Introduction A functionally single cardiac ventricle seen on foetal ultrasound scan carries a guarded prognosis. The antenatal diagnosis of anomalous pulmonary venous connection (APVC) remains challenging, if there is no associated structural cardiac abnormality. Antenatally, a combination of complex cardiac anomaly with suspected isomerism should raise the possibility of associated total anomalous pulmonary venous connection (TAPVC). There needs to be a high index of suspicion for TAPVC, in functional single ventricle and suspected isomerism, as this carries a very grim outcome postnatally. We illustrate foetal echocardiographic findings of suspected TAPVC and review outcomes of antenatal versus postnatal diagnosis of TAPVC with functional single ventricle. Methods We retrospectively reviewed our database over 13 years, focusing on foetal cardiac diagnosis, pregnancy outcomes, management and outcomes of livebirths with diagnosis of TAPVC with functional single ventricle. Results Thirteen patients were included in the review. For the nine antenatal patients, three pregnancies were terminated and six babies were born alive (four babies had compassionate care, two babies had cardiac surgery). One baby is alive at 8.5 years, after Fontan surgery. For the four postnatal patients, three babies had compassionate care (one alive at age 8.1 years) and one baby had cardiac surgery (died age nine weeks). Ten of the 13 patients have right atrial isomerism. Of these 10 patients, only two are alive. For the three non-isomeric babies, only one baby is still alive. There is heterogeneity of the type of TAPVC diagnosed with no particular group that offered better survival. Conclusion Antenatal diagnosis of TAPVC, even in the context of functional single ventricle remains challenging. If isomerism is suspected, targeted evaluation of pulmonary venous connection should be done. This combination of cardiac lesion carries a very grim outcome. The ability to make this diagnosis antenatally will add to the information and counselling given to these parents.

Gestational Trophoblastic diseases consist of a broadspectrum of conditions ranging from an uncomplicated partial hydatidiform molar pregnancy to stage -IV choriocarcinoma with cerebral metastasis. Incidence of hydatidiform mole with a co-existing live fetus varies between 0.005 to 0.01 % of all pregnancies. We report a case of partial molar pregnancy with alive term IUGR (intrauterine growth retardation) foetus. Diagnosis was made by sonographic findings of molar changes at her 28 weeks of gestation. Anomaly scan showed no fetal abnormality. At 40 weeks of pregnancy patient went to spontaneous labour and delivered a severely IUGR baby weighing 1.5 kg and it was morphologically normal. There was a single placenta; one third of it was replaced by molar tissue. As the singletone alive pregnancy with partial molar changes is extremely rare occurrence we reported the case here.J Bangladesh Coll Phys Surg 2016; 34(3): 164-167

From a forensic pathologist’s perspective, there are several aspects of the perinatal postmortem that are particularly important. If a fetus is found abandoned, the pathologist needs to ascertain the fetal age, the appropriateness of growth, if the baby was born alive or dead, and the possible causes of death. In cases of litigation for perinatal deaths occurring in hospitals, access to the obstetric and neonatal notes (if the baby is born alive and dies a few hours or days later) is fundamental to reach a correct interpretation and conclusion. The most important points to consider in cases of intrapartum death are the roles of asphyxia and trauma in the causation of the baby’s death. Timing of the fetal death in relation to delivery may also be an important point in these cases. Finally, intrapartum lesions should always be considered in the differential diagnosis of possible child abuse in babies aged two months or less.

Thanatophoric Dysplasia (TD) is a congenital, sporadic and most lethal skeletal dysplasia caused by new mutation in FGFR3 gene. Authors report such a rare case of a term alive baby with dysmorphic features, born to an unbooked, 40 years old G4P3+0 with non-consanguineous marriage; admitted at 9 months of gestation to present hospital with complain of pain abdomen for 2 days. Patient delivered a term female baby of vaginally which had delayed cry after birth, Admitted in NICU immediately with respiratory distress. The baby looked dysmorphic and suggested TD as most likely diagnosis. The case is being reported for its rarity and for high importance of early booking and anomaly scan. Early diagnosis is important since it provides alternative options of termination of pregnancy when an affected foetus is detected.

This essay considers Billie Holiday’s performance of “One for My Baby,” as an affective lamentation that both moves and creates movement—into another kind of relation, another kind of loving, another kind of bodily intensity. When Billie sings to the bartender, a proxy for an absent other and lost love, “I’m kind of a poet/And I got a lot of things to say/And when I’m gloomy, you simply gotta listen to me/Till it’s all talked away . . . So make it one for my baby and one more for the road/That long, long road,” we experience self-storytelling that creates a relation marked by an a sense of vitality, dependent on mutual vulnerability, and animated within a field of power. Holiday’s “One for My Baby” becomes an occasion for performing queer affective intensity—a corporeal and emotional embodiment that leaves us feeling alive, open, and possible.

