Abstract:
How can marginalised communities organise a project to yield significant social change? This thesis theorises the resources which enable such community organisation to work. Participation, empowerment and conscientisation are understood, not through a logic of quantity which creates linear dimensions, but through a logic of concrete qualities. A pragmatist approach is taken, to define our constructs in terms of the actions being undertaken by participants, within specific, qualitatively distinctive domains. Activity theory is used to theorise participation as a process of collective activity, which is supported by shared rules, a division of labour and shared goals, and which is challenged by divergences of interest. A community case study of the Sonagachi Project, a successful HIV prevention project run by sex workers in Kolkata (India), is used to investigate participation. The case study is based on interviews and group discussions with sex workers and Project workers (sex workers employed by the Project), and observation of the daily activities of the Project. Sex workers relate to the Project as a source of support in solving their individual problems, gaining new powers, but not acting as collectivity members. Project workers are constituted as collectivity members, whose action interlocks with that of their colleagues, through participating in the politicising discourse of the Project, which states that sex workers should be granted “workers’ rights”, and through learning the rules of participation in meetings and the hierarchical division of labour. To be allowed to operate, the Project has to carefully adjust to local power relations, with madams, political parties, and funding agencies, in collaborative-adversarial relationships. In conclusion, the scope of participation is defined as producing significant, yet circumscribed, local change. To intervene in a fractured community is a political process in which the provision of new resources is both necessary and potentially divisive.

Abstract:
Cancer is the second leading cause of death in America. It’s been suggested that regular physical activity (PA) can improve health outcomes in cancer survivors. An estimate from BRFSS data (2009) suggested that 47% of all cancer survivors met recommended guidelines and that this estimate was not different from the population at large (48%). Several factors were examined from these BRFSS data to determine whether subgroups of survivors existed who might benefit from interventions aimed at improving their PA status. The purpose of this investigation was to obtain more recent estimates of adherence to established PA guidelines for cancer survivors. Data from 2015 NHIS were obtained from the CDC website. Of the survivors, 40% met PA guidelines. Additionally, 79% were 54 years or older, more likely to be female (60%), predominantly white (80%), with more than 2 comorbidities (41%), and with some form of functional limitation (66%). Compared to a study based on 2009 BRFSS data, an even smaller proportion of survivors met PA guidelines in this study. This might be due to differences in age distributions and no limitation of the analysis according to time since diagnosis. Targeted interventions to increase activity in cancer survivors continue to be warranted.

Abstract:
Victorian inquisitiveness about sleep and dysfunctions of sleep is exemplified in novels published during the fifty-year period from Jane Eyre (1847) to Dracula (1897). This inquisitiveness foreshadows modern medical sleep science and immerses the reading public in a body of popular literature that subverts the concept of "normal" sleep. My dissertation explores the ways in which Victorian fiction brings physiological and psychological female concerns to the fore through the plot devices of sleep and sleeplessness. I examine the Victorians' diverse interpretations of illness, physical and sexual vulnerability, moral insanity, criminality, and anxiety to determine the thematic and narratological ways in which these issues are linked to sleeping and waking states. Drawing on feminist literary criticism, cultural historicism, and medical insight from the early nineteenth-century to the present, I argue that Charlotte Brontë, Wilkie Collins, and Bram Stoker use sleep and wakefulness as vehicles to navigate gendered fluctuations of power and loss. Jane Eyre, The Woman in White, and Dracula each present sleep as a gendered space in which power is contested. I argue that sleeplessness and restlessness are the methods women adopt, either on purpose or unintentionally, to realize self-sufficiency and protect themselves from patriarchal jurisdiction and other social restrictions on women. Women must reject their instinctual desires for a certain amount of sleep so that they can maintain agency and authority over their bodies and narratives. Implicit in the novels is the idea that deep sleep is a mechanism for achieving health and moral strength of character. However, explicitly and without apology, the novels use the trope of sleep for women as a violent instrument of loss, infection, powerlessness, and weakness. The cultural and medical artifacts of the time suggest that deep, indulgent sleep is the only way to achieve or maintain health. Yet Victorian authors write sleep as a sure road to incapacitation and subjugation. Brontë, Collins, and Stoker demonstrate that a woman's mind is only as healthy as her sleep, while her body is always safer awake.

