Abstract:
Background The prevalence of risk factors for poor outcomes from aneurysmal subarachnoid hemorrhage (SAH) varies widely and has not been fully elucidated to date in Vietnam. Understanding the risk and prognosis of aneurysmal SAH is important to reduce poor outcomes in Vietnam. The aim of this study, therefore, was to investigate the rate of poor outcome at 90 days of ictus and associated factors from aneurysmal SAH in the country. Methods We performed a multicenter prospective cohort study of patients (≥18 years) presenting with aneurysmal SAH to three central hospitals in Hanoi, Vietnam, from August 2019 to August 2020. We collected data on the characteristics, management, and outcomes of patients with aneurysmal SAH and compared these data between good (defined as modified Rankin Scale (mRS) of 0 to 3) and poor (mRS, 4–6) outcomes at 90 days of ictus. We assessed factors associated with poor outcomes using logistic regression analysis. Results Of 168 patients with aneurysmal SAH, 77/168 (45.8%) were men, and the median age was 57 years (IQR: 48–67). Up to 57/168 (33.9%) of these patients had poor outcomes at 90 days of ictus. Most patients underwent sudden-onset and severe headache (87.5%; 147/168) and were transferred from local to participating central hospitals (80.4%, 135/168), over half (57.1%, 92/161) of whom arrived in central hospitals after 24 hours of ictus, and the initial median World Federation of Neurological Surgeons (WFNS) grading score was 2 (IQR: 1–4). Nearly half of the patients (47.0%; 79/168) were treated with endovascular coiling, 37.5% (63/168) were treated with surgical clipping, the remaining patients (15.5%; 26/168) did not receive aneurysm repair, and late rebleeding and delayed cerebral ischemia (DCI) occurred in 6.1% (10/164) and 10.4% (17/163) of patients, respectively. An initial WFNS grade of IV (odds ratio, OR: 15.285; 95% confidence interval, CI: 3.096–75.466) and a grade of V (OR: 162.965; 95% CI: 9.975–2662.318) were independently associated with poor outcomes. Additionally, both endovascular coiling (OR: 0.033; 95% CI: 0.005–0.235) and surgical clipping (OR: 0.046; 95% CI: 0.006–0.370) were inversely and independently associated with poor outcome. Late rebleeding (OR: 97.624; 95% CI: 5.653–1686.010) and DCI (OR: 15.209; 95% CI: 2.321–99.673) were also independently associated with poor outcome. Conclusions Improvements are needed in the management of aneurysmal SAH in Vietnam, such as increasing the number of aneurysm repairs, performing earlier aneurysm treatment by surgical clipping or endovascular coiling, and improving both aneurysm repairs and neurocritical care.

Abstract:
The objective of this study is to explore the best coupling form of water and nitrogen in arid areas of northwest China. Field experiment was conducted in the oasis region of Shiyang River Basin, and the interactive impact of water and nitrogen nutrition on use of water and nitrogen of spring wheat was investigated. The results showed that nitrogen rate and irrigation in jointing and heading stages impacted on spring wheat yield remarkably. With nitrogen rate (168 kg/hm2), irrigation (90 mm) in jointing stage, and irrigation (70 mm) in heading stage, grain yield was higher. Water use efficiency would be high, when the irrigation in tillering and filling stages both were 30 mm. Soil mineral nitrogen accumulation was mainly nitrate nitrogen, which was positively correlated with nitrogen rate in earlier stage. Nitrogen rate and irrigation in jointing and filling stage impacted on plant nitrogen accumulation remarkably, plant nitrogen accumulation came to maximum when nitrogen rate (168 kg/hm2), irrigation (90 mm) in jointing stage and irrigation (70 mm) in filling stage were applied. And nitrogen use efficiency came to maximum under nitrogen rate (56 kg/hm2), irrigation (90 mm) in jointing stage. Considering high grain yield and use of water and nitrogen, the primary suggestion on mode of irrigation and nitrogen rate of spring wheat in Shiyang River Basin were nitrogen rate (168 kg/hm2), irrigation (90 mm) in jointing stage and irrigation (70 mm) in tillering, heading and filling stages.