The presently reported patient was a 37-year-old married primipara with peripartum depression comorbid with bonding disorder. Care anxiety and a lack of affection towards her baby first appeared at around the time of delivery, and the patient developed major depression at one month after the birth of her healthy baby. At first, standard treatment for major depression including the use of antidepressants, electroconvulsive therapy, and supportive psychotherapy were provided. However, bonding problems appeared to impede and obstruct the amelioration of depression. Although treatment methods for bonding disorder have not yet been established, Kangaroo Care was introduced to facilitate skin-to-skin contact. We also educated her in better parenting behavior and provided repeated motivational interviews with her family because a lack of partner and social support and personal temperament (low self-directedness and cooperativeness) were thought to be related to her bonding disorder. This case suggests the effectiveness of Kangaroo Care, which promotes a humanizing maturation of both baby and parent alike, for mothers with postpartum depression and comorbid severe bonding disorder.

Sirenomelia also known as Mermaid syndrome is a rarely encountered fusion anomaly of the caudal region of body often associated with Potter’s facies, single umbilical artery and various visceral abnormalities which make it irreconcilable with life. Here we report a case of sirenomelia delivered in our tertiary care hospital by lower segment cesarean section to a 24 year old third gravida with no previous live issues. No high risk factors could be identified with the mother including diabetes mellitus. Baby was born alive with Potter’s facies. Both the lower limbs were merging into each other like a mermaid’s tail (hence called mermaid syndrome). Sex of the baby could not be identified and the urogenital and anal orifices were absent. Umbilical cord stump had two umbilical arteries. The baby died after 20 hours of life. There appears to be no definitive modality for diagnosing sirenomelia in the antenatal period. However, if a patient has consistently low AFI without any history of leaking per vaginum, high resolution USG or colour Doppler should be done at the earliest to look for the cause.

This study aims to analyze a phenomenal song entitled “Baby Shark” composed by the Pinkfong, a South Korean media startup. The song has become viral over children and adults alike. This research is undertaken to find out; the composer’s attitudes, the social relation among the participants, and the context of the situation in the discourse. The object of the study is obtained from the whole lyrics. Then it is analyzed through Appraisals and Critical Discourse Analysis by connecting every single line to the intentional marketing and social context. Finally, the conclusion shows that “Baby Shark” is a song about local wisdom education in Korea. The composer implicitly admires the shark and uses it as metaphor of Korean people mindset. The composer intends to give his full support and persuades the listener about hard working culture in Korea

AbstractAs the patron saint of the Indian agate bead industry, Baba Ghor is very important in any reconstruction of its history. The facts about him are quite scanty; we can only hypothetically reassemble them and understand his myth. Abbas, or Habash, a scion of the Malwa Ghors, died in a skirmish near Ratanpur in the early 15th century, probably fighting Ahmed Shah of Gujarat. He was buried on the hill which had long been sacred and was once graced with a fine temple of Makkhan Devi, the Mother Goddess. Her temple was likely destroyed by Ahmed's troops, not those of the Malwa Ghors. Ghor's grave became a place of pilgrimage, first serving the waxing Muslim strength in the area by providing an approved focus of worship. In time it became even more important to the Siddis, who appropriated Ghor as one of their own. He gave the Ratanpur Siddis respectability: in turn they serve his memory. The legends of Baba Ghor and the Ratanpur Rakshisha are not mere fantasy, for they serve the truth as symbols. Ghor represents the coming of Islam, the loss of the old gods, the destruction of the temples, and the forgetting of the old ways. A new dispensation came to Ratanpur and the agate bead industry, and as a result the age-old commerce changed its focus as Cambay replaced Limodra as the lapidary center. Ghor is alive for the Siddis and other Muslims in whose hands the industry is still concentrated. In a very real sense Ghor did encounter the Ratanpur Rakshisha. The Indian agate bead industry has never been the same since, nor can it be understood without taking their battle into account.

