Abstract:
Abstract High titer (HT), ≥1:32, anti-E, anti-c and anti-D Rh antibodies can cause hemolytic disease of the newborn (HDN). Maternal IVIG therapy may reduce need for intrauterine and exchange transfusions in HDN. Minimal data exists regarding optimal timing of therapy initiation, dose and frequency and fetal outcomes. Late administration of IVIG therapy beyond gestational age of 16 weeks have not been shown to improve fetal outcome. We describe the clinical course of 3 pregnant women with Rh AI. Two women with high titer Rh AI were treated with early IVIG dose of 1 gm/kg/week. A third woman with low titer Rh AI did not receive IVIG. Fetal status was monitored non-invasively with weekly Fetal Doppler ultrasound measurement of peak systolic velocity (PSV) of the middle cerebral artery (MCA). Patient 1 was a 38 year old Caucasian female with high anti- D (> 1:500) and history of HDN; second baby required 3 exchange transfusions and third baby required 3 intrauterine transfusions during pregnancy and 2 exchange transfusions post delivery. IVIG weekly infusion was started on week 11. Amniocentesis done at week 21 showed that baby was Rh(D) positive and the Kleihauer-Bethke test was negative. Serial fetal MCA PSV done weekly till labor induction did not show evidence of significant fetal anemia. The IVIG infusions were completed on 33rd week and the patient delivered healthy baby by cesarean section on the 35th week. The baby did not receive any intra uterine or exchange transfusions. Patient 2 was a 37 year old caucasian woman with anti-c AI. The patient’s husband is c antigen positive and she had seroconversion during her third pregnancy which manifested as positive antibody screen in a routine type and screen with her third delivery which was uneventful. She subsequently had a miscarriage of her fourth pregnancy at 9 weeks. Her peak anti –c titer was 1: 128. She received IVIG infusion from week 11 to week 33 with stable anti-c titer of 1: 64. Serial MCA PSV assesments did not reveal significant fetal anemia. She had a normal vaginal delivery on week 34 with a healthy baby not requiring any exchange transfusions. Patient 3 was a 41 year old caucasian woman with low titer anti-E AI (1:8). No IVIG was administered as her titers remained low during her pregnancy course. Her titers were monitored every 2 weeks; peak antibody titer was 1:8. Fetal anemia was ruled out with Serial MCA PSV assesments. She delivered a healthy baby by Ceserean section at 38 weeks, 4 days. Abstract:
During the impact of fragment L filtergrams of high spatial and temporal resolution were obtained in the methane band centered at 892 nm with the solar vacuum tower telescope (VTT) located at Izaña, Tenerife. The VTThas an aperture of 0.70 m and a focal length of 45 m. Images were taken every 20 s in the prime focus with a CCD camera operated in 512 х 512 pixel mode. One pixel corresponds to 0.175 arcsec. The spatial resolution varied between 0.5 and 1.5 arcsec due to seeing effects. The exposure time was 2.5 s. At 22:20:52 UT, four minutes after the impact time based on Galileo images, a first brightening above the dark southeast limb of Jupiter was recognized. A first maximum was reached after five minutes. The subsequent decline of brightness was interrupted at 22:31:52 by a sudden bright flare with a lifetime of 90 s. Within our resolution the two recorded emission features are cospatial. The flux of this flare was (3.4 ± 0.2) х 10-4 of Jupiter’s total flux at this wavelength. The feature became almost invisible 19 minutes after its first detection. At 22:43 UT, 26 minutes after the impact, a new brightness enhancement rotated into view which could be identified with the dark impact signature (debris cloud) observed in the visual wavelength range.

Abstract:
Purpose: Cervical cancer remains a real public health problem in Togo. This study was undertaken to sensitize women and to determine the prevalence of precancerous and canrous lesions of the cervix in the Togolese woman. Material and methods: This is a retrospective descriptive study of the results of a consecutive series of Pap smears performed in the Laboratory of Pathological Anatomy and Cytology CHU Tokoin Lome 2011-2013. Results: Epidemiological, 512 FCV were interpretable. The average age of women was 37.6 years, extreme 19 and 87 years. By occupation, we noted a predominance of unemployed women (351cas, 68,6%), followed by women traders (103 cases, 20,1%) . The distribution by marital status was possible to observe a predominance of married women (234 cases, 45, 7 %). In terms of reproductive history, we found a mean gravidity and parity respectively 3, 5 and 2. We noted 22 women living with HIV, 77.2% were infected with HIV1. Cytopathological, 512 smears selected, we observed 44 abnormal smears (8,6%). It was indeterminate lesions ASCUS / ASCUS (8 cases), endocervical hyperplasia (3 cases), low-grade lesions (17 cases), high-grade lesions (10 cases) and invasive carcinoma (6 cases). Conclusion: The detection rate of cervical cancer is low among the Togolese woman. The health professionals should take the habit of asking the FCV any woman having a consultation.

