Two camptothecin derivatives, 10-cyclohexyl-7-methyl-20(S)-camptothecin and 7-methyl-10-morpholino-20(S)-camptothecin, were synthesized and their differences in solubility were investigated using four chosen solvent systems. Based on our results, 10-cyclohexyl-7-methyl-20(S)-camptothecin exhibited higher solubilities than 7-methyl-10-morpholino-20(S)-camptothecin in polar aprotic solvents. However, these two camptothecin derivatives did not exhibit apparent differences in solubility between 5% dimethyl sulfoxide (DMSO)/95% normal saline co-solvent system and 5% dimethylacetamide (DMAC)/95% normal saline co-solvent system. To rationalize their differences in solubility, we also tried to perform a DFT-B3LYP study to investigate their interaction with one water molecule.

Abstract:
A cross sectional study was conducted (2010–2013) to determine the diversity of hemoprotozoa among bats of Bangladesh. Microscopic examination of blood smears (N=533; Pteropus medius (377), Rousettus leschenaultii (111), Megaderma lyra (45)) revealed 9% of bats (95% confidence interval CI: 7–12%) were positive for hemoprotozoa. The overall prevalence of hemoparasites among P. medius was 5% (n=20, 95% CI: 3–8%); where Babesia sp. was 3% (n=12, 95% CI: 2–5%) and Hepatocytis sp. was 2% (n=8, 95% CI: 1–4%). Moreover, 13% of R. leschenaultii were positive (n=14, 95% CI: 7–20%) where prevalence of Babesia sp. was 10% (n=11, 95% CI: 5–17%) and prevalence of Hepatocystis sp. was 3% (n=3, 95% CI: 1–8%). Twenty-nine percent (n=13, 95% CI: 16–44%) of M. lyra harbored hemoparasites, among which 20% (n=9, 95% CI: 10–35%) were Babesia sp. and 9% (n=4, 95% CI: 2–21%) were Hepatocystis sp. The study indicates bats remain important hosts for various zoonotic parasites and suggests further research.

AbstractObjectiveThe study aimed to highlight the determinants of overnutrition (overweight plus obesity) in fishing communities and establish if these were the same as reported elsewhere in Ghana.DesignCross-sectional study.SettingThe study was conducted in Idun, Ola and Duakor fishing communities in Cape Coast, Ghana.SubjectsAdults (n 252) aged 20 to 50 years.ResultsResults showed that 32 % of participants were overweight/obese (BMI ≥ 25·0 kg/m2). Participants’ mean age was 31·7 (sd 1·0) years, they had 13·7 (sd 8·1) mean years of formal education, their median monthly income was $US 7·4 (interquartile range $US 3·3, 20·0) and their median daily energy intake was 7·3 (interquartile range 5·3, 9·8) MJ. Significant associations (P < 0·05) were found between BMI and gender, age, years of education, fat intake and marital status. Females were almost eight times more likely to be overweight/obese than males (adjusted OR = 7·7; 95 % CI 3·6, 16·4). Persons aged ≥40 years were about six times more likely to be overweight/obese than those aged 20–29 years (adjusted OR = 6·1; 95 % CI 2·6, 14·1). Married people were nearly three times more likely to be overweight/obese than singles (adjusted OR = 2·8; 95 % Cl 1·4, 5·7). People with more than 13 years of formal education (adjusted OR = 0·3; 95 % CI 0·1, 0·9) and people with >30 % fat contribution to daily energy intake (adjusted OR = 0·3; 95 % CI 0·1, 0·6) had reduced odds of being overweight/obese.ConclusionsOvernutrition was prevalent in the fishing communities and associated with factors such as age, gender, marital status, educational status and fat intake.

Abstract:
This chapter reports on a study that investigated how graduate students in the Faculty of Communication and Information Science at NUST were approaching integration in their mixed-methods research dissertations. There has been a concern that lack of expertise of what mixed-methods research is restricts the integrative capacity. Using a research synthesis method, the study investigated three graduate programmes, namely Master's degrees in Library and Information Science, Records and Archives Management, and Journalism and Media Studies from 2016 up to 2018. A total of 95 dissertations were reviewed, and 40 employed mixed-methods research design. It was discovered that integration was commonly done at methods and interpretation levels. Integration of qualitative and quantitative data sets resulted in confirmation (83), expanding understanding (27), and discordance (31). Graduate students dealt with discordant findings by either ignoring the discordance (20), seek corroboration with existing literature (7), or give priority to the quantitative strand (4).