Abstract:
Cerebrovascular accidents (CVAs) are a leading cause of morbidity and mortality in the United States. Metabolic derangements such as hypoglycemia, infections, brain masses or lesions, neurodegenerative disorders, neuropathies, myelopathies, seizures, syncope, types of migraines, and many other disorders may mimic CVA. Our case presents a 38-year-old female who was evaluated in the emergency department with a three- hour history of headache, lethargy, left- sided upper extremity weakness, facial droop, and dysarthria. A CVA workup was initiated and she immediately underwent a computed tomography (CT) scan of the head which revealed no intracranial hemorrhage. She was unable to provide a thorough medical history due to lethargy, however she was able to answer yes/no questions to screen for tissue plasminogen activator (tPA) qualification. She qualified based on her screening results and was administered tPA for her presumed ischemic CVA. She was then monitored in the intensive care unit for twenty-four hours. Due to reoccurrence of headache and left-sided weakness, as well as recent administration of tPA increasing risk of hemorrhage, she underwent two subsequent negative non- contrast head CT scans to rule out bleeding during that time. On hospital day two, magnetic resonance imaging (MRI) of the head, neck, and spine were performed which were also negative for infarct or hemorrhage. A more detailed history from our patient revealed previous migraine headaches, but her left sided weakness and dysarthria were new symptoms. With this information, it was suspected that she was suffering from a hemiplegic migraine, a rare mimic of CVA. Prochlorperazine, diphenhydramine, valproic acid, and corticosteroids were administered for migraine treatment, which aborted her symptoms entirely. Topiramate was then started for migraine prophylaxis. Daily low-dose aspirin was also initiated due to inability to fully rule out CVA/transient ischemic attack (TIA). An outpatient neurology follow up was scheduled on discharge. In clinical practice, hemiplegic migraines and CVA/TIA may be difficult to differentiate as symptoms often overlap. A detailed history and physical exam with careful attention to associated symptoms and timing of symptom onset is essential to formulating a correct diagnosis. This must be done quickly, as tPA is a high-risk medication with a narrow time window for administration. In conclusion, not all disease processes have an available “gold standard” diagnostic test to differentiate similar diagnoses. MRI of the brain is usually performed to differentiate ischemic CVA from TIA; however, imaging is not useful to differentiate hemiplegic migraine from TIA. Therefore, performing a thorough history, physical exam, and chart review is paramount to provide patients with the correct treatment as well as prevent adverse outcomes. It is the responsibility of the clinician to make difficult decisions weighing the risks and benefits of providing various treatments or interventions, and to know the complications of those treatments. Disease processes mimicking CVA must be considered in all patients, as treating an incorrect diagnosis can have devastating effects.