Background: Eclampsia is defined as the development of seizures that cannot be attributed to other causes and /or unexplained coma during pregnancy or puerperium in a woman with pre-eclampsia. The purpose of this hospital based observational study is to evaluate the extent of the problem and to estimate the associate of foetal and maternal complications Methodology: The observational study was carried out Department of Obstetrics and Gynaecology in 250 bedded general Hospital Tangail from January 2015 to November 2017. All on the diagnosed patients of eclampsia were included in this study. Results: Regarding types of eclampsia 187(62.3%) patients had antepartum eclampsia, 78(26.0%) had postpartum eclampsia and 35(11.7%) had intrapartum eclampsia. Majority (57.0%) patients had 5-9 times of convulsion, 65(21.7%) had >10 times and 64(21.3%) had <5 times of convulsion. Most (95.0%) patients had cesarean section and 15(5.0%) had normal vaginal delivery (NVD). Maternal alive was found 293(97.7%) and death was 7(2.3%). Male baby was found 203(67.7%) and female baby was 97(32.3%). More than three fourth (75.7%) babies had weight 2.5-4.0 kg. Alive babies were found 282(94.0%), neonatal death was 8(2.7%) and intrauterine death was 10(3.3%). Abstract:
Congenital rubella syndrome (CRS) is a rare illness in infants that result from maternal infection with rubella virus during pregnancy. The case reported is a primigravida at 37.3 weeks of gestation with the complaints of headache, decreased urine output, bilateral pedal edema, puffiness of face and mild hydromnios suggesting pregnancy-induced hypertension (PIH). The previous data showed that the fetus had bone anomalies. Controversially the mother had rubella immune before 12 weeks of pregnancy but the report of nuchal translucency (NT) scan taken during 12 weeks of pregnancy was normal. On history mother denied any evidence of rubella infection during or recently before pregnancy which shows it was asymptomatic. She was treated for PIH. By lower segment caesarean section (LSCS), she delivered an alive boy baby who was acrocyanotic and had deformity in all long bones and enlarged liver. Placenta was found to be abnormal. The baby was under observation and the investigation confirmed CRS. Baby had bradycardia and was intubated on following days and found baby’s vocal cord shape was abnormal. Baby had seizures and could not tolerate weaning from ventilator. Before further investigations, the baby was discharged against medical advice. Though very rare, CRS was found to have many consequences to the fetus. Hence it is significant to rule out rubella infection for mother during or recently before pregnancy as a part of routine antenatal checkup as many of them are asymptomatic. All women should be insisted about getting vaccination for rubella minimum 28 days before planning for conception and should be abandoned for those who were already pregnant.

Baby dumping is one of the social problems which are being discussed openly among the Muslim community in Malaysia. It became a big issue when the act of baby dumping has become more frequent at the beginning of 2010 until 2012. Baby dumping occurs because of pressures of life among those who are weak in their faith. Abandonment is causing the child to be referred to as adoptee (anak pungut) when found alive. An appropriate term of adoptee is being used to abandoned baby as seen from the point of responsibility. Unlike to foster child (anak angkat), the way of surrender and acceptance is between the two sides of parents and new guardians. This article discuss the law perspectives towards the abandoned baby, adoptee and how they should be take care of. Some suggestions presented solution to curb as well as to overcome the problem of abandoning baby as an immediate action for the child custody or upbringing of the child. Berbeza dengan anak angkat kerana cara penyerahan dan penerimaan adalah antara kasih sayang kedua belah pihak ibubapa dan penjaga. In archaeological and textual evidence alike, the psš-kf-knife consistently occurs as part of the same collection of objects. In the Pyramid Texts, these objects are presented in a sequence that is the earliest attested form of the ‘opening of the mouth’ ceremony. The speeches accompanying the presentations suggest that the psš-kf and the objects associated with it were the equipment for a ritual mimicking birth and childhood, and that the role of the psš-kf in this process was to cut the umbilical cord of a newborn baby. Further archaeological, textual, and iconographic evidence is adduced to support this interpretation.

Potter’s syndrome is a rare congenital disorder characterized by characteristic atypical appearance of a newborn due to the oligohydroamnios experienced during the intrauterine life. Its incidence varies from 1 in 2000 to 1 in 5000. 40 years old G7P4L1 A2 at 37 weeks of period of gestation was admitted at a tertiary care institute with labour pains. Her previous obstetric history included three full term vaginal deliveries, two of the babies expired in postnatal period of 2 months and 7 days age respectively. One was alive and healthy. In fourth pregnancy she underwent emergency LSCS, but baby expired 5 months after birth. She had history of 2 abortions which were spontaneous and not followed by curettage. She also gave history of intake of some drugs at 6th week of gestation prescribed by a local Baba to have a male child. Her ultrasonography revealed fetus with bilateral renal agenesis. On per abdomen examination, clinically liquor was very much reduced, Fetal heart sounds were not heard. She delivered vaginally a still born fetus with ambiguous genitalia, anal atresia and malformed limbs. The fetus had typical facial appearance of “potter facies”. Potter’s Syndrome or Sequence is a rare fatal disorder. Apart from typical Potter’s facies, pulmonary hypoplasia, skeletal deformities are associated with this condition, severity depends on the degree and duration of oligohydramnios. Therefore, it is very important to pick up this condition during antenatal ultrasound at earlier gestation so that timely decision regarding further workup and pregnancy termination can be undertaken. This case report emphasizes upon the importance of regular antenatal check ups and examination in each and every patient as it picks up the suspicious cases which can lead to further workup, definite diagnosis of the condition and timely decision regarding management.

