Full textIn this study were analyzed the 100 Over The Counter drugs (OTCs) more sold in pharmacies and drugstores of the Peru, in he years 1997 and 1998, with the objective of determining the characteristics of the consumption and their value therapeutic. The OTC List was obtained of the International Marketing Services - Pharmaceutical Market Perú (IMS–PMP). The final sample considered as drugs per year was 74. For the classification of the drugs it was used , the Anatomical-therapeutic classification - Chemical (ATC) and the methodology of the value intrinsic therapeutic that considers effectiveness and risks. Most of the specialties 71,08% (1997) and 70,89% (1998) showed non high value. The therapeutic classes prevailed were: those for Respiratory Tract (R), Digestive and Metabolism Tract (A), Skin (D) and System Skeletal Muscle (M). The group R commanded the fixed-dose, and the group S (Sensory organs) in single dose, the subgroup R05 (cough and cold preparations) was the highest sales. The analysis of the economic impact and quality therapeutic confirm the hypothesis that the therapeutic contribution of OTCs drugs sold in the country is not significant and its consumption represents unnecessary expenses in the population. Key words: Analysis A.B.C., Classification A.T.C., Value Intrinsic Therapeutic, OTC drugs, Ethical Market, Popular Market, Total Market.

Full textIn the present work are given the basic notions for the execution of Bioavailability and Bioequivalence studies. The bioavailability studies allow to know the magnitude and rate of absortion of the active principle that reaches systemic circulation whereas the studies of comparative bioavailability allow to evaluate the bioequivalence of two medicines with the same active principle. On the base of System of Biopharmaceutical Classification of the drugs it is possible to establish that for demonstration of the bioequivalence of medicines included in the class 1 and class 2, the profiles of dissolution are recommended; whereas for class 3 and class 4 it is needed to determine the bioequivalence by means of measurement of seric levels of drug. Keeping in mind the aspects mentioned previously, it was taken of the list of medicines included in the National Petitory of Essential Medicines those that they would require bioequivalence studies to determine their interchangeability, being found a total of 61 active principles. This list devised in base to aspects as: sanitary risk, therapeutic margin and pharmacokinetic characteristics of the medicine. Likewise devised a basic protocol for the execution of Bioavailability and Bioequivalence studies, in which the main phases for their execution are indicated; also the document of free and reported consent for the volunteers is proposed. Finally a Unit for the execution of bioequivalence studies is proposed, the profiles of the professionals that would conform the multidisciplinary team are proposed and the diverse installations with the ones that should count the Unit are mentioned

Full textIn the present work, pranoprofen (PF)-loaded poly (lactic-co-glycolic) acid (PLGA) nanoparticles (NPs) were prepared and optimized by the solvent displacement technique. The optimized formulations were those prepared at aqueous phase pH values of 4.5 and 5.5, PF concentrations of 1.0 and 1.5 mg/mL, surfactant concentration of 5.0 and 10 mg/mL and PLGA concentration of 9.00 and 9.50 mg/mL. Under these conditions, the optimized NPs (PF-F1NPs y PF-F2NPs) showed a size appropriate particle size for topical administration (350 nm) and high entrapment efficiency (80%). Lyophilization assays was performed to ensure the long- term stability of these systems. The physicochemical properties of the NPs before and after freeze-drying were similar between them. The evaluation of the interactions between the drug and the polymer revealed that there is no evidence of any chemical interaction. The optimized PF-NPs formulations were incorporated into Carbomer hydrogel (HG_PF-F1NPs and HG_PF-F2NPs) or hydrogel with 1% or 3% azone (HG_PF-F1NPs-Azone and HG_PF-F2NPs-Azone), in order to obtain semisolid formulations that prolonging the contact of the PF with the eye or skin. The morphological analysis of the PF-NPs after incorporated into hydrogel revealed that the diameter of particle was similar to those of the NPs suspension. The in vitro release assay of PF from PF-NPs, HG_PF-NPs and HG_PF-NPs-Azone formulations revealed that these formulations exhibited sustained release behaviour with a slower release of PF compared to the free drug solution. The results obtained from the ex vivo corneal permeation and in vivo anti-inflammatory efficacy suggest that the ocular application of the semisolid formulations with a 1% azone could be appropriate in the treatment ocular oedema. In addition, these formulations showed an optimal ocular tolerance by HET-CAM method and Draize test. Otherwise, the results observed from the ex vivo transdermal permeation and in vivo anti-inflammatory efficacy assays of the PF indicate that the administration of the HG_PF-F2NPs may be more effective in the treatment of edema on the skin surface respect to the others formulations. No signs of skin irritancy were detected for all the semisolid formulations without or with a 3% azone.

