Abstract:
Standard allergy testing relies on identifying the IgE antibody specific for the allergen in question. Two classic methods of doing this are the immediate wheal-and-flare skin prick tests (a small amount of antigen is introduced into the skin and evaluated at 15 minutes for the presence of an immediate wheal-and-flare reaction) and in vitro testing. Allergy testing that does not have a clear scientific basis includes cytotoxic testing, provocation-neutralization testing or treatment, and "yeast allergy" testing. Allergy-related conditions such as asthma, chronic rhinitis, urticaria and angioedema, anaphylaxis, food allergy, stinging insect allergy, and drug allergy are reviewed.

Abstract:
Anesthesiologists and sedationists often use propofol as the main anesthetic agent for brief procedures, such as esophagogastroduodenoscopy with biopsy for eosinophilic esophagitis (EoE). Pediatric patients presenting for endoscopy often have a history of reflux as well as drug and food allergies. Specifically, patients with EoE often have sensitivity to egg and soy, as well as milk and dairy products, tree nuts/peanuts, and seafood (fish/shellfish). Propofol use is often cited as a contraindication in patients with hypersensitivity to egg and soy. Current literature does not support avoiding propofol in egg- and soy-allergic patients. Most practitioners, however, continue to avoid propofol in patients with a history of egg anaphylaxis due to lack of evidence supporting its safe use in this population.

Abstract:
The term ‘anisakiosis (anisakidosis)’ or ‘anisakiasis’ collectively defines human infections caused by larval anisakids belonging to the nematode family Anisakidae or Raphidascarididae. Anisakis simplex, Anisakis physeteris, and Pseudoteranova decipiens are the three major species causing human anisakiosis. Various kinds of marine fish and cephalopods serve as the second intermediate hosts and the infection source. Ingestion of viable anisakid larvae in the fillet or viscera of these hosts is the primary cause of infection. The parasite does not develop further in humans as they are an accidental host. Clinical anisakiosis develops after the penetration of anisakid larvae into the mucosal wall of the alimentary tract, most frequently the stomach and the small intestine. The affected sites undergo erosion, ulceration, swelling, inflammation, and granuloma formation around the worm. The patients may suffer from acute abdominal pain, indigestion, nausea, vomiting, and in some instances, allergic hypersensitive reactions. Symptoms in gastric anisakiosis often resemble those seen in peptic ulcer or gastric cancer, and symptoms in intestinal anisakiosis resemble those of appendicitis or peritonitis. Treatments include removal of larval worms using a gastroendoscopic clipper or surgical resection of the mucosal tissue surrounding the worm. No confirmed effective anthelmintic drug has been introduced, though albendazole and ivermectin have been tried in vivo and in vitro. Prevention of human anisakiosis can be achieved by careful examination of fish fillet followed by removal of the worms in the restaurant or household kitchen. Immediate freezing of fish and cephalopods just after catching them on fishing boats was reported helpful for prevention of anisakiosis. It is noteworthy that anisakiosis is often associated with strong allergic and hypersensitivity reactions, with symptoms ranging from isolated angioedema to urticaria and life threatening anaphylactic shock.

Abstract:
This chapter describes therapeutic plasma exchange, as well as cytapheresis for hyperleukocytosis and essential thrombocythemia, as well as harvesting haematological stem cells (HSC) for transplantation. Instrumentation and techniques are mostly density centrifugation, much less column adsorption for antibodies or membrane filtration for noxious molecules. Pathophysiology of apheresis is dealt with in great detail with emphasis on prevention and treatment of side effects, much more critical in the intensive care unit (ICU) setting. Main manifestations are: hypocalcaemia due to chelation by anticoagulants, hypo- and less hypervolaemia, allergic reactions to sedimenting and volume replacement starches or plasma, depletion of coagulation factors, vitamin K, immunoglobulins, lymphocytes with long lifespan and platelets. Wash-out of drugs for comorbid or underlying conditions occurs inadvertently. Main indications for plasma exchange are thrombotic thrombocytopenic purpura (TTP)/haemolytic uraemic syndrome (HUS) with plasma or cryo-poor supernatant (based on RCT), hyperviscosity syndromes, post-transfusion purpura (PTP) and auto-immune haemolytic anaemia (AIHA), where all other treatments have failed. In cold agglutinin disease, cryoglobulinaemia, coagulation factor inhibitors and ABO incompatible HSC transplants, plasmapheresis has proven useful. Myeloma with renal failure does not seem to benefit significantly from plasma exchange (randomized controlled trials proven).

Abstract:
A bewildering feature of so much contemporary political violence is its stunning impersonality. Every major city centre becomes a potential shooting gallery; and every metro system a potential bomb alley. Victims just happen, as the saying goes, to ‘be in the wrong place at the wrong time’. Killing Strangers tackles the question of how such violence became ‘unchained’ from inter-personal relationships. It traces the rise of such impersonal violence by examining violence in conjunction with changing social and political realities across Western Europe and North America since the late eighteenth century. In particular, it traces both ‘push’ and ‘pull’ factors. On the one hand, the rise of the modern state with its titanic bureaucratic resources of monitoring and coercion forced the violence of opponents into niche forms. On the other hand, social and technological changes offered fresh opportunities to cause mayhem in startlingly new ways. Both forces are necessary for any understanding of why contemporary political violence takes the forms that it does.

Der Bamberger Dom, das bedeutendste Bauwerk der Stadt, besteht seit gut 1000 Jahren. Prof. Dr. Walter Sage, nachmalig erster Inhaber des Lehrstuhls für Archäologie des Mittelalters und der Neuzeit (AMANZ) an der Universität Bamberg, führte dort von 1969-72 großangelegte Ausgrabungen durch. Er ließ nahezu das gesamte Hauptschiff öffnen, dazu große Bereiche der Seitenschiffe. Man traf Fundamente aller Bauphasen an, dazu viele Bestattungen und eine große Zahl von Kleinfunden. Diese Funde stammen zum Teil von der Innenausstattung des ersten Doms, dessen grundsätzliche Boden- und Wandgestaltung somit gut rekonstruierbar ist. Die wissenschaftliche Aufarbeitung der Funde – 50 Jahre nach ihrer Bergung – war Anlass und Inhalt einer Sonderausstellung im Historischen Museum, bewerkstelligt vom Lehrstuhl AMANZ in Zusammenarbeit mit dem Historischen Verein und dem Historischen Museum Bamberg, den Eigentümern der Funde und unterstützt vom Erzbistum Bamberg. Mit dieser Ausstellung und dem Begleitheft, beides hauptsächlich von Studierenden erarbeitet, feiert der Lehrstuhl AMANZ zudem sein 40-jähriges Bestehen. Bamberg Cathedral, the most important building in the city, was built more than 1000 years ago. Prof. Dr. Walter Sage, who later became the first professor of medieval and postmedieval archaeology at the University of Bamberg, carried out large-scale excavations from 1969-72. He researched almost the entire nave as well as large areas of the aisles. Foundations of all construction phases were found, as well as many burials and a large number of small finds. Many finds were part of the interior of the first cathedral, allowing a reconstruction of the floor and the wall design. 50 years after the excavations the analysis of these finds is part of a new scientific project. Together with the 40th anniversary of the chair of medieval and postmedieval archaeology this is the occasion for a special exhibition in the Historical Museum Bamberg. The exhibition and this booklet were realized in cooperation with the Bamberg Historical Association, who owns the finds today, and with the support of the Archdiocese of Bamberg. Conception, exhibition texts as well as most of the article were prepared by students of the chair of medieval and postmedieval archaeology.