Objective: Sleep disturbance is common among adolescents around the world. Our study aimed to determine the prevalence of sleeping disorders among in-school adolescents in Ilorin, Nigeria.Methods: With a cross-sectional design, sleep quality was assessed among 512 in-school adolescents using the Pittsburgh Sleep Quality Index (PSQI). A cut-off score of 5 was used. Data analysis was done with the Statistical Package for Social Sciences version 22.Results: Three out of every five adolescents were poor sleepers (PSQI global score > 5). Only a few respondents (19%) had optimal sleep (≥ 9 hours), and more than half had inadequate sleep (< 7 hours). Poor sleep was associated with male gender, being the first born and residence at home. Logistic regression analysis showed that living at home, as against hostel living, was predictive of poor sleep.Conclusion: Most respondents had inadequate sleep. Support from parents and other stakeholders could improve sleep among adolescents, as poor sleep is quite common in this age group. These concerted efforts would promote adolescent mental health in north-central Nigeria.

Abstract:
BACKGROUND. Pneumonia resulting from SARS-CoV-2 infection is characterized by the development of certain radiological patterns such as “ground glass” and others, which can only be detected using high-resolution computed tomography (HRCT) of the chest. The use of CT to combat the coronavirus disease (COVID-19) pandemic varies worldwide. In scientific publications, there is no consensus on the information content of CT for COVID-19 pneumonia, the timing of its implementation at the onset of the disease and control examinations. The long-term consequences of the disease, the possibilities of software processing of CT images of the chest cavity in order to study the dynamics of the process, predict outcomes and evaluate the effectiveness of the therapy, have not been sufficiently studied.
 OBJECTIVE. To determine the possibilities of digital software processing to increase the information content of CT images and justify its use in determining the dynamics and stage of development of COVID-19 pneumonia.
 MATERIALS AND METHODS. For image analysis, we used the Dragonfly program, which was provided free of charge for non-commercial scientific research by Object Research Systems (Montreal, Canada). The program allows you to carry out segmentation, mathematical and statistical processing of images, to build conventional and segmented histograms. To use it, it is necessary to reformat DICOM files of CT into so-called raster files (TIFF, JPEG). Further analysis of images of CT slices is performed using gray scale (from 64 to 512 gradations), which correlates with density values on the Hounsfield scale, which allows you to more accurately determine the structure of the organ under study.
 RESULTS AND DISCUSSION. The analysis of CT for COVID-19 pneumonia with the “ground glass” pattern showed that standard densitometry does not allow to determine the morphofunctional difference between the studied areas of “ground glass” and to determine the stage of its development. The segmentation histogram based on digital software processing of the same “ground glass” areas of the patient with COVID-19 pneumonia has changed significantly with a clear definition of a significant difference in the density of the pattern in dynamics, on the basis of which it can be concluded that the patient is undergoing a process of recovery and COVID-19 pneumonia is at the resolution stage.
 CONCLUSIONS. Carrying out digital software image processing with a CT segmentation histogram allows you to determine the dynamics and stage of development of COVID-19 pneumonia, evaluate the effectiveness and need for therapeutic measures. In order to conduct an objective control of the inflammatory process in COVID-19 pneumonia, it is necessary to conduct a HRCT of the chest at discharge from the hospital, especially in patients who were treated in the intensive care unit. If there are significant changes, follow-up studies should be carried out after 3 and 6 months, or depending on clinical indications.

Bacteria of the Rickettsia genus are agents of Brazilian Spotted Fever (BSF), a zoonotic disease which is difficult to diagnose, evolves quickly and can result in death. Antibodies against Rickettsia spp. in horses were studied, by means of Indirect Immunofluorescence Assay (IFAT ≥64), in 150 blood samples taken from animals in two Santa Catarina mesoregions (Planalto Serrano and Vale do Itajaí). The overall occurrence of Rickettsia spp. antibodies in horses was 18.66%, with cross-reactivity occurring in all positive samples for at least two of the species tested. Separately, according to the species, 25 (16.66%) samples were positive for R. rickettsii, 15 (10%) for R. parkeri, 22 (14.66%) for R. amblyommii, 23 (15.33%) for R. rhipicephali, 16 (10.66%) for R. bellii and 19 (12.66%) for R. felis. Only two animals resulted in a conclusive serodiagnosis, one for R. bellii and the other for R. rickettsii, at maximum dilutions of 1:4096 and 1:512, respectively. The occurrence of antibodies against Rickettsia spp. in horses from two mesoregions in the state of Santa Catarina indicates the movement of BSF agents in these sentinel animals and confirms the importance of studying spotted fever in the state of Santa Catarina.

