Abstract From solid phase peptide synthetic approaches of mMIF(93-115) the C-terminal sequence turned out to be a typical „difficult sequence“ peptide. Various stepwise syntheses were performed with different resins and coupling reagents. The Peptide was synthesized in 90% yield by solid phase fragment condensation of protected mMIF(93-108) to mMIF(109-115) ‘2-Chlorotrityl’ resin bound.

Abstract:
Study Design: Survey. Objectives: To characterize national practices of and shortcomings surrounding intraoperative assessments of spinal alignment. Methods: Spine surgeons in the US were surveyed to analyze their experience with assessing spinal alignment intraoperatively. Results: 108 US spine surgeons from 77 surgical centers with an average of 19.2 + 8.8 years of surgical experience completed the survey. To assess alignment intraoperatively, 84% (91/108) use C-arm or spot radiographs, 40% (43/108) use full-length radiographs, and 20% utilize the T-bar (22/108). 88% of respondents’ surgical centers (93/106) possessed a navigation camera and 63% of respondents (68/108) report using surgical navigation for 40% of their deformity cases on average. Reported deterrents for using current technology to assess alignment were workflow interruption (54%, 58/108), expense (33%, 36/108), and added radiation exposure (26%, 28/108). 87% of respondents (82/94) reported a need for improvement in current capabilities of making intraoperative assessments of spinal alignment. Conclusions: Corrective surgery for spinal deformity is a complex procedure that requires a high level of expertise to perform safely. The majority of surveyed surgeons primarily rely on radiographs for intraoperative assessments of alignment. Despite the majority of surveyed surgical practices possessing navigation cameras, they are utilized only for a minority of spinal deformity cases. With the majority of surveyed surgeons reporting a need for improvement in technology to assess spinal alignment intraoperatively, 3 of the top design considerations should include workflow interruption, expense, and radiation exposure.

Abstract:
Background: Systemic candidiasis is associated with a high crude mortality rate, even with first line antifungal therapy. C. albicans is the predominant cause of invasive fungal diseases which is a serious public health issue. The main objective was to assess the reliability of different media for germ tube production in Candida albicans isolated from various clinically diagnosed pulmonary samples. Methods: All Candida isolates were identified and speciated by conventional methods such as Gram’s staining, germ tube test, chlamydospore formation on corn meal agar, sugar fermentation test, sugar assimilation test, and growth on Hi-chrome candida agar. Results: Out of 108 clinical isolates of Candida albicans, 5 different methods were used for germ tube production. Pooled human sera showed 93/108 (86.1%) was the most sensitive method wherein YEPD (yeast extract peptone dextrose) broth 91/108 (84.7%) was the reliable and easy method for detection of germ tube, followed by trypticase soy broth 81/108 (81.4%); peptone water 80/108 (74.7%) and 2% sucrose 71/108 (65.7%).Conclusions: YPED broth is found to be a better serum free substrate and subsequently for the presumptive differentiation of C. albicans from non-albicans candida (NAC), without the extensive time required for the preparation and testing of pooled human serum. Furthermore, this medium is commercially available, more stable, effective, and is not bio hazardous.

Abstract:
The objective: to determine causes of frequent detection of disseminated tuberculosis in HIV-infected patients taking into account the behavioral characteristics of patients, to propose additional measures to enhance tuberculosis detection.Subjects and Methods. 108 questionnaires filled out by TB/HIV co-infected patients staying in specialized hospitals in 3 regions of the Russian Federation were studied. We filled out the first 18 questionnaires ourselves while talking to patients, to assess whether the questions were clear to patients. The questionnaire included 28 questions with prompted answers; in addition, patients could give their own answers. Using the questionnaire, we found out whether the patients were seeking medical help in a timely manner, units to which they referred, awareness of tuberculosis before developing it, and exposure to tuberculous infection.Results. The majority of interviewed patients with previously diagnosed HIV infection did not undergo medical examination at the Center for AIDS Prevention and Control (CAIDSPC). When complaints associated with tuberculosis appeared, 57/108 (52.8%) respondents referred to the clinic at their place of residence. In 56/108 (51.8%) patients, 2 or more months passed from the moment of referral for medical help to diagnosis and initiation of tuberculosis treatment. 7/108 (6.5%) respondents reported history of tuberculosis, and 4/108 (3.7%) respondents reported exposure to a tuberculosis patient. After developing tuberculosis, 20/108 (18.5%) respondents were exposed to other HIV-infected patients. After detection of HIV infection, 93 (86.1%) patients indicated that they had not been made aware of tuberculosis.

