Full text
Abstract:
A survey was made of the Southern River Otter's (Lutra provocax Thomas) present distribution and its decline during the last century. North of 36S the otter's decline began after 1880. Between 39S and 43S the decline is patchy. South of 43S the decline began around 1930. The reasons for the decline are a combination of habitat destruction, disturbance and hunting. Between 39S and 4330'S otter populations were found in seven discrete regions. Re-establishment by the otter is unlikely owing to the separation of the basins and habitat destruction. Tourism and settlement in regions with permanent otter populations could have a significant impact if the density of vegetation cover on river and lake banks is not maintained. Conservation of river otters in Chile should involve three approaches; education, recovery and re-establishment.

Abstract:
Heat resistance experiments were carried out with Listeria monocytogenes which had been grown at three different temperatures (30, 39, and 43°C). Heated whole milk was inoculated with L. monocytogenes and then passed through a high-temperature short-time system at 72, 69, 66, and 63°C for a minimum holding time of 16.2 s. Heated cells were recovered both aerobically and anaerobically using four different methods: direct plating, most probable number, cold enrichment, and warm enrichment. Significant differences in recovery of L. monocytogenes were observed depending on the growth temperature. Cells grown at 43, 39, or 30°C, held 1 d at 4°C, and then heated at 69°C showed an overall decrease in numbers of approximately 2.1, 2.8, and 4.1 logs, respectively. Cells grown at 39°C and then held 3 d at 4°C appeared to be the most heat sensitive. Although cells grown at 43 and 39°C were capable of surviving at the minimum high-temperature short-time temperature (72°C), those grown at 30°C were not. In some instances, anaerobic incubation enhanced the recovery of L. monocytogenes, as compared to cells recovered aerobically, although these differences were not statistically significant. While L. monocytogenes can survive minimum pasteurization treatment (71.7°C/16 s) under certain conditions, common methods of handling, processing, and storing fluid milk will provide an adequate margin of safety.

Abstract:
Variants of the Cx43 gap junction protein have been detected on Western immunoblots by using an antipeptide antibody to the N-terminus of the protein. In heart ventricle, atrium, brain, retina, and uterus, different yet characteristic ratios of a broad 43-kDa band and a 39- to 40-kDa doublet were observed. These proteins (in lens epithelium, testes, and spleen) or their messages (in stomach, duodenum, kidney, and lung) were also detected in several nonexcitable systems but at consistently lower levels than found in electrically excitable tissues. The reproducible heterogeneity in electrophoretic mobility of Cx43 seen in different tissues does not appear to be due to proteolysis, since both the 43-kDa band and the 39- to 40-kDa doublet were recognized by an N-terminal as well as a C-terminal antibody. Furthermore, Northern (RNA) blots from different tissues show that both polypeptide profiles arise from indistinguishable transcripts. The conversion by alkaline phosphatase treatment of a predominantly 43-kDa profile (in heart) to a 39- to 40-kDa profile (characteristic of brain and protein translated in vitro from the RNA) suggests that the observed electrophoretic heterogeneity arises from tissue-wide differences in the phosphorylation state of Cx43.

Abstract:
One of the causes of maternal mortality and morbidity is pregnancy induced hypertension (PIH). One of many hypertension management in pregnancy with non-pharmacological is hydrotherapy, which is soaking the feet using warm water for 20 minutes. This study aims to determine the differences in systolic and diastolic blood pressure in the hydrotherapy group with a temperature of 40o - 43 o compared to temperatures of 37 o - 39 o. The study using Quasi-Experimental non-randomized pre-test and post-test with control design on 22 hypertensive pregnant women using purposive sampling technique. Research instruments included observation sheets, sphygmomanometer, stethoscope, and digital water thermometer. Univariate and bivariate data analysis using the Wilcoxon and Mann-Whitney tests. This study showed difference of systolic blood pressure p value 0,000 and difference of diastolic p value 0,013 (p <0,05), therefore there were differences in systolic and diastolic blood pressure in the hydrotherapy group with a temperature of 40 o - 43 o compared to 37 o - 39 o. Hydrotherapy using temperature of 40 o - 43 o is better to use than temperature of 37 o - 39 o. Midwives can use hydrotherapy as an early treatment alternative for hypertensive pregnant women

