Abstract:
Altered sensorium is a frequent presentation in emergency department, but the exact etiology of many altered sensorium patients is unknown. Patients present with vague symptoms, thus altered sensorium diagnosis and treatment are highly challenging for emergency physicians. The aim of this study is to provide a framework for the assessment of altered sensorium patients. This assessment should allow providers to better understand the etiology of mental status changes and therefore improve diagnostic skills and management. This is a retrospective record based study. The research duration was from January, 2016 to December 31, 2016. The study involved 9,800 emergency patients, in which 524 (5.34%) were altered sensorium patients. Demographic characteristics, clinical manifestations, assessment approaches, causative factors, emergency treatments and outcomes were collected. In 524 patients with altered sensorium patients recruited, accounting for 5.34% of all emergency department patients, 316 (60%) were male, and 208 (40%) female. Etiologic factors were neurological (n=225; 42.93%) & nonneurological (n=299; 57.07%). Patients with altered sensorium pose a diagnosing difficulty for ED physicians. The most frequently encountered diagnostic categories causing altered sensorium patients were primary CNS disorders, intoxication, organ system dysfunction, and endocrine/metabolic diseases. Altered sensorium is an important warning signal for ED patients because of its potentially fatal and reversible effects. Prompt evaluation and treatment are essential to decrease morbidity and mortality associated with altered sensorium patients.

Abstract:
Background: Apart from head injury many patients present to the tertiary care hospital in unconscious state, the etiology of which is obscure in most of the cases. The present study was conducted with on objective to provide insight into the clinical features and diagnostic methods to know the aetiology of patients with non-traumatic cases of altered sensorium and to study the outcome of these patients.Methods: This is an observational study on 100 patients of altered sensorium of non-traumatic origin during the period from October 2012 to September 2014 conducted in the Department of General Medicine, MKCG Medical College Hospital, Berhampur, Odisha, India. All patients were selected for the study based on the inclusion exclusion criteria. Detailed history, clinical evaluation, laboratory investigations like neuroimaging studies etc. were carried out. Statistical data analysis was done using Graph pad Prism 6 and Microsoft Excel. P value <0.0001 was considered statistically extremely significant.Results: Out of 100 patients of altered sensorium, 64 were males and 36 were females. All patients were in the age group of 19 to 89 years. Cerebrovascular accident was the most common aetiology of altered sensorium followed by metabolic encephalopathy and infection. Altered sensorium in patients with CVA carries a high mortality. Metabolic causes and younger age indicated a better prognosis, patients with low (Glasgow Coma Score) GCS score of 3 to 4 had poorer prognosis.Conclusions: The results suggest that clinical assessments yield accurate predictive information about the potential for recovery in cases of altered sensorium. So, this study concludes that empirically based estimates of prognosis in the neurologically severely ill provides great reassurance to those involved in a decision-making process, including patients’ families and physicians.

Pitutary macroadenoma and microadenoma has their own clinical presentations. These conditions are not very common and they may present with varied symptoms to the ED. There are instances when these conditions are incidentally detected by emergency radiological investigations. However in most of the instances the patient may present with vague complaints like dizziness and headache and in some rare instances the patient may resent with poor GCS. The cause may be attributed to the raised ICP due to growing adenoma or sometimes due to other effects.

Catatonia is a neuropsychiatric syndrome that represents an uncommon clinical end point of a number of primary psychiatric and neuromedical conditions. The common pathophysiology may be associated with reduced dopaminergic activity within the frontal–subcortical circuitry. The incidence of catatonia was considerably higher before the introduction of neuroleptics, which reduced its association with poorly controlled schizophrenia. Catatonia is a syndrome with a spectrum from benign to malignant, the latter being a medical emergency with a significant mortality rate. Treatment is with benzodiazepines, which can be sufficient in less severe cases, and in more resistant or malignant cases with life-threatening clinical features, electroconvulsive therapy is sometimes applied.

In the United Syates, chronic kidney disease (CKD) affects approximately 5% to 10% of the general population. It is estimated that about 20 million Americans have some degree of CKD. Central nervous system (CNS) abnormalities are common in patients with CKD, especially in those individuals with end stage renal disease (ESRD) who require renal replacement therapy, such as dialysis or transplant. Neurological symptoms in patients with CKD may range from mild altered sensorium and cognitive dysfunction to tremors and coma. By the time patients require renal replacement therapy, some patients may have uremia, a clinical syndrome with protean manifestations.

