Abstract:
Background and aims: Oral mucosa is prone to various lesions. Some of these lesions can be early signs of a particular disease or even malignancy in the body. The aim of this study was to investigate the frequency distribution of oral biopsies from 2009 to 2019. Methods: In this analytical descriptive cross-sectional study, 621 records of the oral cavity biopsies performed in the pathology department of Shahid Sadoughi hospital were analyzed. Age, gender, type of lesion, site of lesion, histopathological diagnosis, year of referral, and medical specialist who performed the biopsy were included in the study checklist. The data were analyzed using chi-square and Kruskal-Wallis test in SPSS version 25.0. Results: Of 621 biopsies, 308 specimens belonged to men (49.6%) and 313 specimens belonged to women (50.4%). The mean age of patients was 46.58±21.08 years, ranging from 6 months to 95 years. The most affected areas were the tongue (33%), lower lip (29.1%) and cheeks (17.7%). squamous cell carcinoma (SCC) (27.9%), inflammatory lesions (26.1%), mucocele (14.8%), and ulcers (8.5%) were the most frequent lesions. There was a significant difference in the age of individuals with different lesions (P<0.001). Conclusion: The most frequent lesion diagnosed in biopsies was SCC. The most frequently affected sites were tongue and cheek. Lesions were more frequent in women and were most frequently seen in the fourth, fifth, and sixth decades of life.

ImportanceTelemedicine can increase access to care, but uptake has been low among people living in rural areas. The Veterans Health Administration initially encouraged telemedicine uptake in rural areas, but telemedicine expansion efforts have broadened since the COVID-19 pandemic.ObjectiveTo examine changes over time in rural-urban differences in telemedicine use for primary care and for mental health integration services among Veterans Affairs (VA) beneficiaries.Design, Setting, and ParticipantsThis cohort study examined 63.5 million primary care and 3.6 million mental health integration visits across 138 VA health care systems nationally from March 16, 2019, to December 15, 2021. Statistical analysis took place from December 2021 to January 2023.ExposuresHealth care systems with most clinic locations designated as rural.Main Outcomes and MeasuresFor every system, monthly visit counts for primary care and mental health integration specialties were aggregated from 12 months before to 21 months after pandemic onset. Visits were categorized as in person or telemedicine, including video. A difference-in-difference approach was used to examine associations in visit modality by health care system rurality and pandemic onset. Regression models also adjusted for health care system size as well as relevant patient characteristics (eg, demographic characteristics, comorbidities, broadband internet access, and tablet access).ResultsThe study included 63 541 577 primary care visits (6 313 349 unique patients) and 3 621 653 mental health integration visits (972 578 unique patients) (6 329 124 unique patients among the cohort; mean [SD] age, 61.4 [17.1] years; 5 730 747 men [90.5%]; 1 091 241 non-Hispanic Black patients [17.2%]; and 4 198 777 non-Hispanic White patients [66.3%]). In fully adjusted models for primary care services before the pandemic, rural VA health care systems had higher proportions of telemedicine use than urban ones (34% [95% CI, 30%-38%] vs 29% [95% CI, 27%-32%]) but lower proportions of telemedicine use than urban health care systems after pandemic onset (55% [95% CI, 50%-59%] vs 60% [95% CI, 58%-62%]), signifying a 36% reduction in the odds of telemedicine use (odds ratio [OR], 0.64; 95% CI, 0.54-0.76). The rural-urban telemedicine gap was even larger for mental health integration (OR, 0.49; 95% CI, 0.35-0.67) than for primary care services. Few video visits occurred across rural and urban health care systems (unadjusted percentages: before the pandemic, 2% vs 1%; after the pandemic, 4% vs 8%). Nonetheless, there were rural-urban divides for video visits in both primary care (OR, 0.28; 95% CI, 0.19-0.40) and mental health integration services (OR, 0.34; 95% CI, 0.21-0.56).Conclusions and RelevanceThis study suggests that, despite initial telemedicine gains at rural VA health care sites, the pandemic was associated with an increase in the rural-urban telemedicine divide across the VA health care system. To ensure equitable access to care, the VA health care system’s coordinated telemedicine response may benefit from addressing rural disparities in structural capacity (eg, internet bandwidth) and from tailoring technology to encourage adoption among rural users.