Abstract:
Abstract One-hundred thirty-nine patients with acute lymphoblastic leukemia (ALL) and hypodiploidy (fewer than 45 chromosomes) were collected from 10 different national ALL study groups and single institutions. Patients were stratified by modal chromosome number into 4 groups: 24 to 29 (N = 46); 33 to 39 (N = 26); 40 to 43 (N = 13); and 44 (N = 54) chromosomes. Nine patients were Philadelphia chromosome (Ph) positive (4 cases: 44 chromosomes; 5 cases: 40-43 chromosomes) and were not considered further. Event-free survival (EFS) and overall survival (OS) of the remaining 130 patients were 38.5% ± 4.4% and 49.8% ± 4.2% at 8 years, respectively. There were no significant differences in outcome between patients with 24 to 29, 33 to 39, or 40 to 43 chromosomes. Compared with patients with fewer than 44 chromosomes, patients with 44 chromosomes had a significantly better EFS (P = .01; 8-year estimate, 52.2% vs 30.1%) and OS (P = .017; 69% vs 37.5%). For patients with 44 chromosomes, monosomy 7, the presence of a dicentric chromosome, or both predicted a worse EFS but similar OS. Doubling of the hypodiploid clone occurred in 32 patients (24-29 chromosomes [n = 25] and 33-39 chromosomes [n = 7]) and had no prognostic implication. Children and adolescents with ALL and hypodiploidy with fewer than 44 chromosomes have a poor outcome despite contemporary therapy.

Abstract:
The topic of the article is relevant because practical experience in pre-trial investigation of criminal offenses related to illicit drug trafficking that affect the morality and health of the population of Kazakhstan shows a serious problem related to the activities of state bodies in counteracting this phenomenon. This article deals with the problems of conducting the deposition of witness testimony on materials of pre-trial investigations in the sphere of illicit drug trafficking and precursors. Analyzed the practical activities of pre-trial investigation in the investigation of criminal offenses in this category. The authors propose a mechanism to simplify procedures for the recognition of narcotic drugs and psychotropic substances as such, through the introduction of an integrated registry of the National Assembly (narcotic drugs) and PV (psychotropic substances), with an active (daily) solved update, formed on the basis of expert opinions. Options have been proposed for eliminating gaps in the conduct of the deposit by the investigating judge, as well as in investigating crimes.

Abstract:
8543 Background: A multicenter phase II study, PIT-2 (Personized Induction Therapy-2), was performed to investigate the efficacy and safety of S-1 plus cisplatin and concurrent thoracic radiation therapy (TRT) followed by surgery in patients with stage IIIA (N2) lung squamous cell carcinoma (LSCC). To date, no clinical trials on the use of induction therapy prior to surgery have focused solely on the treatment of N2 LSCC. Methods: Eligible patients were 20 to 75 years old and had stage IIIA (pathologically proven N2) LSCC, performance status of 0-1, and no prior treatment. The patients received induction therapy consisting of three cycles of S-1 and cisplatin plus concurrent TRT (45 Gy in 25 fractions) followed by surgery. S-1 was administered orally at 40 mg/m2 twice per day, on days 1 through 14 along with an intravenous infusion of cisplatin (60 mg/m2) on day 1. The treatment cycles were repeated every four weeks. The primary endpoint was 2-year progression-free survival (PFS) rate and key secondary endpoints included overall survival (OS), the objective response rate (ORR) as defined by the Response Evaluation Criteria in Solid Tumors version 1.1, pathological complete remission (pCR) rate, feasibility, and toxicity. Results: Of the 45 patients registered between December 2013 and September 2018, 43 received induction therapy. Of the 43 patients, 39 (91%) underwent surgery (25 lobectomies, 10 bilobectomies, three pneumonectomies, and one wedge resection). The complete resection rate was 95% (37/39). Median follow-up time was 35.7 months. The 2-year PFS and OS rates were 67% (90% CI: 54-78%) and 70% (95% CI: 53-81%), respectively. The ORR and pCR rates were 86% (37/43, 95% CI: 76-96%) and 39% (15/39, 95% CI: 23-54%), respectively. Grade (G) 3 or 4 toxicities during the induction therapy in 43 patients included neutropenia (40%), anemia (9%), thrombocytopenia (7%), and hyponatremia (7%). Severe surgical complications in 39 patients included G3/4 pneumonia (5%), G3 bronchopleural fistula (5%), and G3 pleural effusion (5%). No G3/4 intraoperative adverse events occurred. There was no 30-day postoperative mortality and one 90-day postoperative mortality in a patient who underwent right pneumonectomy and developed pneumonia after discharge. Conclusions: Induction therapy using S-1 plus cisplatin and concurrent TRT followed by surgery is a promising treatment for patients with stage IIIA (N2) LSCC. Clinical trial information: 000012413.

