In appropriately selected cases, palliative therapeutic strategies can be adapted to those special features of cancer biographies that indicate an atypical course of disease. Elucidating these features, and adapting multimodal treatment strategies to them, can lead to significantly superior effects when compared to the routine application of conventional treatment algorhythms. A case of regionally metastactic bladder cancer is presented that documents the value of repeat debulking-surgery and repeat radiotherapy leading to unexpected short-term and long-term treatment results.

Abstract:
One of the most significant fruits for human consumption is the banana. Fruit consumption not only promotes health but also lowers the risk of heart disease, stroke, digestive issues, hypertension, some cancers, cataracts in the eyes, skin ailments, cholesterol reduction, and, perhaps most importantly, boosts immunity.The study included secondary data, which is information gathered from online resources like Kaggle. Ten categories of bananas will be identified from the 531 total varieties of bananas used as a train dataset: Ambon bananas, Stone bananas, Cavendish bananas, Kepok bananas, Mas bananas, Red bananas, plantains, Milk bananas, Horn bananas, and Varigata bananas. The development of information technology for image object recognition has become a very intriguing topic along with the rapid advancement of society, and it is undoubtedly directly tied to information data. In order to examine Naive Bayes, Support Vector Machine, and Neural Network techniques for classifying banana types, researchers will use the SqueezeNet Deep Learning model to extract features from photos. The study's findings will provide empirical evidence for the distinctions between each algorithm's accuracy, recall, and precision. Based on the collected results, the Neural Network (NN) method is the best in terms of classification, with accuracy of 72.3%, precision of 72.1%, and recall of 72.3%.

The article examines the problem of automatic object recognition using a video stream as a digital image. Algorithms for recognizing and tracking objects in the video stream are considered, methods used in video processing are analyzed, and the use of machine learning tools in working with video is described.The main approaches to solving the problem of recognizing moving objects in a video stream are investigated: the detection-based approach and the tracking-based approach. Arguments are made in favor of the tracking-based approach, and, in addition, modern methods of tracking objects in the video stream are considered. In particular, the algorhythms: Online Boosting Tracker - one of the first object tracking algorithms with high tracking accuracy, MIL Tracker (Multiple Instance Learning Tracker), which is a development of the idea of learning with a teacher and the Online Boosting algorithm and the KCF Tracker algorithm (Kernelized Correlation Filters Tracker) - a method that uses the mathematical properties of overlapping areas of positive examples.As a result, the advantages and disadvantages of the considered methods and algorithms for recognizing and tracking objects for various applications are highlighted.

Women with a history of breast cancer are common at centers for cancer genetic risk all over Europe. Given limited health care resources, managing this demand, while achieving good value for money coming from health services, is generally a major challenge. This paper recapitulates and summarizes the available methods of the risk assessment of familial breast cancer. After a systematic review of the literature Gail-model, Claus-model and BOADICEA-model were selected, as well as softwares (LINKAGE software; MENDEL v3.3 software) available in the application of these algorhythms are also summarized. Comparisons were made between the models concerning their advantages and disadvantages. The really reliable methods of risk estimation of familial breast cancer are always based on the analysis of the pedigree structure and allow the estimation of the patient’s probability of carrying a susceptibility gene under a particular genetic model, given her family history. For this method the knowledge of BRCA mutation status is absolutely indispensable. Background: The Republic of Indonesia is a country that is vulnerable to disasters. Disaster preparedness is an important component in disaster management in order to reduce mortality or morbidity. One component of the community component that plays a role in disaster preparedness is health workers including nurses. The role and competence of disaster management must be considered and enhanced for community nurses to ensure the safety of the community in dealing with disasters starting from the prevention phase, the mitigation phase, the preparedness phase, the emergency response phase and the recovery phase. Algorhythms is an effective method in increasing understanding in a training or simulation.This study was conducted to analyze the effectiveness of the algorithm based algorithm can increase competency of disaster preparedness management in community nurses in the city of Surakarta. Methods: This type of research is a comparative experimental study with a pretest-post test with control group design. The population in this study were all community nurses in the city of Surakarta as many as 110 people. The sampling technique was a total sampling of 97 people (88%). Data is tested by Difference Test. Results: There was an increase in the value of roles and competencies in all groups. Difference in value improvement in the treatment group is (role = 2.56 and competence = 2.76). While the increase in value in the control group is (role = 2.05 and competence = 1.88). Value of P = 0.00. Conclusion: There are significant differences in the value of roles and competencies between the treatment and control groups. Increasing the value of the role and competence of the simulation group with algorithm learning is higher than the control group.