3013 Background: ABBV-621 is a potent tumor-necrosis factor-related apoptosis-inducing ligand (TRAIL) receptor agonist fusion protein that induces apoptotic cell death, particularly in DR4/5 expressing tumor models. Methods: Patients (pts) with previously treated solid tumors and ECOG 0–2 were administered ABBV-621 (2.5–15 mg/kg IV) on day (D) 1 (dose level [DL] 1) or D1D8 (DL2 and beyond) of each 21-day cycle. Dose escalation (DE) was guided by a Bayesian continual reassessment method. In addition to PK studies, blood-based PD markers of apoptosis (M30, M65) and drug binding were assessed. Results: As of 14 December 2018, 57 pts were enrolled in the DE portion, of which 30% had pancreatic, 23% colorectal cancer, and 47% other tumor types; 13 were KRAS mutant. Median age was 61 yrs. 60% were male; pts had a median of 4 prior regimens (range 1–10). Pts per DL: 2.5 (5 on D1, 16 on D1D8), 3.75 (12), 5 (6), 6.5 (6), 8.5 (4), 11 (4), and 15 mg/kg (4). Median duration of ABBV-621 exposure was 2 cycles (range 1–11). Seven pts had dose-limiting toxicities: respiratory failure (5 mg/kg; Grade 5, the only treatment-related death), blood bilirubin increased (3.75, 6.5 mg/kg), nausea (3.75 mg/kg), fatigue (3.75 mg/kg), increased ALT (2.5, 3.75, 6.5, 15 mg/kg), and increased AST (6.5 mg/kg). Summary of AEs is shown in Table. Clinical trial information: NCT03082209. A partial response (duration 20 weeks) was observed in a pt with pancreatic cancer (2.5 mg/kg D1D8). 27 pts had stable disease (6 pts for > 12 weeks). ABBV-621 PK was linear (mean ± SD clearance was 1.79 mL/h/kg ± 0.44) with a terminal half-life of 36.7 ± 5.55 h (n = 49). ABBV-621 bound to decoy receptors on neutrophils for up to 168 h; the duration of binding was dose-dependent. M30 and M65 increased at 8, 24, and 48 h following ABBV-621, but effect was independent of dose. Conclusions: ABBV-621 shows evidence of antitumor activity and effect on blood-based markers of apoptosis, with acceptable toxicity (MTD not reached). NCT03082209.[Table: see text]

The research is concerned about studying the absorption spectrum of the solution coumarin dye C47. The chloroform solvent was used with C47 dye in three different concentrations 10-4, 10-5 and 10-6 M. The laser dye solution was prepared by dissolving the required amount of dye in chloroform alcohol, while studying absorption spectrum before and after irradiation with gamma ray by cobalt-60 source 60Co at exposure time, which are 0, 4, 6 and 18 hours with different absorbed doses 0, 136, 204 and 612 Gy. The results show that red shift in the absorption spectrum was increased by increasing the concentration of laser dye solutions , while the increase of gamma dose led to increase the red shift after irradiation, as the exposure period and irradiation dose increased from gamma ray at 60Co source. Furthermore, the decrease in the concentrations of laser dye solutions shifts the absorption spectrum peak towards the short wavelength blue shift.

В условиях Краснодарского края в 2021–2022 годах проводились исследования по применению 6-фурфуриламинопурина (0,075 г/л) в трёх дозах (0,5, 0,75 и 1,0 л/га) на землянике садовой сорта Клери, в результате которых было выявлено положительное влияние препарата как на биометрические показатели растений и элементы структуры урожая, так и на количество и качество полученной продукции. Отмечено, что значительный прирост по элементам структуры урожая был получен при обработке регулятором роста растений на основе 6-фурфуриламинопурина в норме 0,75 л/га. Так, увеличение среднего количества ягод на цветоносе в 2021–2022 годах составляло 31,1%. Масса ягод земляники возросла на 6,1–13,7% при среднем значении в контрольном варианте 14,37 г. Полученные в опытных вариантах ягоды отличались лучшими вкусовыми качествами и более сбалансированным биохимическим составом. Так, сумма сахаров в ягодах земляники садовой в вариантах с применением 0,075 г/л 6-фурфуриламинопурина возросла на 3,3–8,4%, а сумма сухих веществ — на 3,4–7,9% по сравнению с вариантом без обработки регулятором роста растений. Содержание витамина С в ягодах земляники увеличилось на 2,2–3,8% при 76,4 мг/100 г в контрольном варианте. Также выявлено положительное влияние регулятора роста на урожайность земляники. Максимальная прибавка урожая за 2 года была получена при применении препарата в дозе 0,75 л/г, она составила 0,5 т/га. Опрыскивание рассады земляники садовой сорта Клери через 2 недели после высадки 6-фурфуриламинопурином (0,075 г/л) благоприятно сказалось на росте и развитии культуры, улучшении продуктивности и качественных показателей ягод земляники, а также способствовало лучшей приживаемости высаженной рассады и снижению стресса у растений This article reports on the application of three rates (0.5, 0.75 and 1.0 l ha-1) of 6-furfurylaminopurine (0.075 g/l) when growing strawberry (Kleri) in the Krasnodar region in 2021–2022. A positive effect was indicated on plant biometry, productivity as well as product quantity and quality. The growth stimulator based on 6-furfurylaminopurine (0.75 l ha-1) significantly improved plant productivity. The number of strawberries per peduncle grew by 31.1% in 2021–2022. The mass of strawberries increased by 6.1–13.7% under the average value of 14.37 g in a control. The application of the growth stumulator resulted in the product of better taste with more balanced biochemistry. Treating strawberries with 0.075 g/l of 6-furfurylaminopurine led to the increase of sugars by 3.3–8.4%, dry matter — by 3.4–7.9%. The content of vitamin C grew by 2.2–3.8% versus 76.4 mg/100 g in the control. The growth stimulator had positive impact on strawberry productivity. The highest yield increase of 0.5 l ha-1 was observed under the application of 0.75 l ha-1 of the preparation. Spraying strawberry with 6-furfurylaminopurine (0.075 g/l) 2 weeks after planting improved plant growth, development, productivity, quality, and stress-resistance.

