Abstract:

The presence of allergy may reduce cancer risk. Literature searches identified 142 epidemiological studies on this association. Data from the American Cancer Society Cancer Prevention Study-II cohort were used to explore the relationship between self-reported asthma and/or hay fever and cancer mortality in 508 318 men and 483 079 women who were cancer-free at baseline. During 18 years of follow-up from 1982--2000, there were 44 524 cancer deaths in men and 36 567 in women. Cox proportional hazards models were used to obtain adjusted relative risks for overall cancer mortality and for cancer mortality at 12 sites. There was approximately a 10% reduction in overall cancer mortality among people with asthma and hay fever. Asthma and/or hay fever were also associated with a reduced risk of cancer at a number of other sites, and some of the associations were modified by gender and smoking status.

Abstract:

Air pollution is an important determinant of population health. The World Health Organization estimates that air pollution is responsible for nearly 2% of all deaths globally. The current research is aimed at determining the specific components of air pollution that are most likely linked to increased risk of mortality. Since one air pollutant is unlikely to be emitted by itself, various mixtures of air pollutants must be investigated. It is possible that the health effects of one pollutant in the mixture might be larger than the health effects of another. The current research focuses on the relationship between multiple air pollutants and mortality in the general population. By examining combinations of pollutants, it is possible to isolate, to a certain extent, the effects of individual pollutants. The population health outcomes investigated include mortality from cardiopulmonary conditions, lung cancer, and all causes combined. Air pollution data from various metropolitan regions in the U.S. were linked to the health outcomes of individuals living in these areas. Characteristics of individuals that may affect the relationship between air pollution and mortality, such as age, sex, smoking history, alcohol use, were obtained from the American Cancer Society Cancer Prevention Study II cohort. Nearly 1.2 million adults were enrolled in this study in 1982, and have been followed up on an on-going basis. This study suggests that sulfate and, more broadly, fine particulate matter may be the most important contributors to excess risk of all-cause, cardiopulmonary, and lung cancer mortality.

Abstract:

Emotional appeals are frequently employed in strategic messaging by nonprofit organizations. In this study, I identify instances of emotional appeals in select print adverts of the Susan G. Komen Breast Cancer Foundation and the American Cancer Society, and use rhetorical analysis to explore how each organization’s tactics are strategic in their appeal to target audiences’ emotions. In doing so, I identify several reoccurring emotional themes—including hope, love, and unity—that engage their target audiences and persuade them to respond to diverse calls-to-action. In order to make these appeals to audience emotion, the adverts employ rhetorical devices such as personification, metaphor, repetition, and imagery. Rhetorical analysis of the two organizations’ strategic messaging suggests that their calls-to-action, at least in print adverts, rely heavily on appeals to audience emotion. Taken together, the data suggest that previous efforts made by these nonprofits have been proven successful.

Breast cancer is regarded as the most common form of cancer in women and it comprises of approximately 23 % of female cancers, while affecting women at any age range. For oestrogen receptor positive patients, tamoxifen is used as a prescribed medication for breast cancer therapy. However, tamoxifen in its natural form is not active to achieve the required treatment and prevention of breast cells proliferation. Since tamoxifen is a prodrug, it need to be converted into its active form, endoxifen, for which it is achieved by the action of the cytochrome P450 enzymes. Cytochrome P450 2D6 (CYP2D6) is a member of cytochrome P450 enzymes for which are superfamily of heme enzymes characterised by their ability to catalyse the oxidative reactions of compounds, including the pathway of tamoxifen metabolism. However, due to polymorphism that lead to inactive phenotypes of CYP2D6 in this gene, there is a challenge of diagnosing if a patient can metabolise tamoxifen or not. The current diagnostic tool, Amplichip CYP450, for CYP2D6 is based on genotypes, and it lead to uncertainness as to whether the presence of functionalCYP2D6 alleles of CYP2D6 may lead to coding of active protein, thus leading to wrong treatment measures and overdose of tamoxifen. Electrochemical techniques have provided reliable, simple, quick, and sensitive methods for the determination of drug metabolism by enzymes. Therefore, it is important to develop a CYP2D6 phenotype-based sensor to detect and tell whether a particular individual can metabolise the drug or not.

Abstract:

Cancer patients report having a high need for cancer information. Several studies show that the majority of patients surveyed report preferring information from the American Cancer Society (ACS). Ranging up to 129 pages, the ACS’ Detailed Guides (DG) are widely distributed throughout the United States, and offer patients an authoritative guide to help patients navigate the difficult terrain of the cancer journey. This dissertation examines the ACS’ cervical, endometrial, ovarian, penile, prostate, testicular, and vaginal cancer guides. Through a rhetorical analysis of the 7 guides, it was shown that the ACS DGs in question foster gendered narratives that strictly limit the type, range, and quality of information offered to cancer patients. These limitations and their resulting exclusions pose serious risks of misinformation and isolation among vulnerable patient populations with high information needs. Three recommendations are offered to help mitigate the issues identified within the ACS DGs, to improve the quality of information offered in the DGs, and to subsequently help improve patient quality of life.

This is a collection of short stories that traces a chronological movement through one family's experience of the mother's illness. Each piece in the collection is meant to be an independent, free-standing short story. Each story is different, told from distinctive points-of-view, angles, and voices. However, every story covers a span of time within the nine months of the family's experience, in the presented order, as part of a collective movement toward the core. All together, the pieces hope to reflect a mosaic of sorts--one that tells a story that cannot otherwise be told.