ABSTRACT Background Medication shortages prevent patients from receiving optimal care. Despite the frequency with which medical trainees care for inpatients, no assessment of their experiences in medication shortage management has been performed. Objective We evaluated trainees' experiences managing medication shortages. Methods We performed a cross-sectional survey of trainees postgraduate year 2 (PGY-2) and above in medicine, anesthesiology, and emergency medicine departments at 2 academic centers in 2018–2019. Categorical and ordinal assessments evaluated shortage awareness, substitution availability, pharmacy and therapeutics committee-based restrictions, communication, and education. Regressions were performed to determine effect of PGY, department, and institution on responses. Results A total of 168 of 273 subjects completed the survey (62% response rate). Most (95%, 159 of 168) reported managing medication shortages during training; 51% (86 of 168) described managing clinically relevant shortages daily or weekly. Seventy-seven percent (129 of 168) noted equivalent alternatives were unavailable at least one-quarter of the time, and 43% (72 of 168) reported clinically necessary medications were restricted at least weekly. Fifty-four percent (89 of 168) and 64% (106 of 167) of respondents discussed clinically relevant shortages with supervising physicians or patients “some of the time” or less, respectively. Most respondents (90%, 151 of 168) reported they would benefit from shortage management training, but few (13%, 21 of 168) reported prior training. Conclusions Although trainees reported frequent involvement in clinically impactful shortage management, medication shortage communication between trainees and supervising physicians or patients appears sporadic. Medication shortage management training is uncommon but perceived as beneficial.

Abstract:
This study presents the development of geopolymer bricks synthetized from industrial waste, including fly ash mixed with concrete residue containing aluminosilicate compound. The above two ingredients are mixed according to five ratios: 100:0, 95:5, 90:10, 85:15, and 80:20. The mixture's physico-mechanical properties, in terms of water absorption and the compressive strength of the geopolymer bricks, are investigated according to the TIS 168-2546 standard. Scanning electron microscopy (SEM) and X-ray diffraction (XRD) analyses are used to investigate the microstructure and the elemental and phase composition of the brick specimens. The results indicate that the combination of fly ash and concrete residue represents a suitable approach to brick production, as required by the TIS 168–2546 standard.

Cucumber (Cucumis sativus L.) seed require soil temperature to be around 20C for efficient germination. This hinders early planting in cool soils. This study was conducted to determine how germinating seed of cucumber cultivars Earlipik 14 and Arkansas Little Leaf at 13.9, 15.6, and 20C in the dark affected protein formation. Seed were removed from moist chambers at 0, 12, 24, 48, 72, 84, 96, 120, and 168 h. Germination, defined as the radicle being at least 5 mm long, was determined at each time. Germinated and ungerminated seed was prepared for polyacrylamide gel electrophoresis. At 20C, 90% to 100% of seed had germinated by 48 h. At 15.6C, 20% to 50% of seed germinated by 168 h, and at 13.9C, ≈2% of seed had germinated by 168 h. For seed incubated at 20C, concentrations of proteins at 70.1 kDa decreased, while those at 37.4, 43.4, and 50 kDa increased after 24 h, which corresponded to formation and elongation of the majority of radicles. These changes were expressed later for seed germinated at 15.6 and 13.9C. Identification of the proteins is being attempted. The importance of these proteins in germination and early development will be discussed.

Zusammenfassung. Theoretischer Hintergrund: Die Eignung der SCL-90-R und von Persönlichkeitsfragebogen für die Differentialdiagnostik verschiedener klinischer Gruppen ist umstritten. Fragestellung: Überprüfung der Eignung der SCL-90-R sowie von drei neu entwickelten Persönlichkeitsfragebogen zur Differentialdiagnostik von Patienten aus sechs diagnostischen Kategorien psychischer Störungen. Methode: Untersuchung von 168 Patienten mit mittelgradigen bzw. schweren Depressionen, anhaltenden Schmerzstörungen, Anpassungsstörungen, Agoraphobien und Essstörungen mit der SCL-90-R, dem Fragebogen zu Kompetenz- und Kontrollüberzeugungen, dem Fragebogen zum selbstbezogenen Verhalten und dem Fragebogen zur habituellen körperlichen Gesundheit. Ergebnisse: Alle vier diagnostischen Verfahren tragen zur Differentialdiagnostik bei. Es lassen sich spezifische Profile der sechs klinischen Gruppen erstellen. Schlussfolgerungen: Die neun Skalen der SCL-90-R messen primär einen starken Faktor. Der klinische Einsatz der drei Persönlichkeitsfragebogen kann empfohlen werden. Neben Symptomen tragen Persönlichkeitseigenschaften zur Charakterisierung klinischer Gruppen bei und erleichtern die Therapieplanung.