To determine the role of endothelin-1 (ET-1) and its receptors in the regulation of calcitonin gene-related peptide (CGRP) release, male Wistar rats were divided into six groups and subjected to the following treatments for 1 wk with or without ABT-627 (an ETA receptor antagonist, 5 mg·kg−1·day−1 in drinking water) or A-192621 (an ETB-receptor antagonist, 30 mg·kg−1·day−1 by oral gavage): control (Con), ET-1 (5 ng·kg−1·min−1 iv), Con + ABT-627, Con + A-192621, ET-1 + ABT-627, and ET-1 + A-192621. Baseline mean arterial pressure (MAP, mmHg) was higher ( P < 0.05) in Con + A-192621 (122 ± 4) and ET-1 + A-192621 (119 ± 4) groups compared with Con (104 ± 6), ET1 (106 ± 3), Con + ABT-627 (104 ± 3), and ET1 + ABT-627 (100 ± 3) groups. Intravenous administration of CGRP8–37 (a CGRP receptor antagonist, 1 mg/kg) increased MAP ( P < 0.05) in ET-1 (13 ± 1), Con + A-192621 (12 ± 1), and ET-1 + A-192621 (15 ± 3) groups compared with Con (4 ± 1), Con-ABT-627 (4 ± 1), and ET-1 + ABT-627 (5 ± 1) groups. Plasma CGRP levels (in pg/ml) were increased ( P < 0.05) in ET-1 (57.5 ± 6.1), Con + A-192621 (53.9 ± 3.4), and ET-1 + A-192621 (60.4 ± 3.0) groups compared with Con (40.4 ± 1.6), Con + ABT-627 (40.0 ± 2.9), and ET-1 + ABT-627 (42.6 ± 1.9) groups. Plasma ET-1 levels (in pg/ml) were higher ( P < 0.05) in ET-1 (2.8 ± 0.2), ET-1 + ABT-627 (3.2 ± 0.4), Con + A-192621 (3.3 ± 0.4), and ET-1 + A-192621 (4.6 ± 0.3) groups compared with Con (1.1 ± 0.2) and Con-ABT-627 (1.3 ± 0.2) groups. Therefore, our data show that ET-1 infusion leads to increased CGRP release via activation of the ETA receptor, which plays a compensatory role in preventing ET-1-induced elevation in blood pressure.

BackgroundThe interrater reliability of 2 new inpatient functional short-form measures, Activity Measure for Post-Acute Care (AM-PAC) “6-Clicks” basic mobility and daily activity scores, has yet to be established.ObjectiveThe purpose of this study was to examine the interrater reliability of AM-PAC “6-Clicks” measures.DesignA prospective observational study was conducted.MethodsFour pairs of physical therapists rated basic mobility and 4 pairs of occupational therapists rated daily activity of patients in 1 of 4 hospital services. One therapist in a pair was the primary therapist directing the assessment while the other therapist observed. Each therapist was unaware of the other's AM-PAC “6-Clicks” scores. Reliability was assessed with intraclass correlation coefficients (ICCs), Bland-Altman plots, and weighted kappa.ResultsThe ICCs for the overall reliability of basic mobility and daily activity were .849 (95% confidence interval [CI]=.784, .895) and .783 (95% CI=.696, .847), respectively. The ICCs for the reliability of each pair of raters ranged from .581 (95% CI=.260, .789) to .960 (95% CI=.897, .983) for basic mobility and .316 (95% CI=−.061, .611) to .907 (95% CI=.801, .958) for daily activity. The weighted kappa values for item agreement ranged from .492 (95% CI=.382, .601) to .712 (95% CI=.607, .816) for basic mobility and .251 (95% CI=.057, .445) to .751 (95% CI=.653, .848) for daily activity. Mean differences between raters' scores were near zero.LimitationsRaters were from one health system. Each pair of raters assessed different patients in different services.ConclusionsThe ICCs for AM-PAC “6-Clicks” total scores were very high. Levels of agreement varied across pairs of raters, from large to nearly perfect for physical therapists and from moderate to nearly perfect for occupational therapists. Levels of agreement for individual item scores ranged from small to very large.

