The complexities surrounding the dying process may distort rational decision-making and impact care at the end of life. Advance care planning, which focuses on identifying the individual's definition of quality of life, holds great potential to provide clarity at the end of life. Currently, young adults are not the intended audience for advance care planning. A quality improvement project engaged 36 college-age adults in structured group advance care planning discussions and evaluated the perceived value of a self-recorded advance directive. Findings from a pre- and postintervention survey suggested that young adults welcomed a conversation about end-of-life care; they wished for more information and expressed that a video-recorded advance directive stimulated thoughts about their own definition of quality of life. Participants' improved self-perception of comfort, confidence, certainty, and knowledge regarding the advance care planning process and end-of-life care indicated young adults may be a willing and eager population for the expansion of advance care planning. In addition to directing advance care planning to a younger audience, a personal video-recorded advance directive may complement the current advance care planning process and aid individuals in defining their quality of life.

Abstract:
A topical problem of improving the fuel supply process in locomotive diesel engines in considered in this article. An ESUVT.01 type electronic control system developed by OOO PPP Dizelavtomatika (Saratov) for a D50 (6 ChN 31.8/33) locomotive diesel engine manufactured by Penzadizelmash is presented. It is shown that fuel efficiency and exhaust gas toxicity indicators can be significantly improved by optimizing the initial fuel supply phase, that is the injection advance angle. Experimental studies are conducted to assess the influence of this angle on the locomotive engine characteristics. Bench tests were carried out on a 1-PDG4D diesel generator consisting of the above-mentioned diesel engine and a traction generator MPT-84/39. As the result of the tests, dependencies of the diesel characteristics on the injection advance angle are obtained, and the need to optimize the injection advance angle according to the diesel operation mode is confirmed. Optimized values of the injection advance angle in the diesel characteristics modes are obtained. Fuel efficiency and exhaust gas toxicity indicators are determined at the optimized injection advance angle.

In order to improve the performance of the diesel engine, the original engine fuel injection advance angle is optimized, and a new advance angle of fuel injection is proposed in this paper. By numerical calculation with simulation of software FIRE, the effect of different combustion chamber structures on the cylinder pressure, temperature, accumulated heat release and the parameters such as NOx mass fraction was analyzed. From the simulation results, the optimized fuel injection advance angle had greatly improved the diesel combustion and emission performance. Finally, via experimental verification, the engine with optimized fuel injection advance angle has better dynamic performance, as well as less emission than original machine.

In the last years, research investigating consumer behaviour and how their decision-making process has advanced and has become an important topic in the marketing society and literature. To advance the research further, this paper presents an extensive literature review of academic publications in the area of buying decision-making process in marketing and its status. Furthermore, the paper presents latest trends and themes that emerge there. Based on 24 journal articles, reports and marketing books, the core models and theories in this area were evaluated and discussed. Moreover, a framework of “moments that matter” in consumer decision-making process and factors that influence them was elaborated for a possibility to influence consumer behaviour in favour of company’s offers. Furthermore, recommendations for marketers were suggested for deeper understanding the consumer behaviour and their buying strategies to empower marketing campaigns and be a success in the market. The paper also suggests several directions for future research related to buying behaviour.

The applicability of this framework is illustrated using real examples from the action research projects and relevant literature.Originality/value“Using data to advance service” is a critical and timely research topic in the service literature. This study develops an original, specific framework for a company’s use of customer-related data to advance its services and create customer value. Moreover, the four projects with industry and government are early CPM case studies with real data.

This study aims to explore the specifics of the advanced organizer learning model that can also be used in learning Islamic education. This research is literature research, which is analyzed using the method of description, interpretation, and data analysis. The discussion in this study includes the understanding of the advanced organizer learning model, its forms and objectives, steps, and advantages and disadvantages. The results showed that this model is a learning model that emphasizes the importance of strengthening students' initial knowledge as a preparation effort to receive new material. There are two forms, namely: Expository Advance Organizer and Comparative Advance Organizer. The model has four objectives, and three Step phases. This model also has some advantages as well as disadvantages.