Yeung, Anna, Ryan Lisk, Jayoti Rana, et al. "Community and Health Care Provider Preferences for Bacterial Sexually Transmitted Infection Testing Interventions for Gay, Bisexual, and Other Men Who Have Sex With Men: e-Delphi Study." Journal of Medical Internet Research 25 (June 29, 2023): e40477. http://dx.doi.org/10.2196/40477.

Full text
Abstract:
Background Canadian clinical guidelines recommend at least annual and up to quarterly bacterial sexually transmitted infection (STI) testing among sexually active gay, bisexual, and other men who have sex with men (GBM). However, testing rates are suboptimal. Innovative solutions are needed to close the gap because there is currently limited knowledge on how best to approach this issue. Objective Our aim was to build consensus regarding interventions with the greatest potential for improving local STI testing services for GBM communities in Toronto, Ontario, Canada, using a web-based e-Delphi process. Methods The e-Delphi method involves using a panel format to conduct successive rounds of prioritization, with feedback between rounds, to determine priorities among groups. We recruited experts separately from the community (GBM who sought or underwent STI testing in the preceding 18 months; conducted between October 2019 and November 2019) and health care providers (those who offered STI testing to GBM in the past 12 months; conducted between February 2020 and May 2020). The experts prioritized 6 to 8 potential interventions on a 7-point Likert scale ranging from definitely not a priority to definitely a priority over 3 survey rounds and ranked their top 3 interventions. Consensus was defined as ≥60% within a ±1 response point. Summaries of responses were provided in successive rounds. We reported the percentage of a priority (encompassing somewhat a priority, a priority, and definitely a priority responses) at the end of the final round of the survey. Results Of the community experts (CEs), 84% (43/51) completed all rounds; 19% (8/43) were living with HIV; 37% (16/43) were HIV negative and on pre-exposure prophylaxis; and 42% (18/43) were HIV negative and not on pre-exposure prophylaxis. We reached consensus on 6 interventions: client reminders (41/43, 95%), express testing (38/43, 88%), routine testing (36/43, 84%), an online booking app (36/43, 84%), online-based testing (33/43, 77%), and nurse-led testing (31/43, 72%). The CEs favored convenient interventions that also maintain a relationship with their provider. Of the provider experts (PEs), 77% (37/48) completed all rounds; 59% (22/37) were physicians. Consensus was reached on the same 6 interventions (range 25/37, 68%, to 39/39, 100%) but not for provider alerts (7/37, 19%) and provider audit and feedback (6/37, 16%). Express testing, online-based testing, and nurse-led testing were prioritized by >95% (>37/39) of the PEs by the end of round 2 because of streamlined processes and decreased need to see a provider. Conclusions Both panels were enthusiastic about innovations that make STI testing more efficient, with express testing rating highly in both the prioritizations and top 3 rankings. However, CEs preferred convenient interventions that involved their provider, whereas PEs favored interventions that prioritized patient independence and reduced patient-provider time.