Apor-celaimellus brevicaudatus sp. n. is characterized by a body length of 1.54–2.14 mm long, lip region offset by a more or less distinct constriction and 14–16 μm broad, odontostyle 13–15 μm long, neck 512 μm long, pharyngeal expansion 255 μm long, uterus 39–57 μm long or 0.9–1.0 times the corresponding body diameter, V = 50–55, tail short and rounded to hemispherical (24–27 μm, c = 66–82, c’ = 0.6–0.7) with inner core hardly reaching the middle of tail length, and male unknown. Aporcelaimellus rotundus sp. n. is characterized by its body 2.01–2.58 mm long, lip region offset by constriction and 19–20 μm broad, odontostyle 22–23 μm long, neck 455–579 μm long, pharyngeal expansion 226–316 μm long, a dis-tinct dorsal cell mass present at cardia level, uterus 28–74 μm long or 0.5–0.9 times the corresponding body diameter, V = 47–52, female tail short and conoid to rounded (27–33 μm, c = 64–90, c’ = 0.7–0.9), and males unknown. Abstract:
A translation and commentary on the spell in the tomb of Neha at el-Qatta previously discussed by H. D. Schneider (1977) and T. J. Allen (2013). The spell was believed to provide Neha, after his death, with magical powers over representations (possibly statuettes) of his “workers-kAwtyw” designed to serve him in the next world. However, as an indispensable condition for the effectiveness of the spell was considered to be the faultlessness of the acquisition of the representations, the latter was asserted in passages resembling an ideal autobiography. The expression mr(y)t(=f) Ds=f “his own household people” in l. 22 is the Middle Kingdom colloquial equivalent of the mr(y)t=f n(y)t Dt=f, which has survived in nomarch Nehery’s tomb at el-Bersheh and on the pedestal of a sculptural group of four figurines from the mastaba of Shepi at Dahshur (CG 512). The latter parallel is of special significance, since in the selection and arrangement of other spells Neha’s burial chamber is also comparable to certain tombs in the Dahshur–Lisht region. Neha was most likely influenced by one of the magical traditions of this metropolitan area. If Neha, like Shepi, had a sculptural group of four “household people”, this could explain his words about acquiring “the fourth one (fdnw)” to serve very early and late extra meals (ll. 23 ̶ 24). The noun nDs in l. 17 and the use of the word Ds with the meaning “own” in connection with people date the composition to no later than the reign of Senusret I.

Background: Opportunistic screening for individuals aged ≥30 years at all levels of healthcare for early detection of diabetes mellitus (DM) and hypertension (HTN) is an integral strategy under the national program to control non-communicable diseases. There has been no systematic assessment of the screening process in primary care settings since its launch. The objective was to determine the number and proportion eligible for screening, number screened, diagnosed and treated for DM and HTN among persons aged ≥30 years in two selected primary health centres (PHCs) in Dakshina Kannada district, Karnataka, India during March-May 2019 and to explore the enablers and barriers in the implementation of screening from the perspective of the health care providers (HCPs) and beneficiaries . Methods: This was a sequential explanatory mixed-methods study with a quantitative (cohort design) and a descriptive qualitative component (in-depth interviews and focus group discussions) with HCPs and persons seeking care. Those that were not known DM/HTN and not screened for DM/HTN in one year were used to estimate persons eligible for screening. Results: Of 2697 persons, 512 (19%) were eligible for DM screening, 401 (78%) were screened; 88/401 (22%) were diagnosed and 67/88 (76%) were initiated on treatment. Of 2697, 337 (13%) were eligible for HTN screening, 327 (97%) were screened, 55 (17%) were diagnosed with HTN; of those diagnosed, 44/55 (80%) were initiated on treatment. The documentation changes helped in identifying the eligible population. Patient willingness to undergo screening and recognition of relevance of screening were screening enablers. Overworked staff, logistical and documentation issues, inadequate training were the barriers. Conclusion: Nearly 19% were eligible for DM screening and 13% were eligible for HTN screening. The yield of screening was high. We noted several enablers and barriers. The barriers require urgent attention to reduce the gaps in delivery and uptake of services.