German social theorist Ulrich Beck has consistently maintained that the logic of social distribution in western cultures has been reconfigured over the last three decades. Beck believes that, in the first industrial modernity, political and economic energies were directed toward the dissemination of ‘social goods’, such as healthcare, employment and wealth. By contrast, in the second modernity - or risk society - the positive logic of goods distribution is displaced by a negative logic of ‘social bads’, exemplified by environmental despoliation, terrorism and nuclear accidents. Critically, whilst the logic of goods is sectoral - some win and some lose, some are protected, some exposed - social bads follow a universalising logic which threatens rich and poor alike. This article interrogates and challenges these core claims by fusing together and developing empirical and theoretical criticisms of the theory of distributional logic. Empirically, it is demonstrated that Beck draws upon a narrow range of examples, is insensitive to continuities in social reproduction and glosses over the intensification of traditional inequalities. Theoretically, the paper asserts that the risk society perspective constructs an unsustainable divide between interconnected modes of distribution, neglects the way in which political discourses can be used to reinforce hegemonic interests and overlooks uneven patterns of risk distribution.

One hundred and eighty-five crossbred (Yorkshire × Landrace) sows of mixed parity were randomly assigned to be fed either a basal diet or the same ration top dressed with either 500 ppm dichlorvos, 250 ppm copper or a combination of these additives. Top dressing was conducted from the time the sows entered the farrowing crate on day 106 of gestation until they were weaned 28 d postpartum. There was no effect of treatment on the number of pigs born alive, born dead or their birthweight. However, preweaning mortality, based as a percentage of pigs found alive, was 20.3% for control sows, 13.8% for sows fed the ration top dressed with copper, 12.3% for sows fed the ration top dressed with dichlorvos and 10.6% for sows fed the ration top dressed with the combination of feed additives (P = 0.02). There was no significant effect of treatment on the percentage of fat, protein, lactose or total solids in sows milk. There was also no effect of treatment on lactation feed intake, sow weight loss through lactation, the interval between weaning and successful service, conception rate, serum copper levels or total cholesterol. HDL cholesterol levels, serum copper, zinc and iron levels were generally higher in piglets from treated sows than controls. It was concluded that feeding high levels of copper or dichlorvos, alone or in combination, during late gestation and lactation may be useful as a means of reducing preweaning mortality in baby pigs. Introduction: Magnesium sulfate is the drug of choice for prevention of seizures in the pre-eclamptic woman. There is no agreement in the published randomized trials regarding the optimal time to initiate magnesium sulfate, the dose to use (both loading and maintenance) as well as the duration of therapy. The objective of this study is to determine whether magnesium sulfate (MgSO4) prophylaxis is needed for up to 24 hours postpartum in all patients with severe pre-eclampsia for the prevention of seizure. Methods: It is a randomized controlled trial done on 60 pregnant women with severe preeclampsia randomized into standard dose regimen and loading dose only regimen. Results: Out of 30 cases in each group 1 (3.3%) patient in standard regimen and 2 (6.7%) patients in loading dose only developed seizure. The occurrence of seizure is not significant statistically. In both regimens there was no maternal mortality. Total of 3 patients needed MICU care and 12 patient developed maternal complications. MgSO4 toxicities were seen only in standard dose regimen that is in 17 (56.7%) of the patients. The median number of IM injections of MgSO4 received in standard dose regimen was 8±2.176. In standard dose regimen 73.3 percent baby were alive whereas in case of loading dose only regimen 93.3 percent of baby were alive after 48 hours of delivery. Conclusions: Single dose of magnesium sulfate is equally effective as standard dose regimen in terms of seizure prophylaxis in severe pre eclamptic women, with added advantage of reduced maternal toxicity and better neonatal outcome. An anencephalic infant, who came to be known as Baby K, was born at Fairfax Hospial in Falls Church, Virginia, on October 13, 1992. From, the moment of birth and repeatedly thereafter, the baby's mother insisted that aggressive measures be pursued, including cardiopulmonary resuscitation and ventilator support, to keep the baby alive as long as possible. The physicians complied. However, following the baby's second admission for respiratory failure, the hospital sought declaratory relief from the court permitting it to forgo emergency life support on the grounds that “a requirement to provide respiratory assistance would exceed the prevailing standard of medical care,” and that “because any treatment of their condition is futile, the prevailing standard of medical care for infants with anencephaly is to provide warmth, nutrition, and hydration.” The United States Court of Appeals for the Fourth Circuit ruled in favor of the baby's mother, citing the federal Emergency Medical Treatment and Active Labor Act (popularly known as the “anti-dumping” act), which contained no “standard of care” exception to the requirement to provide “treatment necessary to prevent the material deterioration of the individual's condition.” An appeal to the United States Supreme Court was rejected. The baby died some two and one-half years later of cardiac arrest during her sixth visit to the emergency department of Fairfax Hospital for respiratory failure.