Abstract:
Introduktion: I Sverige beräknas 25 procent i åldersgruppen 16–29 år lida av någon form av psykisk ohälsa följt av 20 procent i åldersgruppen 30–44 år (Folkhälsomyndigheten, 2019d). Enligt United Nations Development Programme (UNDP) (u.å.b) så är covid-19 pandemin den största hälsokrisen i vår tid och många av de välkända riskfaktorerna för psykisk ohälsa, så som stress, ensamhet och oro, kan bli allt mer förekommande. Syfte: Studien syftar till att inhämta information kring vilket samband det finns mellan covid-19 pandemin och psykisk ohälsa i åldersgruppen 18–44 år i Sverige. Metod: En kvantitativ tvärsnittsstudie som inhämtat data genom en webbenkät där ett bekvämlighetsurval har använts. Enkäten besvarades av 157 respondenter av båda könen i åldrarna 18–44 år och resultaten analyserades med hjälp av en deskriptiv analys, korstabeller samt chi-två tester. Resultat: Resultaten visar att en ökning med 7,1 procent har skett bland de som uppger att de upplever psykiska besvär dagligen/några gånger i veckan innan pandemin (19,7 procent) jämfört med under pandemin (26,8 procent). Vidare uppger 52,2 procent av respondenterna att deras psykiska mående har försämrats till följd av pandemin. Slutsats: Den här studien visar på att det psykiska måendet kan ha försämrats till följd av covid-19 pandemin bland respondenterna. Då ett bekvämlighetsurval har använts så är studiens resultat ej generaliserbara till resterande befolkningen i åldersgruppen 18–44 år. Vidare studier inom ämnet som inkluderar fler deltagare kan vara nödvändiga för planering av framtida arbete för att förbättra den psykiska hälsan i denna åldersgrupp under och efter pandemin.
Introduction: It is estimated that 25 percent of youth between the ages 16-29 and 20 percent of the population between the ages 30-44 in Sweden suffer from some sort of mental illness (Folkhälsomyndigheten, 2019d). The covid-19 pandemic is the biggest health crisis of our time and many of the well known risk factors in mental illness, such as stress, loneliness and anxiety could be on the rise, according to the United Nations Development Programme (UNDP) (u.å.b). Aim: The purpose of the study is to collect information around the connection of the covid-19 pandemic and mental illness in the age span 18-44 in Sweden. Methods: A quantitative cross-sectional study that has collected data through an online survey where a convenience sample has been used. The survey was answered by 157 respondents of both sexes between the ages 18-44 and the result was analyzed with the help of descriptive analysis, cross tables and chi-square tests. Results: The result shows that there has been an increase of 7,1 percent amongst respondents that experience mental problems daily/a few times a week before the pandemic (19,7 percent) compared to the time during the pandemic (26,8 percent). Furthermore, 52,2 percent state that their mental well-being has worsened as a result of the pandemic. Conclusion: This study shows that the mental well-being of the respondents may have been worsened as a result of the covid-19 pandemic. The result of the study is not generalizable amongst the population of Sweden in the age span 18 to 44, as a convenience sample has been used. Further studies in the field that include more respondents may be necessary in the planning of future work of improving the mental health of those age group during and after the pandemic.

Abstract:
Heart weight is frequently used with other criteria at autopsy to assign hypertension as a cause of death in the absence of definitive signs of any other pathology that could have caused death. The extent to which heart weight is influential in that assignment is unknown. A major problem is to determine if heart weight is abnormal There are no universally accepted limits of heart weight in normally healthy persons. Although many studies have addressed this issue, few have been well designed and/or implemented and results are inconsistent. The purpose of this study was to determine 'normal' heart weight of healthy males aged 25-44 years who died of trauma, and to determine a criterion, based on heart weight, that classifies hearts as enlarged or not with higher sensitivity and/or specificity than the currently used criterion of 400 grams The data for this study were obtained from the Community Pathology Study, conducted in New Orleans over a ten-year period beginning in 1969. Of the 1292 male deaths that were autopsied and investigated during this time period, only the deaths due to trauma resulting from accidents, suicides, homicides, and poisoning (805 cases) were used to determine 'normal' limits of heart weight The mean heart weight of normal subjects was 363.50 $\pm$ 68.82 grams. Heart weight increased with age and there were no racial differences in each of the four five-year age groups considered; blacks tended to have lower heart weights but the differences were not statistically significant. Age, body weight, and trunk length were significant predictors of heart weight in normal persons. The upper limit of heart weight that discriminates 'normal' from 'diseased' hearts satisfactorily is 420 grams. This criterion should be used only when evidence of disease pathology is absent at autopsy, and it indicates the absence of CHD or hypertension with greater accuracy than their presence. On the basis of the findings it is believed that the actual misclassification that occurs as a result of using heart weight as a criterion to assign hypertension as a cause of death at autopsy is minimal