Polyunsaturated fatty acids (PUFA) were administered to 168 patients over a period of 6 months in an open-label uncontrolled study. In 129 patient available for study, 86% experienced reduction in severity, frequency and duration of migraine attacks, 22% became free of migraine and more than 90% had reduced nausea and vomiting. Self-medication changed to simple analgesics in the majority except in 14% of patients without improvement.

This dissertation has for object of study the Provisional Measure 168/90 which served as the political and economic instrument for a specific context, namely, the Collor administration. It will analyze, therefore, the legal limits existing at the time of the mentioned Provisional Measure, weighing the economic needs and the legal system in force, and will account for the decisions of the courts that evaluated the constitutionality and legality of the seizure of assets. The Provisional Measure 168/90, put in place to serve economic and political interests during the Collor administration, with the goal of comparing such measures with the existing legal limits in order to come to a conclusion on the illegality of these facts.
A presente dissertação tem por objeto de estudo analisar a Medida Provisória 168/90 que serviu de instrumento político-econômico para um contexto específico, qual seja, o governo Collor. Analisar-se-á, portanto, os limites jurídicos existentes à época da mencionada Medida Provisória, sopesando as necessidades econômicas e a ordem jurídica vigente, fazendo, inclusive, menção às decisões dos Tribunais que apreciaram a constitucionalidade e legalidade do bloqueio de haveres. A Medida Provisória 168/90 foi manejada em razão de interesses econômicos e políticos durante o governo Collor, de modo a cotejar tais medidas com os limites jurídicos existentes, a fim de concluir sobre a ilicitude desses fatos.