Scrub typhus is an acute febrile illness rarely diagnosed in pregnancy. Here we report a case of scrub typhus in the third trimester of pregnancy, its associated complications and its management to ensure a favourable fetomaternal outcome. Mrs X, a 22- year old primigravida, was diagnosed with scrub typhus at 38 weeks of pregnancy after developing high- grade fever for 12 days. An atypical eschar was seen on one foot. Her ultrasonography revealed foetal tachycardia and severe oligohydramnios. She was started on tab azithromycin 500 mg OD but remained febrile even after two days. Fever subsided at dose of 1gm/day. When she was afebrile for 48 hours and adequately hydrated, labour was induced and she delivered vaginally a full-term alive baby of 2.2 kg. Vertical transmission was not observed in the baby. Scrub typhus is an easy-to-treat cause of acute undifferentiated febrile illness in the Indian subcontinent, and should always be a differential diagnosis even in the absence of a pathognomic eschar. Timely treatment ensures a favourable fetomaternal outcome.

Premature babies who are born alive before 37th week of gestation or babies born with weight criteria of less than 2500 grams need to be treated in an infant incubator because they are very susceptible to viruses in open spaces and will provide protection to newborns by providing premature babies stable temperature and humidity. Stable temperature regulation is needed to prevent hypothermia or hyperthermia that will occur in premature babies. Care of the baby in an infant incubator causes the separation of mother and baby. Mothers who have premature babies are found to lack trust in caring for their babies compared to mothers who have term babies. The purpose of this study was to analysis of temperature on baby incubator control system. In order to get a stable and precise temperature, the temperature sensor is positioned according to the standard of the baby incubator at the hospital. This research uses LM35 and DS18B20 temperature sensors which will be processed using Atmega 328 Microcontroller and will be displayed on a PC (personal computer). Based on the measurement results obtained an average value of min difference for sensor readings on the LM35 has the largest value of 0.22°C and the average max value difference has the largest value of 0.38°C and the average value of the min difference for the sensor readings on the LM35 has a value the largest is 0.39°C and the average difference in the value of max has the largest value that is 0.55°C. It can be concluded that the LM35 sensor is better than the DS18B20 sensor because it has smaller oscillations and also good accuracy. The results of this study can be implemented in infant incubators in accordance with established standards and can be produced at affordable prices in the domestic market.

BackgroundThe 2030 Agenda for Sustainable Development aims to reduce neonatal mortality to at least 12 per 1000 live births. Most of the causes can be prevented or cured. Access to quality healthcare during pregnancy and labour is the key to reduce perinatal deaths, and maternity waiting homes (MWHs) may have an impact, especially for women who live far from the healthcare system. We conducted a case–control study to evaluate the effectiveness of MWH in reducing perinatal mortality in a secondary hospital in Ethiopia.MethodsWe did a nested case–control study from January 2014 through December 2017. The enrolled cases were mothers whose childbirth resulted in stillbirth or early neonatal death. The controls were mothers with an alive baby at 7 days or with an alive baby on discharge. We collected demographic, anamnestic, pregnancy-related and obstetric-related data. The effectiveness of the MWH on perinatal death was assessed by a logistic regression model, adjusted for all other variables investigated as potential confounders. We also did a sensitivity analysis to explore the role of twin pregnancies.ResultsWe included 1175 cases and 2350 controls. The crude analysis showed a protective effect of the MWH towards perinatal mortality (OR=0.700; 95% CI: 0.505 to 0.972), even more protective after adjustment for confounders (adjusted OR (AOR)=0.452; 95% CI: 0.293 to 0.698). Sensitivity analyses showed a consistent result, even excluding twin pregnancies (AOR=0.550; 95% CI: 0.330 to 0.917).ConclusionMWHs appear to reduce perinatal mortality by 55%. Our findings support the decision to invest in MWH to support pregnant women with higher quality and more comprehensive healthcare strategy, including quality antenatal care in peripheral primary care clinics, where risk factors can be recognised and women can be addressed for admission to MWH.

Primary and secondary spinal tumours with cord compression often represent a challenging condition for the patient and clinicians alike, even more so during pregnancy. The balance between safe delivery of a healthy baby and management of the mother’s disease bears many clinical, psychological, and ethical dilemmas. Pregnancy sets a conflict between the optimal surgical and oncological managements of the mother’s tumour and the well-being of her foetus. We followed the CARE guidelines from the EQUATOR Network to report an exemplificative case of a 39-year-old woman with a 10-year history of breast cancer, presenting in the second trimester of her first pregnancy with acute onset severe thoracic spinal instability, causing mechanical pain and weakness in lower limbs. Neuroradiological investigations revealed multilevel spinal deposits with a pathological T10 fracture responsible for spinal cord compression. The patient was adamant that she wanted a continuation of the pregnancy and her baby delivered. After discussion with her oncologist and obstetrician, we agreed to perform emergency spinal surgery—decompression and instrumented fixation. The literature search did not reveal a similar case of spinal metastatic breast cancer undergoing spinal instrumentation and delivery of a healthy baby a few months later. Following the delivery, the patient had further oncological treatment, including chemotherapy and radiotherapy. The paucity of such reports prompted us to present this case and highlight the relevance of a multidisciplinary approach involving obstetrician, oncologist, spinal surgeon, and radiologist to guide the optimal decision-making process.