Abstract:
Spent oil based drilling fluid and cutting wastes are global liabilities due to their hazardous hydrocarbon content which impacts negatively on flora, fauna, and global carbon footprint. The formulation of two demulsifiers to ensure chemically enhanced phase separation of this waste into oil, water and solid components was successfully carried out in addition to recycling the solid phase into PA6 nanocomposite materials. Initial characterisation of the untreated waste was carried out by Fourier Transform Infra Red (FTIR) for total petroleum hydrocarbon (TPH) analysis, Inductively coupled plasma optical emission spectrometry (ICPOES) for quantitative elemental analysis and Energy dispersive xray analysis (EDXA) for qualitative elemental composition amongst other characterisation methods. The analysis showed that the sample had a high hydrocarbon load of 662,500mg/kg and a high heavy metal load for Pb of 122mg/kg. No As, Cd, Hg were detected. The demulsifier formulations were composed of isopropanol, sodium dodecyl sulphate, poloxamer, sodium chloride, chitosan in 0.2M acetic acid and deionised water for demulsifier S4 and addition of phosphoric acid for demulsifier S3. Hydrocarbon reduction on the extracted solid phase nanofiller S3 and nanofiller S4 was 98.6% and 98.5% respectively after demulsification. The demulsified spent oil based drilling fluid solid extracts were below OSPAR regulation of < 1% oil on cutting by weight. However, recycling of the recovered solid was carried out in order to achieve environmentally sustainable management of the waste in Polyamide 6 (PA6) nanocomposite manufacture/fabrication. The formulation of different blends of PA6 nanocomposite materials from untreated, demulsifier treated and thermally treated drilling fluid and cuttings was successfully achieved. Nanocomposite leaching test showed Pb immobilisation. The flexural and compressive - modulus and strength of the PA6 were markedly improved in the presence of the nanofillers and glass fibre. This was attributed to the reinforcement, exfoliating, stiffening, rigidity effect of the nanofillers. S6 (untreated drilling fluid) nanofillers significantly improved the mechanical properties of PA6. This was attributed to the increased interfacial bonding between the fillers and the polymer matrix as a result of the petroleum hydrocarbon present in the sample. The Thermogravemetric analysis (TGA) results showed that nanocomposites PA6/S3 and PA6/S3/GF30 had improved the thermal stability of PA6 by 13.6% and 38.8% respectively compared to PA6/S2 and PA6/S2/GF30 (simulated commercial nanocomposite materials) that improved PA6 by 9.7% and 35.8% respectively.

AbstractThe second ASCL conference was held in February 2020, about 1 month after the first case of coronavirus disease 2019 (COVID-19) was documented in the United States and before evidence of community spread. Since then, COVID-19 has become a global pandemic that has disproportionately infected, hospitalized, and killed ethnic and racial minorities in the United States (Jacobson et al., Am J Prev Med 61(5):644–651, 2021; Moore et al., MMWR Morb Mortal Wkly Rep 69:1122–1126, 2020; Shiels et al., Ann Intern Med. https://doi.org/10.7326/m21-2134, 2021; American Cancer Society, Cancer facts & figures 2021. American Cancer Society, Atlanta, 2021). According to a report of provisional life expectancy estimates from the National Center for Health Statistics, the life expectancy for US Hispanics decreased 3 years (81.8–78.8) between 2019 and 2020. As a result, the life expectancy advantage held by Hispanics over non-Hispanic whites narrowed by 60%, suggesting poorer health and mortality outcomes for the US Hispanic population. It is estimated that 90% of this decline in the life expectancy gap is the result of mortality caused by COVID-19 (Arias et al., Provisional life expectancy estimates for 2020. National Center for Health Statistics, Hyattsville, 2021). Some speculate that this disease disparity exists because Hispanics and other underrepresented groups receive the greatest exposure to the virus. They are more likely to live in densely populated areas and multigenerational households; use public transportation; and have essential public-facing jobs in the service and healthcare sectors, where working from home is not an option (American Cancer Society, Cancer facts & figures 2021. American Cancer Society, Atlanta, 2021; Hooper et al., JAMA 323(24):2466–2467, 2020; Balogun et al., JAMA Oncol 6(10):1531–1532, 2020). If infected, they are also more likely to experience severe symptoms of COVID-19 because of comorbidities such as diabetes, obesity, cardiovascular disease, asthma, and other chronic conditions (Hooper et al., JAMA 323(24):2466–2467, 2020).

Abstract:
Terrorism is a pain point with serious social, economic, legal or political consequences in Turkey and in the world. Aside from the aforementioned consequences and the atmosphere of fear, anxiety and uncertainty they create, they also have a devastating effect on the violation of fundamental human rights. When the emergence of terrorism and terrorist organizations is evaluated, it is seen that they have their own “legitimate” social, economic, legal or political justifications. Therefore, while terrorist organizations create their own legitimate, they instrumentalize ideological, economic, cultural, ethnic or religious issues without any legal or ethical boundaries. In terms of criminal law, this instrumentalization activity has an important place in the emergence of the concepts of crime and criminality. Apart from struggling the crimes committed by the organization in order to achieve its goals, planned and systematic propaganda activities constitute an important part of this struggle. In the research within this framework articles 7/2 and 6/2 of the Anti-Terror Law No. 3713 will be evaluated.