The objective of this study is to investigate the potential of novel neural network architectures for measuring the quality and quantity parameters of silage grass swards, using drone RGB and hyperspectral images (HSI), and compare the results with the random forest (RF) method and handcrafted features. The parameters included fresh and dry biomass (FY, DMY), the digestibility of organic matter in dry matter (D-value), neutral detergent fiber (NDF), indigestible neutral detergent fiber (iNDF), water-soluble carbohydrates (WSC), nitrogen concentration (Ncont) and nitrogen uptake (NU); datasets from spring and summer growth were used. Deep pre-trained neural network architectures, the VGG16 and the Vision Transformer (ViT), and simple 2D and 3D convolutional neural networks (CNN) were studied. In most cases, the neural networks outperformed RF. The normalized root-mean-square errors (NRMSE) of the best models were for FY 19% (2104 kg/ha), DMY 21% (512 kg DM/ha), D-value 1.2% (8.6 g/kg DM), iNDF 12% (5.1 g/kg DM), NDF 1.1.% (6.2 g/kg DM), WSC 10% (10.5 g/kg DM), Ncont 9% (2 g N/kg DM), and NU 22% (11.9 N kg/ha) using independent test dataset. The RGB data provided good results, particularly for the FY, DMY, WSC and NU. The HSI datasets provided advantages for some parameters. The ViT and VGG provided the best results with the RGB data, whereas the simple 3D-CNN was the most consistent with the HSI data.

Abstract:
This chapter discusses issues concerning the limitation of freedom of religion or belief, including related issues of interpretation. First, religious manifestation is different from holding, adopting, or changing religion or belief in that the latter is not subject to any limitation whatsoever. Second, national security is mentioned as a limitations ground in articles 12 (liberty of movement), 13 (expulsion), 14 (fair trial), 19 (freedom of expression), 21 (peaceful assembly), and 22 (freedom of association), but not in article 18 of the ICCPR. Third, traditions and limitations intersect in two ways; a single tradition cannot determine limitation on the grounds of morals, and limitations should not target a single tradition. Lastly, the exercise of assessing ‘necessity’ and ‘proportionality’ should not sideline the importance of upholding the protection of the relevant human rights. Esta evaluación fue ejecutada por el equipo científico y técnico de AGROSAVIA, en el Centro de Investigación La Suiza y la Universidad Industrial de Santander, desde el Laboratorio de Alimentos del CICTA encontrándose como resultado que los genotipos en estudio presentan un comportamiento productivo y sanitario de interés comercial (TCS 06, 01, 11, 13, 03 y 49), así como un potencial en su composición funcional (TCS 01, 04, 06, 13 y 47) y atributos sensoriales (TCS 01, 02, 05, 13, 19 y 06) además se desarrolló un modelo de predicción de perfiles. Toda la información consignada en este manuscrito sirve como base para investigadores, gremios, industriales, agricultores y comunidad académica, en la evaluación de nuevos genotipos de interés y ratificar a Colombia como país productor de cacao fino y aromatizado reconocido por la International Cocoa Organization (ICCO).

Abstract M<sub>3</sub> derived mutants from two bread wheat varieties, namely, 'Giza 186' and 'Saha 93', were screened for resistance to the rust Ug99 at two locations in Njoro (Kenya) and in Tihama (Yemen). At Tihama, two mutants of 'Giza 186' (G-M2-2010-1-28 and G-M2-2010-41-52) and four mutants of 'Saha 93' (S-M2-2010-16-12, S-M2-2010-21-13, S-M2-2010-22-14 and S-M2-2010-27-15) were seen to be resistant at both seedling and adult stages while their parents were resistant at seedling stage and susceptible at adult stage. In Kenya, the resistance score of the mutants was slightly different from those obtained at Tihama. The mutants G-M2-2010-1-28 and G-M2-2010-41-52 were stable in their level of resistance recorded at Tihama, but only two mutants of 'Saha 93' (S-M2-2010-16-12 and S-M2-2010-27-15) were resistant at both growth stages. S-M2-2010-22-14 and S-M2-2010-21-13 were resistant at the seedling stage while susceptible at adult stage. Further selection on these mutants for yield potential, agronomic performance and yellow rust disease resistance, as well as on selected mutants of both 'Giza 186' and 'Saha 93', at M5-M6 stages identified superior mutant lines compared with the two parents 'Saha 93' and 'Giza 186'. These included the line Erra-010-GM2w-41-52-40, which ranked first in yield (3768 kg/ha), followed by the lines Erra-010-SwM2-16-12-19, Erra-010-GM2w-1-28-18 and Erra-010-SwM2-22-14-6. Moreover, it can be concluded that Erra-010-GM2w-41-52-40 and Erra-010-SwM2-16-12-19 are highly recommended for their resistance to stem and yellow rust diseases as well as for yield potential and preference by farmers. Therefore, efforts are in progress to increase their seeds for dissemination over a wide range of farmers and wheat areas where rust diseases are an epidemic, and for registration of the lines as improved mutant varieties.