Thus, climatologists seek out effective ways of communicating results of their research to various constituencies—a task made difficult by the complexity of the concept of climate. The current standard for communicated variability of climate on the global scale is a map based on the Köppen-Geiger classification (KGC) of climates, and maps of change in average annual temperatures and total annual precipitation for communicating climate change. The ClimateEx (Climate Explorer) project (http://sil.uc.edu/webapps/climateex/) communicates spatial variability and temporal change of global climate in a novel way by using the data science concept of similarity-based query. ClimateEx is implemented as a web-based visual spatial search tool. Users select a location (query), and ClimatEx returns a similarity map that visually communicates locations of places in the world having climates similar to the climate at a query location. ClimateEx can also inform about magnitude of temporal climate change by calculating a global map of local magnitudes of climate change. It offers personalized means of communicating climate heterogeneity and conveying magnitude of climate change in a single map. It has the advantage of relating climate to a user’s own experience, and is well-suited for communicating character of global climate to specialists and nonspecialists alike.

The ritual conducted is intended to keep the baby away from various kinds of illnesses or disturbance of natural surroundings. This study aims to reveal the meanings of verbal signs, nonverbal, manifest and latent in the ritual. The disclosure of meanings of verbal and nonverbal signs in Savukh Rangin ritual applied theory of social semiotics by Peirce (1975), which emphasizes on the signs of icons, indexes, symbols, and a means manifest and latent meaning according to Berger (2010). The analytical method used is descriptive qualitative analysis. The data collecting techniques used include interview, participatory, and observation techniques. The research results shows that the use of verbal and nonverbal signs referring to the lexical, significance, referential, associative, manifest, and latent meanings. The meanings are mediated systematically by all participants in the ritual. More dominant meanings in the ritual refer to the associative and referential meanings.

Baby care in an incubator is a method of caring for infants by putting the baby in a device that serves to help create adequate ambient temperatures with normal temperatures. Providing optimal nutrition is important in the management of premature and very low weight babies. Prevention of infection is the most important part of every newborn component. The purpose of this study was to determine the relationship between post natal care and premature baby health at the Siti Fatimah Makassar Hospital. This study is a cross sectional study, the population in this study were all babies born at gestational age less than 37 weeks and who were temporarily undergoing treatment at Siti Fatimah Hospital Makassar. Sampling using purposive sampling, obtained 30 samples according to inclusion criteria. Data collection is done in a primary way with direct observation techniques. The collected data is then processed and analyzed using Microsoft Excel computer programs and statistical programs (SPSS) version 6.0. Data analysis includes univariate analysis by looking for frequency distribution, bivariate analysis with chi-square test (x <0.05), to determine the relationship between variables. The results of the bivariate analysis found a relationship between postnatal care with premature infant health (p = 0.033). So it can be concluded that there is a relationship between post natal care with premature baby health at the Siti Fatimah Makassar Hospital. Therefore, paramedics should make efforts to continuously improve health services and carry out care of premature babies properly and apply optimal care facilities. For further research, researchers can conduct studies on other treatments provided to premature babies.

The Department of Meteorology (now Atmospheric Sciences) at the University of Utah faced reductions in state funding in 2008 that reduced support for nontenured instructors at the same time that the faculty were becoming increasingly successful obtaining federally supported research grants. A faculty retreat and subsequent discussions led to substantive curriculum changes to modernize the curriculum, enhance course offerings for undergraduate and graduate students, and improve the overall efficiency of the academic program. Maintaining discipline standards and existing teaching loads were important constraints on these changes. Key features of the curriculum revisions for undergraduate majors included eliminating a very rigid course progression; shifting the emphasis from required courses to elective courses; offering many courses only every other year; and relying on half-semester short courses to survey subject areas rather than focusing in depth on fewer ones. The curriculum changes were evaluated through surveys and individual and focus group discussions of students and faculty. While the feedback suggests that the changes overall were beneficial, the transitional period during which the changes were implemented was difficult for faculty and students alike. Faculty members have opportunities now to adjust courses based on their experiences gained teaching these courses in their new format. The feedback from students and faculty suggests that building improved relationships and interactions among co-enrolled undergraduate and graduate students is the greatest need in order to improve the classroom learning environment.