El objetivo de la investigación es determinar si la identidad/imagen que posee Ciudad Victoria, Tamaulipas favorece la creación de la marca Ciudad Victoria, en Tamaulipas México, por medio del hexágono de Anholt. Para ello, se aplica un cuestionario a 315 ciudadanos, para el método, se utiliza un análisis factorial por medio del software estadístico SPSS. Los resultados muestran que los 6 elementos del hexágono de Anholt, son elementos de suma importancia y de forma representativa para el ciudadano, en la creación de la marca de Cuidad Victoria.

In the settlement of Gorodnaya, at the beginning of 2020, 71 patients with thyroid diseases (nodular goiter, postoperative hypothyroidism, thyrotoxicosis, hypothyroidism) were registered, including 8 patients for the first time detected in 2019. According to the 2020 study, 7 people fell ill with thyroid cancer in the village - 1 man and 6 women, which amounted to 14.3% and 85.7%, respectively. The highest incidence rates were registered among men aged 51-60 years, and among women - in the age groups 51-60 years old and 61-70 years old. At the age of 21-30 years, thyroid gland pathology was registered in one person.

With a large capacity thermal storage system using phase change material (PCM), Concentrated Solar Power (CSP) is a promising technology for high efficiency of solar energy utilization. In a thermal storage system, a dual-media thermal storage tank is typically adopted in industry for the purpose of reducing the use of the heat transfer fluid (HTF). While the dual-media sensible heat storage system has been well studied, a dual-media latent heat storage system (LHSS) still needs more attention and study; particularly, the sizing of volumes of storage tanks considering actual operation conditions is of significance. In this paper, a strategy for LHSS volume sizing is proposed, which is based on computations using an enthalpy-based 1D model. One example of 60MW solar thermal power plant with 35% thermal efficiency is presented. In the study, potassium hydroxide (KOH) is adopted as PCM and Therminol VP-1 is used as HTF. The operational temperatures of the storage system are 390°C and 310°C, respectively for the high and low temperatures. The system is assumed to operate for 100 days with 6 hours charge and 6 hours discharge every day. From the study, the needed height of the thermal storage tank is calculated from using the strategy of tank sizing. The method for tank volume sizing is of significance to engineering application.

Wear of materials is a serious problem facing industry especially in mechanical applications where moving parts are continuously subjected to friction. Hard coatings prepared by a variety of processes are nowadays considered as effective solutions to protect components against wear. Examples of such processes are: thermal spray coating, vacuum-based coating and hardfacing. In this paper, we study the mechanical, tribological and corrosion properties of two hard coating systems: CoCr Stellite 6 (ST6) hardfacing on 316 stainless steel and NiWCrB Colmonoy 88 (C88) thermal spray coating on Inconel 718. The effect of gas nitriding on the microstructure and wear performance of these coating systems is investigated. X-ray diffraction, energy dispersive spectroscopy and scanning electron microscopy were used for microstructural analysis. Micro-indentation technique was utilized to measure the surface and cross-sectional hardness of the coatings. Rockwell indentation technique was used to evaluate coating adhesion in accordance with CEN/TS 1071-8. Pin-on-disk tests were conducted to assess the tribological performance of the coatings. Microstructural analysis showed that ST6 has a cobalt matrix in the form of dendrites reinforced with metal carbide particles whereas C88 has a Nickel matrix reinforced mainly with metal boride particles. ST6 and C88 improved significantly the wear resistance of their corresponding substrates. This is mainly due to good adhesion and high hardness of the coatings; HR15N values of ST6 and C88 were almost 85 as compared to 61 and 80 for 316 and INC substrates, respectively. ST6 was found to improve significantly the corrosion resistance of 316 whereas C88 decreased the corrosion performance of INC. Moreover, nitriding treatment was found to improve significantly the wear resistance of 316 and INC, however, in the case of ST6, nitriding was beneficial in terms of wear resistance only at relatively low load.

