Abstract:
In order to expand hepatitis C virus (HCV) screening, a change in the diagnostic paradigm is warranted to improve accessibility and decrease costs, such as utilizing dried blood spot (DBS) collection. In our study, blood from 68 patients with chronic HCV infection was spotted onto DBS cards and stored at the following temperatures for one week: −80 °C, 4 °C, 21 °C, 37 °C, and alternating 37 °C and 4 °C; to assess whether temperature change during transportation would affect sensitivity. Sample was eluted from the DBS cards and tested for HCV antibodies (HCV-Ab) and HCV core antigen (core-Ag). HCV-Abs were detected from 68/68 DBS samples at −80 °C, 4 °C, 21 °C, and 67/68 at 37 °C and alternating 37 °C and 4 °C. Sensitivity of core-Ag was as follows: 94% (−80 °C), 94% (4 °C), 91% (21 °C), 93% (37 °C), and 93% (37 °C/4 °C). Not only did temperature not greatly affect sensitivity, but sensitivities are higher than previously reported, and support the use of this assay as an alternative to HCV RNA. We then completed a head-to-head comparison (n = 49) of venous versus capillary samples, and one versus two DBS. No difference in core-Ag sensitivity was observed by sample type, but there was an improvement when using two spots. We conclude that HCV-Abs and core-Ag testing from DBS cards has high diagnostic accuracy and could be considered as an alternative to HCV RNA in certain settings.

95 Background: There is little evidence that chemotherapy (CT) impacts on outcome of patients with MSEC. We designed an ongoing randomized phase 2 trial to detect a progression-free survival benefit of CT continuation over CT discontinuation in disease-controlled and ECOG≤2 patients at 6 weeks after an initial CT treatment. The aim of the present study was to identify predictors of disease control at 6 weeks (DC6wkx) in MSEC patients receiving platinum-based CTs as first-line treatment for metastatic disease. Methods: Among 68/70 evaluable patients included between 1/2011 and 7/2013 who received at least 1 CT cycle, 58 were evaluable for disease assessment at 6 weeks. Ten patients were not evaluable (early death: 4, patient’s decision: 2, concomitant disease 1, early progressive disease 1, other reasons: 2). Baseline demographic, clinical, biological, and tumor characteristics were tested for prediction of DC6wkx. Significant variables for DC6wkx were identified with the chi-squared test and logistic regression. Results: Baseline patients characteristics were as follows: median age: 61.5yo; male: 57/68; ECOG 0/1/2: 13/42/13; metachronous/synchronous MSEC: 38/30; number of metastatic sites 1/2/>2: 35/20/13; metastatic location: lung 36, liver 23, bone 11, nodes 37, other 11; prior exposure to CT: 37/68; time from previous CT exposure: ≤ 6m 6/37, 6-12m 14/37, > 12m 17/37; gr>2 dysphagia (Atkinson) 19/67; BMI<18.5kg/m²: 13/68. Current CTs were FU-CDDPq3w 2/68, LV5FU2-CDDPq2w 15/68, FOLFOX 51/68, and patients received the following number of cycles 1/2/>2: 5/7/54. DC6wkx rate was 65.7%, with 16/68 PR (23.5%) and 28/68 SD (42.2%). Albumin (p<0.01), BMI (p<0.02), bone metastases (p<0.005), gender (p<0.047) and ECOG status (p<0.05) were predictive of DC6wkx. Normal or overweight BMI, grade 0 albumin, and no bone metastases, were predictive of DC6wkx in multivariate analysis. Conclusions: DC at 6 wks was 65.7% in MSEC receiving platinum-based CTs as first line treatment for metastatic disease. Normal or overweight BMI, normal albumin, and the absence of bone metastasis were significant predictors of DC6wkx in this prospective phase 2 trial. Clinical trial information: NCT01248299.

Abstract John 2:17 quotes Ps 68:10: “Zeal for your house will consume me.” Interpreters disagree about whether consume portrays Jesus’s zeal overwhelming him during the temple incident or leading to his death. They also disagree about whether John alludes metaleptically to the whole psalm, especially the rebuilding of Jerusalem in Ps 68:36–37. This article argues that consume portrays Jesus’s death. It substantiates that John alludes to the whole psalm, not only the rebuilding of Jerusalem in 68:36–37, but also the table becoming a trap and the pouring out of wrath in 68:23, 25. These echoes suggest that Jesus embodies the judgment of God in the temple incident, the suffering of the psalmist in his death, and the restoration of Jerusalem in his resurrection. The story from the Psalter is thus reconfigured in the temple incident: God rebuilds the forsaken city by identifying with Israel’s exile in the crucified body of Jesus.