Replacing the connecting rod with a lumped two-mass system causes an error, which influences the inertia rolling moment, the thrust force between the piston and the cylinder liner, and the loading on the main bearings. Dimensionless relationships have been found that relate the inertia error due to the connecting rod simplification (the inertia error) to the errors of the forces and moments that are created by it. Additionally, the results of a statistical study of 19 SI connecting rods indicate that the mass moment of inertia of the two mass system is −2.65% to 22% higher than that the experimentally measured moment of inertia of the connecting rod, with an average error value of 9.65%.

Introdução: As infecções fúngicas ocorrem com grande frequência na rotina clínica de pequenos animais, devido a fatores como resposta imune do hospedeiro e ampla distribuição do agente no ambiente. Estas infecções são conhecidas como micoses, sendo classificadas como superficiais, subcutâneas e sistêmicas. Objetivo: Objetivou-se com presente trabalho identificar os fungos isolados de amostras clínicas de cães e gatos com infecções fúngicas. Material e métodos: No período de setembro/2020 a março/2021, realizou-se o levantamento das fichas e laudos dos animais relacionados aos casos de infecções fúngicas, obtidos no Centro de Diagnostico Veterinário (VETLAB). A partir das análises dos arquivos, obteve-se dados como raça, sexo e idade. Além disso, foi possível identificar os micro-organismos isolados por meio de amostras colhidas. Resultados: Foram avaliados 52 animais, sendo 57,6% (30/52) cães e 42,4% (22/52) felinos. Os cães possuíam idades entre três meses a 15 anos e os felinos de um mês a oito anos. Em relação aos cães os de maiores incidências no período foram das raças Yorkshire e SRD com 16,66% (5/30), Spitz e Pug com 13% ( 4/30), Pinscher e Chiuahua 6,6% ( 2/30) e os da raça Pastor Maremano, Bulldog Francês, Jack Russell Terrier, Branco, Maltês, Lulu da pomerânia, Dachshund e Bulldog Inglês com 3,33% (1/30) cada. Tiveram predominância nos felinos, SRD com 86,37% (19/22) e Persa com 13,63% (3/22). Destes 52 animais, foram colhidas amostras de pele, pelo e conduto auditivo. Houve maior ocorrência de Penicillium sp. 23,07% (12/52), Microsporum sp. 13,4% (7/52), Microsporum canis sp. 11,5% (6/52), Aspergillus niger 7,69% (4/52), Dermatófito e Malassezia sp. com 5,76% (3/52), Aspergillus sp. 3,84% (2/52), Trichophyton sp. 1,92% (1/52) e em 25% (13/52) das amostras não houve o crescimento fúngico. Conclusão: Diante dos resultados, observou-se que os fungos do gênero Penicillium sp. e Microsporum sp., foram mais prevalentes nas afecções fúngicas. Devido ao aumento das infeções, ressalta-se a importância da realização de cultura fúngicas e o antifungigrama, permitindo um melhor diagnóstico e adequação da alternativa terapêutica.

Introdução: A sífilis é uma infecção sexualmente transmissível sistêmica, crônica, curável e exclusiva do ser humano. A transmissão ocorre principalmente por contato sexual. Todavia, também pode ser transmitida verticalmente para o feto durante a gestação de uma mulher com sífilis não tratada ou tratada de forma inadequada. Configura-se como um grave problema de saúde pública, afetando a saúde e a qualidade de vida das pessoas. Em todo território nacional, foi instituída a notificação compulsória de sífilis congênita por meio da Portaria n. 542, de 22 de dezembro de 1986, a de sífilis em gestantes, por meio da Portaria n. 33, de 14 de julho de 2005, e a de sífilis adquirida, por intermédio da Portaria n. 2.472, de 31 de agosto de 2010. Objetivo: Descrever como a pandemia de COVID-19 intensificou a subnotificação da sífilis adquirida. Métodos: Trata-se de um estudo epidemiológico descritivo elaborado por pesquisas na base de dados públicos de notificação de sífilis adquirida no ano de 2019 a 2020 no Distrito Federal, disponibilizados pelo Departamento de Informática do Sistema Único de Saúde do Brasil. Resultados: Em 2019 foram notificados 1.954 casos de sífilis adquirida no Distrito Federal. Em 2020 foram notificados apenas 898 casos. Essa brusca diminuição segue uma tendência nacional, visto que no Brasil, em 2019, foram notificados 152.915 e, em 2020, os dados preliminares apontam apenas 49.154. Conclusão: Com a reorganização dos serviços de Atenção Primária à Saúde para enfrentamento do novo coronavírus, houve uma diminuição da oferta de testes sorológicos para infecções sexualmente transmissíveis. A intensificação da subnotificação da sífilis adquirida demonstra que, durante o período analisado, houve falha na detecção de novos casos, o que prejudica as ações de enfrentamento dessa infecção sexualmente transmissível em razão da falta de dados fidedignos para o planejamento de ações efetivas de controle do agravo, especialmente no contexto da pandemia.

