The 52nd meeting of the European Pancreatic Club was organized and conducted online in Paris on July 2–3, 2020. It is the first time the Congress took place online, with about a thousand of pancreatologists having participated in it. The high scientific, organizational and technical level of the congress should be noted. This review presents the most interesting research results in the following sections: anatomy of the pancreas, acute early chronic, autoimmune pancreatitis, cancer and other malignant pancreatic diseases, functional pancreatic insufficiency, enzyme replacement therapy, pancreatic steatosis, microbiota in pancreatic pathology. Dugic et al. (Sweden) conducted a systematic review “The effect of anatomical variants of the Wirsung’s duct on exocrine function and risk of pancreatic pathology”. Studies of the MEDLINE database from 1960 to 2019 were analyzed; data of 3234 patients were included. A classification of anatomical variants of the pancreatic duct system was compiled. A lecture by T. B. Gardner (Great Britain) on pancreatic steatosis drew great attention. The problem of pancreatic steatosis is surrounded by a number of unresolved issues; the terminology of pancreatic steatosis itself is unclear. Under conditions of oxidative stress, cytokines released from adipocytes trigger the local inflammation and dysfunction of the affected pancreatic cells. The pathogenesis of pancreatic steatosis requires further studies. The symposium on exocrine pancreatic insufficiency in pancreatic cancer played the central role in the congress. Professor E. Dominguez-Munoz (Spain) gave a lecture within the framework of the symposium. 80% of patients with pancreatic cancer have sarcopenia and/or cachexia. Sarcopenia and cachexia in pancreatic adenocarcinoma are the most obviousamong all tumor localizations. The pathogenesis of cachexia and sarcopenia in pancreatic cancer was analyzed in detail; the necessity of enzyme replacement therapy with Creon was substantiated. Particular attention was paid to the pathogenesis of pancreatic insufficiency after surgical treatment (pancreatic resections, pancreatectomy); the features of replacement therapy were highlighted. On the seventeenth National People's Congress of CPC, the strategic goal of the construction of ecological civilization was first proposed. The construction of ecological civilization is new requirement for our country to realize well-off society. From the meaning and nature of ecological civilization, this paper tries to establish an index system of ecological civilization in county administrative areas, which is composed by the environment, ecological economy, ecological community, ecological awareness and Eco-system and includes twenty-six representative indexes. At last this system is used combined with AHP method to evaluate the level of ecological civilization in Haian county Jiangsu province. The result shows that the integrated score of ecological civilization of Haian county in 2010 is 0.6373, indicating that the ecological civilization level is at the initial state. On the five sub-systems, the level of ecological awareness is poor and this is the major factor that limits the level of ecological civilization of Haian.

Abstract Results are provided for the Guiding Vote on the seven formal proposals to amend the International Code of Nomenclature for algae, fungi, and plants to be decided by the Fungal Nomenclature Session (FNS) of the XI International Mycological Congress in July 2018. Te ballot for the Guiding Vote was provided online. Tere were 520 valid ballots, submitted by mycologists from 42 countries, belonging to 23 eligible groups and societies, along with authors of proposals. Tw o proposals F-005 and F-006, both concerning DNA sequences as types, exceeded the 75 % No vote that is the threshold above which proposals are considered rejected by the FNS unless formally re-introduced. Two options for amendments to future procedures for the Guiding Vote are proposed: adding eligibility via publication of a nomenclatural novelty among fungi and removing eligibility via membership of IMA MMOs.