Hepatitis E virus (HEV) is one of the 7 viruses with mainly hepatic tropism. HEV determines 20 million new infections worldwide every year, 3.4 million acute hepatitis E and 44,000 deaths in 2015 (3.3% of the mortality due to viral hepatitis). Transmitted by the digestive tract mainly (fecal- orally, particularly by water infected with feces), the virus reaches the liver where it does not have a direct cytolytic effect, but immunological phenomena, especially cellular, activated by the replication of the virus in the hepatocytes. Clinically, over 95% of cases of HEV infection are asymptomatic and sel- limiting; in immunocompetent patients in tropics HEV can cause acute hepatitis with clinical features. On rare situations the infection can result in a severe, fulminant hepatitis with acute liver failure. In immunocompromised patients (organ transplant recipients, hematologic malignancies, HIV-infected) HEV may determine chronic hepatitis. In pregnant women or the elderly people or people with underlying liver disease HEV can cause fulminant forms which can become fatal (E.g.: 30% deaths among pregnant women in some parts of the world). Acute and chronic E hepatitis may be accompanied by extrahepatic manifestations: neurological, kidney, pancreatic, hematological diseases, autoimmune diseases with a pathogenesis not fully elucidated.

Abstract:
ABSTRACT Background: Breastfeeding peer support helps lactating women to sustain breastfed and reduce cultural barriers. This study aimed to examine the effects of maternal age, maternal education, maternal employment, type of labor, knowledge, attitude, family support, and social capital on breastfeeding practice. Subjects and Method: A cross sectional study was conducted at 25 integrated health posts in Karanganyar, Central Java, from August to September 2019. A sample of 200 lactating mothers with children aged 7-12 months was selected by stratified random sampling. The dependent variable was exclusive breastfeeding. The independent variables were maternal age, maternal education, maternal employment, type of labor, knowledge, attitude, family support, and social capital. The data were collected by questionnaire and analyzed by a multipe logistic regression. Results: Exclusive breastfeeding increased with maternal age 20-34 years (b= 1.96; 95% CI= 0.53 to 3.39; p= 0.007), maternal education ≥Senior high school (b= 1.47; 95% CI= 0.08 to 2.87; p= 0.038), good knowledge (b= 1.73; 95% CI= 0.33 to 3.12; p= 0.015), positive attitude (b= 1.61; 95% CI= 0.28 to 2.94; p= 0.017), strong family support (b= 1.39; 95% CI= 0.13 to 2.65; p= 0.030), and strong social capital (b= 1.34; 95% CI= 0.01 to 2.68; p= 0.049). Exclusive breastfeeding decreased with mothers working outside the house (b= -2.95; 95% CI= -4.37 to -1.53; p<0.001) and sectio cesarean labor (b= -1.57; 95% CI= -3.06 to -0.08; p= 0.039). Abstract:
The thermal oxidative stability of several aircraft fuels at high temperatures has been evaluated in a flowing test device. The fuels studied include Jet A, JPTS, and JP-7. The tests evaluated solid deposition under various conditions, typically at maximum fuel temperatures of ∼480 °C (900 °F) at a pressure of 69 atm. Under these conditions, the dissolved oxygen in the fuel is completely consumed and the thermal-oxidative reactions are driven to completion. At test times up to 50 hours, both surface and bulk (filter) deposition was usually an approximately linear function of test time, after an initial induction time of low deposition which varied from 0 to ∼20 hours depending upon fuel quality. In tests with flow rates from 12 to 200 mL/min (1–21 lb/hr), the deposition rate (expressed in ppm) was fairly constant. These data give some guidance for extrapolating results from small flow-rate bench-scale tests to higher flow rates more realistic to aircraft fuel systems, for conditions where the oxygen is completely consumed. For conditions where the oxygen is not completely consumed, the relationship between surface and suspended bulk deposits can become quite complex. Two dispersant additives widely examined in the JP-8+100 program were studied, with the additive significantly decreasing the slope of the deposition-vs-time curve (the deposition rate) for Jet A fuels.

