Jeanette Allen and Panos Vostanis describe the set-up and preliminary evaluation of a seven-week training programme for foster carers and supervising social workers based on attachment theory. It has long been recognised that parenting children with complex and challenging needs in a family setting is extremely demanding, with research indicating that foster carers' lack of confidence in managing such demands is a significant factor in placement breakdown. This study suggests that attachment theory provides a useful model to help carers understand and respond to the complex difficulties presented by children with a history of abuse and trauma, and that the provision of a conceptual framework may be a significant factor in improving foster carers' confidence to manage the difficulties presented by the children in their care. The clinical implications of this evaluation are discussed.

Abstract. Coronal mass ejection (CME)-shock compression of the dayside magnetopause has been observed to cause both prompt enhancement of radiation belt electron flux due to inward radial transport of electrons conserving their first adiabatic invariant and prompt losses which at times entirely eliminate the outer zone. Recent numerical studies suggest that enhanced ultra-low frequency (ULF) wave activity is necessary to explain electron losses deeper inside the magnetosphere than magnetopause incursion following CME-shock arrival. A combination of radial transport and magnetopause shadowing can account for losses observed at radial distances into L = 4.5, well within the computed magnetopause location. We compare ULF wave power from the Electric Field and Waves (EFW) electric field instrument on the Van Allen Probes for the 8 October 2013 storm with ULF wave power simulated using the Lyon–Fedder–Mobarry (LFM) global magnetohydrodynamic (MHD) magnetospheric simulation code coupled to the Rice Convection Model (RCM). Two other storms with strong magnetopause compression, 8–9 October 2012 and 17–18 March 2013, are also examined. We show that the global MHD model captures the azimuthal magnetosonic impulse propagation speed and amplitude observed by the Van Allen Probes which is responsible for prompt acceleration at MeV energies reported for the 8 October 2013 storm. The simulation also captures the ULF wave power in the azimuthal component of the electric field, responsible for acceleration and radial transport of electrons, at frequencies comparable to the electron drift period. This electric field impulse has been shown to explain observations in related studies (Foster et al., 2015) of electron acceleration and drift phase bunching by the Energetic Particle, Composition, and Thermal Plasma Suite (ECT) instrument on the Van Allen Probes.

Zusammenfassung Einleitung Seit 1907 haben sich international verschiedene Transpositionsmanöver bei einer Abduzensparese etabliert. 1935 beschrieb OʼConnor als Erster eine komplette Vertikalmotorentransposition, die Foster 1997 mit einer posterioren Sklerafixation der Transplantate verstärkte. Die durch die „Foster-Naht“ verstärkte Kompletttransposition der geraden Vertikalmotoren gehört zu den wirkungsvollsten Operationstechniken zur Verbesserung der Abduktion. Ziel dieser Studie war es, bei Patienten mit einer Abduzensparalyse das Ergebnis einer solchermaßen verstärkten Muskeltransposition zu beurteilen. Methoden und Ergebnisse Die durch eine Foster-Naht verstärkte komplette Transposition der geraden Vertikalmotoren wurde im Jahr 2014 bei 5 Patienten mit einer Abduzensparese durchgeführt. Zwei der Patienten erhielten eine Botoxinjektion in den M. medialis: ein Patient 3 Monate postoperativ und ein Patient während der Operation. Einer der Patienten hatte 1 Jahr zuvor eine kombinierte Horizontalmotorenoperation erhalten. Bei 3 der Patienten, die eine reine Transposition mit Foster-Naht erhielten, wurde der präoperative Fernschielwinkel (Median: + 56,6 PD, Streubreite: + 40 – + 80 PD) im Mittel um 39,6 PD reduziert (Median; Streubreite: 34 – 50 PD), die Abduktion wurde im Mittel um 3 mm verbessert (Streubreite: + 2 – + 4 mm). Bei den anderen 2 Patienten, bei denen die Transposition der Vertikalmotoren mit Vor- bzw. Zusatzoperationen an den Horizontalmotoren verbunden war, wurde der präoperative Fernschielwinkel (+ 25 bzw. + 126 PD) um 20 bzw. 81 PD reduziert. Die Adduktion wurde um 4 bzw. 8 mm verbessert. Zwei Patienten entwickelten nach der Transpositionsoperation eine dezente vertikale Abweichung für die Ferne bis maximal 4 PD. Bei keinem der Patienten traten Komplikationen auf, eine Vorderabschnittsischämie wurde nicht diagnostiziert. Die Hebung bzw. Senkung wurde bei allen Patienten nur sehr gering beeinflusst. Diplopie im Aufblick oder Abblick trat nicht auf. Zusammenfassung Eine komplette Transposition der Vertikalmotoren, verstärkt mit der posterioren Skleranaht, ist eine wirkungsvolle und sichere Operationsmethode und kann in vielen Fällen eine zusätzliche Rücklagerung des M. rectus medialis ersparen. Die Hebung bzw. Senkung wird durch diese Technik nur sehr gering beeinflusst. Diversen Studien zufolge ist die Gefahr einer Vorderabschnittsischämie gering.
