Abstract:
Acute pesticide poisoning is a common mode of intentional self harm. Oral ingestion is the usual mode of poisoning. However, inhalation, accidental or occupational transdermal exposure leading to acute or chronic poisoning can be the other route of poisoning. It has been seen that the purpose of poising is suicidal intensity in most of the cases. We report an unusual case where the victim had acute pesticide poisoning through transdermal route that was intended for non suicidal purpose. The patient was managed successfully with immediate decontamination and adequate antidote.

Background: Organophosphate (OP) poisoning is a significant global health concern, especially in developing countries where these chemicals are widely used in agriculture and household pest control. Children are particularly susceptible to accidental OP poisoning due to their inquisitive nature and immature physiology. This case report presents a child with acute muscarinic symptoms following accidental ingestion of an OP insecticide, emphasizing the importance of early recognition and treatment. Poisoning is one of the reasons for which Emergency Medical Teams are called. They range from mild and asymptomatic incidents to those complicated by shock and Sudden Cardiac Arrest. The causes of poisoning may be related to accidental contact with a potentially poisonous substance, but also to deliberate use or exposure to poison for suicidal purposes. Some of these events are classified as situations subject to the assessment of forensic specialists. Rescue procedures in case of poisoning depend on the type of poison, the course of poisoning and the clinical signs presented by the patient. M

Prior to the 1950s, there existed no formal system for poison prevention or treatment in the United States. Estimates place the number of pediatric poisoning fatalities at over 400 per year at that time. After World War II, urbanization and modern technological methods brought forth over 250,000 different brand name products on the market. Health care professionals presented with cases of acute poisoning usually had little knowledge of what ingredients were contained in these new products making it difficult, if not impossible, to treat these patients. It is a very common plant poison in rural South India, with a poor outcome. Increase in age, underlying chronic diseases of kidneys and heart decides the outcome of the case. Suicidal or accidental ingestion of the plant poison causes hypokalemia, arrhythmias, metabolic acidosis, ARDS and eventually death. Death is mainly due to its alkaloid Cleistanthin A and B and oduvan.

OBJECTIVES Accidental poisoning in children is a serious public health problem in developing countries, especially where chemicals used in agriculture are ingested. Paraquat is a herbicide which is still used in Pakistan. We report a case of fatal unintentional paraquat ingestion by 3 years old child who could not survive. Lack of care in the storage of paraquat lead to the poisoning but initial inappropriate management was also one factor in the fatal outcome. Detailed history and provision of poison containers are important in the proper management of patients with poisoning.