This article calls into question the frequently used negative moral labels assigned to corruption by describing gift giving as a form of narrative entrepreneurship that bridges ontologies between public service organizations. To effectively make the comparison, this article utilizes a unique methodology to explore corruption: the layered account autoethnography. The empirical setting of this story is a jointly operated military corrections facility in Iraq. It illustrates how gaps were perceived in the U.S.–Iraqi joint bureaucracy creating a space for play, and how corrupt behavior metaphorically bridged these gaps. Engaging in a minute form of gift giving provided remarkable insight into how partner organizations respond to traditional and corruption-friendly practices. This article is of benefit to practitioners and academicians alike because it illustrates at an individual level how corrupt bureaucracies function and how to promote successful interactions between alternate organizational ontologies.

Abstract:
A few years ago a battered infant was admitted to a California hospital. After a period of observation and testing, the physicians concluded that the infant had been beaten so badly that his brain was almost completely destroyed, leaving him permanently unconscious. The hospital had just adopted a policy specifying that life-sustaining treatment for permanent unconsciousness was futile and, therefore, not indicated. According to this policy, after suitable subspecialty consultations and deliberations, including efforts to gain parental agreement and documentation of unanimous ethics committee support, the patient's physician had the authority to discontinue life-sustaining treatment. The infant's physician wished to do this. The mother, however, who was the prime battery suspect, insisted that the baby be kept alive.

Abstract:
In this essay I construct a literary genealogy that situates William Dean Howells in the middle of a call-and-response literary conversation with popular women writers about race, gender, and genre. Since Howells correlated racial questions with realism, his only novel that treats intermarriage, An Imperative Duty (1891), offered Howells an opportunity to deploy his presumably objective, scientific, realist knowledge about race in order to challenge women's romantic miscegenation plots found in Margret Holmes Bates's The Chamber over the Gate (1886) and Alice Morris Buckner's Towards the Gulf (1887), two novels that he had recently read and reviewed. Yet the tragic mulatta stereotype, a stock figure of romanticism and sentimentality that was resistant to scientific discourse, ruptures Howells's goal of representing the figure according to the tenets of realism. In Iola Leroy (1892), Frances Ellen Watkins Harper cunningly recasts the tragic mulatta stereotype both to critique Howells's project and to represent the potential of black womanhood. Knowledge of Bates and Buckner can change critical conversation about the influence of women writers on Howells, the understanding of the role of the racialized woman in his fiction, and his conception of the link between the romantic mulatta and realist representation. Likewise, Harper takes issue with Howells's supposed ironic sophistication about race, and in Iola Leroy she rewrites many of his views in order to show the ways that miscegenation is at once a novelistic and a national problem.

Abstract:
Abstract This paper investigates the circumstances of 1,075 fatalities from flash flooding recorded from 1996 to 2014 across the United States. This study provides insights into the situations of the fatality events as determined by the victims’ profile and activity and the spatiotemporal context of the flooding. A reclassification of the individual fatality circumstance (i.e., location and/or activity) is performed to explore statistically the timing, the duration, and location of the flash flood event and the age and gender of the victims. In agreement with other studies, more than 60% of the reported fatalities were related to vehicles involving mainly males. A geospatial analysis indicated these were most common in southern states. Further, 21% of fatalities occurred outdoors, typically in neighborhoods near streams, where the victims were exhibiting high-risk-taking behavior, such as cleaning out drains and even playing in the floodwaters. Human vulnerability varies dynamically on a subdaily basis and depends on social and natural factors of the flash flood. For example, most campsite-related fatalities were associated with very fast-responding flash flood events (less than 5-h duration), occurred more commonly after midnight, and impacted younger females and males alike. On the other hand, fatalities related to inundation of permanent buildings were most commonly associated with longer-duration events and impacted the elderly. Situational rather than generic examination of vulnerability is required to realistically capture risky cases during short-fuse flood events. The circumstances in which people perished in flash floods suggest that situational rather than generic examination of vulnerability is required to realistically capture risky cases during short-fuse flood events.