Abstract Four monoclonal antibodies, SBU.II 28-1, 37-68, 38-27, and 42-20, each recognizing a distinct, non-overlapping subset of sheep class II molecules, were used to purify class II molecules from a single sheep. Four class II alpha subunits designated 28-1 alpha, 37-68 alpha, 42-20 alpha, and 38-27 alpha and five class II beta subunits designated 28-1 beta, 37-68 beta 1, 37-68 beta 2, 42-20 beta, and 38-27 beta were compared by N-terminal sequence analyses. Two distinct alpha subunits were identified; the 28-1 alpha, 37-68 alpha, and 42-20 alpha subunits all had identical N-terminal amino acids sequences, which exhibited about 75% homology with HLA-DR alpha and mouse E alpha polypeptides. In contrast, the 38-27 alpha sequence exhibited about 80% sequence homology with HLA-DQ alpha and mouse A alpha polypeptides. In general, sheep beta subunits displayed insufficient sequence homology to enable correlation with human beta-chain sequences; however, the 38-27 beta-chain sequence showed homology with the HLA-DQ beta sequence. The conserved sequence surrounding the site for N-linked glycosylation within human/mouse beta polypeptides (residues 19 to 21) was not present in sheep beta sequences and in contrast with the beta-chains of mouse and man, sheep beta polypeptides contained between 1 and 3 positionally variable cysteine residues (residues 13 to 15 inclusive). Individual sheep beta subunits exhibited extensive sequence heterogeneity and each consisted of a unique population of beta polypeptide species. At least 16 different beta polypeptide sequences were identified from a single sheep and the existence of no fewer than nine non-allelic beta genes was inferred from the sequence data. We have previously provided evidence suggesting that the sheep has multiple major histocompatibility complex class II alpha and beta genes related to those of all three HLA-D subregions. The present results suggest that a number of these genes encode HLA-DQ-like heterodimers and that a sheep DR-like alpha gene product is shared with the products of a large and heterogeneous sheep beta gene family.

ABSTRACTOrientia tsutsugamushiis the etiological agent of scrub typhus, a mite-borne, febrile illness that occurs in the Asia-Pacific region. We conducted strain characterization ofO. tsutsugamushiisolates from chiggers obtained from rodents based the nucleotide sequence of the 56-kDa outer membrane protein gene. With the use of PCR, a total of 68 DNA sequences of 56-kDa antigen genes were amplified. Phylogenetic analysis revealed that there were at least six definable clusters among the 68 isolates: 37% Karp-related strains (25/68), 27% TA763 strains (18/68), 12% JG-related strains (8/68), 19% Kato-related strains (13/68), 4% divergent strains (3/68), and 1% representing a Gilliam prototype strain (1/68). Overall, theO. tsutsugamushigenotypes exhibited a high degree of diversity, similar to that seen in strains from the rest of the areas where scrub typhus is endemic. Moreover, the 56-kDa protein sequence similarity betweenO. tsutsugamushiisolates from mites and those from human patients (H. Y. Lu et al., Am. J. Trop. Med. Hyg. 83:658-663, 2010) were striking, thus highlighting potential risk factors for this emerging zoonotic disease.

This retrospective study aimed to assess the prevalence of ultrasonographic detected adrenomegaly in clinically ill cats, evaluating the final established diagnosis, describe adrenal ultrasound findings and if the adrenomegaly was suspected or incidental. Abdominal ultrasonography reports of cats presenting to a veterinary teaching hospital between October 2018 and February 2021 were retrospectively reviewed. Cats showing adrenomegaly (one or both glands having a dorsoventral axis >4.8 mm) were selected and medical records respectively evaluated. Nine-hundred and eighty-three ultrasonographical reports were selected, of which, 68 (7%) disclosed adrenomegaly. European/Domestic Short-Hair (62/68; 91%) male (44/68; 65%) castrated (35/44; 80%) cats were overrepresented. Adrenomegaly was an incidental finding in 62/68 (91%) cats while in 6/68 (9%) it was identified in the context of investigating a potential adrenal disease. Concerning established diagnosis, chronic kidney disease was overrepresented (25/68; 37%), followed by endocrinopathies (20/68; 29%). Adrenomegaly was bilateral in 53% (36/68) of cases. In unilateral cases (32/68; 47%), it was more prevalent on the left side (23/32; 72%), with a normal-sized contralateral adrenal gland. Left adrenal demonstrated a larger size and a tendency to oval shape. This study assesses the prevalence of adrenomegaly in clinically ill cats, reinforcing it can be an incidental ultrasound finding.

Abstract:
379 Background: Somatostatin analogue functional imaging with Ga-68 Dotatate PET/CT has demonstrated superiority in lesion detection in patients with NETs. The effect of this novel imaging modality on US clinical practice and its usefulness in different types of NETs is not well described. We describe the impact of initial NETSPOT imaging on diagnosis and management in NET patients at a large urban medical center. Methods: Consecutive patients diagnosed with NETs and referred to our institution who received an initial Ga-68 Dotatate PET/CT between 07/2017-09/2018 were included. Imaging was reviewed and compared to prior available CT, MRI, and/or In-111 Pentetreotide scans. Results: Among 101 patients, 51/50 were female/male, tumor origins were gastroenteropancreatic (GEP) (75%), Unknown Primary (UP) (13%), lung (8%), thymic (2%), and other (2%). All tumors were histologically well/moderately differentiated and 37/51/3/10 were G1/G2/G3/Unknown, respectively. Initial imaging with Ga-68 Dotatate PET/CT revealed additional metastatic disease in 37 of 77(48%) patients with prior evidence of metastatic disease. Most common sites were distant lymph nodes (18), bone (15) and liver (9), peritoneal/pleural (4). A previously UP tumor was identified in 3 patients. No patients with metastatic lung carcinoids (6 atypical, 2 typical) or thymic NETs (2 atypical/G2) NETs had evidence of Ga-68 Dota PET/CT uptake above reference liver SUV levels. Results of imaging altered patient management as follows: 14 initiated systemic therapy due to documentation of progression, in 6 surgical therapy was altered, in 4 biopsy/other management was changed. In 11 patients with no tumor Ga-68 Dotatate uptake, decisions about use of PRRT and somatostatin analogues was altered. Conclusions: In this series, Ga-68 Dotatate PET/CT altered diagnosis and management in 35/101 NET patients. Among GEP and UP NETs, Dotatate imaging diagnosed primarily new nodal, bone, liver and pleural/peritoneal metastases missed by other imaging modalities. Results support the routine use of Ga-68 Dotatate PET/CT in the care of patients with advanced and early stage NETS.

