Full textVarje år insjuknar många människor världen över i sjukdomen diabetes mellitus och antalet insjuknande per år väntas öka. Sjukdomen är i de flesta fall kronisk och för med sig flertalet komplikationer. En av dessa komplikationer är amputation av nedre extremiteter på grund av sämre blodförsörjning. Syftet med denna studie var att beskriva hur diabetespatienter upplever sin livskvalitet efter en amputation. Studien är en litteraturstudie som genomfördes enligt Polit, Beck och Hungler (2005). Resultatet redovisas i tre huvudkategorier; fysisk livskvalitet, psykisk livskvalitet och social livskvalitet. Studien visar att livskvaliteten för de diabetespatienter som amputerats är lägre än för de diabetespatienter som känner sig friska men bättre än för de diabetespatienter med pågående fotsår. Livskvaliteten efter amputation påverkas av en rad olika faktorer till exempel; rörlighet, smärta, psykisk status, protesanvändande och patientens sociala nätverk.

Full textMotor imagery, which refers to the internal representation of a movement, has the potential to impact activity-dependent plasticity. The relevance of motor imagery, as a technique allowing substantial motor performance gains and motor recovery in the field of rehabilitation, has been validated by numerous experimental studies in stroke patients and persons suffering from spinal cord injury. By highlighting the possibility of integrating motor imagery within the framework of lower-limb amputees’ rehabilitation programs, and revealing promising preliminary data regarding locomotion recovery, the results of this thesis broaden the range of people which might benefit from this technique. Data further suggest that motor imagery should specifically focus on functional and bilateral tasks, and that concomitant use of transcranial direct current stimulation should contribute to potentiate the impact of motor imagery on locomotion recovery of lower-limb amputees

Full textProsthetic devices allow people with lower limb amputation to recover their autonomy. Currently, this autonomy is limited in several everyday life situations such as walking on a slope or in stairs. Embedded sensors, frequently integrated inside the prostheses, record kinematic and dynamic data during locomotion. In this context, this PhD thesis aims to contribute to the development of methods for the processing of data provided by these technologies to quantify and improve the adaptation of the prosthesis to the environment. These methods were applied and evaluated in the context of the use of a new prosthetic knee-ankle system controlled by microprocessor (SPCM) developed by Proteor to facilitate the locomotion in situations such as slope or downstairs walking for people with above knee amputation. The first part of the work was devoted to the quantification of the adaptations allowed by the SPCM in different situations of daily life (flat, slope, staircase). The second part of the thesis focused on the estimation of gait parameters (walking speed and slope inclination) and the classification of 4 everyday life situations (flat, slope ascent and descent, stair descent) using machine learning algorithms that can be computed by the SPCM in real time. The body of this work will make it possible to better quantify the user real life activity. This quantification could help the user or the medical team in the activity monitoring but also the manufacturer to improve the adaptation of the prosthetic components to the environment and to verify the SPCM actual use

Full textI Sverige amputeras årligen cirka 2500 personer, merparten av dessa är benamputationer. Traumatiska amputationer är en av de mest kostsamma skadorna idag.
Syftet med studien var att beskriva upplevelsen av att bli
traumatiskt amputerad i en olycka eller efter en operation. Studien genomfördes som en litteraturstudie
där 11 artiklar analyserades. Resultatet kategoriserades
utifrån Cullbergs faser; chockfasen, reaktionsfasen,
bearbetningsfasen och nyorienteringsfasen. En traumatisk amputation är en stor förändring, inte bara för
den drabbade patienten utan även för närstående och
vänner. Reaktioner efter amputationen var ilska, rädsla,
osäkerhet och saknad av extremiteten. Patienterna
upplevde att deras kroppsbild förändrades och att de var
oförberedda på smärtan som kom efter amputationen.
För de flesta patienterna var positivt tänkande, hopp och
stöd från omgivningen viktigt för tillfrisknandet. Med
protesens hjälp blev patienterna mer självständiga och
självbilden förändrades till det positiva. Mer kvalitativ
forskning om patientens upplevelse i samband med
traumatisk amputation efterfrågas.

Full textThe aim of the literature review was to describe how people who have undergone amputation perceive their quality of life. The descriptive literature study was based on 14 scientific articles with both quantitative and qualitative approach. The literature searches were carried out in the databases Medline (via PubMed) and CINAHL. The main results showed that amputation affected individuals from a physical, mental and social perspective. Common factors that affected the quality of life negatively were pain and difficulty with the mobilization. Anxiety, depression and decreased self-esteem were common mental health problems. Difficulties in adjusting to the new identity and social isolation were common problems. The use of prosthesis gave the feeling of being normal and the physical ability to handle activities in daily life. The social adjustment and relationships with family and personal surroundings was negatively changed after the amputation. The individuals learned to adapt in social situations which eased the negative emotions and uncertainty. Social relationships were experienced as important for support and encouragement in the rehabilitation and minimize the negative emotions. The conclusion highlighted the importance of the nurse's knowledge about common complications that can affect quality of life, in order to deal with problems before they occur.