Pada umumnya hewan donor disuntik dengan preparat follicle stimulating hormone (FSH) danPregnant mare's serum gonadotropin(PMSG) atau kombinasi (Pregnant mare's serum gonadotropin) dan human chorionic gonadotropin (hCG).Tujuan Penelitian iniadalah untuk mengetahui sejauh mana prestasi beranak babi dara (calon induk) dengan menggunakan hormon pregnant mare's serum gonadropin (PMSG) dan human Chorionic Gonadotropin (hCG).Penelitian ini menggunakan Rancangan Acak Lengkap (RAL) terdiri atas dua perlakuan (menggunakan dan tanpa menggunakan hormon PMSG dan hCG) dengan masing-masing 21 ulangan. Semua data dianalisa dengan analisis sidik ragam (ANOVA) Steel dan Torrie (1993).Variabel yang diamati untuk adalah litter size, berat lahir anak babi yang dilahirkan dan mortalitas anak babi yang lahir sampai disapih. Hasil penelitian menunjukkan bahwa jumlah anak babi yang lahir hidup dalam satu kelahiran ada perbedaan sangat nyata bahwa yang lahir dari induk Superovulasi dibandingkan dengan induk yang tidak disuperovulasikan, rata-rata 9.19-6.66 ekor. Dan juga berat lahir rata-rata 1.53-1.40 kg Demikian juga mortalitas anak rata-rata 14.36-29.19%. Hasil penelitian menunjukkan bahwa penggunakan hormon PMSG dan hCG dapat memperbaiki litter size lahir, bobot lahir, dan menekan persentase mortalitas ternak babi. Superovulation is an effort to increase the degree of ovulation by injection of gonadotropin hormones in the body of a female animal. The objective of this research was to determine the extent of achievement farrow dara (prospective parent) using pregnant mare's serum hormone gonadropin (PMSG) and human chorionic gonadotropin (hCG). The research using completely randomized design (CRD) consisting of two treatments (using and without using PMSG and hCG hormone) with 21 replications each. All data were analyzed with analysis of variance (ANOVA) Steel and Torrie (1993). Variables was observed for litter size, birth weight piglets born and piglets born mortality until weaning. The results showed that the number of piglets born alive in the birth there is a very real difference that is born of a parent superovulation compared with the parent who does not to superovulated on average 9.19–6.66 tail. And also the average birth weight 1:53 to 1:40 kilograms likewise the average child mortality 14.36–29.19%. From the results of the study concluded that the use of PMSG and hCG hormone can improve birth litter size, birth weight and reduce the percentage of mortality of pigs. In January 1581 a short black-letter tract in the tiny octavo format favoured by many Elizabethan publishers of sensational news appeared from the press of a London printing house based just outside the City in the Strand. Like other ‘three-halfpenny’ pamphlets of its ilk, this particular piece of ephemera was a pious tale of the prodigious and strange, but true. It described the ‘wonderfull worke of God shewed upon a chylde’ by the name of William Withers in the small Suffolk town of Walsham-le-Willows. On the previous Christmas Eve, the eleven-year-old boy had fallen into a deep trance for the space often days, ‘to the great admiration of the beholders, and the greefe of his parentes’. At the end of this time he regained consciousness and proceeded to deliver a series of vehement prophetic denunciations of contemporary sin and immorality, calling the people to ‘spedie repentance’—without which, he announced, the day of destruction was surely at hand. If amendment of life was not quickly forthcoming, the Lord would presently shake their houses on their heads and cause the earth to open up and swallow them alive.

Aims of this article consists in analysis of the rate of the most important reasons of miscarriages and in estimation of effectiveness of pregravid therapy of wemen with miscarriages in the past who were patients of Center in 2014. The most important reasons were the infection of genital tract (60 %), inherited thrombophilia (95 %) and AFS (3,2 %), endocrine diseases with secondary or initial insufficiency of corpus luteum (35 %), chronic endometritis (60 %). There were presented results of the investigation of the significance of insulin resistance in the pathogenesis of miscarriages and the efficiency of the treatment with metformin patients with prediabetes. There were analysed the course and results of pregnancy in 354 women with miscarriages in the past. In 92 % of women the pregnancy complete by delivery of alive baby. This results demonstrate the effectiveness of pregravid preparation of women with miscarriages in the past.

Takayasu’s arteritis is a chronic idiopathic vasculitis affecting aorta and its primary branches, commonly seen in young women of Asian or oriental descent during child bearing age. Incidence is 2.6 cases/million/year. It affects females in the reproductive years accounting for almost 80% of the cases. A 34 year female G2P1L1 with 9 months of amenorrhea with previous LSCS known case of takayasu arteritis, epilepsy with right side hemiparesis with aphasia. She proceeded to have elective LSCS with ventouse delivery under epidural anaesthesia and delivered alive healthy female baby weight 2.4 kg and patient was transferred to intensive care unit for observation for 48 hrs. patients intrapartum post partum period was uneventful and discharged after 1month with cardiology fitness. Pregnancy with takayasu arteritis (pulseless disease) requires a multidisciplinary management from gynaecologist, cardiologist, neurologist for favourable maternal and fetal outcome.

The ethical problems are heightened when the parents, or the single mother, are incompetent to make decisions themselves, for example, because of drug addiction. In such cases, when the baby is premature and suffering the effects of the drugs the mother has taken, and the mother herself is either no longer available for consultation or so damaged by her own addiction that she is not a reliable decisionmaker, the usual trend In the United States is to initiate treatment and continue until it is virtually certain that the infant will die.