Abstract:
Unit times and unit costs for initial and ongoing property mapping and parcel indexing have been established from time/task and expenditure records at the regional office of LRIS in Bathurst from June 1, 1979, the date it was opened, to March 31,1986. During the first five years of this period, the initial property mapping and parcel indexing for the 108 000 parcels in Restigouche, Gloucester, and Northumberland counties was completed; during the latter two years of the period the property mapping and parcel indexing were kept up-to-date by incorporating the results of the 24 953 changes that occurred. Person time was normalized to the Property Mapper I level, and expenditures were converted to 1981 Canadian dollar equivalents. The time, 93 minutes, required to make each change was somewhat less than the time, 108 minutes, for the initial property mapping and parcel indexing; similarly, the unit cost per change, 1981C$17.63, was less than the cost, 1981 C$26.95, for the initial compilation.

Abstract:
Object Noninvasive neuroimaging techniques are increasingly identifying unruptured intracranial arterial dissections (IADs) at examination for headache or ischemic symptoms. Approximately 3% of cases of aneurysmal subarachnoid hemorrhage (SAH) are caused by IADs in Japan, but the natural history of unruptured IADs is not known. Methods Clinical data obtained in 190 patients with 206 IADs were retrospectively analyzed on the basis of long-time follow-up of geometry and clinical event. The IADs were divided into an unruptured group and SAH group depending on the patient's clinical status at the initial diagnosis. Day 0 was defined as the day preceding diagnosis of IAD—that is, the day of symptom onset. This was retrospectively determined from the clinical history. Results The 206 IADs included 98 unruptured lesions and 108 SAH. In both groups, the vertebral artery was the most frequent site. In the unruptured group, 93 IADs were followed for a mean of 3.44 years. The mean interval between symptom onset (Day 0) and neuroimaging diagnosis was 9.8 days. Subsequent geometry change was seen in 78 (83.9%) of 93 IADs. Major change was almost completed within 2 months, and complete normalization was seen on neuroimaging in 17 (18.3%) of 93 IADs, with the earliest on Day 15. Rupture of the IAD in the unruptured group occurred in only 1 patient on Day 11. In the SAH group, 84 of the 108 patients complained of preceding headache before onset of SAH. In 81 (96.4%) of the 84 patients, SAH occurred on Day 0–3 with the latest on Day 11. In all patients in the unruptured and SAH groups, the latest day of SAH from the onset of preceding headache was Day 11. Conclusions Most IADs causing SAH bleed within a few days of occurrence. Most IADs that are unruptured already have little risk for bleeding at diagnosis because of the repair process. Intracranial arterial dissections may be much more common than previously thought, and the majority may occur and heal without symptom manifestation.

Abstract:
A study was conducted to investigate the antimicrobial effect of sodium lactate (NaL) (0, 1.5, 3.0, and 4.5%) on the survival of Escherichia coli O157:H7 in 93% lean ground beef. Samples inoculated with a mixture of four strains of E. coli O157:H7 (107 to 108 CFU/g) were subjected to immersion heating in a water bath stabilized at 55, 57.5, 60, 62.5, or 65°C. Results of statistical analysis indicated that the heating temperature was the only factor affecting the decimal reduction times (D-values) of E. coli O157:H7 in 93% lean ground beef. The change in temperature required to change the D-value (the z-value) was determined as 7.6°C. The thermal resistance of this organism was neither affected by the addition of NaL nor by the interactions between NaL and temperature. Adding NaL to ground beef to reduce the thermal resistance of E. coli O157: H7 is therefore not recommended.

