies in the globalization context

A presente dissertação trata da relação estabelecida entre as personagens femininas dos Anais, de Tácito, e a construção da imagem de Nero como um mau imperador. A partir da identificação de dispositivos retóricos relativos às mulheres, principalmente nos livros XIII ao XVI dos Anais, parte do relato referente aos acontecimentos do principado de Nero (54-68 d.C.), buscamos compreender como Tácito utilizou-se das personagens femininas como recurso retórico na elaboração de críticas a este imperador. Tais críticas indicam valores morais pelos quais se orientava a sociedade romana. Neste sentido, analisamos como as interações entre as personagens e a composição de exempla (exemplos) evidenciam modelos de vícios e virtudes. A fim de fundamentar nossa análise, exploramos questões a respeito da condição social e política em que viviam tais mulheres, principalmente aquelas que pertenciam à domus Caesarum (Casa dos Césares). ___________________________________________________________________________________________________________________________________________________
ABSTRACT: This dissertation deals with the relationship established between the female characters of the Tacitus‟s Annals, and the making of the Nero‟s image as an evil emperor. Identifying rhetorical devices relating to women, especially in the Annals‟s books XIII to XVI, as part of the narrative regarding the events of the Nero‟s principate (54-68 AD), aims to understand how Tacitus had used the female characters as a rhetorical tool to censure the emperor. These criticisms indicate moral values which the Roman society was embeded. In this way, we analyse how the interactions between the characters and the composition of exempla (examples) show models of vices and virtues. In order to substantiate our analysis, we will explore questions about the social and political conditions in which these women have lived, especially those who belonged to domus Caesarum (Caesars household).

Abstract:
La investigación mostrada en esta tesis tiene como base el contexto y necesidades de las personas mayores de 55 años en la actual sociedad de la información, comunicación y del conocimiento. Se examina, a través de diferentes Proyectos Europeos de innovación, de qué modo una actuación socio-educativa mediada por la tecnología afecta el bienestar y calidad de vida de la persona mayor, entre otros factores, y de cómo el estudiante evoluciona desde la adopción hacia la inclusión digital. Como resultado se propone una actuación educativa dirigida a personas mayores hacia la inclusión digital partiendo de la adopción de las TICs de una forma útil y provechosa en diferentes fases o pasos que se detallan en propuestas pedagógicas que son evaluadas y validadas.
The research done in this thesis is founded on the context and needs of citizens over 55 years of age in the current society of information, communication and knowledge. An analysis id performed through different European Innovation Projects, to show how a technology-mediated socio-educational action affects the well-being and quality of life of the elderly, among other factors. We also show how the student evolves from adoption to Digital inclusion. Este enfoque tradicional ha perdido interés por parte de los estudiantes y cada día se observa que los jóvenes son más apáticos y se encuentran menos interesados en el patrimonio cultural y material de su territorio.Teniendo en cuenta lo anterior, se hace necesario diseñar experiencias de aprendizaje que faciliten el desarrollo de procesos de Educación Patrimonial que motiven a las personas a interactuar con su patrimonio. Una alternativa para el diseño de este tipo de experiencias de aprendizaje es el uso de la Realidad Aumentada que sobrepone información digital en el mundo real facilitando la interacción real entre usuarios y el patrimonio
Heritage Education is the process that enables people to understand and appropriate their tangible and intangible heritage. Traditionally, Heritage Education takes place in educational environments (within the school), where the teacher transmits the necessary knowledge to the students. This traditional approach has lost interest among students, and young people are becoming more apathetic and less interested in the cultural and material heritage of their territory. Bearing this in mind, it is necessary to design learning experiences to facilitate the development of Heritage Education processes and to motivate people to interact with their heritage. An alternative for the design of this type of learning experiences is the use of Augmented Reality in the context of Heritage Education, taking into account that Augmented Reality overlays digital information in the real world, facilitating real interaction between users and heritage.