Document NO.1 of the Central Government and the report of the eighteenth National Congress has aroused widespread concern in academic circles with its emphasis on rural micro-management body, while research on spatial organization of organic agricultural enterprise under the paradigm of enterprise network is still insufficiency. The paper analyzed the organic agricultural spatial organization of headquarters, processing factories and bases in China by the end of 2012, which reflects the overall development level of the enterprise network is low, and the organizational structure shows an inverted "T" type distribution. Heilongjiang, Zhejiang, Shandong and Sichuan provinces scale the highest level. Regional organizations are quite different on spatial distribution, while strong networking provinces mainly gather in the east coast, northeast and west regions, and due to the strength of the dominant organizational functions, provinces can be divided into seven types, such as headquarters-type, base-type, headquarters-processing type. Natural resources, geographical embeddedness, institutional environment, flexible production level and regional innovation capability are the five drivers of spatial organization, and measures can be taken to develop organic agriculture from four aspects of policy guidelines, strategic planning, marketing planning and technical guidance.

Abstract:
Introduction. Diffuse large B cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphomas (NHL). The use of chemoimmunotherapy (R-CHOP) in DLBCL has improved Overall Survival (OS). However, there are among 45-55% of patients who will die due to relapse, progression (refractoriness) or toxicity to treatment. Genomic classifications are difficult to use in clinical practice, especially in lain America, due to the need of trained personnel and cost. So, we continue using the International Prognostic Index (IPI) and its variations (e.g. revised-IPI, NCCN-IPI) for prognostic purposes. However, other biological variables have been reported to be prognostic, such as serum beta-2-microglobulin (B2M), lymphocyte/monocyte ratio (LMR), neutrophil/lymphocyte ratio (NLR), platelets/lymphocyte ratio (PLR) and serum albumin (SA). These factors have been associated with burden of disease, inflammation and nutritional status. Aim. Therefore, we performed a retrospective analysis of the databases of two Latin American groups to determine which biological variable is the most powerful factor prognostic of OS. Methods. A total of 1,250 patients were analyzed from two databases [(Grupo de Estudio para el Linfoma Mexicano (GELMEX) and the Grupo de Estudio para el Linfoma Latino Americano (GELL)], where 525 patients met the following inclusion criteria: DLBCL diagnosis by immunochemistry; complete data on absolute lymphocyte, absolute monocyte, absolute neutrophil and absolute platelet count, serum B2M and SA; and complete data on traditional variables for calculating risk groups for IPI, NCCN-IPI & R-IPI. The LMR, NLR and PLR was obtained. We evaluated the AUC of the biological variables and the differences among each one of them (including cut-off). Kaplan-Meier curves (KMC) were estimated and subsequently, the inference of OS was evaluated by Hazard Ratio (HR) Cox-regression in univariate/multivariate analyses by forward model. All variables with p<0.05 were considered independent variable for the multivariate analysis. Outcomes: Clinical characteristics of the cohort (n=525) were as follows: median follow-up time was 32 months (range: 0.2-132), 52% were male, 60% had more than 60 y/o (≥75 y/o, 21%), one third (32%) had a bad performance status (ECOG ≥2), 63% had advanced stage (III-IV), 49% had high serum lactate dehydrogenase (LDH; ratio 1.1-2.9, 41%; ratio ≥3, 8%), 21% had ≥2 extranodal sites involved (28%, NCCN-specific sites) and 43% had bulky disease. SA had the best AUC comparing with other variables (see figure 1). 