Abstract:
Silver nanoparticles (NPs) films with different nominal thicknesses were deposited on the anodic aluminum oxide (AAO) templates with pore diameter about 200 nm by thermal evaporation. These NPs films were used as the surface enhanced Raman scattering (SERS) active substrates. The microstructure and surface morphology of the films were studied by X-ray diffraction and scanning electron microscopy, respectively. The SERS activity of the films was investigated by Raman scattering of adsorbed rhodamine 6G (R6G) at different concentrations. The results revealed that the average diameter of Ag NPs in different samples is 58 nm, 75 nm, 93 nm and 108 nm, respectively. Ag NPs film on AAO template is very suitable as a SERS active substrate and can detect R6G molecules with a concentration of 10-8 M. The intensity of the SERS spectra is related to both the Ag NPs size and interparticle distance. The sample with an average particle diameter of 93 nm has the highest SERS enhancement factor (1.3 × 106).

Abstract:
Abstract With the aim of assessing the impact of climate change on surface water resources, a conceptual rainfall-runoff model (the tank model) was coupled with LARS-WG as a weather generator model. The downscaled daily rainfall, temperature, and evaporation from LARS-WG under various IPCC climate change scenarios were used to simulate the runoff through the calibrated Tank model. A catchment (4648 ha) located in the southern basin of the Caspian Sea was chosen for this research study. The results showed that this model has a reasonable predictive capability in simulating minimum and maximum temperatures at a level of 99%, rainfall at a level of 93%, and radiation at a level of 97% under various scenarios in agreement with the observed data. Moreover, the results of the rainfall-runoff model indicated an increase in the flow rate of about 108% under the A1B scenario, 101% under the A2 scenario, and 93% under the B1 scenario over the 30-year time period of the discharge prediction.

Abstract:
ABSTRACTLambs were allocated to three treatments: control (C), monensin (M) and lasalocid sodium (L) in a concentrate diet containing 36 mg copper per kg food. After 2 months deaths from copper toxicity occurred and the trial was terminated. All lambs were slaughtered. The mean liver copper concentration (mg/kg dry matter) of 1907 in the M treatment was higher than the mean concentrations of 1342 and 1229 in the C and L groups respectively (P < 0·01). Total liver copper was 234, 260 and 230 mg and the retention of dietary copper in the liver was 93·5, 108·5 and 93·3 g/kg for the C, M and L treatments respectively. These differences were not significant. From reduced liver size (as a proportion of body weight) and high copper levels in the kidneys, it was concluded that the group given monensin had accumulated copper to a level closer to the toxicity threshold than the other two groups.

Abstract:
The purpose of the study is to establish the types of emotions that are expressed by primary school teachers, their frequency, and the situations that trigger them. Teachers were observed by primary education students during their practical work at school. The observations of 108 teachers in 93 primary schools in Slovenia were analysed. The results show that teachers express a variety of emotions with the unpleasant ones prevailing. Anger was the most frequently expressed emotion. The situations that triggered emotions were varied. Among them, students’ discipline and academic achievement triggered the majority of pleasant and unpleasant emotions in teachers.

Abstract:
The dynamic simulation model ‘PNUTGRO’ was used to predict groundnut growth and yield from 1989 to 1993 at Ludhiana, India. The simulated flowering, pegging and physiological maturity dates, leaf area index (LAI), pod and seed yields and shelling percentage of groundnut were compared with actual observations for the commonly grown cultivar, M-335. The simulated phenologic events showed deviations of only −3 to +3 days for flowering, −3 to +2 days for pegging and −4 to +2 days for physiological maturity of the crop. The model estimated the LAI to be within 95–108% (mean 101·5%) and shelling percentage to be within 93–108% (mean 100·5%) of the actual values. The model predicted the pod yields from 89 to 111% (mean 100%) and seed yield from 90 to 110% (mean 100%) of the observed yields. The results obtained with the model for the five consecutive crop seasons revealed satisfactory predictions of phenology, growth and yield of groundnut and hence the model ‘PNUTGRO’ can be used for prediction of groundnut production in the central plains of the Indian Punjab.