El aprendizaje móvil (m-learning) se encuentra todavía en su infancia y grandes esfuerzos se deben hacer para investigar el cambio de paradigma educativo, desde la forma de enseñanza tradicional de 'un modelo único para todos' a un aprendizaje adaptativo que se pueda entregar a través de dispositivos móviles. De esta manera, el siguiente desafío ha sido identificado por esta implicación: ¿Cómo se puede implementar el diseño instruccional con el fin de beneficiarse de las características del m-learning y lograr la adaptación y personalización del proceso de aprendizaje personal en diferentes contextos? Un importante factor para lograr un m-learning personalizado y adaptable ha sido el procesamiento de la información contextual de los estudiantes. This study is focused on the different ways the character of the Emperor Nero is depicted by the biographer Suetonlus and the historiographer Tacitus. The study is Introduced with an historical survey of the life of Nero. Thereupon a discussion follows on the life experiences of the authors, Suetonius and Tacitus, as well as the different style and genre used by each, and suggestions on how their writings could have been influenced by these factors. Two aspects of Nero's life are discussed at length, namely his relationship with his mother (and issues relating to that). as well as his interest in and practising of the arts (focusing on Nero as a poet, singer and charioteer). The discussion of these topics is done in the following manner: firstiy Suetonius' account of the events are given, followed by the account by Tacitus. These versions are then compared and where different, the candidate suggests possible reasons for the difference of opinion or presentation between the authors. Various characteristics of the Emperor Nero are referred to. The main conclusions can be categorised into three sections: - First and foremost the authors had different life experiences. Suetonius held various secretarial positions at the court of Caesar Hadrian which SUbsequently influenced his writings. He Was not however a senator or interested In politics and military affairs, which explains the fact that he does not write about political upheavals and does not contribute political motivations for certain actions of characters under discussion. Tacitus on the contrary, was a senator during the reign of Domitian. The atrocities committed by the Emperor and the slowly diminishing power of the Senate influenced the mind and writings of Tacitus to a great extent.

Abstract:
By combining literary criticism, philology, and contemporary psychoanalysis, this dissertation offers an innovative interpretation of Neronian court literature (Seneca, Lucan, and Petronius). I argue that the works of these three authors thematize and embody a problematic relation between the human subject and language. Language is not conceived or represented as an inert tool that can be easily appropriated by the speaking subject, but rather as a powerful entity that may, and often does, take control of the human subject, directing it from without. Besides analyzing how Seneca, Lucan, and Petronius portray the relation between the human subject and language in the internal plots and characters of their works, I also explore the relation between these three authors themselves and language. My conclusion is that this relation is defined by unresolved ambiguities and neurotic tensions, and I suggest that this might be a consequence of the traumatizing circumstances that the three examined authors endured at Nero’s court.

Abordagens que utilizam jogos para auxiliar no processo são consideradas boas alternativas. Neste sentido, este trabalho aborda a realização e os resultados de uma Revisão Sistemática da Literatura no intuito de explorar, identificar e analisar os projetos de jogos digitais existentes que propõem-se a engajar o paciente na realização das sessões. Os resultados reafirmam problemas de engajamento e motivação dos pacientes (68, 81% dos trabalhos trazem esta problemática), além de mostrarem uma grande diversidade de projetos realizados, com uma vasta gama de métodos de interação entre o paciente e o jogo, além de diversos recursos empregados para a promoção do engajamento no paciente, como a própria utilização dos jogos em si (43%) ou ainda questões relacionadas à dificuldade da atividade (37%).

We have shown earlier that abnormal platelet aggregation in spontaneously hypertensive rats (SHR) is not caused by prostaglandins (Thromb. Res. 41, 555-566, 1986). In this study platelets from SHR and normotensive (Wistar Kyoto, WKY) rats were used to examine the role of phosphoinositides (Pins) and protein phosphorylation in increased platelet activation in hypertension. Thrombin (0.05 U/ml) induced rapid hydrolysis of phosphatidylinositol-4,5-bis-phosphate (PIP2), phosphatidyl-inositol-4-phosphate (PIP), and phosphatidylinositol (PI) in (32p)-pO4 labeled platelets. However, significantly greater hydrolysis of PIP2 and PI was seen in SHR platelets than in WKY platelets (see Table). Thrombin also caused two- to three-fold increased accumulation of phosphatidic acid (PA) in SHR platelets than in WKY platelets (see Table).Thrombin caused phosphorylation of 18,000 Dalton (P18) and 47, Dalton (P47) proteins in SHR and WKY Platelets. Significantly increased phosphorylation of P47 was seen at 5, 15, 60 and 240 seconds of incubation with thrombin in SHR platelets (60%, 68%, 98% and 91%) than in WKY platelets (13%, 37%, 44% and 47%). The extent of P18 phosphorylation was same in both SHR and WKY platelets. Aspirin (500 uM) did not affect phosphorylation of P47 or P18 in SHR or WKY Platelets. These data lead us to suggest that increased turnover of Pins and increased phosphorylation of P47 are involved in abnormal platelet aggregation in SHR (Supported in part by the COHC grant #86-01-A and the Ohio University College of Osteopathic Medicine).