5-y OS rates by SA (>3.2 mg/dL vs. ≤3.2 mg/dL) were 72% vs 34% (Log Rank p<0.0001), by B2M (<4 µg/dl vs. ≥4 µg/dL) were 68% vs. 45% (p<0.0001), by LMR (≥2.2 vs. <2.2) were 67% vs. 47% (p<0.0001), by NLR (<4 vs. ≥4) were 70% vs. 52% (p<0.0001) and by PLR (<475 vs. ≥475) were 65% vs. 44% (p<0.0001). In the univariate Cox regression analysis, SA ≤3.2 mg/dL was associated with HR 3.41 (CI95% 2.47-4.64), B2M ≥4 µg/dL with HR 2.13 (CI95% 1.57-2.88), LMR <2.2 with HR 2.11 (CI95% 1.56-2.87), NLR ≥4 with HR 1.92 (CI95% 1.45-2.55), and PLR ≥475 with HR 2.17 (CI95% 1.52-3.1)]. The multivariate Cox regression analyses (Forward model) showed that SA ≤3.2 mg/dL, B2M ≥4 µg/dL and high NLR ≥4 retained their prognostic value for worse OS (HR 2.87, CI95% 2.1-3.90; p=<0.0001; HR 1.40, CI95 1.02-1.94; p=0.036; and HR 1.43, CI95% 1.06-1.92; p=0.01, respectively). SA ≤3.2 mg/dL was the only independent factor associated with worse OS when adjusted by IPI (HR 2.53, CI95% 1.75-3.67; p<0.0001), NCCN-IPI (HR 2.51, CI95%1.81-3.5; p<0.0001) and R-IPI (HR 3.12, CI95%2.34-4.16; p<0.0001). Conclusion: Low SA emerges as an easy-to-use, important biological factor prognostic of worse survival, independent of the IPI, R-IPI and NCCN-IPI scores, in Latin American patients with DLBCL treated with chemoimmunotherapy. ABSTRACT BACKGROUND AND AIMS The efficacy of prophylactic central neck dissection (pCND) in patients with papillary thyroid carcinoma (PTC) is still unclear. The aim of this prospective randomized controlled study was to evaluate the clinical strengths and weaknesses of pCND. METHODS Between May 2009 and September 2015, a total of 1,134 patients with clinical N0 PTC were randomly assigned to two groups. Group A was treated with thyroidectomy alone and group B was treated with thyroidectomy + pCND. When the surgeon detected extrathyroidal extension or suspicious lymph nodes during the operation, frozen biopsy was performed to confirm the metastasis and the patients were excluded. We analyzed the clinicopathologic characteristics, postoperative complications, and recurrence. RESULTS Of the 1,134 patients, 110 were excluded because they had evidence of either capsular invasion or lymph node metastasis proven by frozen biopsy during the operation (rate of dropout: 9.7%). A total of 1,024 patients were included, 504 patients in group A and 520 patients in group B. In group B, 67 patients had ipsilateral central lymph node metastasis in frozen biopsy that were converted to total thyroidectomy. Clinicopathologic characteristics of the two groups were not significantly different. The median follow-up period was 20 months (1–77 months). Recurrence was detected in three patients (0.3%) in the lateral lymph node. Disease-free survival was not statistically significantly different between the two groups (p = 0.561). However, the incidence of temporary hypoparathyroidism in group B was significantly higher than that in group A (group A: 13.1%, group B: 20.8%, p = 0.001), while the permanent hypoparathyroidism, vocal cord palsy, chyle leakage, bleeding, and wound infection were similar between groups A and B. CONCLUSION Prophylactic central neck dissection did not reduce locoregional recurrence in clinical N0 PTC, but it significantly increased the incidence of transient hypoparathyroidism. But the assessment of effectiveness of pCND in reducing recurrence and mortality requires a long-term follow-up.

In this paper we examine the relevance of stocking rate research to the complex and highly variable ecosystems that make up most rangeland enterprises. We review a number of stocking rate experiments that have been conducted in both rangelands and more intensively grazed, improved pastures and demonstrate a fundamental difference in the nature of the stocking rate - animal production relationship between the two environments. Animal production in rangelands is less sensitive to increases in utilisation rate than in improved pastures, at least in the short to medium time frame of most experiments. These differences can largely be explained by factors relating to the much greater spatial and temporal variability of rangelands such as: inter-annual and seasonal fluctuations in vegetation composition and quality; long-term vegetation changes; and spatial and temporal patterns of diet selection in complex vegetation. Together, these factors limit the application of linear stocking rate models to complex rangeland environments for prediction of animal production responses. We suggest that dynamic simulation models, which incorporate the spatial and temporal variability of rangelands, may be the best way of developing simple but useful management principles for setting stocking rates that are more appropriate than simplified regression relationships.