In today’s digital age, effective detection of unwanted emails, commonly known as ”spam”,has become a priority for individuals and organisations alike. As email inboxes fill up withunsolicited messages, it has become evident that the predefined rules and heuristics used bytraditional spam filters have lost their effectiveness. This persistent problem poses challengesat both the personal and business level.Despite efforts to protect email accounts with anti-virus, which in many cases come at a cost,spam remains a growing concern. For businesses, implementing costly firewalls can be an un-necessary burden. The problem of spam persists, and its impact on the efficiency and securityof email communication is indisputable.The primary objective of this paper is to investigate and evaluate machine learning algorithmsspecifically designed to address the challenge of automatic spam detection. This is achieved byusing text classification techniques applied to mail servers and personal computers. In particu-lar, three key algorithms are examined: Random Forest, Decision Tree and Naive Bayes, withthe intention of determining their applicability in both environments.This study relies on two essential research methodologies. First, feature selection, a crucialprocess that identifies the most relevant variables in mail classification, including keywordsand word frequencies, is carried out. In addition, performance evaluation, which uses metricssuch as accuracy, recall and F1-score, is employed to understand the performance of MachineLearning models in detecting spam and legitimate emails.The results of this study are presented in the form of comparative tables showing the hit andmiss rates of the three models evaluated. Notably, it is determined that the Random Forestmodel, when applied in conjunction with tokenisation techniques, exhibits superior efficiencycompared to the other two models.The choice of the right Machine Learning model is critical to ensure efficiency in email classifica-tion, and this study provides a solid basis for making informed decisions in the implementationof email security systems in real world business environments. Spam detection, supported bymachine learning algorhythms, remains an evolving field and offers a promising solution toaddress a persistent problem in the digital world.

The Federal clinical recommendations on treatment of pulmonary tuberculosis with multidrug-resistanсe to micobacteruim tuberculosis contain of algorhythm of actions to correct negative side effects appearing in the process of chemotherapy after linezolid prescription. According to this algorhythm, if the negative side effects in form of myelosuppression (inhibition of blood development) or neuropathies are developing, administration of linezolid can be abolished, and the drug prescription can be recovered beginning the dose of 300 mg per day after normalization of blood analysis, under blood analysis control, and then with 2-fold increase of oral dose (300 mg twice a day) to prevent repetitive development of negative side effect. In this case, dispensary prescription of linezolid 300 mg is permitted for tuberculosis treatment. In the paper, a comparative analysis of usage of different doses of linezolid for tuberculosis treatment with argumentation to use median (300 mg) doses of linezolid has been done.

Aim. To evaluate standard 12-lead ECG indices for the differential diagnosis of wide QRS tachycardias with right bundle branch block (RBBB) pattern. Materials and methods. Study analyses the 244 ECG indices in 111 patients (79 males and 32 females, age 53±17 years) with RBBB tachycardias, who underwent electrophysiological studies. First step includes retrospective analysis of QRS characteristics in 20 patients with ventricular tachycardias (VT), 24 pts with aberrant supraventricular tachycardias (SVT+RBBB) and 14 pts with antidromic SVTs (WPW). ROC- and multifactorial analyses were performed to develop diagnostic ECG algorithms. The prognostic accuracy of the algorithms was subsequently evaluated on a prospective group of patients with RBBB tachycardias (n=53). Results and discussion. ECG criteria of RBBB VTs were: 1) the presence Q-wave in lead II, 2) the duration interval R(peak)-S(end) >100 ms in lead V5. ECG criteria for antidromic SVTs with RBBB were: 1) the duration of the R wave in lead I ≥80 ms, 2) the absence of split (M-sharp) R-waves in lead V2, 3) the absence notch in ascending S wave in lead aVL. The accuracy of the algorhythm for diagnostic of VTs with RBBB was 83% (sensitivity 100%, specificity 73%). The accuracy of the algorhythm for diagnostic of antidromic SVTs with RBBB was 91% (sensitivity 85%, specificity 96%). Conclusion. The proposed algorithms are based on new ECG criteria for the differential diagnosis of wide QRS complexes tachycardias with RBBB pattern, unlike the previous algorithms.

Abstract Primary hyperparathyroidism (PHPT) is the third most common endocrine disease after diabetes and osteoporosis. It is more common in women and its prevalence increases with age. PHPT is associated with parathyroid adenoma or cancer or paraneoplastic parathyroid hormone (PTH) production. PHPT can be asymptomatic or present with nephrolithiasis, peptic ulcers, pancreatitis, bone lesions and rarely – as a palpable formation in the cervical area. We present a 73-year-old female patient with cystic cervical lesion with high levels of PTH in the evacuated fluid contents. We discuss the differential diagnosis and the diagnostic algorhythm in cystic cervical lesions.