Abstract:
ЦЕЛЬ: изучить коморбидность больных с сахарным диабетом 2 типа (СД2), получающих лечение по по- воду переднего острого инфаркта миокарда (ОИМ) с зубцом Q. МАТЕРИАЛЫ И МЕТОДЫ: в исследование вошли 90 пациентов с передним ОИМ с зубцом Q, получаю- щие гликлазид (n=30, 1-группа), глимепирид (n=30, 2 группа) и глибенкламид (n=30, 3 группа). Изучались липидограмма, гликированный гемоглобин (HbA1C), креатинин, альбуминурия, осматривались неврологом, окулистом, рассчитывался индекс массы тела (ИМТ). РЕЗУЛЬТАТЫ: мужчин было 43 (47,7%), а женщин 47 (52,3%). Возраст мужчин составлял 60±9 [57-62] лет, женщин 53±8 [50-55]. В 1 группе ИМТ составил 30,4±2,1[29,6-31,1] кг/м2, во 2 группе ИМТ 31,6±2,3 [30,7-32,4], а в 3 группе- 31,2±3,9 [29,8-32,6] кг/м2. При проведении анализа корреляционной взаимосвязи ИМТ от стажа СД2 была установлена высокой тесноты обратная связь (р<0,001, rxy= -0,782). При увеличении стажа СД2 на 1год следует ожидать уменьшение ИМТ на 0,402. Полученная модель объясняет 61,2% наблюдаемой дисперсии. При оценке связи ИМТ и возраста была установлена умеренной тесноты обратная связь. При увеличении возраста на 1 следует ожидать уменьшение показателя ИМТ на 0,319. При оценке связи ИМТ и дозы препарата сульфонилмочевины (ПСМ) связь отсутствовала. При увеличении дозы ПСМ на 1 следует ожидать увеличение ИМТ на 0,019. У всех пациентов регистрировалась дислипидемия: ЛПНП в 1 груп- пе 3,2±0,4[3,06-3,34], во 2 группе 3,2±0,6 [2,99 -3,41], в 3 группе 3,3±0,7 [3,05-3,55]; холестерин в 1 группе 5,7±1,0 [5,34-6,06], в 2 группе 5,9±0,9 [5,58-6,22], в 3 группе 6,1±0,9 [5,7-6,42]. У всех больных была выявлена артериальная гипертензия (АГ). Во всех группах диагностировались хроническая болезнь почек (ХБП). В 1 группе было с ХБП С2А1 5 пациентов (16,7%), 17 больных с ХБП С3а (А1-А2) (56,7%) и 8- ХБП С3б (А1-А2) (26,7%). Во 2 группе ХБП С2А1 3,3% (n=1), ХБП С3а (А1-А2) 86,7% (n=26), ХБП С3б (А1-А2) 10% (n=3). В 3 группе ХБПС2А1 40%(n=12), ХБП С3аА1-А2 36,7%(n=11), ХБП С3бА1-А2 23,3% (n=7). Из 90 пациентов со скоростью клубочковой фильтрации (СКФ) более 50мл/мин/1.73 м2 было 14, а с менее 50 мл/мин/1.73 м2-16 больных. Чем больше стаж СД2, тем меньше СКФ был. Был проведен анализ для выявления связи между СКФ и дли- тельности СД2. Исходя из полученных данных при сопоставлении показателя стажа СД более 6 лет и по- казателя СКФ менее 50 мл/мин/1.73 м2, были установлены статистически значимые различия (p = 0,004, p < 0,001). Анализ показал, шанс иметь СКФ меньше 50 при наличии СД2 более 6 лет были выше в 37,500 раз, по сравнению с СД менее 6 лет, различия шансов были статистически значимыми (95% ДИ: 3,638 – 386,513). HbA1C составил 9,3±1,2 [9,1-9,6]. Корреляционный анализ показал, что при увеличении стажа СД2 на 0,446 следует ожидать увеличение HbA1C на 1. При увеличении HbA1C на 1 следует ожидать увеличение СКФ на 1,007. При увеличении HbA1C на 1 следует ожидать уменьшение ИМТ на 0,518. При осмотре глазного дна выявились в 1 группе 80% (n=24) больных с ретинопатией, во 2 группе 80 % (n=24), в 3 группе 66,6 % (n=20). Всем больным был выставлен диагноз диабетическая полинейропатия. ВЫВОДЫ: больные с СД2 и передним ОИМ с зубцом Q имели осложнения и заболевания, которые говорили о наличии и риске значимых сердечно-сосудистых осложнений.