Abstract:
Background Infection is the most common cause of morbidity and mortality in patients undergoing myleosuppressive therapy with the risk of infection being heightened during the neutropenic phase. Fluoroquinolones are most often utilized as prophylaxis, specifically levofloxacin or ciprofloxacin; however, there is increasing resistance among these agents. The objective of this study is to compare the efficacy of ciprofloxacin and levofloxacin when used prophylactically in hematopoietic stem cell transplantation patients. Study design A retrospective cohort study conducted at a 443-bed tertiary teaching county hospital from 1 January 2005 to 31 September 2016. Methods Patients aged 18–89 who were admitted and received levofloxacin or ciprofloxacin post hematopoietic stem cell transplantation were evaluated. Results The patient population (N = 151) was predominantly male (93 vs. 58) and the median (IQR) age was 57 (20.1) years. There were 108 patients undergoing autologous hematopoietic stem cell transplantation compared to 43 undergoing allogenic hematopoietic stem cell transplantation. Significantly fewer patients who received levofloxacin (11/43, 25.6%) developed neutropenic fever compared to patients who received ciprofloxacin (61/108, 56.5%, p = 0.0006). Also there were significantly more positive blood cultures in the ciprofloxacin group (36/108, 33.3%) compared to the levofloxacin group (4/43, 9.3%); the majority of which were Gram-positive organisms (p = 0.0025). Conclusion Prophylaxis with levofloxacin was associated with a lower incidence of febrile neutropenia and bacteremia when compared to ciprofloxacin in hematopoietic stem cell transplantation patients.

Abstract:
Background: The TNI-93 is a quick memory test designed for all patients regardless of their education level. A significant proportion of patients with Alzheimer’s disease (AD) are illiterate or poorly educated, and only a few memory tests are adapted for these patients. Objective: In this study we aimed at assessing the diagnostic value of the TNI-93 for diagnosis of patients with biologically confirmed amyloid status. Methods: We included all patients who had an analysis of AD cerebrospinal fluid biomarkers, a neuropsychological assessment including a TNI-93 and an anatomical brain imaging at Avicenne Hospital between January 2009 and November 2019. We compared the TNI-93 scores in patients with amyloid abnormalities (A+) and patients without amyloid abnormalities (A-) according to the AT(N) diagnostic criteria. Results: 108 patients were included (mean age: 66.9±8.5 years old, mean education level: 8.9±5.2 years). Patients from the A + group (N= 80) were significantly more impaired than patients from the A- group (N= 28) on immediate recall (A+: 5.9±2.8; A-: 7.4±2.6; p = 0.001), free recall (A+: 3.5±2.7; A-: 5.9±2.8; p ≤ 0.001), total recall (A+: 5.7±3.5; A-:7.8±2.8; p ≤ 0.001), and on number of intrusions during the recall phase (A+: 1±1.8; A-: 0.1±0.3; p = 0.002). ROC curves revealed that the best scores to discriminate A + from A- patients were immediate recall (Area under curve (AUC): 0.70), number of encoding trials (AUC: 0.73), free recall (AUC: 0.74), and total recall (AUC: 0.74). Conclusion: The TNI-93’s immediate, free, and total recalls are valuable tools for the 39 diagnosis of AD.