This article proceeds from the observation that, in 21st century cities, algorithmic technologies engage people as bodily beings in the production of space in ways that warrant theoretical discussion on urban infrastructure and infrastructural power. While human corporeality is an increasingly prominent issue in critical (media) infrastructure studies, my argument in the article is that the structural role of corporeality in the pervasively computed urban context remains undertheorised. A key starting point demanding reconsideration concerns the ontological separation of human embodiment from the materiality of infrastructure. To overcome this separation, I direct attention to urbanites’ mediated bodily habits and routines, stressing their importance in infrastructural constitution. The power-infused interrelational dynamic that these routines enact is addressed in the article by developing a conceptualisation that combines views of classic social and urban thinkers with more recent, particularly nonrepresentational, theorisation. As a methodological bridge towards empirically investigating how mediated bodily routines ‘infrastructure’, I propose a reworking of Henri Lefebvre’s notion of rhythm in terms of the algorithmic qualities of contemporary cities.

An improvement of the forecast of pink salmon runs is one of the most actual problems in management of salmon stocks of Kamchatka. Algorhythm of Random Forest with the use of iterative selection of most important climate-oceanological predictors seem to be promising. Three authenticly influental predictors have been found for the model of Random Forest at the error of the forecast of 28%. Verification of the obtained results used alternative methods of selection of the best predictors. Obtained results have demonstrated possibility of the use of considered climate-oceanological predictors in modeling the forecasts of stock abundance dynamics of pink salmon in Western Kamchatka.

Kontusionen [tiefes dissezierendes Hämatom]), oder nach Radiotherapie oder Kryotherapie wegen Hautkrebs am Unterschenkel), im Rahmen kongenitaler Krankheiten auftretende Ulzera (beispielsweise bei Klinefelter-Syndrom oder Sichelzellanämie), Hautnekrosen als Folge von Medikamenten-Toxizität (z. Aims and Objectives: In recent publications on acl-ruptures and especially on failure of acl reconstruction there comes a strong focus on posterior tibial slope (PTS). ACL reconstructions with a PTS of >12° have an 8 times higher risk of recurrent instability and reconstruction failure. But many questions stay unclear so far-When do we have to correct the tibial slope? How do we correct it? What about simultaneous frontal axis deviations? In this publication a new algorhythm is presented. Materials and Methods: The following aspects have to be evaluated Is the PTS the only dimension of the deformity or do we have to correct the frontal axis simultaneuosly? Performing a anterior closed wedge extension osteotomy: when do we go distal the tuberosity and when do we perform a tuberosity osteotomy and use it as “bio plating”? Osteosynthesis only screws or always plate? Are there indications for a contineous correction, f.e. with a hexapod? Whats the role of preoperative range of motion of the knee (especially extension)? Always tunnel filling in the same surgery? What about PCL insufficiency and low PTS? Results: An algorhythm is presented giving a treatment path for the different questions mentioned. The procedures are shown step by step in clinical examples and surgery documentation for every pathway. Conclusion: Posterior tibial slope plays an critical role in ACl recontruction. In primary ACl tear a slope correction is probably not indicated. In ACL reconstruction failure a analysis of the PTS needs to be done and correction needs to be discussed. Simultaneuous varus deormities need to be corrected by openwedge valgisation - extension high tibial osteotomy (HTO), while as isolated PTS elevation is subject to an anterior closed wedge extension HTO. Preoperative range of motion needs to be respected not to create hyperextension. Osteosynthesis can be perormed with only screws using the tibial tubercle as “bio-plating”. In cases of former bone-tendeon-bone (BTB) ACL reconstruction a tibial tubercle osteotomy should be avoided and a infratuberositeal osteotomy should be performed and stabilized with plate osteosynthesis. In severe postraumatic cases contineous correction of the slope with fixateur externe, f.e. hexapodes, needs to be performed.

Abstract:
Aims: to compare the efficacy and safety of mitral valve (MV) prosthesis using different surgical accesses (median sternotomy and lateral thoracotomy) in overweight patients; to evaluate the influence of the choice of access on complications, quality of life, survival and cardiac function, as well as to determine the risk factors for complications in combined surgical treatment of coronary heart disease and MV prosthesis. Methods: Patient groups were formed depending on the type of surgical access: 50 patients underwent MV prosthesis through median sternotomy, and 80 patients underwent MV prosthesis through lateral thoracotomy. The study design was a retrospective cohort study. Surgical interventions were performed between 2015 and 2023. 130 overweight patients (BMI >30 kg/m2 ) with MV lesions were included in the analysis. Quality of life before and after surgery was assessed using the standard SF-36 questionnaire. Operative risk was assessed using Euroscore II scale, comorbid background was evaluated using Charlson comorbidity index. Statistical methods were used to analyze quantitative features: Student’s t-test, Mann-Whitney test, and mixed general linear models. Results: The use of lateral thoracotomy, in comparison with median thoracotomy, contributed to a more pronounced reduction in pain intensity and improved quality of life in most of the parameters assessed by the SF-36 questionnaire, including physical, lifestyle, social and emotional functioning, as well as physical pain, general health and vitality. Conclusion: According to the results of the study, MV prosthesis using lateral sternotomy can be considered as a preferable method in overweight patients, as it is characterized by shorter duration of surgery, artificial circulation, artificial ventilation, and duration of hospitalization. Both types of access provide pronounced improvement of hemodynamic parameters and reduction of the functional class of chronic heart failure according to NYHA without statistically significant differences between the groups. However, when combined surgical treatment of ischemic heart disease is necessary, the choice of access should be based on the assessment of the risk of progression of the underlying chronic kidney disease.

