Abstract:
ADHD is a neurodevelopmental disorder with symptoms that start during childhood or early adolescence and commonly persist into adulthood. In adults, similar symptoms and impairments are seen as in children, although overt levels of hyperactivity-impulsivity tend to decrease, inattention tends to persist, and age-appropriate presentation of symptoms needs to be considered. Emotional self-regulation, sleep problems, excessive spontaneous mind wandering and behavioural self-regulation often co-occur with adult ADHD and are independent sources of impairment. In establishing the diagnosis of adult ADHD it is important to be aware of potential symptom overlap with other common mental health disorders such as anxiety, depression, and personality disorder. There is a risk that ADHD is mistaken for these other disorders. In most cases ADHD can be accurately assessed and diagnosed in adults through the use of diagnostic interview assessments that provide a detailed clinical assessment of the diagnostic criteria for ADHD.

Abstract:
The treatment of ADHD in adults is very similar to that recommended for children and adolescents. There have been many more clinical trials evaluating the efficacy and safety of the drugs used in ADHD treatment on children than adults. Pharmacotherapy for adults with ADHD has been shown to be effective. The National Institute of Health and Care Excellence (NICE) guidelines recommend pharmacotherapy as first-line treatment for adult ADHD. The NICE guidelines consider stimulants (methylphenidate and dexamfetamine) as the first choice for treatment of adult ADHD, and the non-stimulant atomoxetine as a second-line treatment option, followed by other non-stimulant medications such as bupropion and tricyclic antidepressants. This recommendation is in line with European Consensus as determined by the European Network of Adult ADHD. Psychological treatment programmes have been developed implementing psychoeducation, cognitive–behavioural therapy, dialectical behaviour therapy, and mindfulness. Typically, psychological treatments have been added to drug treatment.

ADHD affects in a distinctive way the employment of service users. This can range from their choice of work, work performance, fitness for work, and requirements for adjustments to the workplace. Task performance, contextual performance, counterproductive work behaviour, and adaptive performance have been linked behind this impairment, and the fitness to work assessment needs to consider both the person with ADHD and the specific job the person is doing. The most evidence-based workplace intervention is treatment of ADHD with medication, although other strategies can be helpful. This, however, does not apply to every person with ADHD and many people are successfully employed.

One out of every 10 adult psychotherapy clients—or more—probably has attention deficit/hyperactivity disorder (ADHD). When psychotherapists miss the ADHD, treatment is frustrating and less effective. This book is for the general mental health professional who wishes to learn how to recognize, assess, and treat adult ADHD. Adults with ADHD are very different, developmentally and symptomatically, from children. ADHD affects adults of all races, gender identities, and backgrounds, and obvious hyperactivity is rarely part of the adult clinical picture. This book provides a full description of adult ADHD symptoms, based on the most current research, including executive functioning problems, emotional dysregulation, atypical reward sensitivity, and problems with time perception. Complete guidance to recognizing adult ADHD is provided, including behavior displayed during psychotherapy, developmental issues, educational challenges, and adaptive functioning problems. Depression, anxiety, self-image issues, and substance abuse are addressed, since these mental health problems are commonly associated with adult ADHD. Guidance is provided to help the mental health practitioner provide psychoeducation about cognitive differences in ADHD, which is essential for client self-acceptance and adaptive functioning. Common psychotherapy problems with ADHD clients are addressed, including chronic lateness to sessions, missed appointments, motivation problems, difficulties with homework, and tangentiality. The principles of cognitive-behavioral treatment of ADHD are provided, along with case examples to illustrate the implementation of these techniques. An overview of medications for ADHD is provided, including their benefits, contraindications, and side effects. Nontraditional treatments are reviewed for their effectiveness.