Objetivo: determinar el comportamiento epidemiológico de las enfermedades más frecuentes, transmitidas por vectores en niños guatemaltecos entre los años 2015-2019. Materiales y métodos: estudio descriptivo, de corte transversal, retrospectivo, con datos del Sistema de Información Gerencial de Salud (SIGSA), del Ministerio de Salud Pública y Asistencia Social de Guatemala, (MSPAS). Se determinaron la distribución geográfica por departamentos, los rangos de edad con mayor número de casos y la enfermedad transmitida por vectores con mayor frecuencia en el país. Resultados: del total de casos reportados de enfermedades transmitidas por vectores, los niños de 5 a 9 años de edad representaron el 37% de las personas afectadas. El dengue fue la enfermedad más frecuente, 68% del total. Conclusiones: la mayoría de los casos de enfermedades transmitidas por vectores reportados del 2015-2019, corresponden a Dengue con un 68%. El rango de edad más afectado fue de 5 a 9 años, con un 37% de los casos reportados. En el departamento de Guatemala ocurrió la mayor proporción en tres enfermedades.

Depression is frequently found in patients with panic disorder. It is known that patients suffering from panic disorder and depression have more severe symptoms of anxiety and avoidance and respond less well to treatment with antidepressants. The General Behaviour Inventory, an instrument that identifies subjects with subsyndromal and syndromal recurrent mood disorders was completed by 108 patients in a panic disorder program. Thirty-four percent (n = 37) were classified as suffering from a mood disorder. Of the mood disordered patients, 25 of 37(68%) (23% of the total) were classified as suffering from bipolar or cyclothymic mood disorder.

Development and characterization of thermosensitive pluronic-based metronidazolein situgelling formulations for vaginal applicationThe purpose of this study was to develop pluronic-basedin situgelling formulations of metronidazole (MTZ) for treatment of bacterial vaginosis, aimed at prolonging the residence time, controlling drug release, enhancing efficacy, decreasing recurrence, and increasing patient compliance. Thein situgel formulations were prepared using different concentrations of pluronic F-127 (PF-127) alone and in combination with pluronic F-68 (PF-68). The prepared formulations were evaluated for their gelation temperature (Tgel),in vitrodrug release, rheological properties, mucoadhesion properties and tolerability by vaginal mucosa in tissue levels. TheTgeldecreased with increasing PF-127 concentration. TheTgelwas modulated by addition of PF-68 to be within the acceptable range of 25-37 °C. With increasing pluronic concentration, thein vitrodrug release decreased, viscosity and mucoadhesive force increased. Histopathological examination of rabbit vaginas from the control and treated groups revealed normal histology of the vagina and cervix. Based on thein vitroevaluation of prepared formulations, thein situgelling liquid formulated with PF-127/PF-68 (20/10 %,m/m) was selected for further clinical evaluation.

This study was conducted on patients with a history of congenital bleeding disorders or with suspected bleeding tendencies. Laboratory analysis revealed Von Willebrand disease (VWD) in 68 (21.3%) of 318 participants with male to female ratio of 0.8: 1 (31 to 37) and median age 17 years (range 2-45 years). Type 3 being the most frequent, 35 (51.4%) of 68, type 2, 20 (29.4%) of 68, and lastly type 1, 13 (19.1%) of 68. A total of 55.8% patients with VWD presented with mucocutaneous bleeding. Menorrhagia was the most common presentation of female patients. Von Willebrand disease (21.3%) was the second common bleeding disorder and the most common coagulation defect among females with menorrhagia. However, the frequency in the study was quite low when compared to the western world. Similarly, low frequency of VWD type 1 might be due to the fact that only symptomatic patients visited us. Further studies are needed as there is limited information on VWD in the developing countries. This will help in the development of expertise for the accurate diagnosis & proper management.

AbstractA field emission scanning electron microscopy analysis was carried out to determine the origin of the oriental printing inks used inThe Annals of the Joseon Dynasty, which are a national treasure of the Republic of Korea and listed in UNESCO’s Memory of the World Register. The average diameters of the carbon black found in the printing inks of all the samples were in the range of 30–500 nm. The carbon black diameters of the printing inks in J-Se 68, J-Se 124, and T-Gwang 37 were 80–130 nm, whereas those of the printing inks in J-Seong 1, T-In 46, and T-Heon 4 were larger than 150 nm. It could be concluded that the printing inks in J-Se 68, J-Se 124, and T-Gwang 37 are lampblack ink, and those in J-Seong 1, T-In 46, and T-Heon 4 are charcoal ink. Furthermore, the results of infrared (IR) and principal component analysis (PCA) of printing inks suggest that there was no difference in the binding medium used in the manufacturing process of the charcoal ink stick and the lampblack ink stick.