Abstract:
BackgroundThalassaemia and variant haemoglobin are the most common severe monogenic disorders worldwide.AimsTo develop prenatal diagnosis programmes for the prevention of the most important haemoglobin disorders and identify healthy carriers of thalassaemia.MethodsSequencing analysis was used to obtain complete data on gene structure and to correlate specific phenotypic expression with mutations, especially for new or very rare mutations in globin genes.ResultsA rare single nucleotide variation, HBB:c.93–23T>C, located in nucleotide 108 of the first intervening sequence of the HBB gene, was identified. This variation was previously reported but its clinical significance was not known. Six heterozygous patients had this nucleotide variation and eight further cases co-inherited it together with other defects in the globin genes. Heterozygous subjects for this substitution showed normal haematological and electrophoretic features, whereas subjects who were compound heterozygotes for this mutation and another defect in globin genes showed the classic phenotype of a healthy carrier.ConclusionThis nucleotide can be considered a single nucleotide polymorphism and not a thalassaemic mutation that reduces the production of haemoglobin. This is another example of a very rare nucleotide variation. Knowledge of this is important so that appropriate genetic counselling can be carried out of a couple potentially at risk, where one of the partners is a carrier of β-thalassaemia and the other is carrier of a nucleotide variation.

We have tested recent suggestion of anomalous sensitivity in highly excited quantum many-body systems. Two independent measurements of cross sections for the 19 F + 93 Nb strongly dissipative heavy-ion collisions have been performed at incident energies from 102 to 108 MeV in steps of 250 keV. In the two measurements we used different, independently prepared, 93Nb target foils with nominally the same thickness. The data indicate statistically significant non-reproducibility of the energy oscillating yields in the two measurements. The observed non-reproducibility is consistent with recent theoretical arguments on spontaneous correlation, slow phase randomization and chaos in highly excited complex quantum systems.

Abstract:
INTRODUCTION: Modern and effective diagnosis of osteomyelitis is the basis for successful therapy; at the same time the problem of establishing this pathology, especially in the early stages of the development of the process, has not been completely resolved.OBJECTIVE: To develop of ultrasound imaging of metaepiphyseal osteomyelitis in young children, depending on the duration of the disease.MATERIAL AND METHODS: 108 children (boys — 65% (70/108), girls — 35 (38/108)) from 1 month to 2 years were examined. Children's age from 2 up to 8 months — 57% (62/108), newborns — 26% (27/108). The age of other children: 12% (13/108) — 9–12 months, 5% (6/108) — 1– 2 years. All children underwent x-ray and ultrasound study. The diagnosis of osteomyelitis was verified by joint puncture (n=102), bone puncture (n=57), drainage of periarticular phlegmon (n=12).Statistics: processing of the study results was carried out using the Statistica 13.0 application package (TIBCO Software Inc, 2017). To assess the effectiveness of ultrasound in the diagnosis of acute osteomyelitis, the diagnostic effectiveness and predictive value of positive or negative results were calculated, and the likelihood ratio was calculated.RESULTS: 3 groups were identified depending on the duration of the disease at the time of admission to the hospital: Ia — 1–3 days, Ib — 4–7 days, Group II — 8–14 days, Group III — 15–21 days from the onset of the disease. DE indicators for patients of group Ia: sensitivity — 81.8%, specificity — 60.0%, accuracy — 75%, PV(+)=81.82%; PV(–)=60.00%; OR=4.50. DE indicators for patients in group Ib: sensitivity — 86.6%, specificity — 66.6%, accuracy — 80.95%, PV(+)=87.5%; PV(–)=60.0%; OR=7.00. DE indicators for patients in group II: sensitivity — 92.98%, specificity — 80.0%, accuracy — 93.44%, PV(+)=98.15%; PV(–)=57.14%; OR=37.86. DE indicators for patients of group III: sensitivity — 91.66%, specificity — 91.0%, accuracy — 91.66%, PV(+)=100.0%.DISCUSSION: Ultrasound imaging of acute osteomyelitis in children of group Ia included nonspecific changes in periarticular tissues and had the lowest diagnostic accuracy (75%). Diagnostic accuracy increased in group Ib to 85% due to initial structural changes in the zone of preliminary calcification and hyaline cartilage of the epiphysis. Disease progression in group II was characterized by more distinct structural changes in the metaphysis with an increase in diagnostic accuracy to 93%. In patients of group III, the highest rates of diagnostic accuracy (91.67%) were obtained due to visualization of destructive changes in the epiphysis, metaphysis and zone of preliminary calcification.CONCLUSION: The ability to detect ultrasound signs of acute hematogenous metaepiphyseal osteomyelitis in children will allow the use of this method in the early diagnosis for the timely selection of an adequate treatment tactics.