Background: stroke is the second leading cause of death and disability worldwide. The most significant factors for early mortality are age, severity of stroke (NIH stroke scale), atrial fibrillation (AF) and hypertension. Objective: elucidate the factors correlated with unfavorable outcome and mortality after ischemic stroke. Design/Setting: retrospective descriptive study conducted at the Botucatu Medical School Hospital. Methods: this study included 515 stroke patients, aged at least 18, admitted to ICU and stroke-unit between January/2017-December/2018. Baseline data, comorbidities and risk factors were collected and relation to unfavorable outcome and in-hospital mortality were evaluated. Unfavorable outcome was defined as dependency (modified Rankin Scale mRs 4–5). Results: overall, in-hospital mortality rate was 15% (77) and unfavorable outcome 36.7% (189). The patients average age was 69.18±13.08, and NIHSS at admission 9.27±8.41. NIHSS at admission and pre-morbid mRs were independently associated with unfavorable outcome, as each NIHSS point was responsible for 22% outcome increase. Both higher NIHSS and AF were independently associated with in-hospital mortality, increasing the death risk 19% and 3.5 times respectively. Conclusion: the main factor associated with overall in-hospital mortality and unfavorable outcome was stroke severity.

Abstract:
The presence of XLFDP in plasma, a consequence of the balance between fibrin generation and dissolution, is an indirect index of the presence of fibrin and raised levels accompany intravascular thrombosis. The potential of an enzyme immunoassay (EIA) for XLFDP, using the monoclonal antibody DD-3B6/22 was evaluated in 104 patients with suspected DVT receiving lower limb venography. Using a reference level of 400 ng/ml of XLFDP, all patients with a positive venogram had elevated levels (sensitivity 100%), although levels were also elevated due to causes other than DVT in some patients with negative venograms (specificity 52%). Higher elevations of XLFDP occurred in proximal than in calf DVT.A second study of another 48 patients receiving venography for suspected DVT confirmed the sensitivity of EIA while the complementary latex "Dimertest", also based upon DD-3B6/22, was positive in 19 and negative in 3 patients with confirmed DVT. In an ongoing study, 130 patients have been followed after abdominal operations with 125I-fibrinogren leg scanning and venography when appropriate. Levels of XLDFP (EIA) were elevated in all patients developing thrombosis. Latex assays were positive in 42/48 patients with positive leg scans. While elevated levels of XLDFP do not specifically define the location or even the presence of a thrombus they reliably indicate the possibility of thrombosis in patients without antecedent medical or surgical illnesses. In complicated surgical patients,an arbitrary cut-off of 1000 ng/ml is more appropriate for screening for the possibility of DVT. A positive latex test is also suggestive, although it is negative in some patients with calf thrombi. Measurements of XLFDP, with high sensitivity but moderate specificity, are potentially valuable in selecting patients for invasive investigations such as venography, and combinations with other noninvasive screening tests for require evaluation

Objectives: To present the results of adopting CBR/FEBRASGO/SBM recommendations for breast cancer screening and diagnosis during 2020 COVID-19 PANDEMIC on mammograms (MMG), breast ultrasound (BUS), breast biopsy (BB), and cancer diagnosis (CD) rates. Methodology: Comparing by month the total number of MMG, BUS, BB (composed of fine needle, core, and vacuum procedures), and invasive and in situ cancers diagnosis (CD) performed at Redimama, a private breast unit from Belo Horizonte Brazil, that adopted CBR/FEBRASGO/SBM recommendations for breast cancer screening and diagnosis during the 2020 COVID-19 pandemic year to 2019 same period. Results: In April 2019, 391 MMG, 714 BUS, 223 BB, and 22 CD were performed, compared with 115 (29.4%) MMG, 313 (43.8%) BUS, 116 (52%) BB, and 11 (50%) CD in 2020. A continuous and fast recovery occurred along the time. In 2019 first semester, 2241 MMG, 4229 BUS, 1214 BB, and 84 CD were performed, compared with 1,903 (88.7%) MMG, 4,227 (99.2%) BUS, 1,044 (86%) BB, and 92 (109.5%) DC in 2020. In 2019, 4,424 MMG, 10,395 BUS, 3,304 BB, and 231 CD were performed, compared with 4,561 (110.79%) MMG, 11,549 (120.72%) BUS, 3,011 (91.13%) BB, and 226 (97.83%) CD in 2020. In 2019, the median size in image (T) by MMG/BUS for invasive cancers (IC) was 18.18 mm, from CD 184 (79.66%) were IC, and 47 (20.4%) ductal carcinoma in situ (DCIS) compared to a T of 18.2 mm, 191 (86.52%) IC, and 35 DCIS in 2020. Conclusion: Adopting the CBR/FEBRASGO/SBM recommendations for breast cancer screening and diagnosis results to recovery the prior pandemic levels. Recovery of MMG and BUS is faster and shows a “J” curve compared with recovery of BB and CD that shows a “´U”` curve with a delay. This strategy should be adopted in Brazil in order to maintain breast cancer screening and diagnosis.