We developed a quantitative workflow for serial PSMA PET/CT to optimise predictive and prognostic imaging biomarker capability for progression free (PFS) and overall survival (OS). Methods: 56 men with mCRPC previously treated with taxane chemotherapy and androgen signaling inhibitor were enrolled, receiving up to 6 doses of LuPSMA and a radiation sensitizer idronoxil (NOX66). 68Ga-PSMA-11 PET/CT was performed at study entry and exit. Traffic Light (TL) quantification workflow was developed to track changes in both tumour volume and intensity at a total body and lesional level. Lesions were classified as responding in green (>30% decline in volume), stable in yellow (<30% change in volume/intensity), progressive in red (>30% increase in volume/intensity, or new). Overall response pattern was categorised as responding ( green/yellow), low volume red (<50% progressive disease) or high volume red (>50% progressive disease). TL workflow results were correlated with PFS and OS. Results: 37/56 men underwent both entry and exit imaging. The median PSA decline was 77% (IQR 34-92%), and 70% (26/37) achieved PSA response >50%. PSA progression occurred in 54% (20/37) at exit imaging. Median PFS was 8.6 months (95%CI 5.6-11.6) and median OS 22 months (95% CI 18.6-25.6). 95% (35/37) had reduction in PSMA SUVmax (-26.1 (IQR +11.7 to -89.4)) and SUVmean (-3.3 (+2.9 to -14.2)). PSMA total tumor volume reduced in 68% (25/37) (median -0.64 liters (range +1.44 to -1.1)). On TL workflow, 24% (9/37) had responding/stable disease ( green/ yellow), 76% (28/37) had progressive disease ( red) of whom 41% (15/37) had low volume progression and 35% (13/37) high volume progression. Men with high volume progression had worse OS compared to responders (HR 0.18 (0.05-0.59), p 0.005), and low volume progression (HR 0.30 (0.11-0.80), p 0.02). 68% (19/28) had progression on both TL workflow and PSA, while 32% (9/28) had progression on TL workflow without PSA progression. In multivariable analysis, TL workflow and PSA progression at time of exit scans were independent predictors of OS (Table). Conclusions: This study demonstrates the feasibility of characterizing lesional response on molecular imaging with a quantification TL workflow. Submaximal concentrations of inositol 1,4,5-trisphosphate (InsP3) rapidly release only a fraction of the InsP3-sensitive intracellular Ca2+ stores, despite the ability of further increases in InsP3 concentration to evoke further Ca2+ release. The mechanisms underlying such quantal Ca2+ mobilization are not understood, but have been proposed to involve regulatory effects of cytosolic Ca2+ on InsP3 receptors. By examining complete concentration-effect relationships for InsP3-stimulated 45Ca2+ efflux from the intracellular stores of permeabilized hepatocytes, we demonstrate that, at 37 degrees C, responses to InsP3 are quantal in Ca(2+)-free media heavily buffered with either EGTA or BAPTA [1,2-bis-(2-aminophenoxy)ethane-N,N,N′,N′-tetra-acetic acid]. Lower concentrations of InsP3 were used to examine the kinetics of Ca2+ mobilization at 2 degrees C, because at the lower temperature the stores were more sensitive to InsP3: the concentration of InsP3 causing half-maximal Ca2+ release (EC50) after a 30 s incubation decreased from 281 +/- 37 nM at 37 degrees C to 68 +/- 3 nM at 2 degrees C. At 2 degrees C, the EC50 for InsP3-stimulated Ca2+ mobilization decreased as the duration of exposure to InsP3 was increased: the EC50 was 68 +/- 3 nM after 30 s, and 29 +/- 2 nM after 420 s. InsP3-stimulated Ca2+ mobilization is therefore non-quantal at 2 degrees C: InsP3 concentration determines the rate, but not the extent, of Ca2+ release. By initiating quantal responses to InsP3 at 37 degrees C and then simultaneously diluting and chilling cells to 2 degrees C, we demonstrated that the changes that underlie quantal responses do not rapidly reverse at 2 degrees C. At both 37 degrees C and 2 degrees C, modest increases in cytosolic Ca2+ increased the sensitivity of the stores to InsP3, whereas further increases were inhibitory; both Ca2+ effects persisted after prior removal of ATP. We conclude that the effects of Ca2+ on InsP3 receptors are unlikely either to be enzyme-mediated or to underlie the quantal pattern of Ca2+ release evoked by InsP3.

583 Background: Lapatinib is an orally bioavailable reversible, dual tyrosine kinase ErbB1 and ErbB2 inhibitor. Trastuzumab, an ErbB2 inhibitor, has been associated with cardiotoxicity. Methods: Left ventricular ejection fraction (LVEF) was monitored every 8 weeks using MUGA or echocardiogram. Cardiac risk factors including age, underlying cardiovascular disease, previous exposure to AC, trastuzumab, or mediastinal/left sided radiation (XRT) were also collected. LVEF data were evaluated for ≥ NCI CTC grade 3, or ≥ 20% decline relative to baseline (& below institutional LLN). Results: 1.3%, 37 of the 2812 subjects who have received lapatinib in all trials to date have experienced decreased LVEF. 22 of 37 patients received lapatinib monotherapy, the remaining 15 received lapatinib in combination with other chemotherapeutic agents. 68% (25/37) of patients were female, median age was 59 years. Decreased LVEF occurred within 9 weeks of treatment onset in 68% of cases. The event resolved/improved in 57% of cases: in 50% of these, lapatinib treatment was continued. Average event duration was 5 weeks. Four of 37 patients were symptomatic (0.1% incidence). Most symptomatic events responded promptly to standard CHF therapy. The majority of patients (92%, 34/37) had confounding factors that may have contributed to the event, including previous exposure to AC, trastuzumab or XRT, and/or medical history. 8 of 34 patients had a positive lapatinib dechallenge (LVEF recovered): 3 were symptomatic (signs of CHF) and 5 asymptomatic. 3 patients had no contributing factors: 1 experienced symptomatic decreased LVEF, the remaining 2 died due to disease progression whilst LVEF decrease was ongoing. 16 of 1307 patients participating in placebo controlled studies experienced decreased LVEF. Of these 8 are known to have received lapatinib, giving an incidence of 1.2% (8/654). Conclusions: Lapatinib-associated LVEF decrease is rarely symptomatic and generally reversible/non-progressive. Incidence was 1.3% in our population, less than the 3–6% incidence of asymptomatic LVEF decreases observed in the general population. Pharmacogenetic studies are ongoing to determine whether there is a link between ErbB2 polymorphisms and lapatinib cardiotoxicity. [Table: see text]