Mit der Entdeckung der freien fetalen DNA (ffDNA), auch zell-freien DNA (cfDNA) genannt, im maternalen Blut wurde der vorgängig entwickelte Ersttrimester-Test (ETT) revolutioniert; der heute bei uns bereits weit verbreitet angewandte «nicht invasiven pränatalen Test» (NIPT) wurde entwickelt. Dessen exzellente Performance im Screening nach Trisomie 21 wurde in zahlreichen Studien bewiesen. Heute wird vor allem die Integration des NIPTs in den ETT diskutiert. Die Weiterentwicklung des cfDNA-Screenings führt zu immer vielfältigeren Testoptionen, welche die schwangere Frau und den behandelnden Arzt zum Teil vor komplexe Entscheidungen stellen. Auch die Interpretation des NIPTs wird dadurch immer komplizierter. Die Möglichkeiten, aber auch Grenzen dieser Screeningmethode zu verstehen ist von grosser Wichtigkeit in der Beratung der Schwangeren.

Abstract:
Veterinary doctors often observe cases of unexplained elevated glucose and ketones in urine of domestic animals without any other signs of diabetes. We studies these effects from the standpoint of the phenomenon of interdependent conditions in animals and humans, described by T.V.Novosadyuk in 2000. She was the first to provide a theoretical and practical foundation for clinical cases of simultaneously developing similar diseases in domestic animals and their owners.
 During the last 5 years we studied health of humans in families where domestic animals are affected by the laboratory abnormalities described above. In vast majority of cases it has been found out that animal owners have diabetes mellitus of variable severity.
 At the same time there were no disorders of carbohydrate metabolism in animal owners in 11 cases. We recommended members of these families to undergo a specialized examination. In all of these cases latent diabetes mellitus was found in humans who had especially close relationships with animals. These findings led to initiation of treatment in humans.
 At the same time animals were treated with a collar with a linen sack attached containing Peganum Harmala 30 globules. Repeated laboratory tests were performed after one month of such treatment. Normalization of laboratory variables was observed in all of the cases.
 Based on the study results we developed an algorhythm of activities that helps to diagnose early and latent forms of diabetes mellitus in domestic animals and their owners. This algorhythm includes:
 - test for glucose and/or ketones in animal urine after correction of feeding and care defects.
 - blood and urine glucose tests in family members of animal owners. In cases of deviations from normal values we recommended them to consult appropriate specialists and begin treatment immediately.
 - animals are given collars with Peganum Harmala 30 globules in a linen sack attached.
 - granules are removed when laboratory values normalize in animals. Control urinalysis is performed every three months durign a year.
 This approach is especially useful in latent early forms of diabetes mellitus when abnormal findings in animals or their owners enable us to suspect a similar disease in another. 
 In such a way, the suggested algorithm is effective for organization of preclinical diagnostics in both domestic animals and their owners. In the future it is possible to enrich not only veterinary but also medical practice with new options for effective patient aid by practical development of the use of animal and human interdependent conditions.

Существует набор методик, с помощью которых происходит процесс аттестации ФШ. В данный набор, помимо прочего, входят процессы контроля критических размеров и исследования дефектов фотошаблона. Результаты данных исследований по большей части зависят от исходного выбора точек для измерений в рабочей области фотошаблона. Данная работа посвящена исследованию влияния различных типов измерительных структур на результат аттестации ФШ, а также разработке алгоритма поиска точек для измерения на кадре изображения ФШ. As is known, photolithography is the most common way to produce IC. In this connection, one of the most important criteria is manufacturing accuracy of the mask set. There is a set of techniques used for realizing a mask validation process. This set includes mask inspection processes such as critical dimension (CD) control and defect analysis. The results of these processes largely depend on the initial choice of points for measurements in the working area of the photomask. This article is dedicated to the study of the influence of various measurement structures on the photomask verification process, as well as to the development of measurement points search algorhythm.

