Abstract:
This work deals with analysis of phonation in patients with Parkinson’s disease (PD). Approximately 90% of patients with Parkinson’s disease suffer from speech motor dysfunction called hypokinetic dysarthria. System for Parkinson’s disease analysis from speech signals is proposed and several types of features are examined. Czech Parkinson’s speech database called PARCZ is used for classification. This dataset consists of 84 PD patients and 49 healthy controls. Results are evaluated in two ways. Firstly, features are individually analysed by Spearman correlation, mutual information and Mann-Whitney U test. Classification is based on random forests along with leave-one-out validation. Secondly, SFFS algorithm is employed for feature selection in order to get the best classification result. Proposed system is tested for each gender individually and both genders together as well. Best result for both genders together is expressed by accuracy 89,47 %, sensitivity 91,67% and specificity 85,71 %. Results of this work showed that the most important vowel realizations for phonation analysis are sustained vowels pronounced with maximum or minimum intensity (not whispering).

Abstract:
The aim of the diploma thesis "Maternal Identification in Anorexia Nervosa" is to map the developmental context of the onset of anorexia nervosa in female patients related with the formation of relationships with immediate caregivers and the detailed focus on the daughter's relationship with the mother. The theoretical part offers a cross-section of psychoanalytic theories that specifically deal with such process. These are mainly the object relations theory, the attachment theory as well as the recent theory of mentalization. The initial premise of the whole work is the femininity itself, the age of the patients when the period of adolescence and female gender are amongst the main risk factors for the development of anorexia nervosa and contribute to the disproportion between male and female patients. The concept of identification between daughter and mother within the separation-individuation process is described both in early childhood and during adolescence. The empirical part focuses on a particular examination of the patient's relationships with primary caregivers, the immediate environment from childhood to adulthood and the factors involved in the development of the disease. Through a semi-structured in-depth interview, the study monitors key moments of psychosocial development in four...

Abstract:
Current status: The recurrence of urinary stones is common, according to research results. Prevention of urolithiasis, especially of recurrent cases, is essential. Information about the disease, causes of urolithiasis and preventive regime are the basic principles. The patient should not only be informed about drinking and dietary regime after removal of the stone, but he should be motivated to fulfil these recommendations. Metaphylaxy is complex treatment regime; aim of whist is prevention of stone recurrence Methods: The research part was based on quantitative survey. Questionnaire, consisting of 22 questions, was used for survey.. The survey was performed at the Department of Urology, Region Hospital Ceske Budejovice since January 2011 till December 2013. Patients, where first stone was diagnosed, were involved. SPSS programme was used to evaluate results of quantitative survey. Medical recodes was used for secondary analysis of data of primary stone patients treated at the Department of Urology, Region Hospital Ceske Budejovice since 2009 till 2013. Microsoft Office Excel 2007 and SPSS programme was used for statistical analysis. The hypotheses were tested by using of Chí quadrat test. Patients: The patients with primary urinary tract stones, diagnosed in 2012 and 2013, were selected for survey. There were included 184 patients in 2012 and 102 patients in 2013. 150 patients had been addressed by questionnaire and 123 patients responded. The secondary analysis of data of patients treated for primary urinary stones at the Department of Urology, Region Hospital Ceske Budejovice since 2009 till 2013, was performed on 644 patients. Results: 644 patients were found as patients treated for primary stone at the Department of Urology, Region Hospital Ceske Budejovice since 2009 till 2013. This is in concordance with research goal: "Find, how many urinary stones patients were diagnosed at the Department of Urology, Region Hospital Ceske Budejovice since 2009 till 2013". The second research goal was: "Identify differences in dietary habits in urinary stones patients." There was fund a difference in meat intake between men and women. Decreasing trend in meat intake according to age was found, too. No difference in sugar intake was found intake between men and women. The third research goal was: " Identify the fluid intake in in urinary stones patients." There was found average fluid intake per day of 1 922 ml in women and 2 127 ml in men. The last research goal was: " Identify willingness of the patients to change their drinking and dietary regime." The willingness to to change drinking and dietary regime is affected by age of the patient, profession, way of stone removal and quality of information about to change their drinking and dietary regime. Conclusions: The drinking and dietary regime, in context with life style information, is basic for prevention of urinary stones. Fluid intake, dietary habits, physical activity, weight reduction, stress release are essential factors. The motivation, including clear and detailed information of the patient is important for compliance. The results of the theses will be offered for publishing in peer reviewed journal Urologie pro praxi.