238 Background: Gallium 68 dotatate (Ga-68 DOTA) provides physiologic imaging and assists in the localization of disease in somatostatin (SSA) receptor positive neuroendocrine tumor (NET) patients. Questions regarding the value of Ga-68 DOTA imaging in the FDA post-approval era for NET patients include: usefulness in disease monitoring, correlation of Ga-68 DOTA avidity to Ki-67 index and its impact on clinical decision making. We present our experience with 200 Ga-DOTATATE scans performed at University of Kentucky. Methods: A retrospective review of the first 200 patients who had undergone Ga-68 DOTA imaging at the Markey Cancer Center from Dec 2016 to Dec 2017 was conducted. Results: 59.5% were females and the median age was 62 (30-84 years). Primary tumor sites included: small bowel 37.5%, pancreas 18.5%, bronchial 14%, colon 3.5%, rectum 2%, appendix 1.5%, adrenal 0.5%, prostate 0.5%, others 3% and unknown primary 19%. Ga-68 DOTA scan influenced clinical decisions in 39% (n = 78) patients. Ga-68 DOTA imaging identified primary tumors in 17 of 38 patients who were classified as NET of unknown primary based on CT imaging. Subgroup analysis of mean standardized uptake value (SUV) for hepatic metastatic lesions revealed 37.3 for G1 (n = 20) as compared to 32.3 for G2 (n = 37) and 17.46 for G3 (n = 4). Mean hepatic SUV of the lesion with the greatest radiolabel uptake in 96 patients was similar irrespective of exposure to SSA LAR; 31.3 versus 27.8 for SSA versus the no SSA cohorts. Conclusions: Ga-68 DOTA imaging impacted clinical decision making in 39% of NET patients (n = 200) and identified the primary site in 17 of 38 patients with unknown primary. Systemic exposure to long acting SSA does not seem to impact quality of Ga-68 DOTA scan.

Between 1 March 1985 and 28 February 1987, 1659 patients attending Glasgow Genito-Urinary Medicine (GUM) Clinics were tested for antibodies to human immunodeficiency virus (HIV). Forty (2.4%) were positive. Thirty-seven of these were homosexual (36/37) or bisexual (1/37) males. The overall prevalence of antibodies to HIV in this group was 3.9% (37/940) and showed no significant increase over the two year period. Following the extensive media campaign in October 1986, the total numbers tested each month rose from an average of around 50, to 144 in December 1986. There was a further rise to 220 in March 1987 coinciding with AIDS week in March 1987. Since then the numbers have again declined. Before the October campaign, 70% of these tested (692/990) were homo/bisexual males. After the campaign the number of heterosexual males and females rose sharply, accounting for 68% of these tested.

It will focus on the way that Darwinian evolutionary thought was translated into 19th Century German geography, particularly in the work of Friedrich Ratzel and his formulation of the concept of Lebensraum. The paper argues that the Ratzelian Lebensraum must be viewed as a concept aimed towards a synthesis between biogeography and anthropogeography. The paper will also trace how the Ratzelian Lebensraum came to play a vital part in Rudolf Kjellén's later formulation of an organic theory of the state. Here the paper will focus specifically on Kjellén's organic taxonomy of the political system. The paper will end with a short discussion on the synthetic aspects of the Lebensraum concept, focusing on the seemingly divergent ways that the Lebensraum concept was mobilised within National socialist ideology and planning.

Forty-four varicella-zoster virus (VZV) isolates from China were genotyped by using a scattered single nucleotide polymorphism (SNP) method, including open reading frames (ORFs) 1, 22, 31, 37, 60, 62, 67, and 68. Based on the analysis of the polymorphic markers in the 8 ORFs, all of the 44 isolates can be placed in genotype J defined by the SNP profiles in ORF22 or clade B defined by the SNP profiles in ORFs 31, 37, 60, 62, 67, and 68. The three consecutive nucleotide (CGG) in-frame insertions in ORF 1 were found in 8 (18.2%) isolates, which has not been described in VZV strains from any other part of the world. A novel synonymous A>G substitution in ORF60 was revealed in 4 (9.1%) of the isolates. In addition, a previously described three consecutive nucleotide (ATC) insertion in ORF 60 was found in all the Chinese isolates but not in the US isolate MLS. The results showed all the 44 strains that belong to genotype J/clade B with significantly high homogeneity, and phylogenetic analysis suggested that the 44 Chinese isolates consist of 4 clusters, but interstrain variations also exist. Overall, VZV isolates obtained in China showed significantly higher genetic homogeneity than isolates reported from other countries.