O hipertiroidismo felino é uma doença multifactorial resultante da produção e excreção excessiva de Tiroxina e Triiodotironina pela glândula tiróide considerando-se a doença endócrina mais comum do gato. Atualmente, são apontados diversos fatores de risco que podem variar consoante a área geográfica em que são descritos. Alguns desses factores são: idade, tipo de alimentação e estilo de vida. O presente trabalho teve como objetivo avaliar alguns dos fatores de risco desta doença na área metropolitana de Lisboa. A amostra recolhida incluiu 44 animais hipertiroides apresentados à consulta no Hospital Veterinário do Restelo, no período de 2014 a 2017. Foram também avaliados 44 animais saudáveis que constituiram o grupo controlo. Os factores de risco avaliados foram: idade, peso, raça, género, estado fértil, estilo de vida, tipo de alimentação, entre outros. Foram encontradas diferenças significativas entre os grupos apenas em relação à idade e ao peso (P< 0,01). Nos restantes factores de risco estudados, não foram encontradas diferenças significativas entre os grupos, provavelmente devido ao facto de os grupos em estudo se mostrarem demasiado heterogéneos. Os animais hipertiroideus eram em geral mais velhos e leves que os animais eutiroideus. Uma vez que não foram obtidos outros resultados estatisticamente significativos que permitam ser interpretados de forma conclusiva e tendo em conta a diferença de idades entre a população controlo e a polpulação de animais hipertiroideus, estes resultados não devem ser considerados válidos nem devem ser generalizados para toda a população de gatos residentes na área da grande Lisboa.
Feline hyperthyroidism is a multifactorial disease resulting from the production and excessive excretion of Thyroxine and Triiodothyronine by the thyroid gland considering the most common endocrine disease of the cat. Currently, several risk factors are pointed out that may vary depending on the geographic area in which they are described. Some of these factors are: age, type of diet and lifestyle. The present study aimed to evaluate some of the risk factors of this disease in the metropolitan area of Lisbon. The sample included 44 hyperthyroid animals submitted to the consultation at the Veterinary Hospital of Restelo, from 2014 to 2017. Forty-four healthy animals that constituted the control group were also evaluated. The risk factors evaluated were: age, weight, gender, lifestyle and type of diet. Significant differences were found between the groups only in relation to weight (P< 0.01). In the other risk factors studied, no significant differences were found between the groups, probably due to the fact that the study groups were too heterogeneous. The hyperthyroid animals were generally lighter than the eutiroideus animals. Since no other statistically significant results have been obtained to be interpreted conclusively and taking into account the age difference between the control population and the polulation of hyperthyroid animals, these results should not be considered valid and should not be generalised to the entire population of cats residing in the greater Lisbon area.

Abstract:
Extrazelulläre Purine und Pyrimidine sind wichtige Signalmoleküle, die über membranständige Rezeptoren, so genannte purinerge Rezeptoren ihre biologischen Effekte vermitteln. In der vorliegenden Arbeit wurden ATP-induzierte Antworten in den Stützzellen des olfaktorischen Epithels mit Hilfe der Calcium-Imaging-Technik charakterisiert.Die Applikation von ATP induzierte Zunahmen der intrazellulären Kalziumkonzentration ([Ca2+]i) sowohl in Anwesenheit, als auch in Abwesenheit von extrazellulärem Kalzium. Anders bei Anwendung von CPA, einem spezifischen Hemmstoff der Ca2+-ATPase des sarkoplasmatischen bzw. endoplasmatischen Retikulums. In diesen Versuchen wurden die ATP-induzierten [Ca2+]i-Zunahmen komplett aufgehoben. Das zeigt, dass die ATP-induzierten [Ca2+]i-Zunahmen in Stützzellen größtenteils, wenn nicht vollständig, durch die Aktivierung von G-Protein gekoppelten P2Y-Rezeptoren ausgelöst werden. Die ermittelte Wirkpotenz purinerger Agonisten war UTP>ATP>ATPγS. Die ATP-induzierten [Ca2+]i-Zunahmen konnten durch die purinergen Antagonisten PPADS und RB2 reduziert werden. Die hemmende Wirkung des purinergen Antagonisten Suramin blieb aus. Zusammen weisen die oben genannten Ergebnisse dieser Arbeit darauf hin, dass extrazelluläre Nukleotide die Stützzellen des olfaktorischen Epithels über P2Y2 / P2Y4-artige Rezeptoren aktivieren. Zusätzlich zeigten die Versuche mit dem Ektonukleotidase-Hemmstoff ARL 67156, dass im olfaktorischen Epithel von larvalen Xenopus laevis eine hohe Ektonukleotidasenaktivität herrscht.