<b><i>Background:</i></b> With increased awareness of type 2 diabetes (T2D) in children and adolescents, an overview of country-specific differences in epidemiology data is needed to develop a global picture of the disease development. <b><i>Summary:</i></b> This study examined country-specific prevalence and incidence data of youth-onset T2D published between 2008 and 2019, and searched for national guidelines to expand the understanding of country-specific similarities and differences. Of the 1,190 articles and 17 congress abstracts identified, 58 were included in this review. Our search found the highest reported prevalence rates of youth-onset T2D in China (520 cases/100,000 people) and the USA (212 cases/100,000) and lowest in Denmark (0.6 cases/100,000) and Ireland (1.2 cases/100,000). However, the highest incidence rates were reported in Taiwan (63 cases/100,000) and the UK (33.2 cases/100,000), with the lowest in Fiji (0.43 cases/100,000) and Austria (0.6 cases/100,000). These differences in epidemiology data may be partly explained by variations in the diagnostic criteria used within studies, screening recommendations within national guidelines and race/ethnicity within countries. <b><i>Key Messages:</i></b> Our study suggests that published country-specific epidemiology data for youth-onset T2D are varied and scant, and often with reporting inconsistencies. Finding optimal diagnostic criteria and screening strategies for this disease should be of high interest to every country. <b><i>Trial Registration:</i></b> Not applicable.

Objective – To determine how content-enriched catalogue records impact the circulation rates of print resources in four subject areas, and to investigate how this additional metadata influences OPAC searching and item retrieval. Design – Analysis of circulation data, bibliographic records, and OPAC search logs. Setting – A library at a four-year undergraduate residential college in the North-eastern United States. Subjects – Bibliographic records for 88,538 titles; data from 7,782 circulation transactions; and 130 OPAC search strings and related circulation data. Methods – In the first part of the study, bibliographic records for print items published since 1990 were extracted from the library’s integrated library system (ILS) in the following Library of Congress (LC) classes: D, E, F, H, J, L, P, Q, R, S, and T. It is assumed that electronic books were excluded from this study because their usage is not tracked in the ILS. These LC classes were chosen to correspond to the subject areas targeted by the researchers for comparison – “history, social sciences, language and literature, and science and technology” (p. 416). The data file included the publication date of the title, as well as values for the MARC fields identified by the researchers as containing content-enriched data. These fields were MARC 505 (an item’s table of contents or list of works included), MARC 520 (summaries or annotations), and MARC 856 (URL to electronic location of related material or electronic copy) (p. 416; Library of Congress Network Development and MARC Standards Office, 2003, 2008a, 2008b). The authors analyzed records for 88,538 titles and determined the total number of records containing each of the MARC fields either singly or in combination. Data relating to circulation transactions for items located in these LC classes from January to May 2009 was also identified. Like the bibliographic records, circulation data was pulled for print items only. The researchers identified 7,782 circulation transactions that met the study criteria for the period in question. In the second part of the study, circulation data for September 22, 2009 was obtained and sorted into the four subject categories identified in Part I of the study. The authors indicate that this date was chosen at random, but do not specify how. Researchers compared the records of the 133 titles borrowed that day from the LC classes studied to the OPAC search logs from September 16-22, 2009 to determine which searches led to the circulation of these items. The authors felt that searches resulting in checkouts on the day in question may have begun earlier in the week. The searches that led to borrowing were recorded and categorized as keyword, title, author, or other searches. If a user entered a title or author name into the keyword field, these were classed as known item searches in the appropriate categories. The authors identified and analyzed 130 searches relating to circulated items. Main Results – In the first part of the study, the number of catalogue records that contained MARC 505, 520, and/or 856 fields significantly increased for titles published between 1990 and 2007, with a slight decrease in 2008. MARC 505 was the most common content- enriched field until 2000, after which the presence of MARC 856 grew significantly. The MARC 520 field was used least often, making it difficult to draw firm conclusions about its impact on circulation. The incidence of enhanced records was very low among older books in the study. Only 14.3% of items published between 1990 and 1994, and 19.3% of items published between 1995 and 1999, had records that