This study assessed the associations of LL-37 and anti-CCP, vitamin D metabolites and vitamin D receptor (VDR) polymorphisms in early inflammatory arthritis (EIA).MethodsSerum LL-37, 25-hydroxy-vitamin D (25OHvitD) and anti-CCP were measured by ELISA in treatment naïve EIA (n = 181). VDR single nucleotide polymorphisms (Fok1, Bsm1, Apa1, Taq1, Cdx-2) and HLADRB1 shared epitope (SE) alleles were detected by DNA amplification. Associations were tested in multivariable models. Median (25%, 75%) or percentiles are reported.ResultsParticipants (70 % female, age 56 [45, 66] years, disease activity score [DAS28ESR3var] 3.7 [2.8, 4.8], 41 % anti-CCP positive, 68 % RA) had low serum 25OHvitD; 20.5 nmol/L (13.9, 29.0). In multivariable models, controlling for age, sex, SE, smoking and vitamin D deficiency, LL37 level (top quartile) associated with anti-CCP seropositivity (OR 22; 95% CI 4 to 104).ConclusionsLevels of circulating LL-37 are associated with anti-CCP seropositivity. LL37 activity may be one mechanism linking infection and toxin exposure to anti-CCP generation.

213 Background: For planning salvage procedures in recurrent prostate cancer accurate diagnostic tools are needed. Till now in patients suspicious for isolated local recurrent cancer invasive biopsy was needed. 68-Ga-PSMA-PET is frequently used to identify even small metastatic deposits. We analyzed the diagnostic value to identify local recurrence in patients after radiotherapy undergoing salvage radical prostatectomy, sRPE. Methods: We retrospectively reviewed patients undergoing sRPE with 68-Ga-PSMA-PET as preoperative staging. All patients underwent sRPE and extended lymph node dissection. Sensitivity, Specificity, pos and neg predictive value had been calculated per patient and per prostate lobe. Results: 37 patients with preoperative 68-Ga-PSMA-PEt could be identified. In all but one at least in one lobe PSMA staining could be identified. Mean tumor volume per prostate lobe was 6.22 (0.1-24)ml. Sensitivity, Specificity, neg and pos predictive value per lobe is 80%, 75%, 32% and 96%. Sensitivity to identify cancer recurrence per person irrespective of the side is 95% with a pos predictive value of 100%. Conclusions: 68-Ga-PSMA-PET seems to have a very high diagnistic value to identify local recurrence after radiotherapy. After validation of our experience an invasive biopsy to proof cancer recurrence might be spared.

Over half, (54%)had emergency surgeries & 37% experienced infections, re-laparotomy and ileus (20% each). Conclusion There is an opportunity to risk-stratify patients at diagnosis based on the risk factors (men, younger age, severe/extensive disease) to a top-down therapy & treat to target strategy to reduce colectomy rates.

Methods: Between 2012 and 2015, 182 evaluable patients were diagnosed with unilateral WT. Patients were staged with abdominal ultrasound/computed tomography and thorax radiograph/CT. Therapy was adapted from National Wilms Tumor Study-5 except for patients with large abdominal masses and/or severe malnutrition who received preoperative doxorubicin, vincristine, actinomycin-D for 4 to 6 weeks. Treatment failure was defined as abandonment of therapy, recurrent/progressive disease (PD), and death: early (< 2 weeks from diagnosis) or toxic (> 2 weeks after diagnosis). Results: 49% were male; 23% were <2 years (median age 3.5y). Stage distribution: I, 8; II, 21; III, 116; IV, 37 cases. For 123/181 (68%) preoperative therapy was warranted (volume 579cm3). Treatment failures were: recurrence/PD 28; abandonment 19; early death 3, and toxic death 2. 2/29 (7%) patients with stage I/II suffered recurrence/PD, 13/116 (11%) stage III, and 13/37 (35%) stage IV. Most failures were metastatic (68%). Abandonment-sensitive 3-years EFS was 68%±0.04% (SE) for the all stages, 77%±0.043% abandonment-censored, and abandonment-sensitive OS was 71%±0.05%. Conclusion: Multicenter protocol-driven therapy tailored to fit regional needs is feasible in patients with WT. Abandonment remains a challenge as do early and toxic deaths due to advanced disease presentations in fragile children. Compared to previous eras 2000-2004 (3-year EFS 50%±4.3% and 2005-2011 (3-year EFS 59%±3.1%) survivals continue to improve. This improvement reflects the engagement of the whole multidisciplinary team including surgery, radiation oncology, and pathology. Participaram da pesquisa 68 sujeitos, com idades entre 17 e 44 anos, sendo 37 homens e 31 mulheres. Foi construída uma escala de atitudes baseada no modelo Likert, contendo 37 afirmativas referentes a sentimentos negativos e positivos, e estabeleceu-se uma equivalência entre as expressões da escala e a atribuição de valor. Houve diferença na média das pontuações dos sentimentos negativos: no tempo de duração da relação, no tempo de término do relacionamento e na relação afetiva após término da relação. De um modo geral, a separação provoca abalo emotivo tanto em homens como em mulheres, nas diferentes situações.