Delirium in the postoperative period, characterized by inattention, disorganized thinking, disorientation, and/or altered levels of consciousness within the first few days after surgery, has been associated with significant increases in hospital stay, functional decline, prolonged cognitive dysfunction, and mortality. It is underdiagnosed without routine assessments with validated tools such as the Confusion Assessment Method (CAM), the 4AT, the Confusion Assessment Method for Intensive Care Unit (CAM-ICU), or the Intensive Care Delirium Screening Checklist (ICDSC). Prevention strategies for postoperative delirium include multimodal pain control, judicious use of medications that affect the sensorium, including benzodiazepines and anticholinergics, maintenance of appropriate volume status, and optimization of the patient’s environment. In patients who develop delirium with severe agitation, antipsychotic and alpha-2 agonist medications may be useful. Because postoperative delirium occurs commonly and is associated with worse outcomes, an understanding of its disease process, risk factors, and management is essential for an anesthesiologist.

Cryptococcal infections account for approximately 8% of all invasive fungal infections in solid organ transplant recipients and typically occur 16 to 21 months after transplantation. Cryptococcal infection should be considered for any transplant recipient who presents with altered sensorium and fever. Analysis should include opening pressure measurement, cryptococcal antigen testing, cell count with differential, and glucose and protein levels. Initial treatment should consist of a lipid formulation of amphotericin B and flucytosine for at least 2 weeks, followed by high-dose fluconazole for 8 weeks and then lower doses of fluconazole for 6 to 12 months or possibly longer.

This chapter discusses how Hamtramck residents engaged in public debates over the adhān, the Muslim call to prayer traditionally broadcast five times a day in Muslim-majority nations. The chapter introduces the concept of the “urban sensorium” to discuss how individuals on both sides of the debate described the adhān as rhythm that either facilitated or compromised harmonious relationships between Muslim and non-Muslim Americans, and how residents engaged in shared listening as a mode of spatial and temporal embodied practice across religious lines. Expressions of Islamophobia fomented by media coverage of the call-to-prayer campaign gave rise to an interfaith alliance in which Hamtramck Muslim and Catholic Americans publicly demonstrated new forms of identification with one another. The chapter considers how Muslim sound altered social and sensory dimensions of city life and how the debates presented opportunities to expand the sensory and cultural boundaries of municipal belonging.

Foreign Body (FB) aspiration incidence follows a bimodal distribution and is commonly seen in children under the age of 5 or adults with advanced age. The type of FB can be organic or non-organic and varies depending on age, region of the world, and socioeconomic status. In children, FB aspiration is often a consequence of development and exploration. Aspiration of FB in adults typically occurs when cough reflex is blunted due to altered sensorium (from drugs, medications, advanced age), trauma to the airway, poor swallowing, or anatomical variations in the GI tract (strictures, Zenker’s Diverticulum, etc. Symptoms can range from cough to asphyxiation depending on the acuity of the aspiration event. Initial management is focused on ensuring there is no airway compromise leading to respiratory failure. Bronchoscopy is the treatment of choice to retrieve the FB. This chapter will review the incidence of FB aspiration in children vs. adults and will focus on diagnosis and treatment including bronchoscopy, role of ECMO, and surgical intervention.

Pesquisas realizadas recentemente revelam que os jogos virtuais esti- mulam o cérebro de uma forma benéfica à saúde do usuário, portanto, esses vêm sendo amplamente utilizados como um auxı́lio ao tratamento de doenças rela- cionadas ao envelhecimento. Além disso, estudos elucidam que o uso de efeitos sensoriais é eficaz no tratamento de psicopatologias. Dessa maneira, este ar- tigo propõe o Jogo do Stroop, um jogo cognitivo baseado no teste de Stroop, com a presença de um efeito sensorial de luz. O jogo foi desenvolvido para a TV digital brasileira, utilizando o middleware Ginga-NCL. Após a realização de testes do Jogo do Stroop com idosos em uma oficina de memória cognitiva, conclui-se que o jogo proposto neste trabalho teve uma boa aceitação entre os voluntários e que o efeito sensorial altera a percepção do jogador, tornando-o mais atrativo.