The next imperative question would be how something that has always been in the very discourse of human achievement stays in the centre stage for generations to come. To tackle this question, it is important to see what religion actually is related to its ability to manifest the self. The question can even be extended further by examining how the self – central to epistemological inquiries – be justified by the presence of consciousness. Constructing answers to this colossal undertaking of religious identity invites a thorough understanding of how human beings can be taken as conscious. The subsequent agenda is to determine whether consciousness lies within or – on the other way around – outside; whether it is naturally personal or else impersonal. Separate regulation of 2 arterial beds – phylogenetically early muscle arterioles (millions of local peristaltic pumps) and phylogenetically late proximal region, heart and elastic arteries – is the basis for the phenomenon that their non-coordination formed at different stages of phylogenesis manifests itself in functional disorders in paracrine cell communities. There is a small number of means to produce any effect on the paracrine cell community function at the level of the organism. Metabolic disorders in these communities can be normalized via the biological reaction of arterial pressure (AP). Pathogenesis of essential arterial hypertension is based on in vivo disorders of the biological functions of homeostasis, exotrophy, endoecology and adaptation. In essential hypertension, primary disorders form at the paracrine cell community level in the distal area of arterial bed, but not in the target organs. Only later, after compensatory activation of the biological reaction of AP and formation of discrepancy between regulations at the organism and paracrine cells communities, the process secondarily involves the target organs: kidneys, lungs and brains which have autonomous hemodynamics systems. The heart is the fourth target organ in essential hypertension. The pathogenesis of essential hypertension is based on phylogenetic discrepancy between metabolism regulation at the levels of the entire body and paracrine cell communities.

The article reveals the results of the study of the linguo–suggestive potential of the football fans discourse. It is found out that the discourse of fans is built in the paradigm of “us – them” and is aimed at forming a certain emotional state of the discursive community, which involves the implementation of linguo-suggestive influence. It is proved that anchoring technique is one of the leading techniques of neuro-linguistic programming in the communication of a fan group as a discursive community. The research presents an algorhythm for its implementation, which has six stages: 1) determining the situation, 2) creating the key anchor-stimulus, 3) finding the image-standard in the minds of the community, 4) forming the image presentation, 5) choosing the channel of broadcasting, 6) ensuring synergy through verbal and nonverbal means. Video recordings, publications and posts of football fans of German lingual culture on the Internet (on the example of protest discourses) became the empirical material for the study of anchoring technique. The research material proved that three types of broadcast channels are used in discourse: visual, audio and kinesthetic. The effectiveness of the impact is achieved through their complex combination and the use of verbal and nonverbal semiotic signs. The article singles out and analyzes the means of spelling, graphic, lexical-semantic and grammatical-stylistic levels due to which the pragmatic effect is achieved.

Abstract:
The graduation profile is one of the key elements for the accreditation standard of higher education. It mirrors the performance of the applied educational system within a period of time. The better it is, the better the accreditation will be. In support of this, a graduation prediction may be conducted to the academic database of the students. It is of pivotal to trace and classify the historical data into the data training and data testing, thus, to predict the on time-graduation. The step is importantly done to help decide the better management of learning processes. This study was therefore done to analyse certain variables applied to predict the on time-graduation using the algorythms of C.45 and K-Nearest Neighbour (K-NN). The data mining was done to the academic database of the students of the Pharmacy study programme, Pharmacy Faculty, Muhammadiyah University of Purwokerto by adding certain variables into the process. The data was then classified into the data training and data testing. Backward selection was done to select the best and most influential variables for the dataset. The study further resulted that by using the algorhythm of C.45 and backward selection, the accuracy of the graduation reached 92.75%. It is different from the acurracy the K-NN and backward selection showed that reached 96.14%. The result confirmed that the KNN showed the better accuracy than the C.45. It considerably benefitted the study programme to make better decisions on increasing the quality of services, in particular that of leraning processes.

The problem of planning of cargo transportation in the logistics chain "supply-production-sales" in terms of high risks has been stated and solved. Risk factors are analyzed and, with the help of experts, their impact on the main transportation performance is evaluated by means of full-factorial experiment planning. It makes possible to identify the significant risk factors and use them in future to take measures on risk minimization and neutralization. In order to create the cargo transportation schedules the problem of getting the most efficient transport route that minimizes the risks has been formulated and solved. The routing algorithm based on simulation for making the cargo transportation plan in terms of high risks is proposed. In order to reach the goal the distribution of "numerical" waves between the different transport hubs is used. It makes possible to find the least risky route for the vehicles. The obtained route of the movement serves as the basis for obtaining schedules of cargo passing through the transport nodes of the system, in which storage, transportation or distribution of cargo flows is possible. The algorhythm will allow the formation of rational traffic routes simultaneously for several cargo flows and delivery and acceptance points of cargo flows. The optimization of costs for cargo transportation by the selection and justification of measures aimed at minimizing risks using Boolean linear programming has been carried out. The proposed approach should be used in the tasks of planning the transportation of goods in the logistical chain "supply - production - sale" in conditions of increased external and internal risks.