Measurements of decompression wave speed in conventional and rich natural gas mixtures following rupture of a high-pressure pipe have been conducted. A high pressure stainless steel rupture tube (internal diameter = 38.1 mm, and 42 m long), has been constructed and instrumented with 16 high frequency-response pressure transducers mounted very close to the rupture end and along the length of the tube to capture the pressure-time traces of the decompression wave. Tests were conducted for initial pressures of 33–37 MPa-a and a temperature range of 21 to 68 °C. The experimentally determined decompression wave speeds were compared to both GASDECOM and PIPEDECOM predictions with and without non-equilibrium condensation delays at phase crossing. The interception points of the isentropes representing the decompression process with the corresponding phase envelope of each mixture were correlated to the respective plateaus observed in the decompression wave speed profiles. Additionally, speeds of sound in the undisturbed gas mixtures at the initial pressures and temperatures were compared to predictions by five equations of state, namely BWRS, AGA-8, Peng-Robinson, Soave-Redlich-Kwong, and GERG. The measured gas decompression curves were used to predict the fracture arrest toughness needed to assure fracture control in natural gas pipelines. The rupture tube test results have shown that the Charpy fracture arrest values predicted using GASEDCOM are within +7 (conservative) and −11% (non-conservative) of the rupture tube predicted values. Similarly, PIPEDECOM with no temperature delay provides fracture arrest values that are within +13 and −20% of the rupture tube predicted values, while PIPEDECOM with a 1 °C temperature delay provides fracture arrest values that are within 0 and −20% of the rupture tube predicted values. Ideally, it would be better if the predicted values by the equations of state were above the rupture tube predicted values to make the predictions conservative but that was not always the case.

Sawmills in the Canadian province of British Columbia (BC) will soon be confronted with a collective wood waste disposal problem (bark, sawdust and shavings) of about 3 Million bone dry tonne/y at an average wet basis moisture content of about 45%. About 40 existing sawmill beehive burners presently incinerate this waste. Emissions from these beehive burners exceed current provincial particulate limits. Markets for the waste — board plants, sawdust digesters, charcoal plants, etc. — are limited. The economics of 25 to 50 MW wood-fired, steam power plants is poor. 6¢/kW.h is needed to finance a plant; the major utility offers 2¢/kW.h. This paper describes a 3 MW to 5 MW, Nuovo Pignone PGT-5 co-generation plant fuelled by the 2000°F (1093°C) exhaust from a Heuristic EnvirOcycler, a two-stage, wood waste incinerator. The exhaust meets BC’s particulate limit of 0.052 grains/dscf (120 mg/Nm3). 37 to 65 Million Btu/h (39 to 68 GJ/h) of waste heat can be recovered from the system exhausts. In this application the PGT-5’s external combustion chamber is replaced by a “recuperator”, i.e., a high temperature, gas-to-air, heat exchanger. Two variations of the basic system are examined. One features a larger than necessary EnvirOcycler to generate additional steam in the waste heat boiler. The other variation discusses heating 1,550°F (843°C) air from the recuperator up to 1,796°F (980°C) with natural gas. The extra power generated can cost as little as 1.9¢/kW.h.