Se realizó un inventario florístico detallado de un bosque andino secundario del noroeste de Colombia, con el fin de documentar la riqueza de especies vasculares. El inventario florístico muestra la existencia de 784 especies de plantas vasculares pertenecientes a 461 géneros y 133 familias. Las familias más diversas fueron las Poaceae (68 especies), Fabaceae (49), Melastomataceae (37), Rubiaceae (37), Orchidaceae (31), Asteraceae (27), Malvaceae( 26), Piperaceae (24), Araceae (21), Moraceae (17) y Polypodiaceae (16). Los géneros más ricos fueron Piper (17 especies), Inga (13), Ficus (11), Paspalum (11), Psychotria (11), Miconia (10), Anthurium (9), Clusia (7), Mikania (7), Peperomia (7), y Solanum (7). Algunas de las especies muestreadas constituyeron nuevos registros para la flora del noroeste de Suramérica. El bosque analizado es florísticamente similar a los bosques neotropicales de tierras bajas.

Background: In coronavirus disease 2019 (COVID-19) patients, increases in high-sensitive cardiac troponin T (hs-cTnT) have been reported to be associated with worse outcomes. In the critically ill, the prognostic value of hs-cTnT, however, remains to be assessed given that most previous studies have involved a case mix of non- and severely ill COVID-19 patients. Methods: We conducted, from March to May 2020, in three French intensive care units (ICUs), a multicenter retrospective cohort study to assess in-hospital mortality predictability of hs-cTnT levels in COVID-19 patients. Results: 111 laboratory-confirmed COVID-19 patients (68% of male, median age 67 (58–75) years old) were included. At ICU admission, the median Charlson Index, Simplified Acute Physiology Score II, and PaO2/FiO2 were at 3 (2–5), 37 (27–48), and 140 (98–154), respectively, and the median hs-cTnT serum levels were at 16.0 (10.1–31.9) ng/L. Seventy-five patients (68%) were mechanically ventilated, 41 (37%) were treated with norepinephrine, and 17 (15%) underwent renal replacement therapy. In-hospital mortality was 29% (32/111) and was independently associated with lower PaO2/FiO2 and higher hs-cTnT serum levels. Conclusions: At ICU admission, besides PaO2/FiO2, hs-cTnT levels may allow early risk stratification and triage in critically ill COVID-19 patients.

85 Background: The purpose of this study was to analyze skeletal events unrelated to bone metastasis in men with castration-refractory prostate cancer (CRPC). Methods: A total of 68 patients with CRPC treated by secondary hormonal therapy (dexamethasone) and/or chemotherapy including docetaxel were analyzed. Primary endpoint was skeletal related events (SREs) after PSA relapse. PSA relapse was defined as 3 consecutive PSA increase and serum PSA more than 4 ng/ml. Zoledronic acid was given for 31 patients with radiographic bone progression from the initiation of docetaxel-based treatment. For 37 patients without radiographic bone progression, no bone targeted agents or radioisotopes were administered during the observation period. Results: During median observation period of 17.5 months (range 1 to 76 months), SREs were observed in 17 of 68 patients (25%). In patients with bone progression, SREs was observed in 8 of 37 (22%) patients, whereas 9 of 31 (29%) patients without radiographic bone progression experienced SREs unrelated to bone metastasis. All patients without radiographic progression had been treated by oral steroids (dexamethasone), which might be related to osteoporotic change in these cases. Conclusions: For CRPC patients without radiographic bone progression, caution needs to be paid for SRE unrelated to bone metastasis. The administration of steroids may cause osteoporotic change, thus preventive administration of anti-osteoporotic agents would be mandatory.

Abstract One Indonesian Air Pollution Standard Index (ISPU) parameter is nitrogen dioxide (NO2), with a minimum concentration of 80 μg/Nm3 in ISPU 0-50. This research aim was to evaluate NO2 concentrations less than 80 μg/Nm3 in ambient air and determine the ISPU number based on NO2 concentration. This investigation was conducted from February until April 2021, utilising secondary information compilation and field estimations. The method of the NO2 concentration test was based on the national standard SNI 7119-2:2017. The run of NO2 concentrations obtained from checking the information in Jakarta and Bogor was 6-51 μg/Nm3, and from inspecting came about 17-40 μg/Nm3. NO2 concentrations from monitoring data and sampling were deficiently compared to the NO2 minimum concentration limit in ISPU. ISPU number for maximum NO2 concentration in Jakarta and Bogor was 32, relatively low than the ISPU NO2 minimum standard of 0-50. The relevant NO2 concentration limits as an initial indicator of ISPU 0-50 were 0-37 μg/Nm3, and for ISPU 51-100 were 38-68 μg/Nm3 based on the NO2 toxicity study. Based on the research findings, it can be concluded that the relevant concentration of NO2 for ISPU 0-50 is 0-37 μg/Nm3, whereas for ISPU 51-100 is 38-68 μg/Nm3.

Incubation at 37 degrees C or treatment of granule membranes of chromaffin cells with Staphylococcus aureus phosphatidylinositol-specific phospholipase C converted from an amphiphilic to a hydrophilic form two proteins with molecular masses of 82 and 68 kDa respectively. Their release is time- and enzyme-concentration-dependent. We showed that they were immunoreactive with an anti-(cross-reacting determinant) antibody known to be revealed only after removal of a diacylglycerol anchor. Furthermore, the action of HNO2 suggests the presence of a non-acetylated glucosamine residue in the determinant. This is one of the first reports suggesting that a glycosylphosphatidylinositol anchor might exist in membranes other than the plasma membrane. We showed that the 68 kDa protein is probably not the subunit of dopamine (3,4-dihydroxyphenethylamine) beta-hydroxylase, an enzyme present in granules in both soluble and membrane-associated forms.

