Abstract:
Aim: To investigate the frequency of different etiologies of acute scrotal pain in males presenting to the emergency department at American University of Beirut Medical Center in Beirut, Lebanon. Materials and Methods: In all, 94 patients between the ages 18 and 40 years presenting to the emergency room at American University of Beirut Medical Center with acute scrotum were recruited. The scrotum was inspected with palpation, and Doppler Ultrasound of the testicle was performed by a radiology resident to confirm the diagnosis. The study results were then collected after the official reports were signed by a board-certified radiology attending with expertise in ultrasound. Results: Of the 94 patients presenting with acute scrotal pain, 23.4% (95% confidence interval (0.70–0.87)) had no ultrasound findings and were considered normal. On the other hand, 54.3% (95% confidence interval (0.45–0.66)) were diagnosed with varicocele, 9.6% (95% confidence interval (0.04–0.16)) were diagnosed with epididymo-orchitis, and 9.6% (95% confidence interval (0.04–0.16)) had a history of trauma to the testicle. Patients presenting with acute scrotum had a pain duration of 13.2 ± 4.5 h, with 10.6% (95% confidence interval (0.04–0.17)) having associated urinary symptoms, 18.1% (95% confidence interval (0.09–0.25)) have had sexual activity within 48 h of presentation, and 10.6% (95% confidence interval (0.04–0.17)) were pre-diagnosed with varicocele. Conclusion: Varicocele is the most common cause of acute scrotal pain in patients presenting to American University of Beirut Medical Center in Beirut. This should direct varicocele to the top of our differential when a patient with acute scrotum presents to the emergency department. Nevertheless, ruling out torsion remains the first step in the workup.

Abstract:
Iraq’s healthcare infrastructure has deteriorated after decades of war and sanctions. A small fraction of Iraqis injured as a result of war have accessed quality care at the American University of Beirut Medical Center (AUMC), some funded by the Iraqi government. This chapter, based on the author’s work as a surgeon treating Iraqi war wounded at the AUMC, describes how powerful elites determine the political value of a war wound, influencing what a person’s injury means to the country as a whole and hence, the kind of treatment the Iraqi government is willing to sponsor. As such, the chapter sheds light on a form of social stratification shaping access to care for war-injured populations in Iraq and around the world, as political elites seek legitimation for their projects.

Abstract:
This chapter explores the development of the department of physics at Yenching University, an American-funded missionary institution in Beijing, China during the Republican period. It shows how the department evolved from a primitive premedical teaching program to a major center of physics education and research. It also reveals the significant role of the Rockefeller Foundation in this development, partly as the sponsor of the Premedical School of Peking Union Medical College. Founded in 1917, the Premedical School shared with Yenching’s science departments its advanced facilities and in 1926 became part of the university. In 1927, the department created a Master of Science program in physics, the first of its kind in China, promoting original research among its faculty and students. Before the Japanese army shut down the university in December 1941, more than ninety Chinese young men and women had completed their study in this department with a research thesis. A considerable number of Yenching graduates went on to earn their doctorates in America or Europe and subsequently returned home, becoming leading physicists in China in the twentieth century. Among them, Kun Huang (黃昆‎, Class 1941) and Chia-Lin Hsieh (謝家麟‎, Class 1943) even won the State Preeminent Science and Technology Awards, the highest scientific honor in China, in 2001 and 2011 respectively.