9 Abstract The aim of this thesis is the role of neuronal antibodies in epileptogenesis. In the first phase of research we focused on 1) characteristics of epileptic seizures in patients with autoimmune encephalitis, 2) implementation of tissue brain assay as novel antibodies detection method in the Czech Republic, 3) profile of chemo- and cytokines in different autoimmune encephalitis stages, 4) genetic risk factors of autoimmune encephalitis development. After mastering the detection methods, we found neural antibodies in 5 % of temporal lobe epilepsy patients. Immunotherapy response in some cases and lack of antibodies positivity in subgroup of temporal lobe epilepsy patients of known aetiology suggests their pathogenic role. Because seropositive and seronegative patients are difficult to distinguish by clinical characteristics in temporal lobe epilepsy patients (statistical significant differences were only in two of 14 characteristics), neural antibodies could be potential biomarker of autoimmune epilepsy. However, the investigation as a screening tool in all temporal epilepsy patient are too high, we focused on determination of clinical risk factors leading to identify the indication of antineural antibodies investigation. In the cohort of patients with focal epilepsy of unknown aetiology antineural...

Abstract:
Substitute family care is a form of care for abandoned or orphaned children when a child is brought up by foster parents in an environment which is similar to a natural family environment. In the Czech Republic, there are various forms of substitute family care, mainly adoption and foster care. Arranging foster care means seeking children who are suitable for placement in foster care and finding correspondent foster parents for those children. There are few different groups of children acceptable for foster care. One of those groups includes children with handicap. Handicap means an obstacle, difficulty, disadvantage or unfavourable situation which ensues from a limited ability or even inability of an individual to assert as well as other people. The aim of this dissertation is to asses foster care situation abroad and to compare foster care for children with handicap in South Bohemia and Pilsner Regions. Based on the aim, two hypothesis were determined. The first one: Variability of different foster care forms in the Czech Republic is not optimal compared to abroad. The second one: Comparison results of data from both surveyed regions will not be fundamentally different. Secondary data analysis method was used for the survey. The data was acquied from available professional literature and foster family files from Foster Care Centre of South Bohemia County Council in České Budějovice and Pilsner County Council in Plzeň. Required data were gathered from foster parents' records and fostered children documentation from 2001 to 2005. The established aim was accomplished. The first hypothesis was confirmed. The second hypothesis was not confirmed. The survey established that some European countries have already introduced further forms or foster care, especially professional foster care. This form of foster care is not available in our country yet but as the interest to become foster parents decreases in the Czech Republic, professional foster care may be the solution of this situation. Furthermore, the survey established that there is hardly any difference among foster parents and fostered children in individual character in both counties. However, there is a distinctive difference in the amount of foster parents who accepted a child to foster care. The same situation arises in the amount of children placed in foster care. Contribution of this dissertation for the health and social section consists in a creation of statistics during the years 2001-2005 and in an estimate of characteristics of foster parents which can assist in searching for new foster parents.