contained MARC 505, 520, or 856 fields. In contrast, the percentage of enhanced records was very high (80.9%) for items published between 2005 and 2008. The authors acknowledged that these stark imbalances created skewed comparison data for items published in these date ranges. As such, they suggested that the data for titles published between 2000 and 2004 offered the most balanced comparison because the numbers of enhanced and non-enhanced records were almost equal. The overall circulation of items with enhanced records published between 2000 and 2004 was 2.9% higher than for items with non-enhanced records, constituting a relative percentage difference of 30.7%. The relative percentage difference in this period was higher for books in science and technology (36.9%), followed by history (34%), language and literature (30.6%), and social sciences (25.7%). Enhanced records also had a positive impact on circulation for items published between 1990 and 2000 over their non-enhanced counterparts; however, this positive growth levelled off for items published between 2005 and 2008, with almost equal circulation rates between items with enhanced and non-enhanced records during this period. The impact of the three MARC fields was examined, and the presence of the MARC 505 field was most associated with increased circulation rates, in part because it was the most commonly used field of the three for the period in question. The number of records with MARC 520 and 856 fields was not sufficient to draw firm conclusions about their impact on circulation. While not the focus of the study, the circulation data also suggested a preference for current titles among all four subject areas, most significantly among the social sciences and science and technology. The second part of the study found that keyword searching was the most common strategy employed by patrons, with 49.6% of the 130 searches examined falling into this category. Keyword searches most commonly led to the borrowing of items from the history LC classes, while title searches were most common in science and technology. Known item searches (title or author) accounted for 45.9% of the overall searches analyzed. However, in most cases, the search terms used that led to a title circulating were found in the title and subject fields, rather than in a content-enriched MARC field. The researchers suggested that this may be due to the appearance of search results in the OPAC (brief rather than full record) and the way relevancy sorting was calculated, as contents notes were not given a high weighting in the OPAC’s formula. Conclusion – The study found that enhanced catalogue records led to higher circulation rates in the four subject areas studied. The increased proportion of content-enriched records in the overall catalogue in recent years suggested that their value had been recognized by the library. The limited role these enhanced fields played in the September 22, 2009 searches suggested that further work on improving how this information is displayed to users in the OPAC and sorted is needed. The researchers identified areas for future research including the role of the publication date and the impact of improvements to the display of content fields in the OPAC on the circulation of items with content-enriched records.

The opioid epidemic has been called the “most consequential preventable public health problem in the United States.” Though there is wide recognition of the role of prescription opioids in the epidemic, evidence has shown that heroin and synthetic opioids contribute to the majority of opioid overdose deaths. It is essential to reframe the preventive strategies in place against the opioid crisis with attention to factors surrounding the illicit use of fentanyl and heroin. Data on opioid overdose deaths shows 42,000 deaths in 2016. Of these, synthetic opioids other than methadone were responsible for over 20,000, heroin for over 15,000, and natural and semisynthetic opioids other than methadone responsible for over 14,000. Fentanyl deaths increased 520% from 2009 to 2016 (increased by 87.7% annually between 2013 and 2016), and heroin deaths increased 533% from 2000 to 2016. Prescription opioid deaths increased by 18% overall between 2009 and 2016. The Drug Enforcement Administration (DEA) mandated reductions in opioid production by 25% in 2017 and 20% in 2018. The number of prescriptions for opioids declined significantly from 252 million in 2013 to 196 million in 2017 (9% annual decline over this period), falling below the number of prescriptions in 2006. In addition, data from 2017 shows significant reductions in the milligram equivalence of morphine by 12.2% and in the number of patients receiving high dose opioids by 16.1%. This manuscript describes the escalation of opioid use in the United States, discussing the roles played by drug manufacturers and distributors, liberalization by the DEA, the Food and Drug Administration (FDA), licensure boards and legislatures, poor science, and misuse of evidencebased medicine. Moreover, we describe how the influence of pharma, improper advocacy by physician groups, and the promotion of literature considered