Patterns, murals, reliefs, and various types of surface articulations have long been an integral part of design. Attitudes towards surface are reflective of a broader climate of spatial production, gauging the necessity and commodity of agglomeration within spaces specifically, and the agency of liminal territories within architecture at large. Commonly deposited on surfaces within prized programs (such as churches, palaces, and mosques), patterns and their respective aggregations reference through their inscriptions deities and assert the influence of various authorities. They are also telling of traditions of making and craft, both analog and digital. Whether such formations assume anthropomorphic qualities or are fashioned around pure geometry and repetition, the agency of surface articulation remains a present yet often contested area in design.1 The resilience of patterns and their resurgence in the digital age is a testament to their impact on our spatial experience.2 From primitive markings to the most complex formal plasticity of fabricated assemblages, topical material applications are intrinsic to asserting value and identity to the spatial volume. Heterotopias by nature, patterns alter and augment the space of occupation. They cater to a different formal metabolism characterized by a unique receptiveness to the users, thus, introducing friction to interior and exterior territories. Now more than ever, the need for this friction between surfaces and their occupants is essential in an era ubiquitous with the flatness of virtual imagery3 and an overall contextual banality.

Introdução: Nas últimas décadas, tornou-se evidente que o segmento populacional que mais cresce no Brasil é composto por pessoas com mais de 60 anos. Paralelamente, houve uma evolução da promoção da saúde e maior conscientização em relação aos cuidados com a boca e os dentes nesta fase da vida. Objetivo: Ressaltar os principais fatores fisiológicos e não fisiológicos responsáveis por alterar as papilas gustativas durante o envelhecimento. Material e métodos: foi realizada uma pesquisa bibliográfica analítica para reunião de informações contemplando artigos nacionais e internacionais, em português e inglês compilados na íntegra nas bases de dados: Scielo (Scientific Electronic Library Online) e BVSMS (Biblioteca Virtual em Saúde) utilizando os descritores “paladar”, “envelhecimento”, “idoso”. Resultados: O envelhecimento é responsável por alterar estruturas e funções do corpo humano como a capacidade mastigatória, fluxo salivar e estímulos sensoriais. O processo de senescência compromete a identificação do cheiro e sabor dos alimentos, e fatores como o uso de fármacos, álcool, tabaco e próteses dentárias podem intensificar essas alterações. Com o avanço da idade, os corpúsculos gustativos presentes nas papilas da língua vão sofrendo um decréscimo significativo, e os principais sabores (doce, salgado, amargo, azedo e umami) vão se tornando menos intensos, comprometendo os hábitos alimentares do idoso. O envelhecimento é um processo irreversível e natural. Dessa forma, compreender as mudanças na função sensorial dos idosos é de extrema importância. As membranas responsáveis por recobrir o nariz tornam-se finas e secas e os nervos olfativos e neurônios receptores acabam se deteriorando. Nesse sentido, a detecção de odores sutis vai se tornando difícil entre os idosos. Além disso, foi visto que alcoolismo, tabagismo, uso de fármacos e próteses dentárias fazem com que os indivíduos idosos estejam suscetíveis à perda da sensibilidade gustativa. Em decorrência disso, a nutrição também é acometida e os efeitos nos pacientes idosos podem ocorrer de maneira mais proeminente. Conclusão: É necessário compreender as alterações que ocorrem na função gustativa durante o envelhecimento e detectá-las a fim de contribuir para uma melhor qualidade de vida dos idosos.

Abstract:
An on demand oil system, based on electrically driven lube and scavenge pumps, for use in gas turbine engines has been developed. The need to optimize gas turbine engine performance, coupled with ‘electrification’ of aircraft systems, as on the Lockheed F35 and Boeing 787 in order to maximize efficiency and flexibility, created an opportunity to develop a ‘smart’ electrically driven lubrication system for gas turbine engines. Spytek’s electric oil system, developed for use on its 400lbt ATG-2 engine platform, ascertains the operating condition of the gas turbine engine, including speed, pressure, bearing temperatures and determines the amount of lubrication required for each bearing zone. The system has the benefit of better thermal control of engine bearings, lower system weight and power use, with flexibility in the placement of the system on the engine/airframe combination. The system has been successfully demonstrated on the Spytek Aerospace ATG-2/J304 gas turbine engine series. Major areas addressed in the development of the system were the selection of reliable, gas turbine engine compatible feed and scavenge pumps, control systems sensoring and feedback, variable feed of oil to the individual sump wells, as well as systems durability and operating parameters. Various gas turbine engine platforms can require altered oil system duty cycles, including pre-oiling or post run down-oiling, high flow oil conditions, features not readily available in traditional mechanical systems but easily implemented using the Spytek on-demand oil system. In the project effort, evaluation of the existing prototype system was used as a system design baseline. Data on pump wear, filter performance and oil supply degradation was available and used in refining the oil system design. The ATG-2 system was modified to take advantage of a full proportional integral derivative controlled oil system with system demonstrations made on a gas turbine engine, demonstrating oil supply on demand capability and full bearing thermal management system control.