Abstract:
Theoretical foundation Chronic obstructive pulmonary disease ranks among the most serious diseases in the population. The COPD prevalence is assumed at up to 7,7%. As the name shows, it is a chronic disease characteristic with typical symptoms including particularly cough, expectoration and noticeable dyspnoea. The symptoms may get worse above their common level in the course of the disease, which we call exacerbation. The presence of unpleasant breathing problems affects different life areas, restricting the individual when performing everyday activities. The patient's psyche is affected as well, including potential incidence of anxieties or depressions. Finally, the patient's subjectively perceived quality of life is significantly worsened. Goal of the thesis The goal of this thesis is to ascertain the areas in which COPD affects the quality of life. Hypotheses H1: COPD patients feel breathing problems. H2: COPD patients feel restrictions in the sphere of common daily activities. H3: COPD patients feel restrictions in the sphere of physical activity. H4: COPD patients feel restrictions in social sphere. H5: COPD patients experience negative feelings. H6: COPD patients perceive their quality of life as bad. Methodology The research investigation was implemented based on quantitative inquiry within the grant Project No. 120/2012/S "Reflection of life quality in nursing". The data collection was performed under use of quantitative method; the research investigation took place with the help of questionnaire technology. Two questionnaires were used: St. George's Respiratory Questionare Czech (SGRQ) and WHOQOL 100 generic standardized questionnaire. The questionnaires were distributed to persons with diagnosed chronic obstructive pulmonary disease. The selection set was made up through quota selection that was observed. The quotas consisted in COPD diagnosis and the respondents' gender. The proportion of men and women was specified at 500 persons, 294 men and 206 women from that number. The research set consisted of 296 men and 207 women, i.e. 503 respondents in total. The investigation took place through addressing general practitioners, specialized outpatients' departments and chest disease wards. Results The results were processed with the help of the SASD program (statistic analysis of social data). The results can be divided into three groups. The first group includes the results characterizing the structure of the research set; the second group includes the graphically represented results, and the third group includes the results processed with the help of descriptive statistics. The last two groups include results from both questionnaires used. The results allowed us obtaining information on perception of breathing problems by the patients, on the sphere of common daily activities, on problems in the sphere of movement, on social sphere, on experiencing of negative feelings and on subjective perception of quality of life. Based on the results, the hypotheses were evaluated as follows: H1: COPD patients feel breathing problems confirmed, H2: COPD patients feel restrictions in the sphere of common daily activities - confirmed, H3: COPD patients feel restrictions in the sphere of physical activity not confirmed, H4: COPD patients feel restrictions in social sphere - not confirmed, H5: COPD patients experience negative feelings confirmed, and H6: COPD patients perceive their quality of life as bad - not confirmed. Conclusion The thesis provides comprehensive view of the spheres affecting the quality of life of patients with chronic obstructive pulmonary disease. It may be used not only for nurses but also for the whole multidisciplinary team caring for the patient. The thesis may be used also as study material for nursing students. The results of the research investigation will be further presented at conferences and published in professional journals.

Abstract:
The aim of this work is to describe cognitive processing of a child with autistic spectrum disorder, to pause on its mechanisms and causes. The first part of the work presents a theoretical background for following interpretation of the data acquired in the research. Firstly it briefly defines autistic spectrum disorders, cognitive functioning of individuals with the disorder and above all two main psychological theories of autism: Theory of Mind (S. B. Cohen) and Weak Central Coherence Theory (U. Frith). The second part of the work recounts a process of the research. Its focus lies in a long-lasting work with a girl with autistic spectrum disorder and a girl from intact population. Title: Experiance with joga for tennis coaches Objectives of work: The aim of my thesis is to analyze the possitive effects of yoga on the mental and physical health of tennis players. The data is gained by quality data collection methods. The secondary objective is, on the basis of information obtained from short interviews with coaches, to create a theoretical model of categorization. Method: In this thesis we used the method of interview and narrative interview. The data collected from the interviews, was used to create a theoretical model of categorization. A method of narrative interview with elements of the interview was used as a key method of collecting data from selected subjects. Results: Following numerous interviews with various subjets including coaches and trainers, I have come to the conclution, that yoga has a positive effect on the mental and physical health of tennis players. The main effects of yoga include: increased confidence and concentration on the court, accelerated healing, reduced pain in the spine (notibly in lumbar region), therefore increasing the flexibility of joints and improving the performance of players. In terms of categorization on the effects of yoga, both in mental and physical health of tennis players, I classified the effects into two major categories: the...