peer-reviewed led to the explosive use of illicit drugs arising from the issues surrounding prescription opioids. This manuscript describes a 3-tier approach presented to Congress. Tier 1 includes an aggressive education campaign geared toward the public, physicians, and patients. Tier 2 includes facilitation of easier access to non-opioid techniques and the establishment of a National All Schedules Prescription Electronic Reporting Act (NASPER). Finally, Tier 3 focuses on making buprenorphine more available for chronic pain management as well as for medication-assisted treatment. Abstract Background Prognosis for patients with MAB lung disease is poor. We sought to examine the potential association between culture conversion and outcomes (progression, mortality) in patients with MAB lung disease. Methods English-language MAB lung disease studies with ≥ 10 patients and reporting mortality and/or microbiological outcomes were identified from Embase, PubMed, relevant congress abstracts, and the Cochrane Library (data cutoff, September 24, 2019) using the National Institute for Health and Clinical Excellence guidance for systematic literature reviews. Two independent reviewers screened 1,551 indexed records; relevant extracted data are expressed as population-weighted means. Results Mean all-cause mortality across 17 studies (N=1,291) was 12.1% (range, 3%–33%); mortality attributable to MAB lung disease was 7.6% (range, 0%–27%; N=526, 9 studies). Culture conversion across 44 studies (N=2,237) was 46.7% (range, 0%–98.6%), with higher rates reported for M. massiliense subspecies (76.9%; N=507,15 studies) than M. abscessus subspecies (35.8%; N=834,18 studies). No direct comparisons were made between mortality and culture conversion; in the 13 studies (N=1,202) that reported both outcomes there was a moderate correlation between increased rate of culture conversion and decreased MAB-attributable mortality (R2=0.60). The most common definition of progression (21 studies) was radiographic worsening supported by persistent symptoms and/or positive cultures. Across 8 studies (N=415) 57.8% patients had improvement while 35.2% progressed with treatment. A broad variance in treatment regimen and duration (range, 32 days to &gt; 3 years) was observed. Limitations include a small number of studies, and inconsistency in methods and outcomes definitions. Conclusion In this systematic literature review, available data suggest that culture conversion was achieved in less than half of patients and was lower in patients with M. abscessus compared with M. massiliense. One third of patients had disease progression despite treatment. Tooling cost is a major contributor to the total cost of small-lot production of sheet metal components. Within the framework of an academic/industrial/government partnership devoted to the development of a reconfigurable tool for stretch forming, we have implemented a Finite Element-based procedure to determine optimal die shape. In the reconfigurable forming tool (Hardt, D. E. et al., 1993, “A CAD Driven Flexible Forming System for Three-Dimensional Sheet Metal Parts,” Sheet Metal and Stamping Symp., Int. Congress and Exp., Detroit, MI, SAE Technical Paper Series 930282, pp. 69–76.), the die surface is created by the ends of an array of square pins, which can be individually repositioned by computer driven servo-mechanisms. An interpolating polymer layer is interposed between the part and the die surface to attain a smooth pressure distribution. The objective of the die design algorithm is to determine optimal positions for the pin array, which will result in the desired part shape. The proposed “spring-forward” method was originally developed for matched-die forming (Karafillis, A. P., and Boyce, M. C., 1992, “Tooling Design in Sheet Metal Forming using Springback Calculations,” Int. J. Mech. Sci., Vol. 34, pp. 113–131.; Karafillis, A. P., and Boyce, M. C., 1996, “Tooling And Binder Design for Sheet Metal Forming Processes Compensating Springback Error,” Int. J. Tools Manufac., Vol. 36, pp. 503–526.) and it is here extended and adapted to the reconfigurable tool geometry and stretch forming loading conditions. An essential prerequisite to the implementation of the die design procedure is the availability of an accurate FE model of the entire forming operation. The particular nature of the discrete die and issues related to the behavior of the interpolating layer introduce additional challenges. We have first simulated the process using a model that reproduces, as closely as possible, the actual geometry of the discrete tool. In order to optimize the delicate balance between model accuracy and computational requirements, we have then used the information gathered from the detailed analyses to develop an equivalent die model. An automated algorithm to construct the equivalent die model based on the discrete tool geometry (pin-positions) is integrated with the spring-forward method, to generate an iterative die design procedure that can be easily interfaced with the reconfiguring tool. The success of the proposed procedure in selecting an optimal die configuration is confirmed by comparison with experimental results.