Full textThis thesis focuses on two main themes. The first one is related to sickle cell disease and the second theme is related to limb amputations. For studying the first theme, this research proposes to examine the knowledge of 178 men and women living in Benin, on the disease, its causes, effects and care. They answered a questionnaire and their responses had been compared to those given by an expert group. It had been found that there was an overall lack of awareness on the disease, and especially its most severe symptoms and treatment. The results reveal that the higher level of education they have, the less misconceptions about the disease they have. In addition, this ignorance was increased by people declaring themselves carrying the genetic trait (4.83) or sickle cell (5.19) than those who reported not being carriers (3.72) or healthy (3.62). The second study about the first theme, investigated whether the 137 respondents living in Benin had integrated adequately the rules of inheritance of the disease, based on the functional theory of cognition. Three distinct clusters emerge with data analysis. In the first group (n=46), respondents considered a higher risk of transmission if the one or the other of two parents was/were affected by the disease. In the second group (n=51), the risk was considered even greater as both parents were seriously ill. In the third group (n=40), the risk was considered as high only if both parents were touched. The second theme, Accidents involving physical injury, has motivated a study which aim was to examine the reasons that may underlie a refusal of amputation for 224 people living in Benin, by the approach of the reversal theory. The results show that the fear of losing personal identity, the fear of hospitals and medical staff, the belief that the surgery was unnecessary, a lack of confidence in the competences and dedication of doctors, the fear of losing others esteem and the loss of spiritual strength, are among the main factors motivating a refusal of amputation

Syftet med denna litteraturstudie var att beskriva livskvalitet hos personer som genomgått en amputation av en extremitet utifrån ett fysiskt, psykiskt och socialt perspektiv. Sökning av artiklar har utförts i databaserna Medline, Cinahl och genom manuell sökning i några av artiklarnas referenslistor. Sjutton artiklar valdes för analysen, som genomfördes på ett systematiskt arbetssätt där fynden i artiklarnas resultat grupperades i tre huvudkategorier och nio underkategorier. Artiklarnas kvalitet granskades med hjälp av checklistor.
Resultatet visade att patienter som amputerats fick hela sin existens förändrad. Faktorer som gav lägre livskvalitet hos patienterna var smärta, sämre fysisk funktion, minskad rörlighet och psykisk ohälsa. En faktor som gav bättre livskvalitet hos patienterna var användning av protes. Bättre livskvalitet hos patienterna uppmättes i områdena arbetsmiljö och tillfredsställelse inom familjelivet, jämfört med icke amputerade personer. Författarna ansåg att efter en amputation så omvärderade patienten olika delar i livet och uppskattade andra saker mer än människor som var friska, som kanske tog dessa för givet.

Abstract
The aim of this literature study was to describe the quality of life of persons who have undergone an amputation of a limb from a physical, mental and social perspective. Search through Medline, Cinahl databases and additional manual search in some of the articles reference lists was conducted. Seventeen articles were chosen for the analysis, which was carried out through a systematic approach and the findings in the articles results were grouped into three categories and nine subcategories. The quality of the articles was reviewed using checklists.
The results showed that patients who had been amputated had their whole existence changed. Factors that resulted in lower quality of life of patients were pain, worse physical function, decreased mobility and mental illness. One factor that resulted in better quality of life for patients was the use of prosthesis. Better quality of life in patients was measured in the areas of work environment and satisfaction in family life compared to the healthy subjects. The authors believed that after an amputation the patient revalued different aspects of life and appreciated other things more than people who are healthy, who might have taken things for granted.

Full textThe aim of the thesis was to analyze the reorganization of the propulsive forces process of a gait initiation when the postural constraints were modified by a unilateral lower limb amputation. The analyze use the reaction forces of the ground and electromyography. The results were collected from three groups of subjects were analyzed: 7 healthy subjects; 6 below-knee amputee subjects and 7 above-knee amputee subjects. Four main original results can be stressed : l- Role of the anticipatory postural adjustments. In the healthy subjects, the anticipatory postural adjustments were predictive of the progression velocity. In opposite, in the amputee subjects the anticipatory postural adjustments were not predictive of the kinematic characteristics of the progression velocity. Their role would be limited to the initiation of the desequilibrium couple which was a necessary condition to initiate the first step. 2- Strategy of generation of the propulsive forces. Strategies depend of the state of the limb which initiate the step. When the gait was initiated with prosthetic limb, propulsive forces were adjusted during the phase of step execution. When the gait was initiated with intact limb, the propulsive forces were adjusted both during the anticipatory postural adjustments and the phase of step execution. 3- Elements determining the propulsive forces strategies : 2 elements determine the choice of the strategies. The capacity to the swing limb to generate propulsive forces and the capacity to the stance limb to control postural equilibrium. 4- Role of ankle plantar flexors during the phase of step execution. Maximal progression velocity was the same whatever the state of the lower limb initiating the limb. This results shows that ankle plantar flexors were not necessary to generate propulsive forces