Abstract The primary goal of this project was to prove the feasibility of a Cyclic Gas Injection/Production (Huff & Puff) enhanced oil recovery (EOR) scheme and evaluate its economic viability in the Viking formation in Saskatchewan, Canada. Historically, the Viking formation has challenging waterflood economics due to limited injectivity into the tight sand reservoir and formation damage caused by movable and swelling clays. The objectives were to test horizontal well injectivity at desired pressures, assess the use of produced gas injection to repressurize the extremely depleted Viking reservoir, evaluate enhanced oil recovery with various inter-well spacings, and to optimize injection and production cycle lengths. This pilot was designed by utilizing analytical evaluation and numerical simulation. The Huff & Puff pilot consists of four 0.7 mile (1100 m) horizontal wells with two different inter-well spacings; 667 ft vs 1333 ft (200 m vs 400 m). The pilot consisted of two parallel horizontal well pairs, which were designed to alternate injection and production cycles. A detailed reservoir surveillance program was implemented to collect extensive data, including reservoir and wellhead pressures, gas composition analysis, production and injection rates of the pilot wells, and the production rates of surrounding wells to evaluate reservoir response and pilot performance. Whitecap has been operating this pilot project since its initiation in September 2021 with technical success. Six Huff & Puff cycles with varying durations, between one to three months on injection, have been completed without major operating issues. Per-well injectivity has reached higher than designed 750 Mscf/d (21 e3m3/d) at desired surface pressures. Through the course of six injection cycles, average reservoir pressure has increased from an initial 174-319 psi (1.2-2.2 MPa) to 623-885 psi (4.3-6.1 MPa). The incremental oil recovery factor for the pilot area is expected to be 1%, which is considerable for a reservoir with typical primary recovery factors expected to be only 6-8%. The gas injection has resulted in production increases in some offset wells, and a higher degree of gas breakthrough has been observed in the section with 667 ft (200 m) well spacing. The 3-month injection cycle has resulted in the largest incremental recovery and cycle time optimization efforts are ongoing. The pilot results are encouraging, indicating that a cyclic gas injection EOR scheme could have potential for further commercial expansion. The pilot has demonstrated Huff & Puff's potential economic viability as an EOR scheme for the tight Viking sand formation, which is one of the most prominent producing formations with low oil recovery factors in the Western Canadian Sedimentary Basin (WCSB). There is no such published pilot information available before. Detailed pilot design, production performance evaluation and learnings are presented in this paper.

An unblinded trial of intravenous stretokinase in early acute myocardial infarction was planned to study wheter the drug produces a clinically relevant benefit in terms of reduction of in-hospital and one year mortality.11806 pts in one hundred and seventy six coronary care units were enrolled over 17 months.Patients admitted within 12 h after o.nSet of symptons and with no contraindications to SK were randomized to receive SK in addition to usual treatment and complete data were obtained in 11712 for what concerns in hospital prognosis.At 21 days overall hospital mortality was 10.7% in SK recipients versus 13%,in controls,an 18% reduction(p=0.0002,relative risk 0.81).The extent of beneficial effect appears to be a function of time from onset of pain to SK infusion(relative risk 0.74,0.80, 0.87 and 1.19 for the 0-3,3-6,6-9 and 9-12 h subgroups).The data of 1-year follow-up concerning 11605 pts(95.3% of the whole population)were available up to December the 31st.1987.4333 pts out of the SK-treated group(74.0%)and 4219 out of the control one (72.1%)were alive,with a significant difference.These results document that the benefit produced by SK in the hospital period remains substantially unchanged.The differences in mortality in favour of SK vs.C remain highly significant specifically for the 0-3 and 3-6 hrs subgroups and is dramatic for patients treated between one hour from onset of symptoms. 503 out of the 637 treated with SK were alive at 1-year follow-up versus 443 out of 641 control group pts:the amplitude of the benefit seems to be further increased in this particular subgroup.The GISSI results document conclusively that an acute thrombolytic treatment with SK in AMI is effective in reducing mortality not only over the short,but also over the medium period.

Abstract:
Objective: The aim of this study was to compare the protein expression of biglycan (BGN) in normal breast tissue and in breast cancer using deep learning and digital HScore techniques. Methods: In this case-control study, 24 formalin-fixed, paraffin-embedded tissues were obtained from pathological archives for analysis. Normal breast (n=9) and breast cancer (n=15) tissue sections were analyzed by immunohistochemistry using BGN monoclonal antibody (M01 – Abnova), clone 4E1-1G7 at dilution 1:300 at pH 6, and 3,3’-diaminobenzidine (DAB) as the chromogen. Photomicrographs of the slides were analyzed using the ImageJ software with “color deconvolution”. After selecting the regions of interest (ROI), deconvoluted panels with DAB only were quantified using arbitrary DAB units. Another set, with higher magnification without ROI selection, was submitted to the inception V3 deep neural network image embedding recognition model. Next, supervised neural network analysis, using stratified 20-fold cross-validation, with 200 hidden layers, ReLu activation, and regularization at α=0.0001 were applied for SDLNN. The sample size was calculated for a minimum of seven cases and seven controls, having a power of 90%, an α error=5%, and a standard deviation of 20, to identify a decrease from the average of 40 DAB units (control) to 4 DAB units in cancer. Ethical approval was obtained from the Hospital de Clínicas de Porto Alegre Ethical Review Board (2019/0337). CAAE 15329119.9.0000.5327. Results: BGN expression (mean±SD) was 6.1±3.9 in breast cancer tissue, while in normal breast tissue, it was 39.6±21.9, using D-HScore (p=0.0017, student t-test, Welch corrected). SDLNN was able to correctly classify 110 out of 129 photomicrographs of the dataset using DAB panels only, with a classification accuracy of 85.3% (95%CI 78.1–90.3%) and the area under the curve of 94.3%. Conclusion: D-HScore and SDLNN revealed that BGN protein expression is reduced in breast cancer tissue, compared to normal tissue. The use of SDLNN seems to be a potential tool for image analysis in histological samples.

