Abstract:
Improvements in cryopreservation techniques have made possible excellent long-term results using aortic valve allografts. Few studies, however, have examined the echocardiographic characteristics of allograft aortic valves and postoperative valve function. Aortic allograft function was eveluated in 16 patients by intraoperative transoesophageal echocardiography, postoperative catheterization, and angiography. The mean(s.d.) coaptation zone length of the allograft leaflet was significantly greater (7.3(1.4) mm) than that of the normal aortic valve (3.2(0.8) mm) (P < 0.01). No regurgitation w

Introduction: In severe recurrent infectious lesions in the right heart, one of the alternative options for bioprosthetics is to implant mitral allografts (donor heart valves) in the tricuspid valve position. Allografts offer numerous benefits, including a configuration that closely resembles a native valve, adaptability to high systemic pressure, absence of foreign elements that can could act as entry points for infections, and antibacterial treatment without lifelong anticoagulant therapy. Objective: To study the feasibility of using a mitral allograft in the tricuspid position based on the a

The indications for cryopreserved allografts in aortic valve replacement are still debatable. We aim to identify factors influencing early and long-term durability of the aortic homograft and to define subgroups of patients with an improved long-term quality of life, survival, and freedom from structural valve degeneration (SVD). We evaluated our series of 210 patients who underwent allograft implantation with a retrospective cohort study design over a period of 20 years. Endpoints were overall mortality, cardiac mortality related to SVD, the incidence of SVD, reoperation, and a composite endp

Renal transplantation remains a mainstay of therapy for end-stage renal disease. Cardiac disease has a high prevalence in this patient population. This study reviews the factors and outcomes associated with cardiac surgery in renal transplant recipients. We performed a retrospective review of all patients at our institution with a functioning renal allograft at the time of their cardiac surgical procedure. Between June 1971 and April 2000, 2343 patients underwent renal transplantation at Vanderbilt University Medical Center. Twenty-six patients with a functioning renal allograft subsequently u

Between April 1990 and August 1996, 138 patients (median age 42 years, range 10 days to 66 years) underwent aortic valve or root replacement with allografts ( n = 70) or autografts ( n = 68). Aortic stenosis was the main indication (80 patients, 58%) followed by aortic incompetence (31 patients, 22%). Twenty-five patients had endocarditis (18%) and two patients had truncus repair (2%). Some 140 allograft valves were inserted (70 pulmonary. 70 aortic). Most of the valves were conditioned and cryopreserved by the European Homograft Bank, Brussels, Belgium. Abstract:
This thesis discusses the theory and application of a variety of statistical methods for addressing specific problems, which arise in medical research. The thesis focuses on three data sets that are used as case studies. These data sets contain information on heart disease surgical outcomes and aortic valve allograft survival. An overview of statistical methods for risk stratification and survival analysis is provided. A new method of outcome specific pruning of classification trees for risk stratification of rare events is discussed, applied and interpreted for one of the case studies. App