Abstract:
Abstract A simple, fast, and robust method was developed for the determination of fumonisin B1 (FB1), fumonisin B2 (FB2), and fumonisin B3 (FB3) in corn-based human food and animal feed (cornmeal). The method involves a single extraction step followed by centrifugation and filtration before analysis by ultra-performance liquid chromatography/electrospray ionization (UPLC/ESI)-MS/MS. The LC/MS/MS method developed here represents the fastest and simplest procedure (<30 min) among both conventional HPLC methods and other LC/MS methods using SPE cleanup. The potential for high throughput analysis makes the method particularly beneficial for regulatory agencies and analytical laboratories with a high sample volume. A single-laboratory validation was conducted by testing three different spiking levels (200, 500, and 1000 ng/g for FB1 and FB2; 100, 250, and 500 ng/g for FB3) for accuracy and precision. Recoveries of FB1 ranged from 93 to 98% with RSD values of 3-8%. Recoveries of FB2 ranged from 104 to 108, with RSD values of 2-6%. Recoveries of FB3 ranged from 94% to 108%, with RSD values of 2-5%.

Aims. Left distal transradial arterial approach (ldTRA) is a new interventional route that spares right radial artery (RRA) for use in haemodialysis and as bypass graft. Vasant Kunj Left dIstal Transradial ArtEry approach (VKLITE) study aimed to assess the feasibility and safety of ldTRA access during coronary angiography (CAG) and percutaneous coronary intervention (PCI). Methods and Results. Between April 2018 and June 2020, 108 patients were enrolled and underwent CAG ± PCI via ultrasound guided ldTRA. Arterial puncture, CAG, and PCI were successful in 96.3% (104/108), 92.1% (93/101), and 94.1% (32/34) patients, respectively. Access site crossover rate was 14/108 (13.0%). Mean puncture, procedure, and haemostasis time (minutes) were 6.7 ± 7.1, 55.7 ± 32.8, and 23.1 ± 11.9. Median total fluoroscopic time was 6.6 minutes (IQR-14.2), and median total radiation dose was 39.2 Gy-cm2 (IQR-97.0). Local haematoma occurred in 11 patients (10.2%) with major haematoma in 1.9%, all recovering within three weeks. Mean pain score was 2.4 ± 2.3, and patient satisfaction score was 9.0 ± 1.3. LdTRA access compared with RRA access (n = 121) showed significantly increased mean procedure time (55.7 ± 32.8 vs. 43.9 ± 26.0 minutes, p = 0.01) and median total fluoroscopic time (6.6 [IQR-14.2] vs. 4.7 [IQR-8.2] minutes, p = 0.02), with similar median total radiation dose (39.2 [IQR-97.0] vs. 43.8 [IQR-54.5] Gy-cm2, p = 0.56). No radial artery loss, dissection, pseudoaneurysm, arteriovenous fistula, or nerve injury was noted. Conclusions. LdTRA access is feasible with few complications during CAG/PCI. Patient comfort and satisfaction makes it a desirable route for coronary interventions.

Abstract:
Objective: The objective of this study was to categorize and quantify the most common causes of joint- or leg-associated lameness by summarizing available information from cases presented to the Iowa State University Veterinary Diagnostic Laboratory (ISU VDL) between 2010 and 2015. Materials and methods: All cases of lameness or locomotor dysfunction in 7- to 40-week-old pigs submitted to the ISU VDL between May 1, 2010 and April 30, 2015 were retrieved. After removing cases that did not meet the inclusion criteria, the remaining cases were individually reviewed and assigned a primary and secondary diagnosis. Results: Of the 1847 cases retrieved, 464 met the inclusion criteria. The 4 most common primary diagnosis categories were Mycoplasma hyosynoviae (93 cases; 20%), metabolic bone disease (86 cases; 18.5%), infectious arthritis due to non-Mycoplasma bacterial infection (81 cases; 17.5%), and lameness with inconclusive findings (101 cases; 21.8%). There were 23.3% of the cases (108 of 464 cases) that had a secondary diagnosis with metabolic bone disease (28.7%; 31 of 108 cases) identified as the most common secondary diagnosis. Implications: This study reinforces the importance of careful clinical examination, proper sampling, and confirming causes with appropriate diagnostic testing for accurate diagnosis of lameness.