Abstract:
ЦЕЛЬ: изучить диагностические возможности разработанного мобильного приложения с точки зрения оценки вариабельности гликемии и времени в целевом диапазоне глюкозы крови. МАТЕРИАЛЫ И МЕТОДЫ: с целью оптимизации самоконтроля пациентов с сахарным диабетом (СД) разработано мобильное приложение для мониторинга глюкозы крови «Dialog GM» (Русанов А.Н., Родионова Т.И.; свидетельство о регистрации ПрЭВМ №2022665169, 11.08.2022). Мобильное приложение используется совместно с глюкометром и рассчитывает следующие параметры гликемического контроля: средние значения гликемии, параметры вариабельности гликемии (SD, CV, MAGE), показатели нахождения в различных диапазонах глюкозы крови (TIR, TAR, TBR). В рамках пилотного исследования проведен анализ историй болезни пациентов, проходивших непрерывный мониторинг глюкозы (НМГ) крови с помощью системы Medtronic iPro2 в ГУЗ Саратовская ГКБ №9. Критерии включения: СД 1 типа, продолжительность НМГ – 6-7 дней, ежедневный контроль гликемии глюкометром 7 и более раз в день на фоне НМГ. Данные дневников самоконтроля пациентов были оцифрованы и загружены в программное обеспечение (ПО) Dialog GM для дальнейшего анализа данных. Полученные результаты сравнивались с аналогичными па- раметрами, рассчитанными на основании НМГ с помощью ПО Medtronic CareLink iPro (эталонный метод). Статистический анализ данных проведен с помощью программы IBM SPSS Statistics, v.26. Чувствительность и специфичность выявления дисгликемии оценивались с учетом критериев Международного консенсуса по НМГ. РЕЗУЛЬТАТЫ: в исследование включено 30 пациентов со следующими характеристиками: возраст – 34,3±9,1 лет; длительность СД – 9,7±7,7 лет; уровень гликированного гемоглобина – 8,5±2,1%; женщины – 70%, мужчины – 30%. Проведен анализ 1653 измерений с помощью глюкометра (7,6±1,0 измерений/ сутки) и 53266 измерений с помощью НМГ. Результаты анализа с применением ПО Dialog GM: средняя глюкоза крови – 8,8±2,3 ммоль/л; SD – 3,3±1,0 ммоль/л; CV – 38,4±10,6%; MAGE – 6,1±2,1 ммоль/л; TIR – 57,0±20,8%; TAR – 35,1±23,4%; TBR – 7,9±7,6%. Результаты анализа на основании данных НМГ: средняя глюкоза – 8,8±2,1 ммоль/л; SD – 3,0±0,9 ммоль/л; CV – 34,1±9,9%; MAGE – 5,8±1,7 ммоль/л; TIR – 60,2±21,6%; TAR – 33,8±23,9%; TBR – 6,1±5,9%. При сопоставлении измерений с помощью двух методов были выявлены статистически значимые различия по уровню SD (p<0,05) и CV (p<0,05), при оценке остальных параме- тров значимых различий не выявлено (p>0,05). Коэффициент корреляции Пирсона (r) для показателей, рассчитанных исследуемым и эталонным методами: средняя глюкоза крови – 0,97; SD – 0,87; CV – 0,92; MAGE – 0,77; TIR – 0,89; TAR – 0,95; TBR – 0,79 с уровнем p<0,05 для всех параметров. ПО Dialog GM позво- лило диагностировать нарушенные показатели гликемического контроля с соответствующими уровнями чувствительности и специфичности: SD – 94% и 77%; CV – 93% и 80%, MAGE – 95% и 71%, TIR – 94% и 70%, TAR – 92% и 75%, TBR – 76% и 86%. ВЫВОДЫ: разработанное мобильное приложение Dialog GM, примененное совместно с глюкометром, может оказаться доступным, высокочувствительным методом исследования вариабельности гликемии и времени в целевом диапазоне глюкозы крови. Планируется дальнейшее исследование и совершенство- вание разработанных методик мониторинга гликемии.

Background: The tens of thousands of academic journal websites that are on the Internet today often do not have a clear organizational structure for their website. If most of them are convenient enough for readers (if the journal is open access), then many have problems informing authors about the conditions, the process of submitting and reviewing the manuscript. The Editorial Offices empirically populate the journal's website with content that can change dramatically (both in terms of website design and content) as the journal develops. Aim of Study: To develop a website model for an academic journal that takes into account the basic requirements for the preparation, publication, and archiving of high quality scientific manuscripts. Material and Methods: The academic journal website model is based on a structural-functional approach. The website content consists of text and integrated applications. This model takes into account the basic requirements for the preparation, publication, and archiving of high-quality open access scientific manuscripts, as well as the indexing of journal articles by leading indexing agencies.