The effects of adverse pressure gradients on the thermal and momentum characteristics of a heated transitional boundary layer were investigated with free-stream turbulence ranging from 0.3 to 0.6%. Boundary layer measurements were conducted for two constant K cases, K1=−0.51 × 10−6 and K2=−1.05 × 10−6. The fluctuation quantities, u′, v′, t′, the Reynolds shear stress (uv¯), and the Reynolds heat fluxes (vt¯ and ut¯) were measured. In general, u′/U∞, v′/U∞, and vt¯ have higher values across the boundary layer for the adverse pressure-gradient cases than they do for the baseline case (K=0). The development of v′ for the adverse pressure gradients was more actively involved than that of the baseline. In the early transition region, the Reynolds shear stress distribution for the K2 case showed a near-wall region of high-turbulent shear generated at Y+=7. At stations farther downstream, this near-wall shear reduced in magnitude, while a second region of high-turbulent shear developed at Y+=70. For the baseline case, however, the maximum turbulent shear in the transition region was generated at Y+=70, and no near-wall high-shear region was seen. Stronger adverse pressure gradients appear to produce more uniform and higher t′ in the near-wall region (Y,+<20) in both transitional and turbulent boundary layers. The instantaneous velocity signals did not show any clear turbulent/non-turbulent demarcations in the transition region. Increasingly stronger adverse pressure gradients seemed to produce large non-turbulent unsteadiness (or instability waves) at a similar magnitude as the turbulent fluctuations such that the production of turbulent spots was obscured. The turbulent spots could not be identified visually or through conventional conditional-sampling schemes. In addition, the streamwise evolution of eddy viscosity, turbulent thermal diffusivity, and Prt are also presented.

Straipsnyje nagrinėjami eksperimentiniai tyrimai, susiję su maisto atliekų skleidžiamų kvapų šalinimų technologijomis. Eksperimentas atliekamas naudojant 20 ml dydžio filtrus su aktyvuota anglimi (7 g), su bioanglimi (2,50 g) ir su keramzitu (6 g), kuris buvo išmirkytas tirpale su mikroorganizmais, taip pat buvo naudojami kontroliniai filtrai – filtrai be užpildo, kad galėtume įvertinti, kokia buvo maisto atliekų skleidžiamo kvapo koncentracija nenaudojant jokių kvapą šalinančių technologijų. Naudojamos maisto atliekos – rūkyta žuvis (10 g), česnakai (10 g), svogūnai (10 g) ir rauginti kopūstai (10 g). Eksperimentas vyko 6 paras 20±1 °C aplinkos temperatūroje. Kvapo koncentracija buvo nustatinėjama dinaminės olfaktometrijos metodu. Tyrimais nustatyta, kad stipriausia kvapo koncentracija buvo po 6 parų (22996 OUE/m3). Efektyviausias filtras buvo su aktyvuota anglimi, vidutinis efektyvumas 95 proc., naudojant šį filtrą didžiausia kvapo koncentracija buvo po 6 parų ir siekė 2105 OUE/m3. Didžiausia maisto atliekų kvapo koncentracija buvo naudojant filtrą su mikroorganiznais, šio filtro vidutinis efektyvumas – 27 proc., didžiausia kvapo koncentracija – 17224 OUE/m3.

Abstract:
Although it has been available for a number of years, Tc-99m RBC venography has never been evaluated in an epidemiological study. We have compared Tc-99m RBC venography to impedance plethysmography (IPG) in 113 consecutive eligible patients with clinically suspected first episode of deep vein thrombosis (DVT). IPG was performed at initial presentation (day 0) and RBC venography within the next 72 hours.RBC venography was performed by labelling in vitro 5ml of patients' red blood cell with 20 millicuries of Tc-99m. Patients with an initially abnormal IPG underwent contrast venography to rule out a falsely positive test result. Patients with an initially normal IPG had the test repeated at day 1, 3, 5 to 7 and 10 to 14. Anticoagulant treatment was witheld in all patients who remained normal by serial IPG testing. All patients underwent a 3 month follow-up period. RBC venography was considered abnormal if there was a 50% or greater decrease in isotope concentration in a deep vein compared to the same vein in the other leg. The sensitivity of RBC venography was 79% with 95% confidence limits (C. L.) from 59% to 92%. Specificity was 61% (95% C. L. from 51% to 71%). Positive and negative predictive values were 37% (95% C. L. from 25% to 51%) and 91% (95% C. L. from 81% to 96%) respectively. None of the patients who remained normal by serial IPG testing died from or had objectively documented venous thromboembolism during the follow-up period. We conclude that : 1)RBC venography is non-specific and, 2) a decision to treat should not be made on the basis of the results of this test alone in view of its low positive predictive value.