Background: The post-stroke persistent headache syndrome is poorly characterized in the literature and recognized in clinical practice. The objective of this study is to describe the clinical and epidemiological profile of patients with persistent post-stroke headache and its pattern. Design and setting: Cross-sectional study at a reference stroke outpatient clinic from Federal University of Bahia. Methods: Patients were actively approached during the usual consultation. Results: From a total of 52 stroke patients, 11 (21.2%) were diagnosed with persistent post-stroke headache, 19 (36.5%) with acute post-stroke headache and 22 (42.3%) without headache. Systemic arterial hypertension was more frequent in the group without headache (95.5%) than in the group with persistent headache (63.6%). On the other hand, the second group presented a greater proportion of sedentary lifestyle (90.9% vs 54.5%) and smoking (45.5% vs 27.3%) than the first group. In patients with persistent headache, the median frequency of pain in the last 3 months was 40.5 days (IIQ 14.25 - 90.0) and the most frequent locations were strictly unilateral (45.5%) and holocranial (36 , 4%), with no individual reporting an alternating unilateral pattern. Conclusions: Corroborating the results of previous studies, our data demonstrated a relevant prevalence of persistent post-stroke headache, and it is possible that previously hypertensive patients are more protected for the development of persistent headache than individuals without hypertension.

Breast cancer is among the main types of cancer in the world. One of the treatment alternatives is chemotherapy, which in turn can cause antagonistic effects related to behavior, reflecting anxiety, and fatigue. Objective: The aim of this study was to evaluate the effects of chemotherapy treatment on anxiety and fatigue in women with breast cancer. Methods: A total of 37 women participated in the study and were distributed into two groups, namely, 19 undergoing chemotherapy for breast cancer (52±13.11 years) and 18 apparently healthy (55.8±8.37 years). All volunteers answered questions from the questionnaire related to the Piper Fatigue Scale, composed of 22 items subdivided into 4 distinct subjective dimensions, namely, affective, sensory, cognitive, and behavioral. Anxiety was assessed using the IDATE anxiety scale, an instrument that consists of two separate self-report scales: state of anxiety and trace state, each has 20 statements in which the subjects must describe how they feel. The significance level was set a priori at p0.05). Behavior fatigue (p=0.08, ES=0.56), affective fatigue (p=0.18, ES=0.44), sensory fatigue (p=0.09, ES=0.55), cognitive fatigue (p=0.34, ES=0.31), general fatigue (p=0.09, ES=0.56), anxiety state (p=0.08, ES=0.56), and anxiety trait (p=0.92, ES=0.03). Conclusion: The results of this study show that chemotherapy treatment between the third and fourth cycles does not directly affect anxiety and fatigue in women with breast cancer.

Introdução: O sistema intrauterino liberador de levonorgestrel (SIU-LNG) e o implante subdérmico com etonogestrel são opções contraceptivas de longa duração (LARC) que apresentam alta eficácia e excelente custo-benefício em pacientes adolescentes. Porém, principalmente nos primeiros meses de uso, são capazes de ocasionar sangramentos e alterações menstruais que podem estimular a interrupção do método por desconforto ou sensação de insegurança da paciente. Objetivo: Analisar o perfil de sangramento das usuárias do SIU-LNG e do implante subdérmico com etonogestrel e a continuidade do uso dos métodos pelas pacientes. Métodos: Trata-se de um estudo qualitativo, realizado por meio da revisão de prontuários de pacientes acompanhadas em um ambulatório de adolescentes que escolheram o SIU-LNG ou o implante subdérmico com etonogestrel como método anticoncepcional, com a inserção do método realizada entre janeiro de 2019 e setembro de 2020. Foram avaliadas a ocorrência de spotting, a presença de menstruação e a permanência com o método em consultas de retorno três e seis meses após a inserção. Resultados: Do total de 118 pacientes avaliadas, apenas 79 (67%) retornaram para o acompanhamento e fizeram parte deste estudo. Delas, 37 optaram pelo SIU-LNG e 42 escolheram o implante subdérmico com etonogestrel como método contraceptivo. A faixa etária das pacientes que optaram pelo SIU-LNG variou entre 13 e 19 anos, e 12 (32%) eram nulíparas. A taxa de amenorreia inicial de 24% vista na primeira consulta, com três meses de uso do método, aumentou para 59% após seis meses. A presença de spotting foi verificada em 29% das pacientes. Apenas três (8%) descontinuaram o método, duas por dismenorreia e aumento de fluxo menstrual e uma por se sentir insegura após episódio de doença inflamatória pélvica. As pacientes que optaram pelo implante subdérmico com etonogestrel também tinham entre 13 e 19 anos, porém 22 (52%) eram nulíparas. A taxa de amenorreia progrediu de 40% em três meses de uso para 81% após seis meses. A queixa de spotting esteve presente em 57% das pacientes durante o período do estudo. Houve desistência do uso do método por oito (19%) pacientes, sendo duas por desejo de gestar, duas por sangramento, duas por cefaleia persistente, uma por aumento de peso e uma por incômodo com spotting. Conclusão: Neste estudo, houve melhor taxa de continuidade do uso do SIU-LNG em comparação com o implante subdérmico e ocorreu maior evolução para amenorreia e spotting entre as usuárias do implante subdérmico. Fatoimportante a ser observado foi a dificuldade de seguimento médico das adolescentes, o que enalteceu ainda mais a importância dos LARC, que asseguram a contracepção independentemente da motivação da usuária.