Introduction The most common diseases in the group of painful hip are transient synovitis, rheumatoid arthritis, infective (septic) arthritis, Perthes disease and slipping of the upper femoral epiphysis. Methodology: The algorhythm covers the first and control examinations in certain time intervals (after: 3-7, 7-15, 21-30 days; as well as 2-4 months). Results and discussion The most frequent feature of painful hip is transient synovitis with 65%, Perthes disease with 13 %, septic arthritis with 6%, rheumatoid arthritis and slipping of the upper femoral epiphysis with 2.5%.The ratio boys and girls was 2.3:1. The average age in the group of the painful hip was 6.8 years, in the group of TS 6.5years. The most frequent clinical signs were limping in 84.2% and hip pain in 79.6%.Through the follow up period the difference of the anterior capsular distance was established for symptomatic hip: the average value on the first exam was 8.1 mm, and on the final exam 4.7 mm 3.6 mm. In transient synovitis, there was no difference in measured values of the anterior and lateral femoral head distance for both hips. The medial duration of synovial effusion, measured ultrasonographically, was 10.6 days, and the duration of the clinical signs was 8.7 days. The prolongated synovitis was recorded in 17.6%, and Perthes disease in 8.4%. The average value of anterior capsular distalnce in these patients was 5.4mm. In group of Perthes disease the values of anterior capsular distance during control examinations showed increase that implicated the lateralisation or extrusion of the femoral head. The values of anterior distance of the femoral head were without significant difference. Conclusion The ultrasonography should be the method of choice in painful hip differentiation regardless of the age. The ultrasonography can replace radiography safely in the primary diagnostic procedure as well as through the control examination.

Abstract BACKGROUND: Multiple cerebral cavernous malformations (mCCMs) are known as potentially epileptogenic lesions. Treatment might be multimodal. Management of patients with mCCMs and epilepsy is challenging. OBJECTIVE: To evaluate (1) algorhythmic therapeutic sequences in patients with epilepsy associated to mCCMs, (2) whether there are predictive parameters to anticipate the development of drug-resistant epilepsy, and (3) seizure after epilepsy surgery compared to conservatively-treated drug-resistant patients. METHODS: All inpatients and outpatients with epilepsy associated to mCCMs from 1990 to 2010 and follow-up >12 months available were retrospectively analyzed. RESULTS: Twenty-three patients matched inclusion criteria. Epilepsy became drug-resistant in 18/23 (78%) patients. No predictors were found for development of drug-resistant epilepsy. Median follow-up for both groups was 7.8 years. Nine patients did not qualify for surgical therapy and were treated conservatively. One patient of this cohort (11%) was seizure-free (International League Against Epilepsy [ILAE] class 1). Surgical treatment was performed in 9 patients; 7/9 (78%) of these patients were seizure-free (ILAE class 1) after epilepsy surgery for at least 12 months compared with 1/9 patients in the non-operated group. In 7/9 cases (78%) the largest CCM was resected. In 8/9 (89%) not all CCMs were resected. CONCLUSION: After initial diagnosis of epilepsy associated to mCCMs, a primary conservative approach is reasonable. Surgical treatment can be successful in a large fraction of cases with drug-resistant epilepsy where an epileptogenic lesion is identified. Cases where surgery is not undertaken are likely to remain intractable.

Introduction An acute limp implies an underlying pathology that causes disruption of the standard gait pattern. Diagnosis and history of disease The differential diagnosis of a limp includes trauma, infection neoplasia, inflammatory, congenital, neuromuscular or developmental disorders. History of disease and clinical examination present first steps in adequate evaluation. A useful approach is to consider the causes of limping from head to foot to avoid overlooking common underlying conditions. The age of patient can further narrow the differential diagnosis, because certain disease entities are age-specific. Laboratory analysis Laboratory analysis should always include complete blood count, erythrocyte sedimetation rate, C-reactive protein, rheumatic factor, PPD 3. In some cases we have to perform tests of enzyme values such as alkaline phosphatase, lactate dehydrgenase, and other subspecial analyses in order to exclude some systemic and autoimmune diseases. Radiologic diagnosis Radiologic diagnosis should include: ultrasonography (doppler ultrasonography and in some cases, arteriography); Plain films (always anteroposterior (AP) and profile or some special angle); computed tomography, magnetic resonance imaging. Conclusion In this paper we suggested an algorhytm for examination of a limping child: 1. Analysis of foot morphology and the gait without socks and shoes. 2. Strength test including walking on toes and heels and running. 3. Evaluation of arthritis, stiffness, pain, skin changes. 4. Points of a maximum pain with ROM testing. 5. Traces of trauma with echimosis. 6. Evaluation of leg length discrepancy. 7. Muscle tonus. 8. Abdominal examination 9. Examination of the spine and back.