Abstract:
O objetivo do presente estudo é estabelecer relações entre a Alteração Auditiva Sensorioneural decorrente da Apneia Obstrutiva do Sono (AOS) e evidenciar os possíveis tratamentos. O caso evidencia homem, 58 anos, IMC 33kg/m2, sinusectomia há 2 anos e histórico de otite média à direita, tabagista, baixa memória, ronco, sono não reparador e sonolência diurna excessiva (SDE), dificuldade no reconhecimento e compreensão de fala no ruído e hipoacusia sem otalgia, em uso de Carbolitium 450mg, Venlafaxina 150mg e Olanzapina 5mg. A polissonografia revelou Apneia Obstrutiva do Sono (AOS) severa, com 163 eventos respiratórios obstrutivos, Índice de Apneia/Hipopneia(IAH) total de 32,2/hora e 101 dessaturações. Implantou o CPAP com melhora na SDE. O último relatório terapêutico revela 33% de uso e IAH 1,5/hora. Na audiometria, apresenta perda auditiva condutiva leve à direita e perda auditiva sensorioneural à esquerda nas frequências de 250Hz, 4KHz, 6KHz e 8KHz. A imitanciometria mostra curva tipo B à direita e curva tipo C com pico rebaixado à esquerda, sem reflexos ipsi/contralaterais. A Apneia Obstrutiva do Sono (AOS) Severa se relaciona à alteração auditiva neurossensorial à esquerda; pois a hipóxia reduz o nível de oxigênio e vulnerabiliza as células ciliadas da cóclea com prejuízo da capacidade sensorial. A alteração auditiva à direita é condutiva e não se associa à apneia. A AOS causa resistência das vias aéreas superiores, disfunção na tuba auditiva e altera a pressão na orelha média, corroborando com a curva tipo C na orelha esquerda.

Abstract:
Introdução: A Cistite Idiopática Felina (CIF) é a alteração mais prevalente entre as Doenças do Trato Urinário Inferior Felino (DTUIF) correspondendo de 55 a 65% de todos os casos. Os animais mais afetados são machos, com idade entre dois e oito anos, obesos, alimentados com ração seca e baixa ingestão hídrica, mantidos em ambiente intradomiciliar e submetidos a fatores de estresse, como disputas territoriais com gatos contactantes. Objetivo: Contribuir com a disseminação de informações sobre a Cistite Idiopática Felina no meio acadêmico. Metodologia: Revisão de literatura realizada em 2022 com base nas informações obtidas no PubMed; Revista Veterinária em Foco; Revista Ciência Animal; Revista Eletrônica Veterinária e Livros. Resultados: Felinos com CIF possuem concentração diminuída de glicosaminoglicanos (GAGs) responsáveis pela formação de uma camada protetora que reveste o urotélio vesical, permitindo a penetração de substâncias irritantes e ativação das fibras de dor. Além disso, o estresse crônico produz aumento da estimulação simpática e respostas adrenocorticais suprimidas, causando estimulação sensorial aumentada e permeabilidade urotelial alterada. Os sinais clínicos frequentemente encontrados são periúria, hematúria, anúria, estrangúria, disúria, anorexia, êmese, apatia, diarreia, lambedura excessiva na região perineal. Pelo fato da manifestação clínica da CIF ser semelhante a outras patologias da DTUIF, como a urolitíase e infecção bacteriana, o diagnóstico é feito por exclusão, correlacionando com os exames complementares, como radiografia, ecografia, cistoscopia. O tratamento é escolhido de acordo com o quadro clínico do paciente, com fluidoterapia para estabilização, cateterismo uretral para desobstrução e lavagem vesical, analgésicos, anti-inflamatórios não esteroidais e correção do manejo. Como prevenção e controle, é utilizado o método MEMO (Modificação Ambiental Multimodal), aumentando a ingestão hídrica, melhorando o bem-estar e qualidade de vida do animal, avaliando e corrigindo as condições ambientais que propiciaram a CIF. Conclusão: É possível concluir que a CIF é uma doença prevalente na rotina clínica, tendo como principal causa o erro de manejo e estresse. Deve-se ficar atento aos sinais clínicos apresentados pelos animais de modo a diferenciar de outras doenças da DTUIF. O tratamento e prevenção são realizados de acordo com o estado clínico do animal, através da estabilização, desobstrução e correção de manejo.