DDAVP has been used for hemorrhage prevention during dental extraction in patient (pts.) with von Willebrand's disease (vWd), usually of type I. However, only about 20 cases have been reported. Moreover, themajority of pts. had basal level ofVIII:C and vWf equal to or higher than 20% so that dental procedures could probably have been performed safely also without any specific treatment.We report the experience with DDAVP for hemorrhage prevention during dental extraction in 14 pts. withtype I vWd (confirmed byplasma multimeric analysis, Prof. Mannucci, Milan) and severely reduced basal level of VIII:C (mean 12.9 ± 3 UI%; range 8-18 UI%) and vWf (< 3 UI%), measured as ristocetin cofactor. VWf:Ag and vWf platelet content (UI%/109 platelets; platelets separated by Ficoll-Hypaque and lysed with TritonX) was in normal range (17-35 UI%and 22-68% in pts.; 17-37 UI% and 24-62 UI% in 18 controls). After DDAVPinfusion (0.4 μg/ml), VIII:Cand vWf activities increasedrespectively 8.5 folds (range 6.4-11.8) and 28.2 folds (range 16-40). Return to basal levels occured with an half-life of60 -80 minutes for all the activities. Bleeding time, prolongedin 8 pts., was ormalizedin 7 cases and shortened in 1. All the pts.referred of previous hemorrhage after tooth extractionperformedin the past. A total of 50 extractions were performed (3.67 teeth/pts). Mild bleeding occured in only 2pts.3-5 days after extraction and waspromptly stopped by repeating DDAVPinfusion.In conclusion, DDAVP has proved effective for dental extraction in type I vWd pts. with severely reduced VIII:C and vWf activities in plasma but normal platelet content.

Background: Meningitis after craniotomy can cause devastating outcomes. Objectives: Estimate the risk of meningitis after craniotomy (MAC). Find the most prevalent pathogens. Assess the impact of meningitis over length of stay and mortality. Find the main risk factor for MAC. Design and setting: Multicentric, longitudinal and quantitative analysis of data collected between 2013-2017 from nine different hospitals from Minas Gerais, Brazil. Methods: Surveillance data was based on NHSN/CDC protocols. Observed outcomes were meningitis, hospital death and total length of stay. Twentythree variables were analyzed in Epi-Info in a two-tailed statistical test with a significance level of 5%. Results: 4,549 patients were analyzed. Risk of MAC was 1.9% (95%CI=1.6%; 2.4%). The mortality rate in patients without infection was 9%, increasing to 33% in infected patients (P<0.01). Length of hospital stay (HS) in uninfected patients (in days): mean=18, median=7, standard deviation=36. HS in infected patients: mean=56, median=37, standard deviation=63 P<0.001).The duration of the procedure ≤4 hours presented a 1.5% risk of MAC compared to 2.5% versus ≥4 hours (RR=1.7; P=0.041). From 88 MAC cases, pathogen was identified in 68 (77%): K.pneumoniae (20%), S.aureus (16%), A.baumannii (13%), P.aeruginosa (9%), Staphylococcus sp . (8%), Acinetobacter sp. (7%), S.epidermidis (5%) among others (20%).Conclusion: MAC risk was 1.9%. Mortality rate was high compared to literature. Meningitis caused threefold increase on HS. Procedure duration ≥4 hours was the main risk factor, presenting RR of 1.7. The most prevalent etiologic agents were K.pneumoniae and S.aureus. Considering the findings, infectious surveillance is paramount for patient safety.

Abstract:
Thrombin (THR) is generated during ARDS, sepsis and DIC. We wondered whether it might augment PMN/endothelial cell (EC) interaction and hence amplify EC damage by inducing platelet activating factor (PAF) (JCI 76:2235-2246;1985). To examine further this premise and the mechanisms involved, we measured intracel1ular calcium (Ca1) in human EC (grown on glass cover slips), in a scanning spectrof1uorometer at 37°C after loading with FURA 2 (4μM). Resting Ca1 was 148±22 nM (Mean±SEM) which increased following THR 0.5u/ml to 458±160 nM at 30s, peaking after 1 min at 559±176 nM, and returning to 273±42 nM by 5 min. Phosphatidyl inositol (PI) turnover was assessed in 3H-myoinositol-loaded EC using water-soluble extracts separated by Dowex anion exchange chromatography. Within 30s of THR (lu/ml) stimulation, PI turnover markedly increased, with production of inositol bi- and tri-phosphates showing a >5 fold rise. Associated with these perturbations, THR-treated EC monolayers enhanced O2- generation by FMLP(10™7)-stimulated PMNs (basal levels of 5.73±0.68 O2-/15 min/106 PMN rising to 8.01±0.85 nM (p<0.05)); moreover this enhancement could be completely inhibited with a newly described PAF antagonist BN 52021. PAF production is dependent upon phosphorylation of an acetyl transferase, and Ca1 flux and PI hydrolysis are events known to be associated with protein kinase activation. THR-EC stimulation would seem, therefore, to initiate a sequence of events involving PMN/EC 'cross-talk' leading to contact activation of marginated PMNs by EC-derived PAF. This is an example of a novel paracrine response, and is consistent with our data showing the potent priming effect of PAF on PMN oxidant responses (Blood 68:88A; 1986) and provides evidence for a previously unsuspected pathway that promotes PMN oxidant-mediated EC injury during sepsis or other THR-generating disorders.