The present thesis deals with the issue of balance disorders in patients with multiple sclerosis (MS) and the effect of individual balance training using biofeedback. It presents an overview of the current knowledge about MS - epidemiology, pathogenesis, diagnostics, types, symptoms and treatment methods. The thesis also describes principles of balance control of the human body, balance deficits in patients with MS and physiotherapeutic treatment. The aim of the experimental part was to evaluate the effect of the Homebalance® system on balance in MS patients and its comparison with conventionally used sensorimotor training. A total of 18 people took part in the research, 9 in the experimental group and 9 in the control group. Standardized tests and functional scales supplemented by standardized questionnaires on balance and gait were used to objectively assess balance. Measurements were performed before and after a series of therapies. After treatment, a significant improvement (p 0,05) was found in some tests and functional scales, but in none of subjective patient reported outcomes. Exercise using the Homebalance® system may be a suitable alternative to classical rehabilitation methods used for balance training in people with MS. Abstract:
Myelodysplastic syndrome (MDS) is a clonal disorder affecting maturation and differentiation of hematopoietic stem cells. MDS is characterized by ineffective hematopoiesis resulting in peripheral blood cytopenia in at least one lineage, and increased risk of development of acute myeloid leukemia (AML). This disorder mostly affects the elderly, with the incidence of 10-50/100000 people per year. Heterogeneity of MDS is further underlined by the presence of both idiopathic primary and secondary therapy-related forms, which arise from accumulation of mutational events induced by cytotoxic therapy (exposure to alkylating agents or after therapy with topoisomerase II inhibitors). In this thesis, we focused on determination of differential gene expression between MDS patients and control subjects using different microarray platforms. Using nylon membranes, we detected 4 genes (ERCC1, FLT1, NME4, PCNA) with increasing expression pattern in advanced MDS stages, which correlated with disease progression. Their strong up- regulation was also observed in patients with de novo AML, suggesting their involvement in the leukemic transformation of MDS. Comparison of gene expression profiles of early and advanced MDS obtained by Agilent microarrays resulted in the set of differentially expressed genes, which might...

The thesis begins with a theoretical part containing a general description of artificial pulmonary ventilation, invasive pulmonary ventilation and a detailed description of non-invasive pulmonary ventilation and chronic obstructive pulmonary disease. This section also focuses on a detailed description of non-invasive pulmonary ventilation in patients with chronic obstructive pulmonary disease. The aim of this thesis is to find out the real usability and success of non-invasive pulmonary ventilation in patients with chronic obstructive pulmonary disease received in 2018 at the department of ARO in the Hospital Jindřichův Hradec. The data was obtained from the patient documentation for 2018 from the ARO department and subsequently entered in the tables. The research found that in 2018, 162 patients had undergone the selected department, of whom 63 patients suffered from chronic obstructive pulmonary disease. Noninvasive pulmonary ventilation was used in 75 patients. Of the total number of non-invasive pulmonary ventilation applications, 30 were used in patients with chronic obstructive pulmonary disease. The success of non-invasive pulmonary ventilation in chronic patients reached 90 %. The main benefit is the fact that the number of patients with chronic obstructive pulmonary disease is increasing and that indeed chronic obstructive pulmonary disease is aggravated in the winter months. Another benefit is the finding that the success and true utility of non-invasive pulmonary ventilation in patients with chronic obstructive pulmonary disease is very good.

The present diploma thesis deals with the topic of communication support in patients with multiple sclerosis in the terminal stage of the disease. Since the number of institutions in the Czech republic which deal with this issue is extremely limited, the questions addressed in the thesis are more topical than ever. In the first part of the thesis, the author introduces to the reader the disease as such, with its clinical picture and pathologies, types, diagnostics, progress, and treatment. The aim of the thesis is to characterize the needs and forms of support for patients with respect to communication and in order to reach this aim the author conducted a survey in an institution in the form of an exploratory research. Answers to the research questions were sought in case studies of patients who were willing to participate in the research and who shared their life stories and provided insight into their intimate struggle to cope with the disease in the best way possible. Their goal was prosaically simple - to keep control over their lives and their human dignity for as long as possible. Abstract:
(ENG) This thesis summarizes the results of a project dedicated to adaptive immune system of patients with partial DiGeorge syndrome caused by deletion of 22q11.2. The introduction sets the DiGeorge syndrome into a broader context of international pathophysio- clinical classification of primary immunodeficiencies and goes into detail describing its history, causes, clinical phenotype, therapeutic options and changes of the immune system. The attached manuscripts illustrate the premature aging of the T cell population, but also impaired development of B cells with low class-switched memory and high naïve subpopulations, along with high serum levels of BAFF, a B cell survival factor. The surprising lack of T independent marginal zone- like (MZ-like) B cells is reflected in decreased natural anti-α-Gal antibodies. The faulty B cell maturation and imperfect germinal center response is not caused by a deficit of follicular helper T cells, which are in fact increased in DiGeorge syndrome patients, and in most cases doesn't lead to hypogammaglobulinaemia. Despite the high incidence of autoimmune disease, in particular thyroiditis and thrombocytopenia, and a trend towards hypergammaglobulinaemia in adolescence and adulthood, we saw normal proportion of regulatory T cells (Tregs) and normal expression of...