Background: Autoimmune encephalitis (AIE) is the main differential diagnosis of infectious encephalitis. Brain MRI is normal in up to 50% of cases and studies indicate that changes in FDG-PET/CT are more frequent and early. Objectives: To describe FDG-PET/CT findings in patients with AIE of Hospital Israelita Albert Einstein (HIAE) from 2015-2020. Design and setting: Retrospective cross-sectional study at HIAE. Methods: Medical records of patients with suspected AIE were reviewed. Laboratory results were compiled, and images were reassessed. Results: Amongst 250 records, we found 7.6% (n=19) of AIE, being 8 seropositive (5 anti-NMDA, 1 anti-CASPR2, 1 anti-MOG, 1 anti-VGKC and 1 anti-LGI1), 5 seronegative and 4 limbic encephalitis. The mean age was 48-22 years, 52% male. In encephalitic patients, the most common manifestations were epilepsy (78%), cognitive changes (63%), and behavioral changes (63%). Only 57% had abnormal MRI. We evaluated 23 PET/CTs at different stages of treatment; of the 9 FDG-PET/CTs performed at initial presentation, 88% were abnormal and 30% had normal MRI. Most frequent patterns found were hypometabolism in frontal (59.1%), temporal (39.1%) and left parietal (39.1%) regions. In follow-up, only 2 patients normalized FDG-PET/CT, with clinical improvement. Conclusions:FDG-PET/CT was altered in 88% of patients. In this series, no typical PET/CT pattern was demonstrated for AIE; the most frequent findings were hypometabolism in cortical areas, which also occur in degenerative diseases. We did not find hypermetabolism, or mixed areas of hypo- and hypermetabolism. The specificity of PET/CT for AIE diagnosis should be evaluated in future studies.

Abstract:
The Northwest Tropical Atlantic Station (NTAS) was established to address the need for accurate air-sea flux estimates and upper ocean measurements in a region with strong sea surface temperature anomalies and the likelihood of significant local air–sea interaction on interannual to decadal timescales. The approach is to maintain a surface mooring outfitted for meteorological and oceanographic measurements at a site near 15°N, 51°W by successive mooring turnarounds. These observations will be used to investigate air–sea interaction processes related to climate variability. This report documents recovery of the NTAS-18 mooring and deployment of the NTAS-19 mooring at the same site. Both moorings used Surlyn foam buoys as the surface element. These buoys were outfitted with two Air–Sea Interaction Meteorology (ASIMET) systems. Each system measures, records, and transmits via Argos satellite the surface meteorological variables necessary to compute air–sea fluxes of heat, moisture and momentum. The upper 160 m of the mooring line were outfitted with oceanographic sensors for the measurement of temperature, salinity and velocity. Deep ocean temperature and salinity are measured at approximately 38 m above the bottom. The mooring turnaround was done on the National Oceanic and Atmospheric Administration (NOAA) Ship Ronald H. Brown, Cruise RB-20-06, by the Upper Ocean Processes Group of the Woods Hole Oceanographic Institution. The cruise took place between 14 October and 1 November 2020. The NTAS-19 mooring was deployed on 22 October, with an anchor position of about 14° 49.48° N, 51° 00.96° W in 4985 m of water. A 31-hour intercomparison period followed, during which satellite telemetry data from the NTAS-19 buoy and the ship’s meteorological sensors were monitored. The NTAS-18 buoy, which had gone adrift on 28 April 2020, was recovered on 20 October near 13° 41.96° N, 58° 38.67° W. This report describes these operations, as well as other work done on the cruise and some of the pre-cruise buoy preparations.