Full text-- The Unit Dose Drug Distribution System (SMDUD) permits an adequate pharmacotherapeutic tracking in the patients, integration of the pharmacist to health’s team and it produces an important economic saving to the hospital. Our study it was designed as objectives: quantifying the devolution percents and identifying the therapeutic groups of bigger consumption and devolution, produced for the SDMDU in the period 2003-2004. Furthermore, identifying errors at examining the medical prescriptions attended for the SDMDU and know the participation of the pharmacist in the system. The method consisted in a retrospective-descriptive analysis of the medical prescriptions, reports of consumption-devolution and surveys guided to the team of health. The results obtained show that the total cost of medicaments and biomedical material were S/. 3 046 014.77, and the devolutions summed up S/. 171 634.14. The therapeutic groups of bigger consumption were the antibacterials with 47% and his devolution of 44%. The principal types of errors found were: the patient's incomplete data 77%, data in the prescription 37% and errors of dispensation 19%. Most of professionals opine that the pharmacist has a little active participation when he integrates to the team of health. It is concluded that: the devolution percents in the period of study went from 6% and in the SDMDU's functioning it is finding faults; thus as, in the provision of medications, errors of dispensation, lack of pharmacotherapeutic tracking and the little recognize participation of the pharmacists.

Donoghue, Craig. "Syntheses of Protein Degraders and Compounds for Targeted Drug Release."

Full textA multicompartmet compliance aid (MCA) is a professionally prepared repackaging storage system used for organizing solid dosage forms prescribed to an individual, usually for periods of up to a week. The use of MCAs is common policy worldwide and helps non-adherent patients to better manage their medication intake. One of the characteristics of a MCA that should be taken into account is permeability to moisture, since moisture is one of the most important environmental factors that can modify the stability of medicines. Since the barrier properties of a MCA cannot be assumed to deliver the same stability assurance as that of the manufacturer’s primary packaging, research is required to assess the effects that repackaging can have on medicine stability and hence on its efficacy and safety. The present Thesis is presented as a compendium of 4 articles published in indexed journals with impact factor. In the first publication, the permeability to moisture of one of the MCAs most commonly used in Spain was evaluated, in order to determine if it met the United States Pharmacopoeia (USP) requirements established for this type of devices. According to the results, the MCA assayed meets the higher protection against moisture (Class A) and therefore it met the conditions of permeability that the USP demands for these types of devices (minimum Class B). The following publications determine the stability of three widely prescribed medicines (omeprazole capsules, enalapril tablets and lamotrigine dispersible/chewable tablets (DCT)) that may be repackaged into MCA if they meet the stability specifications when stored inside. Given the results obtained, we can conclude that it would be acceptable to store omeprazole capsules and enalapril tablets for at least 60 days and lamotrigine DCT for up to 14 days into a USP Cass A MCA at 25°C±2°C/60% RH ±5% RH. This stability information is essential for pharmacists to ensure the suitable selection of MCA users and the optimal use of MCAs, and to provide transport and suitable storage conditions. Having such new information available will contribute to ensuring patient safety by facilitating more informed decision-making and improved drug quality management.

Medication use carries risks, either due to adverse events or lack of compliance. Drug related problems (DRPs) can occur with varying degrees of severity and this can result in an emergency department (ED) visit. The aim of the study was to determine the proportion of PRMs that motivate the consultation to ED in patients 18 years of age or older. A prospective observational study in the SU of a university hospital was carried out between April and December 2015. The information was collected through a tab and the cases were subsequently analyzed by a multidisciplinary team. The DRPs were classified according to the methodology of the University of Minnesota. Other variable evaluated was the change in compliance with pharmacotherapy in patients who reported taking medications on a regular basis, using Morisky-Green-Levine test during the consultation in the ED and 30 days post discharge by telephone call. A total of 994 patients were studied, 61% were women and the mean age was 50 ± 20 years. Of the 943DRPs detected at admission, 195 (20.7%) were DRPs that motivated the consultation to the ED. Compliance with pharmacotherapy increased more tan 180%, from 32% to admission to the consultation to 60% at 30 days after discharge. Future multicenter studies could assess reality in other regional/national settings and compare results with public and private centers