Abstract:
Abstract Background: SWOG S0777, a randomized phase III trial, compared bortezomib, lenalidomide and dexamethasone (VRd) with lenalidomide and dexamethasone (Rd). The primary end point was progression-free survival (PFS) using a pre-specified one-sided stratified log rank test at a significance level of 0.02. The stratification factors were International Staging System (ISS) stage (I, II or III) and intent to transplant (yes or no), among a total of 6 strata. Overall response rate (ORR), overall survival (OS) and safety were secondary end points. Results of the primary analysis of this trial were published in Lancet (2016, Volume 389, Issue 10068, 519 - 527). Methods: This updated analysis includes 460 patients evaluable for survival endpoints: 225 eligible and analyzable patients were randomized to Rd and 235 patients to VRd. Rd patients received lenalidomide 25 mg/day on days 1-21 and dexamethasone 40 mg/day on days 1, 8, 15 and 22 of a 28-day cycle. VRd patients received lenalidomide 25 mg/day on days 1-14 and dexamethasone 20/mg/day on days 1, 2, 4, 5, 8, 9, 11 and 12 plus bortezomib 1.3 mg/m2 IV push on days 1, 4, 8 and 11 of a 21-day cycle. All patients received aspirin 325 mg/day and VRd patients received VZV prophylaxis per institutional standard. The 6-month induction was six 28-day cycles of Rd and eight 21-day cycles of VRd followed by Rd maintenance for all patients until progression, unacceptable toxicity or withdrawal of consent. Results: Between 2008 and 2012, 525 patients from 48 institutions were randomized. Patient data have been rigorously updated for this analysis based upon a May 15, 2018 datalock. The median follow up is now 84 months (7 years). The median PFS is 41 months for VRd and 29 months for Rd: stratified hazard ratio (96% Wald Confidence Interval) was 0.742 (0.594, 0.928) and one-sided stratified log-rank P-value 0.003 (see Figure 1). The median OS for VRd is still not reached with median OS for Rd being 69 months: stratified hazard ratio (96% Wald Confidence Interval) was 0.709 (0.543, 0.926) and stratified two-sided P-value was 0.0114 (see Figure 2). The impact of age was assessed in several ways including using 3 cutoffs (<65 years; 65-75 years; >75 years) and multivariate cox proportional hazards regression analyses adjusted for age. As in prior analyses, both PFS and OS were improved with VRd versus Rd adjusting for age (P-values: 0.013 [PFS]; 0.033 [OS])). Depth of response was assessed incorporating new serial data and additional bone marrow results. The CR plus VGPR was 74.9% for VRd versus CR plus VGPR of 53.7% for Rd (P-value 0.006 for response differences using a stratified Cochran-Mantel- Haenszel analysis). The median duration of lenalidomide plus dexamethasone maintenance was 17.4 months. The number of second cancers was 19/235 (8%) with VRd and 16/225 (7%) with Rd. Conclusion: The addition of bortezomib to lenalidomide dexamethasone for induction therapy in previously untreated myeloma results in a statistically significant and clinically meaningful improvement in PFS as well as better OS with follow up of 7 years. VRd had an acceptable safety and tolerability profile and continues to represent an appropriate standard of care irrespective of age.