This study was designed to investigate students’ motivation orientation in the learning of electric current circuits in secondary school physics after exposure of Science Process Skills advance organizer. The study adopted the posttest only design. Students were exposed to various electric current circuits making activities using electric current components before a physics lesson which was conventionally taught before being post tested. A students’ motivation questionnaire was then administered thereafter. A total of seventy two (72) form two secondary school physics students participated in the study. Data were collected using a six item students’ motivation questionnaire (SMQ) with a reliability of 0.799. Principal Component Analysis was used to reduce student’s responses of each of the six items to manageable variables for easy interpretation of students’ motivation orientation in the learning of electric current circuits in physics. The results of the study indicated that students’ exposure to the science process skills advance organizer made them develop confidence in the physics course, they were excited by the activities that had taken place before the physics lessons especially the experiments they engaged in themselves. The results also indicated students were stimulated when they made conclusions of the experiments exposed to them before the lesson and they were happy when they worked through experiments in groups. This study concludes that science process skills advanced organizer can be used to motivate students in the learning of Physics. The study recommends that teachers of Physics be encouraged to construct relevant science process skills advance organizers for other topics of Physics taught at secondary schools. The study also recommends that publishers and physics education experts publish current research on science process skills advance organizers that can be applied in other subjects.

This research aims to analyze the effectiveness and efficiency of the accounting information system for the down payment for the operational services process using Macro VBA. This research uses a descriptive qualitative research method. The study was conducted at PT Semen Indonesia (Persero) Tbk. Involving four informants in April 2024. The primary data used is gathered on-site through observation, interviews, and documentation. The results obtained from this research include a description of the flowchart and procedures for confirming operational advances. The operational advance confirmation system using Macro VBA has been running according to the guidelines. The process involves flowcharts of the divisions involved. PT Semen Indonesia (Persero) Tbk uses Macro VBA to improve work efficiency and effectivity in the operational advance confirmation process. The implementation of Macro VBA in the advance confirmation process has proven to increase work efficiency and effectivity by speeding up the data processing and reminder-sending processes. Additionally, the use of Macro VBA successfully reduced the need for manual labor, allowing employees to allocate their time to other tasks.

Three-dimensional computation of spray and combustion in 16V240ZJ diesel engine was performed by CFD numerical simulation tool FIRE. Because of the unceasing change of the temperature and the mixture in the cylinder, traditional performance-prediction method can only supply the limited information. The appearance of numerical simulation brought a bright future for the design and development of the new diesel engine. Combustion is an extremely complex process ,involving flow field, fuel injection and various combustion reactions. By calculus simulation, we have a thorough understanding of the combustion process of 16V240ZJ diesel engine. Through the flow field and concentrative analysis in the cylinder, the effect of advance angle of fuel on combustion process and formation of NOx and soot was investigated. The results show that the advance angle of fuel has a great effect on spray atomization and fuel-air mixture, and consequently influences the combustion and emission. By simulating , we can get the optimal advance angle of fuel for 16V240ZJ diesel engine is 15°BTDC, namely 345°CA, at which, the emissions are fewer ,and the efficiency is better .The CFD simulation has a direct value for optimizing the chamber’s structure and improving the combustion system.

Taking the complex geological structure of Tunbao Coal Mine in Xinjiang as an example, in view of the problems such as the inability to effectively advance and accurately detect the faults in front of the head‐on and adjust the roadway slope in advance during the tunneling of the coal mine, geophysical advance exploration must be carried out before the tunneling to meet the purpose of reducing the impact of complex geological structures, realizing the fine exploration of the coal mine, and also highly catering to the requirements of intelligent construction of the coal mine. In the specific work process, through the comparative analysis of different detection technology principles and advantages and disadvantages, combined with the coal face structure and geophysical characteristics of Tunbao Coal mine, analysis of the use of trough wave advance detection tunneling scheme, finally summed up the application of three trough wave advance detection methods, to provide a certain reference for similar mines.

Advance care planning is a process that supports conversations about the values that matter most to patients and their family members. The documentation of advance directives and code status in a patient’s electronic health record (EHR) is a critical step to ensure treatment preferences are honored in the medical care received. The current approach to advanced care planning documentation in electronic medical records often remains disparate within and across EHR systems. Without a standardized format for documentation or centralized location for documentation, advance directives and even code status content are often difficult to access within electronic medical records. This case report launched our palliative care team into partnership with the Information Technology team for implementation of a centralized, standardized, longitudinal, functional documentation of advance care planning and code status in the electronic medical record system.

