Full textDuring the time of health expenditure control, the Local Drug Committee of Tenon hospital implemented and assessed the impact of clinical practice guidelines developed according to the methodology recommended. The aim was to improve the quality of the prescriptions of albumin, anti 5 HT3 and Granulocyte-Colony Stimulating Factors (G-CSF). Implementation of clinical practice guidelines with specific prescription order forms summarizing the guidelines increased significantly physician's compliance with the guidelines. The success of clinical practice guidelines depend on several factors including the method for their implementation and physician adherence to the guidelines

Full textDe plus en plus d’études font l’éloge d’un nouveau composé injectable servant au traitement de l’arthrose, l’acide hyaluronique (viscosuppléance). Ces effets sur les symptômes de l’arthrose sont rapportés comme étant similaires à ceux des infiltrations intra-articulaire de corticostéroïdes, mais sa popularité grandissante est liée à sa nature biologique et à son mécanisme d’action qui pourrait durer jusqu’à 6 mois. Une injection de viscosuppléance à l’intérieur de la capsule articulaire pourrait-elle amener des résultats bénéfiques équivalents voire supérieurs à d’autres composés? L’objectif de cette étude est de vérifier par l’analyse de la douleur, des capacités physiques et de la biomécanique de la marche si, en comparant avec un placebo, l'infiltration intra-articulaire d’acide hyaluronique dans la hanche de patients atteints de coxarthrose, influence leur qualité de vie. À ce jour, très peu d’études ont effectués une telle comparaison et jusqu’ici, la viscosuppléance ne semble pas particulièrement se démarquer d’une solution aqueuse de remplacement. L’échantillon visé de l’étude est composée de 40 participants âgés de 40 à 70 ans. Cet échantillon a ensuite été assigné à un groupe traitement (viscosuppléance) ou à un groupe placebo. À ce jour, 7 participants ont entamé l’étude. Chaque patient a été évalué sur différents paramètres cliniques et biomécaniques relatifs à la marche et aux activités de tous les jours et ce, sur une période de suivi de 6 mois, divisée en quatre évaluations à des fins de suivi. Un moniteur d’activité physique a aussi été remis à chaque patient en début d’étude. Au fil des suivis, les participants ont montré de nombreux progrès, tant au niveau de la douleur ressentie et de l’état fonctionnel, qu’au niveau de la vitesse de marche et de la longueur de foulée. Toutefois, aucun indice laisse croire à un effet supérieur de l’acide hyaluronique, lorsque comparé à un placebo.

Full textThe aim of this work was to study the pain sensitization processes and to evaluate therapeutic strategies to prevent or to treat such a process in rats as well as in humans. By using a behavioural approach in rats, we have demonstrated that, as previously reported with inflammatory pain, rats operated under high doses of fentanyl developed higher level of postoperative hyperalgesia ut also presented stronger morphine tolerance after surgery. The development of hyperalgesia could lead to a change in the animal's state that would be also enhanced by preoperative high doses of fentanyl. The change in the animal's state, noticed after taloxone injection, could be associated with latent pain sensitization. Electrophysiological data we gathered in vivo at D1 in rats reported that operated rats (on D0), as compared to sham ones, had WDR neurons in dorsal horn that showed a higher electrical response to mechanical stimulations of their receptor field including the surgical area. Perioperative high doses of fentanyl induced an enhancement of this late postoperative mechanosensitivity of the WDR neurons. From a therapeutic viewpoint, in animals, we tested peroperative administration of different anti-NMDA agents : ketamine, nitrous oxide and sevoflurane. Ketamine and nitrous oxide were able to reduce not only postoperative hyperalgesia induced by high doses of fentanyl, but also acute postoperative morphine tolerance. In humans, for abdominal surgery, our study reported that per and postoperative administration of ketamine permitted to reduce not only intensity and extension of hyperalgesia around the wound, but also morphine consumption for the 48 postoperative hours. Without analgesic effects per se, antihyperalgesic agents could oppose to the development of the pain sensitization improving thus the management of acute postoperative pain and decreasing the chronicisation of acute pain

In-depth phytochemical study of French Polynesian Tamanu oil was performed for the first time. 16 coumarin derivatives named "Tamanolides". AntiVIH agents, calanolides A and B, were isolated for the first time in C. Inophyllum emphasizing the pharmacological interest of this species

Full textToday, the use of animal mediation as a form of treatment in nursing homes has become more and more common. Our study assesses and measures, largely through the use of the Neuropsychiatric Inventory for Nursing Home (NPI-NH), how the presence of a dog in Alzheimer’s patient psychotherapy affects psycho-behavioral disorder (BPSD) levels in French men and women with a mean age of 85 years living in an institution. Our study focuses on well-being and on building positive emotion in the elderly with dementia, particularly through regaining self-esteem, stimulation, remobilization and maintenance of preserved cognitive abilities. Full textThe repellents are natural or synthetic molecules whose aim is to prevent theapproach of arthropods to avoid transmission of vector-borne diseases. A typicalexample is the transmission of Leishmania infantum responsible for canineLeishmaniasis, which is contracted after a sandfly bite on a dog. The arthropod repellentsmarketed for topical application have all a short action duration, requiring repeatedapplications of basic formulations (solution).The formulation of active molecules having repellent properties, incorporated intonanoparticle systems and having a prolonged action in time was selected. Nanoparticles arecolloidal carriers interesting in the pharmaceutical technology field due to their ability to formcomplexes with hydrophobic molecules, such as repellent molecules (DEET, Picaridin®,IR3535®...). Targeting, protection against degradation and control of the release are the mainadvantages provided by the nanoparticles containing an active ingredient.The nanoparticle physicochemical characteristics (size and zeta potential) permitting theirstorage in the upper dog skin layers and a along the hairs were determined. For that reasoncationic nanoparticles of 200 nm in diameter were formulated. This original formulationinspired from the nanoprecipitation process has allowed us to obtain one single stepconcentrated suspensions (above 10% of active molecules in the final product) and withoutstabilizer addition. A correlation between the release profiles and the effectiveness of modelinsect (Drosophila) has been verified. Therefore the percentage of polymer regulates therelease of encapsulated active molecules. Repellent efficacy of nanoparticulate formulationgreater than 15 days has been validated experimentally. The scale transposition of thenanoprecipitation process makes conceivable an industrial development for the formulation ofan innovative arthropod repellent having a long lasting effect