Abstract:
This paper investigates applicability of cassiterite to dating ore deposits in a wide age range. We report in situ LA-ICPMS U-Pb and Pb-Pb dating results (n = 15) of cassiterite from six ore deposits in Russia ranging in age from ~1.85 Ga to 93 Ma. The two oldest deposits dated at ~1.83–1.86 Ga are rare metal Vishnyakovskoe located in the East Sayan pegmatite belt and tin deposits within the Tuyukan ore region in the Baikal folded region. Rare metal skarn deposits of Pitkäranta ore field in the Ladoga region, Fennoscandian Shield are dated at ~1.54 Ga. Cassiterite from the Mokhovoe porphyry tin deposit located in western Transbaikalia is 810 ± 20 Ma. The youngest cassiterite was dated from the deposits Valkumei (Russian North East, 108 ± 2 Ma) and Merek (Russian Far East, 93 ± 2 Ma). Three methods of age calculations, including 208Pb/206Pb-207Pb/206Pb inverse isochron age, Tera-Wasserburg Concordia lower intercept age, and 207Pb-corrected 206Pb*/238U age were used and the comparison of the results is discussed. In all cases, the dated cassiterite from the ore deposits agreed, within error, with the established period of magmatism of the associated granitic rock.

Abstract:
Objectives To present a neuroradiographic imaging algorithm for patients presenting with pulsatile tinnitus. Methods This was a retrospective review of patients presenting to a tertiary care academic medical center from 1993 to 2007 with a chief complaint of pulsatile tinnitus. Clinical presentation and diagnostic imaging data were analyzed. Results Of the 108 patients identified, 93 patients had subjective pulsatile tinnitus and 15 patients presented with objective pulsatile tinnitus. In patients with subjective pulsatile tinnitus, 27/93 (29%) had positive radiologic findings, with 71% of cerebral angiogram studies and 57% of MR studies revealing anatomic abnormalities responsible for the pulsatile tinnitus. In patients with objective pulsatile tinnitus, 9/15 (60%) had positive radiologic findings, with 80% of cerebral angiogram studies and 57% of MR studies revealing anatomic abnormalities responsible for the pulsatile tinnitus. Magnetic resonance sensitivity in subjective and objective pulsatile tinnitus was 67% and 57% respectively. Conclusions Magnetic resonance is an excellent first-line diagnostic imaging modality in the assessment of pulsatile tinnitus. However, regardless of the subjective or objective nature of pulsatile tinnitus, cerebral angiography should be considered in patients with a negative MR and disabling pulsatile tinnitus.

Abstract:
Erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis are related disorders of skin and mucous membranes, which are typically associated with antecedent medication use or infection. We review 108 cases of erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis from Texas Children's Hospital, Houston, Texas, from 1981 to 1991, and illustrate the characteristic skin and mucosal lesions. In addition, we describe in detail two unusual cases requiring intensive airway management. Head and neck manifestations were present in 4 of 79 patients (5%) with erythema multiforme and 26 of 28 patients (93%) with Stevens-Johnson syndrome. In Stevens-Johnson syndrome, mucosal involvement of the lip (93%), conjunctiva (82%), oral cavity (79%), and nose (36%) were most common. Antecedent medication use was identified in 59% of erythema multiforme patients and 68% of Stevens-Johnson syndrome patients. We note a striking increase in the number of cases in our series caused by cephalosporins. Fifty percent of Stevens-Johnson syndrome patients required supplemental hydration or alimentation because of the severity of the oral cavity involvement. The head and neck mucosal manifestations largely respond to local care, and the routine use of prophylactic antibiotics or systemic steroids is not recommended.