7 Background: In Colorado, there is no legal designation as to who is authorized to make healthcare decisions for an adult unable to make decisions for themselves. Completing an advance directive is increasingly associated with better end-of-life outcomes. Failure to complete an advance directive leads to fewer days on hospice and expensive, end-of-life care. It was hypothesized that patients who completed a Medical Durable Power of Attorney (MDPOA) document early may have higher adoption of advance care planning and earlier hospice at end-of-life. Methods: The Rocky Mountain Cancer Centers (RMCC) is the largest private oncology group in Colorado with multiple offices across the state. The RMCC-Pueblo clinic developed and piloted a new patient process where, at registration, all new patients were invited to complete a MDPOA while also completing other new patient forms. The Medical Durable Power of Attorney (MDPOA) is a document that appoints a healthcare decision maker (“healthcare agent”). Data on 4,441 patients from Nov. 1, 2017, to June 30, 2018, were extracted from the electronic health record (EHR), including code status, advance care planning counseling appointment, advance directive completion, and outcomes. Results: During this pilot, advance directive capture within the EHR rose from 15.4% to 21.4%. While the number of patients who elected hospice enrollment at the end of life was about the same, the length of hospice stay increased from an average of 14 to over 20 days, a 50% improvement. Conclusions: A MDPOA as a routine part of the new patient intake process is feasible and scalable. It opens the door to future ACP discussions and improved hospice utilization. Because of these findings, we are currently expanding this process to all RMCC practice sites. The results suggest that early completion of an Advance Directive lead to ongoing and healthy discussions about the patient’s end-of-life preferences for care, including appropriate hospice enrollment.

The appeal process against the Korean customs offices’ assessment of customs duties can be divided into a pre-assessment procedure and a postassessment procedure in reference to the date of assessment. The pre-assessment review falls under the pre-assessment procedure, whereas an appeal, request for adjudication, and administrative litigation fall under the post-assessment procedure. While the pre-assessment review may be omitted, the request for adjudication is a post-assessment procedure which must take place before filing an administrative litigation, which consists of a trial, an appeal, and the final appeal. Strategies for reduction in tax risk include avoidance of taxation risk through advance ruling to the Korea Customs Service, utilization of the ACVA (a system for an advance agreement between the customs authority and taxpayer on the method of determination of the customs value), and request for rectification following adjustment of customs value of imported goods.

e18637 Background: We discovered four out of five Medical Oncology patients admitted to the hospital December 2019 and January 2020 were discharged to hospice and died within a week. None of those patients had advance directives. In November and December 2019, all oncology patient charts were reviewed and only 7% (12/174) had advance directives. We believe an established advance directive could have helped avoid unnecessary hospitalization, improved end of life care and reduced wasteful health care costs. This led to the question: Would process changes in a rural cancer center lead to an increase in completion and documentation of Advance Directives, thereby providing more patient goal congruent care? Methods: Data collection and process mapping was performed revealing current processes and overall barriers. This was analyzed and countermeasures were developed leading to process changes and interventions. The interventions included assigning dedicated staff to “own” the advance directives education, follow up and completion, training and certification of staff, and education of the entire Cancer Center on the new process and interventions. The new process included a standardized order set to cue a referral to the dedicated staff, standardized patient education and follow up and documentation consistencies. Results: The initial data collection revealed an advance directive completion rate of 7% in new oncology patients. Process inconsistencies and communication deficiencies, with redundant screening and no follow up, created significant barriers to advance directives. Repeat data collection after creating a position for dedicated staff to educate patients and help with advance directive completion revealed a completion and documentation rate of 6% and thus, was insufficient to lead to change. When a consistent process with formal communication to the dedicated staff was implemented, improvements were noted. Our most recent data collection revealed a 36% completion and documentation rate in advance directives. Conclusions: Lack of effective advance directive programs potentially lead to end of life care incongruent with patients’ wishes and wasteful of health care resources. An advance directive ensures that if a patient is unable to clearly communicate their wishes, these wishes are still honored. In addition, the advance directive can help avoid unnecessary hospitalizations, improve end of life care and reduce wasteful health care costs. As a rural cancer center with a limited service area, we are challenged with a small sample size. Practice will be needed to ensure consistency and efficacy of the current interventions in order to support continued improvement in completion and documentation rates of advance directives. This project will be expanded to address advance directives for all patients.

Aims: To explore the views and experiences of health professionals of the advance care planning process with children and young people. Methods: An online questionnaire was used to collect data, which were analysed thematically. Findings: Key findings related to barriers and facilitators to initiating and documenting advance care planning: understanding the process and the condition of the patient; how advance care planning works in practice; and access to relevant, affordable training options. Conclusion: Additional training and standardised documentation can help support the initiation and use of advance care planning, reduce misperceptions, and generate greater confidence in participating in the process. A larger multidisciplinary team, with better communication, will support improved relationships between professionals which will filter down to the families.

A workable model for physicians to use with patients along with appropriate guidelines is discussed in the context of the physician-patient relationship. Examined are variables involved in the establishment of guidelines for use by the physician in the office practice and inpatient settings. Key management and clinical factors for initiating and implementing an advance directive are identified for the practicing physician. Responsibility for the initiation of education regarding the advanced directive is discussed from the patient perspective. Effective use of advance directives with patients requires that a physician have an adequate knowledge base to appropriately guide the patient throughout the treatment process, and that any efforts are coordinated with other healthcare providers.

