Scapho-trapezio-trapezoid (STT) arthrodesis is a relatively common procedure performed for severe STT arthritis, scapholunate instability and Kienböck's disease. Twenty-eight of 31 patients who had undergone this procedure were reviewed at a mean of 65 months following surgery (range 24 to 97 months). Four patients underwent wrist arthrodesis; two for failed STT fusion; and two for uncontrolled pain related to pre-existing carpal osteoarthritis. Isolated injuries of the scapho-trapezial ligament complex are not well recognized. The ligament complex comprises the stout scapho-trapezial ligament, the floor of the flexor carpi radialis (FCR) tendon sheath and the scapho-capitate ligament. Between August 1991 and May 1992, we diagnosed and treated four cases of partial chronic post-traumatic lesions of this ligament complex. Trapezio-lunate external fixation for scaphoid fractures has been investigated experimentally and clinically. Six below-elbow fresh amputation specimens in which proximal, waist and distal scaphoid fractures were created, displaced, reduced and externally fixed were used for an experimental study and after 10,000 extension-flexion movements of the wrist, no displacement was observed. We treated 36 scaphoid fractures in 34 patients with trapezio-lunate external fixation. According to the Herbert and Fisher classification, there were 16 type B1, 14 type B2, four type B3 and two type B4 fractures. A relationship between scapho-trapezio-trapezoid (STT) osteoarthritis and static dorsal intercalated segment instability (DISI) was suspected. The radiographs of 697 wrists in patients over 50 years were surveyed. In 63 wrists STT osteoarthritis was found in isolation, and isolated DISI was found in four wrists. A combination was found in 16 wrists. This was statistically significant (0.02< P<0.05). It is likely that the static DISI occurs as a consequence the STT osteoarthritis.

Anatomical studies show that the abductor pollicis longus acts primarily as a radial deviator of the wrist and as the main stabilizer of the basal joint of the thumb. An advancement and reinforcement technique of this muscle is used in conjunction with a standard silastic replacement arthroplasty for osteoarthritis of the trapezio-metacarpal joint. Excellent postoperative stability is achieved together with good pain relief and improvement in strength.

20 patients with 22 Caffinière prostheses in the trapezio-metacarpal joint were evaluated at a median of nine years after operation. The indication was degenerative osteoarthrosis in 20 cases and rheumatoid arthritis in two cases. We found 18 of the 22 (82%) original prostheses still in place, with satisfactory pain relief and good function. Three hands had been revised due to aseptic loosening, with replacement of two cups and one total prosthesis. Two of the revised prostheses were functioning well 6½ and 10 years after the revision.