Abstract:

This thesis describes patients in acute postoperative pain as well as patients with acute cancer-related pain in palliative care, and their experiences and perceptions of pain management in relation to HRQOL and the multidimensionality of pain. A combination of qualitative and quantitative methods was chosen. Data were collected using interviews and questionnaires; APS, SF:36 and a new developed questionnaire PC-PPQ measuring care related to pain management in palliative care. For assessing pain VAS and Pain-o-Meter were used. The study group consisted of 100 patients on their second postoperative day, and of 75 patients with cancer-related pain from two palliative care teams. The result showed that at the time of the interview 29 of the patients with postoperative pain reported a pain > 3 on VAS and 79 reported VAS > 3 as worst pain past 24 hours. The higher the intensity of pain the less satisfied the postoperative patients were with the nurses´ way of treating their pain. Thirty-three patients stated that they had received information regarding the importance of pain relief. Patients with postoperative pain as well as patients with cancer-related pain had been prescribed analgesics mostly a combination of Paracetamol, NSAID and opioid. Of the 75 patients with cancer-related pain and in palliative care 22 patients reported pain >3 on POM-VAS and 47 patients reported >3 on POM-VAS as worst pain past 24 hours. Twenty-eight patients reported an average pain > 3 on POM-VAS past 24 hours. Twenty-four patients used the words troublesome or tiring when describing their affective pain. Sensory pain was described as prickling or sore by 15 patients. The patients perceived their pain as “aching all over” and expressed a wish for pain relief as well as a fear for increased pain. HRQOL especially physical functioning decreased for patients with average pain > 3. Being cared for by a nurseled or a physician-led palliative care team indicated no statistically significant differences for patients´ HRQOL or pain intensities. The patients had experienced a statistically significant better care after being referred to a palliative care team, despite that pain control had not been optimized. Patients expressed a need for communication, planning and trust in order to improve pain management. Continuity of care and the opportunity to talk increase the patients feeling of security, as well as improved their perceived pain control. Structured ongoing discussion concerning pain management from an early stage of the disease or already preoperatively can provide an important intervention to meet the results of this thesis. Pain assessment covering the multidimensionality of pain, and pain treatment plans including both pharmacological and non-pharmacological treatment are further important interventions.

Abstract:

Introduction. Sexually transmitted infections (STIs) continue to be highly prevalent among young women, and STIs continue to be a challenging health issue on college campuses. Studies have shown that the highest prevalence of human papillomavirus (HPV) is among young adult women, ages 20 to 24, including female college students. While the HPV vaccine has proven to be highly effective in preventing certain high-risk types of HPV, it is not effective in preventing all types of HPV or other STIs. Practicing other safer sex behaviors, in addition to condom use, also can help individuals protect themselves and their partners from acquiring HPV and other STIs. Purpose. Relationships between knowledge about HPV and the HPV vaccine, beliefs about HPV, acceptance of the HPV vaccine, and intentions to practice safer sex behaviors were assessed to determine if female college students who had a high acceptance of the HPV vaccine were also more likely to practice safer sex behaviors. Methods. A convenience sample of 2,706 undergraduate female college students, ages 18 to 24, from three U.S. public universities, completed an online survey that measured the following study variables: knowledge about HPV; knowledge about the HPV vaccine; acceptance of the HPV vaccine, specifically influential factors and barriers to vaccination; as well as attitudes, normative beliefs, control beliefs and intentions for practicing safer sex behaviors. Results. The majority of participants had a high level of knowledge of HPV (70.4%) and the HPV vaccine (73.7%). Over one-third of the participants (37.3%) received at least one dose of the HPV vaccine. Most participants thought they had a low susceptibility to HPV, as 54% thought they were unlikely to contract HPV; however, most (53.6%) thought that HPV would be a serious problem for them. The safer sex behavior that participants thought was the easiest was refusing to have sex with a partner that would not use a condom (51.8% “Strongly agree”) and the safer sex behavior that participants thought to be the most difficult was asking a partner to get tested for STIs (54.5% “Disagree”). Attitudes, normative beliefs, and control beliefs combined to strongly predict intentions (R = .730, p < .001), and attitudes was the strongest predictor for intentions to practice safer sex behaviors (β = .666), 95% CI [.649, .711]. There was a significant positive correlation between vaccine acceptance and intentions to practice safer sex behaviors (r = .087, p < .001), including likelihood of getting vaccinated against HPV (r = .098, p < .001). Conclusions. Findings from this study demonstrate the need for university student health centers to provide information about the HPV vaccine as part of broader STI prevention and social marketing campaigns, targeting female college students who are single, as well as those in monogamous relationships. Furthermore, because young men can be carriers of HPV and the HPV vaccine is now available to them, follow-up studies are needed to determine acceptance of the HPV vaccine among male college students as it relates to HPV knowledge, knowledge of the vaccine, and intentions to practice safer sex behaviors.

Abstract:

This thesis investigates volunteering as performance. In exploring this topic I discuss a dynamic between self-interest and selflessness in the observable performance of service through the social mechanisms of volunteerism. I argue that self-interest is a prominent motivation for volunteering, but its overt performance is kept in check by norms that emphasize selflessness. My argument centers on addressing this lack of acknowledgement toward self-interest within vernacular culture. My research draws examples from an individual, organizational, and global volunteer perspective. Ethnographic research was conducted for this study with a student group that organizes one of the American Cancer Society’s Relay For Life fundraisers. Within this organization, I conceptualize volunteering as a performance that requires a social actor to not just “do” service, but also “show do” and/or “explain show do” their behavior in front of an audience. This presentation culminates in a cultural performance where participants at Relay For Life perform a narrative of selflessness. Expanding my discussion of volunteering to a global perspective, my last chapter addresses volunteer tourism. I argue that the self-interest of both volunteers and volunteer travel companies reduces the recipients of volunteer tourism to essentialized and exociticized cultural "Others." I advocate for the overt acknowledgement of self-interest not only because self-interest is present, but also because it is a central dynamic that constructs volunteerism as performance.