Abstract:
Possible connection between latent toxoplasmosis and schizophrenia is a very interesting and medically important topic. In this thesis I tried to map current state of knowledge in the interdisciplinary research of schizophrenia and Toxoplasma gondii and their possible connections as well as to show differences in responses between Toxoplasma-positive and Toxoplasma-negative subjects using simple computer-administered tests of prepulse inhibition of startle reaction (PPI). Such differences would suggest another similarity between schizophrenia patients and subjects with latent toxoplasmosis as the sensorimotor gating responsible for PPI was found to be disrupted in schizophrenia patients. Side goal of the study was to test newly developed PC software for testing PPI and to determine its applicability in further research. Subjects for the tests were recruited among adepts of professional military service; 409 subjects completed the test of acoustic PPI and 276 subjects completed the test of visual PPI. All the subjects were tested on presence of specific anti-Toxoplasma IgG in their blood serum. Both tests revealed significant (p<0.001) differences between responses on prepulse-preceded stimuli and plain stimuli without prepulse, no significant results were, however, gained for the effects of latent...

Abstract:
Normotensive hydrocephalus (NPH) is a neurodegenerative disease that occurs mainly in patients of high age. The disorder causes accumulation of cerebrospinal fluid (CSF), which leads to enlargement of ventricles and pressure exerted on cerebral structures. Clinical manifestations (gait disorders, development of dementia, incontinence) can be easily confused with symptoms of other neurodegenerative diseases; unlike other such disorders, however, NPH can be treated by surgery, if diagnosed in time. Patients are indicated for the procedure by a lumbar drainage test. There are currently no reliable laboratory biomarkers known that could be the basis of NPH diagnostics. In the past, steroids proved to be linked to neuronal activity in neurodegenerative diseases with the help of specific diagnostic markers. An instrumental method has been developed for the purposes of this thesis and it was used to gauge the level of certain steroids in CSF in a sample group of NPH patients and a control sample group of healthy individuals. A significant difference has been found in levels of aldosterone and cortisone. Aldosterone was higher in NPH sufferers, while cortisone levels were higher in the control group. It is crucial to differentiate patients with NPH from patients with similar clinical manifestations during...

ThLic. Bc. Barbora Šmejdová Title of the thesis: C. S. Lewis and the Language of Modern Apologetics Abstract The dissertation thesis is devoted to the question of what kind of language we should use in contemporary apologetics to make Christian message communicable to unbelievers. This question is approached through the work of C. S. Lewis. After the introduction and biography of the author, the thesis provides the analysis of Lewis' gnoseological starting points. In this chapter, we are trying to show that Lewis is able to pay attention to the present accent on subjective perspective without getting trapped in relativism. The next chapter focuses on Lewis' theory of language. For his view, the theme of myth and metaphor is central. Together with Lewis, we come to the conclusion that human language is metaphorical, but we can still touch truth. This journey is not easy, though. To be able to make readers approach truth, the author has to live in truth. That is why the next chapter is devoted to the theological interpretation of imagination and shows that authentic Christian imagination is an integral part of each good apologetic text. The last chapter is focused on the genres of apologetics and, based on Lewis' work, presents their benefits and restrictions. Keywords C. S. Lewis; language of apologetics;...