Being a correctional entity, the Malaysian Prison Department through its vision and mission is responsible to provide a safe detention and rehabilitation. One of its objectives is to ensure suitable and effective correctional programmes for all categories of inmates. One of the programmes set up by Malaysian Prison Department is to have a collaboration with higher education providers to offer tertiary education for inmates in order to equip them with knowledge and skills to pursue work after release. One of such engagement is the provision of the undergraduate and postgraduate programmes for inmates in selected Malaysian prisons by Open University Malaysia (OUM). OUM conducts undergraduate and postgraduate degree programmes for prison inmates. This study aims to determine the factors influencing prison inmates’ decision to pursue higher education in the prisons. 37 respondents from a total of 68 prisoners-students are involved in this study. The Stakeholder Theory and the Theory of Planned Behaviour were adopted as the underpining theories as they are related to attitude and planned behaviour. The Stakeholder Theory enables us to comprehend how a diverse range of stakeholders impact prisoner learning and comprehend the expectations and duties of distinct stakeholders regarding prisoner learning. Whereas, the Theory of Planned Behaviour postulates that an individual’s behaviour is determined by their intention to engage in such behaviour. Factor analysis was utilised to identify if these three factors (student attribute, social influence and financial aid) influenced prison inmates’ decision to pursue Open University Malaysia’s programmes in prisons. Results of the multiple linear regression indicated that there was a significant effect between student attribute and decision to study, which means that inmates' own characteristics will motivate them to continue learning which can lead to future success in their pursuit for excellence in tertiary education.

Abstract:
Pro-UK is a single chain urokinase-type plasminogen activator (scu-PA) which has fibrin selective thrombolytic properties. However, quantitative data on pro-UK binding to fibrin and on the mechanism of its fibrin enhanced activation of plasminogen have been difficult to obtain. In the present study, a well defined fibrin network constructed on glutaraldehyde-activated PVC plates (Anal. Biochem. 153 : 201-210, 1986) and highly purified pro-UK (99 % scu-PA) were used. Binding was investigated as follows : varying dilutions of pro-UK in the presence of a trace amount of I-labeled pro-UK in buffer without or with glu-plasminogen, plasmin or oC -thrombin and in urine, plasma or serum, were incubated overnight at 4°C and then 2 h at 37°C in the fibrin plates. After washing, the wells were cut out and counted in a gamma-counter. The labeled pro-UK and the effect of enzymes on scu-PA were investigated by SDS-PAGE and autoradiography. In parallel experiments, the activity of the fibrin bound and unbound products was investigated spectrophotometrically by adding glu-plasminogen and a synthetic substrate selective for plasmin. The binding of pro-UK to fibrin was 1.7 ± 0.1 % in buffer and 0.2 ± 0.08 % in plasma, as determined from isotopic and spectro-photometric measurements. This binding is similar to that of (0.13 ± 0.05%) two-chain urokinase (plasmin-transformed scu-PA), but is extremely low compared to the specific binding of tPA (68 - 4%). By contrast, in urine, 11.2 ± 4.47 % binding of pro-UK to fibrin was observed. Thrombin did not modify the binding but transformed scu-PA into a two-chain molecule which had lost activity. These data indicate that pro-UK has little affinity for fibrin under these conditions but that some binding may be induced by a co-factor which is present in urine. Confirmation that thrombin degrades scu-PA was obtained and it is suggested that this effect may help to regulate fibrinolysis.