Full textAmputation affects men and women of all ages, classes, races and religions. The patients’ reactions are, however, entirely unique. This is a literature study comprising fourteen scientific articles which make up the results. The psychosocial effects of amputation can be daunting and considering the mountain of literature on the subject, it is therefore necessary to start by explaining some of the research on stress, crisis, and coping. Brief descriptions of Lazarus’s and Cullberg’s work have been selected to represent a sample of existing theories which have grappled with these psychosocial concepts. The purpose of this study is to describe the various coping strategies utilized by patients who have undergone amputation. The results consist of seventeen coping strategies which are divided up into five theme groups (positive coping, avoidance coping, social support, maladaptive coping, and religion). Evidence is provided which describes how the various coping strategies can actually manifest themselves in reality as witnessed by amputee patients. In conclusion, amputees use various coping strategies at different stages of recovery to deal with the trauma of their loss; these strategies can be observed and identified in both clinical and out-patient settings.

The objective of this thesis was to determine the motions needed in a prosthesis that would enable a transhumeral amputee professional golfer, Michael Gibson, to play golf with similar dynamics to those of a two-armed golfer. Although he plays golf well using only one arm, his swings tend to have less power and are less consistent than his two-armed colleagues. Significant user testing was carried out using various prototypes with Gibson. Analysis was performed with Gibson's feedback, video comparisons of swings, and data from both motion capture and flight analysis software. Not only were differences in the dynamics of Gibson's swing and a two-armed golfer's swing studied, but the root causes of the differences were understood. It was determined that a prosthesis that enables wrist cock, forearm rotation, and slight elbow compliance would increase Gibson's golf performance.

Full textBackground: Diabetes mellitus is a public disease that is seen on a global scale. The illness can cause several complications and amputation of the lower extremity is one of them. To lose a limb as a result of the disease cause both physical and psychological distress, which can cause positive or negative experiences for the patient. Aim: To describe the patient's experience about amputation of the lower extremity as a result of the diagnose diabetes mellitus. Method: A literature review based on 11 qualitative scientific articles. Results: The results show that the patient’s experience is individual and include both positive and negative aspects. The experiences involved mobility, emotions, support, information and education. The support had a determinant effect on the patients whole amputationprocess. This was also crucial for patients ability to adapt and accept the changes in their life. Conclusions: The study showed that the patients felt a lack of support from the people around them, which had negative effects on their experience. When the nurse has a deeper understanding for the patient it might facilitate to be a supportive function for the patient.

Full textBackground: Going through an amputation is a life-changing event followed by challenges physically and mentally for the affected person not least after discharge from hospital. Aim: The aim of this literature review was to compile previous qualitative research of patients’ experience of rehabilitation after amputation of lower extremity after discharge from hospital. Method: Scoping review. Result: Based on twelve scientific articles five main themes and fifteen subthemes were identified. This study illustrated how lack of information exists from healthcare professionals to the patient on different levels and trust towards the staff is important. Lack of financial resources reduced the adherence to rehabilitation and had a direct correlation to patient survival. The cost of medical care was described as a matter of life and death where free health care was crucial for socio-economically vulnerable groups. Conclusion: The conclusion illuminated the importance of the partner and family for how the patient deal with the rehabilitation process. It is important that the nurse is responsive when care transition occurs from hospital to home environment as the burden of care for the family increases and the patient is the most vulnerable.

Full textIn order to promote the patient’s physical health the nurse also have to work with the mental state of the patient. A nurse often meet individuals who find themselves in many different stages of life. When it comes to amputation of a limb both function and body image is lost and the individual has to find a way back to a new life situation. Coping strategies are used by staff, patients and relatives and are important tools in order to manage a demanding situation. The aim of this literature review was to investigate which coping strategies patients who undergone an amputation are using and the meaning coping has for rehabilitation. Increasing knowledge concerning coping strategies will help nurses to aid patients in their mental rehabilitation and gain a better understanding about the reactions she meets. The method was a literature review and contains a summary of ten scientific articles with both quantitative and qualitative methods within the chosen subject. The results showed that coping strategies are important for psychosocial adjustment. A problem solving strategy showed positive effects in the individual’s life leading to for example increased ability to adjustment. Avoidance was common when dealing with an amputation and contributed to low psychosocial wellbeing as well as difficulties adjusting. Seeking social support improved the individuals experience of pain and symptoms of depression. This study will hopefully increase the nurses knowledge about coping strategies and contribute to an increase of understanding about the mental process patients that are amputated as well as their relatives experience.