Abstract:
The main objective of this dissertation is to study the relationship between the mathematician Claudius Ptolemy and his social and intellectual context in the Roman Alexandria of the second century CE. Against the communis opinio (and the much more common silence) on this issue, my main conclusion is that Ptolemy consciously engaged in the competition for gaining a place in the paideia of his time, both by using techniques related to the rhetorical practices of his time, and by seeking the support of a powerful patron. Indeed, I argue that the dedicatee of many of Ptolemy’s works, a certain Syrus, may be indentified with a powerful Roman with links to Alexandria and with probable mathematical interests. Unlike Galen, an author for whom such a scenario has been shown and studied in modern bibliography, Ptolemy does not offer us direct insights on his writing context. However, this is normal in Greek mathematical authors, who tend to write in a highly formulaic style specific of this kind of texts, providing very few biographical evidence. In consequence, the methodology I use in this study is based on indirect strategies, basically consisting in analyzing parts of Ptolemy’s works which may show the author’s concern for self-presentation and for the external projection of his work. The first chapters deal with the works which are considered to be earlier in Ptolemy’s production, the Canobic Inscription, Harmonics, and On the Criterion and the Ruling Principle. In these three works a main scientific topic (astronomy, harmonics, and knowledge theory) is followed by a non-argumentative section linking the main part with Platonic traditions attested in earlier writers, mainly Middle Platonic developments inspired in the Timaeus. The Pythagorizing Platonist Eudorus of Alexandria could have been an important source for Ptolemy in this aspect, since there are many parallels between material drawn from his work by Plutarch and some passages in the last parts of Ptolemy’s inscription and the Harmonics. As regards the text on knowledge theory, it is interesting that, although concomitant with the topic of mathematics, and overlapping with the epistemology developed in the Harmonics (of Stoic, Aristotelian, and Platonic tradition, in the line of Antiochus of Ascalon), mathematics is not even alluded in there. In the same manner as in the preface of the Almagest, where mathematics appears only very late in the text, Ptolemy seems to present himself mainly as a philosopher. Another telling feature of the text on the criterion is its style: it is neither didactic nor ‘seriously’ philosophical. I mean that the text does not present the clarity of a manual kind of work, but neither the deep engagement with philosophical issues of a genuine philosophical text: Ptolemy does not cite authors or works, and he does not develop long arguments. At the same time, the text presents throughout many verbal echoes of the Timaeus. Imitation of the Timaeus could be related to rhetorical practice, which Ptolemy probably received to some degree as every educated citizen in the empire. I also analyze an external feature of some of Ptolemy’s works, especially prominent in the three earlier ones: in such treatises the text appears divided in nearly equally long portions, perhaps related to harmonic intervals. Comparing with rhetors, this could be interpreted as the voice and the gesture, the external qualities which must reflect a harmony suitable to the contents of the discourse. It seems that Ptolemy gradually frees himself from such formal concerns in later works, perhaps because he had acquired by then a more stable position: already in the Almagest the impersonation of a Pythagorean and Platonic philosopher appears only as an introduction restricted to the preface and the epigram, rather than as a development of the main scientific part.
L’objectiu de la tesi doctoral és estudiar la figura del matemàtic Claudiu Ptolemeu en el context social i intel•lectual del seu temps i del seu ambient, és a dir, l’Alexandria romana del segle II dC. La tesi argumenta que Ptolemeu no va ser aliè al clima general de competició de coneixements que es dóna en aquesta època, ben estudiat en la bibliografia recent en el cas de Galè. Ptolemeu, en contrast amb aquest autor, no proporciona en la seva obra dades evidents sobre el seu entorn, però això és habitual en els textos matemàtics grecs, on l’estil està molt fixat i on les digressions com les que trobem en Galè gairebé no hi tenen cabuda. Així doncs, la metodologia usada en aquest estudi és indirecta, consistent en cercar possibles indicadors de la relació entre Ptolemeu i els seus contemporanis. La primera part de la tesi analitza en profunditat aspectes d’aquest tipus presents en les tres obres de Ptolemeu considerades més primerenques, la Inscripció de Canopus, els Harmònics, i Sobre el criteri i la facultat prinicipal. En cadascuna d’aquestes obres s’observa una secció final en la qual Ptolemeu tracta d’integrar la recerca científica de la part principal dins de tradicions platòniques ja testimoniades en autors anteriors, derivades sobretot de desenvolupaments de l’anomenat Platonisme mitjà alexandrí (especialment Eudor d’Alexandria) a partir del Timeu. Es podria dir que Ptolemeu fa servir aquest platonisme pitagòric per crear una màscara filosòfica en aquests tres textos, una personificació que és possible trobar, més deslligada de la investigació principal, també en l’Almagest. La curiosa divisió dels textos que es pot observar en vàries obres, especialment en les tres primerenques, podria tenir a veure amb aquesta escenificació. En general aquest actitud es pot comparar amb les pràctiques retòriques de l’època, en les quals la imitació i l’aparença externa són importants elements estratègics. Ptolemeu podria haver usat aquestes eines per presentar la matemàtica com a coneixement imprescindible, com la ‘veritable’ filosofia, dins de la paideia grecoromana. Un altre aspecte important del context de Ptolemeu és el seu dedicatari Syrus, per al qual proposo una identificació amb un individu de l’elit romana, testimoniat en vàries fonts, que hauria exercit de protector per a Ptolemeu.

Abstract:
In this thesis I address the proliferation of representations of lesbians in North American mainstream media in the early to mid-1990s. Through a focus on a specific object/subject, Martina Navratilova, I investigate the discourse of progressiveness which accompanies this proliferation. As well, I argue that the shift in the way lesbians are being represented is indicative of a new cultural approach to lesbian sexual difference. This approach, "a homosexuality of no importance" (D. A. Miller), is a strategy of partaking of, and enjoying, lesbian difference while at the same time denying its importance and relevance. My theoretical analysis is conducted within the terrain of lesbian/feminist and "queer" theories of gender and sexual identity, lesbian feminist writings on visibility and representation, and recent Marxist and feminist writings on women in sport. A discussion of the conditions, or terms, of lesbian visibility in mainstream media is key to my analysis.

Abstract:
The turbine blades of Gas Turbines operating with “dirty” fuels are sometimes covered by a very thin liquid film, which originates from the condensation of the alkalic sulfates (mainly) in the flue gases. These films may influence drastically the collision coefficient of the impinging (ash) particles. This, in turn, influences the future trajectories of these particles and their adhesive properties, especially in the rotor blades where the Coriolis aceleration becomes a significant factor in the particle kinetic energy absorption process. The study reports on the capture film height and the variation of the collision coefficient with the film height. The experimental conditions correspond to those encountered in rotor blades, where the surface tension, the wave-making process and the boyancy all contribute to this energy absorption. The results indicate that the model based on the interaction of these factors gives a goodexplanation of the energy absorption process, except for the very thin height where particle apparent mass effects dominate the process.