Abstract:
The Ligase Detection Reaction (LDR) is a mutation detection technique used to identify point mutations in deoxyribonucleic acid (DNA). A microscale Ligase Detection Reaction (LDR) device was designed and manufactured in polycarbonate. There are at least two mixing stages involved in the LDR identification process. Various micromixers were simulated in Fluent (v5.4, Lebanon, NH) and several test geometries were selected for fabrication. Passive diffusional micromixers were made with aspect ratios from 7 to 20. The mixers were made by SU-8 lithography, LIGA, laser ablation and micromilling to characterize each fabrication method. It was found that LIGA was best for making the micromixers, but was the longest process. The micromixers were fabricated and are being tested using fluorescent dyes. For a successful reaction temperatures of 0°C, 95°C and 65°C were needed. A stationary chamber method was used with thermal cycling in which the sample held while the temperature is cycled. Finite element analysis showed uniform temperatures in the rectangular 1.5 μl chambers and that air slits can effectively separate the thermal cycle zone from the 0°C cooling zone and the mixing region. A test device was laid out and micromilled with the temperature zones. A commercial thin film heater and a thermoelectric module were used with a PID controller to obtain the required process temperatures. Heating from 65°C to 95°C took 10 seconds, while cooling from 95°C to 65°C also took 10 seconds. The residence times at the required temperatures can adapt to changes in the LDR as parameters and reactant concentrations are varied.

Thermal explosions result when local temperature-dependent heat generation exceeds heat loss via conduction. The temperature dependence of the heat source term is directly related to the material’s chemical kinetics, and hence the chemical kinetics has a direct impact on the thermal explosion times of a material. Much success has been gained in past work to accurately model thermal explosions in various explosives using multi-step Arrhenius chemical kinetics models. However, the generation of these kinetics models is time consuming and complex. Therefore, a methodology has been developed that allows for calibration of a single-reaction global kinetics model using One Dimensional Time to Explosion (ODTX) experimental data, which combines an iterative approach with a steepest descents optimization. This methodology has been applied to calibrate kinetic parameters for the widely-used explosives RDX (1, 3, 5-trinitrohexahydro-striazine), HMX (octahydro-1, 3, 5, 7-tetranitro-1, 3, 5, 7-tetrazocine), LX-10 (95% HMX, 5% Viton binder), and PBXN-109 (64% RDX, 20% Al, 16% binders). The average error between experimental and simulated ODTX and STEX data using this technique is approximately equivalent to that using the traditional multi-step models, and the time required for calibration of the global kinetics model has been reduced from months to hours.

Abstract:
Increased leukocyte adhesion to the endothelial lining of blood vessels is an essential event in inf laaaation and the pathogenesis of certain vascular deseases. The aonocyte product interleukin-1 (IL-1) has been shown to enhance the adherence of huaan periferal blood polyaorphonuclear leukocytes (PMN) to huaan uabilical vein endothelial cells (EC). In this study AD6, a couaarin derivative which inhibits platelet aggregation and coronary throabus foraation in experimental aniaals, was found .to inhibit PMN adhesion to control and IL-i-treated EC. Suspensions of washed 51Cr labeled PMN were added to cultured huaan uabilical vein EC which have been treated either with buffer or IL-i (10 units/ml for 4 hours). The aaount of PMN adherent to 105 EC art ter 5 min incubation was about 2.1×105 to control and 4×105 to IL-i-treated EC. When AD6 was added to the aediua during the adhesion experiaent PMN adhesion was inhibited in a concentration dependent way. The ainiaal active concentration of the drug was 0.5μM( 20-30% inhibition to both control and IL-1 treated EC) but inhibition was aaxiaal at SOOjiM ( 80-90 % inhibition to control and 65-80 % to IL-i-treated EC ). AD6 also blocked (90-95 % inhibition at 500μM) PMN adhesion to plastic wells. The drug effect on PMN required at least 5 min incubation to be apparent and declined after 30 min. AD6 pretreataent of either PMNs or ECs followed by washing reduced PMN adhesion to unstimulated or IL-i-treated EC.The effect was however aaxiaal when both cell types were treated with AD6, thus indicating that the inhibitory activity of AD6 was directed at both ECs and PMNs and night be cumulative.