Full textBackground: An amputation implies a removal of a body part. Most amputations transact at the lower extremities. In Sweden approximately 2250 lower extremity amputations are made a year. The causes of amputation may be vascular diseases, diabetes or trauma. The profession of a nurse has a major role in the relation to the patient. Aim: The aim was to investigate patients’ experiences after an amputation of lower extremity. Method: To approach the aim a literature review was performed by the method of Friberg. Ten qualitative original articles were collected by the databases PubMed and Cinahl Complete. Primary keywords were amputation, lower extremity and life change events. Limitations were English and peer reviewed. Results: Four themes were identified: Experience of psychological and physical changes, To experience changes of identity and loss of independents, Changes of self-perceptions and the feeling of being vulnerable and How to recover after an amputation. The result showed that the patients’ experienced a loss of independence which resulted in a feeling of being a burden. The physical ability was impaired and contributed to a feeling of being isolated and lonely. Conclusion: The patients’ experienced emotional shock and a fear for the future. Not being able to perform to the fullest produced a loss of identity and independence. Support from family, friends and healthcare professionals has a central value for the recovery. To be amputated results in a transitioning and a new phase in life.

Full textBakgrund: Krigsrelaterade amputationer respresenteras av en unik grupp av patienter på grund av sårens komplexitet samt omfattning av flera svåra skador. För att förebygga och förhindra vårdlidande och lidande för patienten, så har denna studie riktat in sig på sjuksköterskans omvårdnad till denna unika patientgrupp. Dessutom innebär det att genomgå en traumatisk amputation en stor omställning för den drabbade, vilket gör att sjuksköterskan har en viktig roll i att bemöta patienten och dess behov.Syfte: Att belysa sjuksköterskans omvårdnad av patienter med krigsrelaterade amputationer.Metod: Vald metod var litteraturöversikt där nio kvalitativa och tre kvantitativa artiklar analyserades.Resultat: Vid analysen identifierades fem teman. Dessa var: ”Initialt omhändertagande”, ”Sjuksköterskans hantering av smärta”, ”Identifiering av sekundära komplikationer”, ”Bemöta patientens upplevelser” samt ”Rehabilitering”.Konklusioner: Sjuksköterskan har en betydande roll vid omhändertagandet av patienter med krigsrelaterade amputationer, från det akuta skedet till uppföljningen efter patientens hemgång. Sjuksköterskan stöttar patienten hos de olika vårdinrättningarna; sjukhus, vårdcentral, psykiatri och hemsjukvård. Genom att sjuksköterskan omhändertar, bemöter och stöttar patienten genom vårdförloppet visar det på ett bra genomfört omvårdnadsarbete av sjuksköterska hos patienten. Fortsatt forskning om krigsrelaterade patienter behövs från ett sjuksköterskeperspektiv.

Full textAn amputation implies both a physical and a psychological impact which is related to a long rehabilitation and adjustment to the new life situation. To optimize the care of the amputee it is important as a nurse to increase the understanding of what the procedure entails. Therefore the aim of this study was to describe the factors that affect the lived experience of health related to a leg amputation. In this literature review 15 articles were used and two major subjects were generated which were of importance regarding the lived experience of health; the importance of support and the importance of the body image. The support implied the external resources in the terms of professional and social support. The body image implied the internal resources in the terms of regaining the body function and the independence. For the amputee it was therefore of importance to utilize health-promoting factors as resources to adjust to the new condition. More research regarding the personal experiences of an amputation is required to increase the nurses’ understanding of the procedure. Research regarding the preoperative information is also required due to the positive effect that it has been proven to inflict on the postoperative processing.

Full textDiabetes progresses towards a public health issue worldwide and the prevalence increases. The illness causes several complications, which lower limb amputation is one of the more lifechanging complications. Patients’ experience of lower limb amputation is of interest to identify, which could increase nurses understanding and knowledge about the patients’ situation and the nurse’s ability to perform a safe and loving care. The aim of this study was to describe patients experience of undergoing a lower limb amputation caused by diabetes. The study was conducted as a general literature study with an inductive approach. Nine articles were found that matched this study’s aim and were reviewed and compiled. Main findings of the study results in three categories; lack of information, need for support and emotional roller coaster. The patients describe a lack of information about life and daily living after the amputation and the process towards being independent. Family and friends describes as very supportive by some patients and others find a equal support in professionals. The emotional roller coaster describes as feelings that are pendulating between injustice, relief, panic and gratefulness after the amputation. Recommendations towards nurses are that they should use standardized and individual care plans as an aid in their daily work around the patient to ensure good and person-centered nursing care.