In order to solve the problems of complicated advanced support process, high labor intensity, affecting the rapid advance of working face and the destruction of roof bolt (cable) by advance single hydraulic prop in ultra kilometer deep mine roadway, the deformation characteristics of roadway surrounding rock is analyzed. Taking the 27304 working face of Wanglou coal mine as the engineering background, numerical simulation, field monitoring, and theoretical calculation were used to analyze the deformation characteristics of roadway surrounding rock within the advanced influence range of 27304 working face. This paper puts forward the active advance support technology scheme, in which grouting anchor cable replaces the existing single hydraulic prop in the advance influence range of the working face in the ultra-kilometer deep mine, and observes and analyzes the deformation and failure characteristics of the surrounding rock of the working face advance roadway. The numerical simulation results show that in the advanced influence range of deep roadway, grouting anchor cable was used to replace the previous single hydraulic prop, and the vertical stress at both ends of the working face decreased by 15 MPa, with a decrease rate of 33.3%; the displacement of roadway roof, floor, and two sides decreased by 10 mm, 55 mm, and 20 mm, with a decrease rate of 40%, 68.75%, and 47.6%, respectively. The field monitoring results show that the roof separation is obviously improved after using grouting anchor cable as the active advance support scheme. It solves the problem of safe and efficient production faced by the ultra-kilometer deep shaft in Wanglou coal mine and provides theoretical and technical support for unmanned double roadway advance support under the condition of safe and efficient mining.

Time-varying process models for micro-machining processes are important as they aid in control of machining parameters. In this research, a state-space-based process model for the temperature and strain generated near the cutting edge of the tool tip is identified using system identification approach. Fiber Bragg grating sensors were placed rigidly near the cutting edge of the tool tip in a micro-turning setup. Subsequently, micro-turning operations were carried out on aluminum and mild steel. The computer numerically controlled program was such that the machining parameters (feed velocity, depth of cut and RPM) change with machining time. The time-varying machining parameters act as inputs to the model, and the dynamic values of strain and temperature serve as model output. A state-space model was generated using the experimental data. Subsequently, a Kalman filter was used to intelligently predict the values of strain and temperature at the cutting edge of tool tip in advance using the model parameters identified by state-space modeling. Experimental results confirm that the time-varying model and the Kalman filter proposed in this research are effective in predicting the strain and temperature in advance with high accuracy. The maximum error in prediction of temperature was 0.4 °C, whereas for strain prediction, the maximum error was 0.3µ∈.

In the process of analyzing advance notification practices in the EU and Japan, the key features of best practices in the organization of advance notification are identified and described.Conclusions. Recommendations for the development of advance notification in Russia are formulated: to increase the number of factors used to determine the content of advance information provided on a mandatory basis. An attempt is made to develop methodological recommendations for verifying information at the stage of advance notification.

This paper describes the genesis of the Rapidform process and its pioneering place in the new developments leading to complete control of the processes of manufacture of prostheses. The materials and geometric considerations involved in the development of a double deformation process under microprocessor control are described. Stages in the development of the system show the advance from the initial application to modular below-knee prostheses through extensions to special suspension systems (supracondylar and suprapatellar) to Syme's and above-knee sockets. The clinical and laboratory results are summarized along with an account of the current aspects of the project, ie advanced clinical trials, testing and analysis. Setting the scene historically for the other computer based modules in this high technology approach to prosthetics, Rapidform has proven to be swift, accurate and economical in its operation. Also, in common with the rest of the suite of equipment, this socket production facility, despite its flexibility and technical sophistication, requires no special services beyond standard single phase mains electricity supply.

Objectives Advance care planning is the process by which people reflect upon their wishes and values for healthcare, discuss their choices with family and friends and document their wishes. Readiness represents a key predictor of advance care planning participation; however, the evidence for addressing readiness is scarce within the renal failure context. Our objectives were to assess readiness for advance care planning and barriers and facilitators to advance care planning uptake in a renal context. Methods Twenty-five participants (nine patients, nine clinicians and seven family members) were recruited from the Southern Alberta Renal Program. Semi-structured interviews were recorded, transcribed and then analyzed using interpretive description. Results Readiness for advance care planning was driven by individual values perceived by a collaborative encounter between clinicians and patients/families. If advance care planning is not valued, then patients/families and clinicians are not ready to initiate the process. Patients and clinicians are delaying conversations until “illness burden necessitates,” so there is little “advance” care planning, only care planning in-the-moment closer to the end of life. Discussion The value of advance care planning in collaboration with clinicians, patients and their surrogates needs reframing as an ongoing process early in the patient’s illness trajectory, distinguished from end-of-life decision making.