ABSTRACT Background: Childbirth is a physiological process which is the process of releasing the product of conception from the uterus. Normal delivery occurs at 37 – 42 weeks of gestation, where the fetus is mature. Labor cannot be separated from labor pain. Several studies have shown that in primitive societies, labor is longer and painful, whereas in advanced societies 7-14% give birth with minimal pain and most (90%) are accompanied by pain and unavoidable pain. The birth ball is a physical therapy ball that helps the delivery of the first stage to help labor progress. A physical therapy ball that helps labor progress and can be used to share positions. One of the movements is to sit on the ball and shake it to comfort and help labor progress. This study aimed to examine the birthing ball therapy on the long-term maternal and labor pain among primigravida mother in private practice midwives, Madiun, East Java. Subjects and Method: This was a cross sectional study conducted in Madiun, East Java. The total of 40 pregnant women were divided into 20 mothers with birthing ball therapy and 20 mothers without birthing ball therapy. The dependent variables were length of the first period of labor and intensity of labor pain. The independent variables were birthing ball therapy. Data were collected using an observation sheet. The data were analyzed using a multiple logistic regression. Results: The use of birth ball was positively associated with the length of the first period of labor (OR = 5.19; 95% CI= 1.06 to 25.4; p = 0.042) and intensity of labor pain (OR = 7.57; 95% CI= 1.53 to 37.3; p = 0.013) and they were statistically significant. Conclusion: The use of birth balls is proven to be effective in shortening stage 1 and reducing pain intensity. Introduction: Germ cell tumours comprise approximately 15-20% of all ovarian tumours. Two third of ovarian tumours in first two decades of life are germ cell tumours. Majority of ovarian germ cell tumours are benign teratomas. The malignant germ cell tumours are usually solid and arise from totipotent germ cells. Over the past 3 decades the clinical outcome of women with ovarian germ cell tumours (OGCT) have significantly improved mainly due to development of more effective chemotherapy regimens. Objective: To study the clinic pathological features, treatment and survival of women with ovarian germ cell tumours. Methods: This is a retrospective descriptive study taken from the case files of patients with histo-pathologically proven ovarian germ cell tumours who were treated in JIPMER over 8 years from 2007 to 2014. Results: There were totally 63 patients with ovarian germ cell tumours over 8 years who were treated in JIPMER. The age at presentation varies from 12 years to 65 years with a median age of 26.5 years. Three were pre pubertal and 1 was post-menopausal. Twenty two women (34%) were unmarried and 5 were pregnant at the time of presentation. Forty eight (76%) of them did not have any menstrual abnormalities. Pain abdomen (55%) was the most common presentation. Ten of them presented with acute abdomen of which 8 were torsion, 1 was ruptured dermoid and 1 was infected dermoid. Another 6 patients had torsion which was diagnosed only during surgery. Majority (68%) were benign tumours (dermoid) and among malignant tumours, there were 6 dysgerminomas, 5 immature teratomas, 5 mixed germ cell tumours and 4 yolk sac tumours. Almost half (22 out of 43) of women with benign tumours were <25 years whereas 3/4th (14 out of 20) of women with malignant germ cell tumours were <25 years. The most common tumour marker which was elevated was alpha feto protein (8) followed by LDH (5). Fertility sparing surgery (salpingo-ovariotomy) was commonly performed which was 95% (41/43) in benign tumours and 60% (8/20) in malignant tumours. Contra lateral ovary was biopsied in only 5 patients with suspected involvement (negative on final HPR). Out of 20 women with malignant ovarian tumours 7 were in advanced stage (Stage III). Majority of them recovered well from surgery, only 12% had post-operative febrile morbidity and one patient had subclavian vein thrombosis on post op D9 which required anticoagulants. 7 of 20 women received chemotherapy (BEP) for 4 cycles. No serious side effects of chemotherapy were noted in these women. 3 out of 20 women with malignant germ cell tumour were lost to follow up. No recurrences have been found in rest of the women and there are no deaths till last follow up. Conclusion: Advances in the field of medicine like effective chemotherapy regimens, improved imaging, precise surgical staging and fertility sparing surgical procedures enable women not only to preserve the reproductive function but also to improve their quality of life.