Pangkalpinang is the capital city of Bangka Belitung Archipelago which has 36 km2 shoreline and is one of the areas affected by anthropogenic activities especially tin minning activities. This study aims to determine the changes of position and rate of shoreline change in 2000 – 2022 using multitemporal imagery in Pangkalpinang. This study used remote sensing method (Algorhytm of Normalized Difference Water Index) and geographic information system with on screen digitation approach using DSAS (Digital Shoreline Analysis). Shoreline position measurement using the NSM analysis and rate of shoreline change using the EPR analysis. Digital image processing result showed that shoreline position at coastal region in Pangkalpinang has changed in 22 years. At sector A (Pasir Padi area), The land mass accretion/ maximum accretion occurred at Serata, Pasir Padi Beach adjacent to Batu Rusa estuary which was 63,6 m with shoreline rate change was approximately 2.89 m/year, while the land mass decreasement/ highest abration occurred in Pasir Padi Beach was 164,5 m with the shoreline rate change was approximately 7.48 m/ year. At sector B (Tanjung Bunga Area) abration occurred was -124.6 m with the shoreline rate change was approximately 5.66 m/year. At sector C (Sampur Beach Area) abration occurred was 81 m with shoreline rate change was 3.68 m/tahun. The average of shoreline change rate in Pangkalpinang affected by abration was 2.5 m/year while the average of shoreline change affected due to accretion was 2.89 m/year whereas there were 2 beaches in Pangkalpinang which did not experience the accretion, namely Tanjung Bunga Beach and Sampur Beach.Keywords: Shoreline, Pangkalpinang, Accretion, Abration, Tin Minning

Abstract:
The aim – to assess the additional prognostic information of metabolic syndrome (MS) components in groups of patients with acute myocardial infarction with segment elevation ST (STEMI), equalized in terms of commonly used acute coronary syndrome (ACS) risk factors. Materials and methods. Retrospective analysis of the 820 cases of STEMI included: evaluation of risk factors according to the scales TIMI, GRACE, PURSUIT, and evaluation of components of the metabolic syndrome at entry (the presence of diabetes mellitus and/or increasing glucose levels > 7 mmol/l, overweight, hypertension, dyslipidemia), as well as the assessment of the indicators of clinical course of hospital period of MI, treatment and results of follow-up of patients, including the information about cases of cardiac death. Results and discussion. Via automated «case-match-control» algorhythm from the basic cohort 2 groups were selected: group 1 (n=41, patients with MS) and group 2 (n=123, patients without MS). Matching criteria included following 13 risk factors: age, height, presence of heart failure, smoking, systemic hypotension at the 1 day of AMI, presence of anterior STEMI, the peak level of the MB-CK and AST, a history of angina and the period of unstable angina before STEMI, the presence of previous MI, baseline heart rate, baseline glomerular filtration rate (CKD-EPI), male gender. Groups were exactly matched by the first 4 matching criteria, and among other criteria maximum mismatch of 3 criteria was allowed (mean mismatch was 1.87 criteria from 13 per pair, and there were no significant differences in groups by each of 13 matching criteria). Otherwise, group 1 was characterized by more severe baseline profile, clinical course of hospital period, but it has the more intensive medical treatment also (including more frequent prescription of ACE inhibitors). According to the follow-up data, patients in group 1 had smaller end-systolic and end-diastolic indexes, more signed improvement in acute heart failure rate, higher heart rate variability and smaller dispersion of repolarisation at the 10th day. Also there was observed a trend toward a lower 3-year mortality (4,9 versus 17,1 %; p=0.05). Conclusions. The presence of MS accompanying STEMI is associated with poorer course of acute period of the disease and, in a contrary, with more favorable course of post-infarction period because of more intensive cardiac therapy in this group of patients.