Full textPyrimidine dimers, the major DNA lesion induced by UV radiations, can lead to the formation of skin cancers. The main objective of this work is to design artificial photolyases able to repair these damages. In order to study the repair of a given lesion, a tool allowing its selective and effective formation is necessary. This work thus presents a study of the photophysical, photochemical and photobiological properties of indoprofen, an agent selective in the formation of thymine dimers. Its ability to induce their photosensitization is in fact due to one of its photoproducts. The repair of photosensitized thymine dimers has then been studied by using porphyrin-like photoactivable compounds as chemical photolyases. An effective photoreversion of thymine dimers by a nickel phthalocyanine on plasmidic DNA has been observed

Full textChemical and biochemical approach to the retinal detachment The retinal detachment may blind if it is not treated by surgical operation. With a chemical and biochemical approach, we have tried to improve the anatomical and functional results of this pathology. We report a clinical study about the results of Oxane HD(r) use in the treatment of the retinal detachment. Oxane HD(r) is a mixed of silicone oil 1300 and a mixed olefin C8F17CH2CH=CHCH2CH(CH3)2 called RMN3. Its density is upper than one. We have showed that the concentration of the Neuron Specific Enolase in the subretinal fluid is correlated with the death of the photoreceptors. Finally, we report that the sphingosine-1-phosphate inhibits the photoreceptors apoptosis during a retinal detachment. We propose a formulation of this molecule for a topic use in an apoptosis corneal model

Full textHerbal medicine refers to medicine based on plant extracts and natural active ingredients. As the number of diabetic patient increase dramatically, many researchers have sought to evaluate the pharmacological action of several traditional plants. The jojoba (Simmondsia chinensis) is a shrub of the family Simmondsiaceae which has many properties linked to a very rich and diversified composition. The objectives pursued during this thesis were to validate the use of jojoba in the prevention of type 2 diabetes and its complications by evaluating in vitro the antioxidant effect of an aqueous extract of jojoba and compare this extract to a pure molecule, the simmondsin, on a pancreatic beta cell line, by demonstrating the efficacy of the various extracts of the jojoba seed on a hepatic cell line following chronic hyperglycemia and hyperinsulinemia and finally by validating in vivo the efficacy of jojoba on rat a model of glucose intolerance induced by high-fat high-fructose diet (HFHF). The results in vitro showed that the various extracts of the jojoba seed tested were not cytotoxic on the cell lines but provided protection against oxidative stress induced by hyperglycemia and hyperinsulinemia. This protection appears to be related to the inhibition of the expression of the p22phox pro-oxidant enzyme. In vivo, the study on the HFHF rat model confirmed the anorectic effect of jojoba combined with a curative effect on complications, in particular liver and kidney damage, which could be linked to its antioxidant properties. This study demonstrated the efficacy of jojoba seeds in the treatment of diabetes and its complications

Full textThe cardiac arrest is mainly of cardiovascular etiology. In the actual context of out-of-hospital cardiac arrests, 20 to 25% of the victims present a ventricular fibrillation. About 3 to 5% of the victims are saved without neurological damage. The chance of surviving a cardiac arrest outside an hospital depends on the early and fast support of the victim. The first active witnesses performing cardiopulmonary resuscitation combined with the use of a defibrillator are an important link to save the victim.Our main objective is to improve survival rate in out-of-hospital cardiac arrest cases. A first way of investigation is to propose an optimal functioning of defibrillator combining wisely the different processes of detection embedded (ventricular fibrillation detection, chest compressions detection, electromagnetic interferences detection), in order to reduce the peri-shock pauses during the resuscitation procedure. In fact, during these pauses, known as "hands-off" pauses, no emergency action is provided to the patient, what is correlated to a drop of the coronary pression, but also to a decrease of the chance of successful defibrillation. That is the reason why, a second way of investigation is based on the prediction of the efficiency of defibrillation. In this context, we propose to use a combination of parameters extracted from electrocardiogram in time, frequency and non-linear dynamics domains. A bayesian classifier using a gaussian mixture model was applied to the vectors of parameters, which are the most predictor of the defibrillation outcome and the algorithm Expectation-Maximization allowed to learn the parameters of the probabilistic model representing the class conditional distributions.All of the proposed methods allowed to reach promising results for both reducing the peri-shock pauses and predicting the efficiency of defibrillation in hope to improve the survival rate in cardiac arrest cases