Laboratory characteristics of seeds of 37 species (41 seed types) from the Strandveld Succulent Karoo were used to predict seed bank types according to a modified key of ). Five seed bank strategies were recognized for this vegetation type, i.e. two with transient and three with persistent seed bank strategies. Of the 37 species investigated, 32% (all perennial species) had transient seed bank strategies, while 68% had persistent seed bank strategies. Seed dispersal of these 37 species was mainly anemochorous, although antitelechoric elements such as myxospermy, hygrochasy, heterodiaspory and synaptospermy were found among these species. The seed bank alone will not be sufficient to restore the vegetation of damaged land in the Strandveld Succulent Karoo, since many of the dominant species in the vegetation do not produce persistent seed banks. Many of these species may, however, be dispersed by wind into revegetation areas from surrounding vegetation. Topsoil replacement, seeding and transplanting of selected species will be essential for the successful revegetation of mined areas in this part of Namaqualand.

The Liverpool integrated care pathway for the dying patient (LCP) facilitates management of dying patients, but does not provide guidance regarding medication. A retrospective audit was performed of patients using the LCP to assess what medications were required, how requirements changed and which of the four symptoms outlined in the LCP were most troublesome (pain, agitation, sickness and respiratory secretions). Over a five-month period, 68 patients died on the LCP, and were included in the audit. The most unstable symptom (stability defined by rescue medication requirements) at the end of life was agitation − 37 of the 68 patients required regular sedatives and 45 patients needed at least one rescue dose of sedative. Sickness was the most stable symptom − 39 patients needed regular antiemetic, but only three patients needed rescue doses. These findings have implications in writing guidelines for symptom management at the end of life.

Esta pesquisa tem como objetivo caracterizar os profissionais que trabalham no contexto das escolas pública e particular, bem como, identificar os agressores e os tipos de eventos violentos vivenciados por esses profissionais. Trata-se de um estudo descritivo, exploratório, comparativo, com abordagem quantitativa, realizado em duas escolas situadas em Natal/RN. A amostra constitui-se de 121 profissionais, sendo 62 da instituição pública e 59 da particular. O projeto foi submetido ao Comitê de Ética em Pesquisa da UFRN e aprovado sob Parecer de nº 149/10. Constatou-se que havia 68 (56,20%) mulheres, 68 (56,20%) professoras; 51 (42,15%) funcionários e 02 (01,65%) diretores, 37 (30,58%) já foram vítimas de bullying, 36 (97,30%) foram agredidos por alunos. Concluiu-se que a maioria dos profissionais eram mulheres, professoras e já sofreram violência na escola. Assim, torna-se fundamental criar estratégias para auxiliar estes funcionários a prevenir e combater o bullying na escola.

The cases of infiltrating bladder cancer seen in the Urology Unit of the Hospital of Noale from 1988 to today are presented. Of the 120 patients observed, 53 underwent radical cystectomy, whilst 67 had conservative treatment. The latter treatment gave a survival rate of 37% and radical cystectomy, 68%. Survival was influenced by the stage of the tumour and the presence of lymph node metastasis. The complications observed after radical cystectomy are reported. Survival data may be compared to those found in international literature.

Osteoarthritis is a group of different diseases with similar manifestations, which are based on the defeat of all of the joint parts (primarily of cartilage and subchondral bone, synovium, ligaments capsule) and periarticular muscles. The purpose of the study is to characterize cartilage structure and periarticular changes of tissue by means of ultrasound and magnetic resonance imaging without X-ray verification of osteoarthritis. Thirty eight patients with X-ray negative stage of osteoarthritis were ex-amined by ultrasound and magnetic resonance imaging. Pathological changes were detected in majority of the patients. The edema of soft tissue, exudate, changes in ligaments, deformation of joint surface were revealed in 71-74%, 11-37%, 76-68% and 58-68% of patients respectively. In general, pathological changes tended to be revealed on magnetic resonance imaging more frequently comparatively to ultrasound examination. Both ultrasound examination and magnetic resonance imaging are comprehensive methods for early detection of knee osteoarthritis. The prognostic value of the changes is to be evaluated.

Abstract Objective To analyze the clinical features, therapeutic management and risk factors for mortality of patients with severe novel A (H1N1) influenza in Shanghai, China. Methods All patients were diagnosed by influenza A (H1N1) virus mRNA detection. Chest CT scan, routine blood, hepatic function, humoral and cellular immunity, sputum smears, and sputum cultures were performed. Logistic analysis was applied to identify risk factors for mortality. Results Total of 68 patients were enrolled in this study, the primary clinical symptoms including cough (66, 97.1%), expectoration (41, 60.3%), and polypnea (41, 60.3%). Altogether, 37 (54.4%) and 11 (16.2%) patients were infected with bacterial and fungal, respectively. CT scan demonstrated that 67 (98.6%) patients had pneumonia. Oxygen therapy, oseltamivir, antibiotic and antifungal drugs were performed in 68 (100%), 66 (97.1%), 39 (57.4%), and 11 (16.2%) patients, respectively. Finally, 4 of 68 patients died. Logistic analysis demonstrated that there was a significant correlation between the percentage of neutrophils and mortality before therapy and direct bilirubin content and mortality after therapy, respectively. Conclusions Patients with severe H1N1 influenza were susceptible to bacterial and/or fungal infection. The risk factors for mortality may be associated with pre-therapeutic neutrophil percentage and post-therapeutic direct bilirubin content.