Abstract Background Iron deficiency (ID) is often a neglected condition in inflammatory bowel disease (IBD), even though it is one of the most common extraintestinal manifestation. Our questions were the follows: If iron deficiency is part of the T2T management as CRP or fecal calprotectin and requires optimisation of the current therapy, how does this affect the medium-term outcome of the disease and the effectiveness of IV iron replacement? If the disease is active, does optimisation of therapy is sufficient to manage iron deficiency or iv. iron supplementation is also necessary? Methods Patients with ID defined by the ECCO guideline (Figure 1.) were randomly included in our prospective one-center, four arms study. Patients were divided into an active (A) and a remission (R) group. Activity was defined by CRP ≥10 mg/l or clinical activity scores CDAI ≥150, pMayo ≥3. Therapy optimisation was performed in the active group, further randomisation was performed into two subgroups: therapy modification alone (A1) or modification with iv. ferric-carboxymaltose (A2). All patients received iron supplementation in the remission group with (R2 – iron T2T group) or without therapy optimisation (R2). Every two months laboratory parameters and clinical activities were assessed until 6 months. Results 65 patients were included (CD 83.4%). Average follow-up was 3.9 months. Number of patients per group: A1:17, A2:23, R1:16, R2:9. Based on our results, iv. iron supplementation with modification in treatment was more efficient, than therapy optimisation alone, as the difference in increase in haemoglobin (p<0.001), haematocrit (p<0.001) and ferritin (p<0.05) was significant after 2 and 4 months the enrollment. In the remission group iv. iron supplementation with therapy optimisation was more effective, than iv. iron alone, based on transferrin saturation (p<0.05), serum iron (p<0.05) and CRP (p<0.05) during the 2 months period. After that period, no further significant difference was measured. Iv. iron seems to be equally effective both in the active and in the remission group, as no significant difference was measured during the 6-month period. No adverse event was observed. Conclusion Our results suggest, that therapy optimisation in addition to iron replacement is more effective in patients with remission and therefore biomarkers of IDA should be involved in the thight monitoring algorhytm to react T2T. However, therapy optimisation has to be associated with iv iron replacement in IBD with IDA and activity without the risk of worsening disease activity. Based on our results, ferric-carboximaltose is an effective and safe therapeutic method in the treatment of IDA associated with active or inactive IBD.

Abstract UCRs are a subset of conserved sequences (100% between orthologous regions of human, rat and mouse genomes) located in both intra- and intergenic regions. There are 481 UCRs described and more than 50% of all the UCRs (256/481) has been classified as non exonic (with no evidence of coding protein), while the other 47% have been designated either exonic (overlap mRNAs of known genes), or possible exonic (with no clear evidence of overlap with protein coding genes) (Bejerano et al, Science 2004). Recently, our group reported that a subset of transcribed UCRs is aberrantly expressed in leukemia (Calin et al, Cancer Cell 2007). Because of the high degree of conservation, the UCRs are likely to be functional and may be involved in the phylogeny of mammals. Since hematopoiesis is regulated by signaling pathways and transcription factors that are highly conserved throughout phylogeny, we hypothesize that UCRs may be differentially expressed in hematopoietic tissues and may play a role in the regulation of this process. In this study we analyzed the UCRs expression in normal hematopoiesis by performing microarray analysis of hematopoietic precursors (HPCs) obtained by culturing human bone marrow (non-mobilized) CD34+ selected cells with different cytokine combinations for 2 weeks to stimulate differentiation to the erythrocyte (E), megakaryocyte (MK), monocyte (M) and granulocyte lineages (G). The following cytokine combinations were used: E (TPO/SCF/IL-3); G (G-CSF, GM-CSF, SCF, IL-3, IL-3); MK (TPO, SCF, IL-3) and M (M-CSF, GM-CSF, IL-6, IL-3 and SCF). Differentiation to the selected lineages was monitored every 3 days using morphology, special staining (benzidine) and flow cytometry analysis using appropriate lineage specific antibodies. Human bone marrow CD34+ and peripheral blood (PB) CD3+ (pan-T cells) and CD19+ (B-lymphocytes) selected cells were obtained from 3 different donors. Total RNA was obtained from the E and M cultures at days 7, 9, 11 and 14; G (days 10 and 14) and MK at day 14 and was hybridized in duplicate to the UCRs microarray chip (OSU version 4). After normalization of the array data with quantiles we analyzed the data using the BRB tools. Unsupervised analyses of the data revealed that samples segregated mainly in 6 main clusters corresponding to the M, E, G and Mk lineages, PB lymphocytes and CD34+ cells. Next we performed a series of two class analysis where we compared the UCRs expression of CD34+ cells vs. differentiated cells (E, G, MK, M, T and B-lymphocytes) one at a time (i.e. CD34+ cells vs. E). In table 1 we showed the top UCRs differentially expressed between in vitro differentiated HPCs/PB lymphocytes and CD34+ cells (+ or − sign reflects if the UCRs is in the sense (+) or antisense orientation (−). In summary we have characterized the UCRs expression during hematopoietic differentiation of HSC and identified distinctive signatures associated with particular lineages. This research has the potential to identify novel regulators of hematopoiesis and may give insights into basic biology of gene expression and cell fate determination.
