Full text
Abstract:
This volume aims to uncover the diverse approach to plants in the Renaissance and seventeenth century that paved the way for a definition of botany as a fully-fledged discipline. Its scope expands beyond the natural historical interest in collections and the fabrication of materia medica: moving from Varchi, Matthioli and Bauhin to Locke, Pinelli, and Linnaeus, among others, the contributions collected here connect practical and theoretical features, dealing with the challenges that characterized any involvement with plants. The authors focus on the linguistic shortcomings, problems in authenticating specimens, and efforts to establish new botanical geographies to favour trades, as well as the pursuits of new methodologies, artisanal technologies, and chymical experiments with plants. Botany thus emerges as a suitable discipline to disclose the complexities and challenges of early modern science in general.

Abstract:
This chapter aims to clarify the terminology of and relations between ontological, epistemological, and semantic aspects of indeterminacy in medical classification systems. Although classifications of diseases and mental disorders are often characterized as having blurred boundaries, there is no consensus on what exactly this means. The following clarification may remedy this shortcoming: from an ontological point of view, disease entities are found to be discrete or continuous, depending on whether realisation gaps occur. From an epistemological perspective, the certainty of a classification depends on how controversial the assessment of its validity is throughout contexts and how much different legitimate interests of classification users vary. Finally, as semantic categories, medical classifications can be defined precisely or vaguely. The chapter analyses how the ontological, epistemological, and semantic levels are interrelated and how the proposed terminological clarifications may help to disentangle discussions about the validity of medical classifications.

Full text
Abstract:
Anorexia-cachexia is a heterogeneous and multifactorial syndrome most likely driven by systemic inflammation and neuroendocrine activation. Key diagnostic features include reduced appetite, weight loss, and muscle wasting. Key clinical problems include management of anorexia without resort to artificial nutritional support, and muscle wasting that cannot be completely arrested/reversed even with such intervention. Assessment should cover domains such as body stores of energy and protein, food intake, performance status, and factors resulting in excess catabolism. Intervention should be early rather than late, informed by the assessment process and focused on a multimodal approach (nutrition, exercise, and pharmacological agents). This chapter aims to discuss these issues and provide (a) the reader with some background principles to classification, (b) a simple approach to patient assessment and a robust algorithm for basic multimodal treatment, and (c) an overview of the evidence base for different pharmacological interventions.

Full text
Abstract:
The 2001 World Health Organization (WHO) classification of hematopoietic and lymphoid neoplasms categorized “the leukemias” into two major groupings—myeloid and lymphoid. Myeloid neoplasms, which are the primary focus of this chapter, include acute myeloid leukemia (AML), myelodysplastic syndromes (MDS), and myeloproliferative neoplasms (MPN). Lymphoid neoplasms are mostly reviewed as part of non-Hodgkin lymphoma in Chapter 40 of this volume, although descriptive patterns and selected etiologic studies are briefly discussed in this chapter because of historical trends. Worldwide, leukemias are ranked eleventh among all cancer types, comprising approximately 2.5% of all malignancies. Exposure to ionizing radiation and certain chemical carcinogens (e.g., cytotoxic chemotherapy, benzene, formaldehyde) are the most consistently associated risk factors for MDS and/or AML. Radiation has been linked with CML, and cigarette smoking with AML. Fewer risk factors have been identified for MPNs. Some evidence implicates increased risks of AML in rubber workers, farmers, and other agricultural workers.

Full text
Abstract:
The intensive care unit (ICU) can be defined as an area reserved for patients with potential or established organ failure and has the facilities for the diagnosis, prevention, and treatment of multi-organ failure. Usually, the ICU is located in close proximity to A & E, the radiology department, and the operating theatres, as it is between these areas that patient flows are greatest. In large urban hospitals, there may be more than one ICU, some of which serve specific patient populations, such as paediatrics, neurosurgery, cardiothoracic surgery, liver failure, and burns. Many hospitals also have high-dependency units (HDUs) that offer higher nurse-to-patient ratios and more advanced monitoring than a general wards does, as well as limited organ support. In the UK, the distinctions between ICU, HDU, and general ward have been abandoned in favour of a classification based on the patient’s needs rather than their location.

Full text
Abstract:
Comparative law plays a key role in the harmonisation of commercial law. This chapter outlines the history of comparative law and the classification of legal systems and goes on to examine the aims of comparative law, with particular reference to the enhancement of an understanding of one's own legal system, the development of transnational commercial law and the role of comparative law in transnational practice. A key part of the chapter is devoted to the purpose and methodology of comparative law. Is the objective to find common solutions or best solutions? Should the approach be formal or functional? The chapter concludes with a discussion of the problems confronting the comparative lawyer: the pitfalls of comparison, the effectivess or otherwise of legal transplants and the extent to which all those engaged in the work of harmonisation can realistically be expected to to be fully equipped as comparative lawyers.

Full text
Abstract:
This chapter is concerned with the question to what extent free word order phenomena are regulated by information-structural (IS) constraints. Progress on this question must combine detailed empirical study with bold theoretical work that aims to test restrictive hypotheses about available syntactic operations, available IS-primitives, and their mapping. The present chapter evaluates four cross-cutting word order generalizations on the basis of a rough classification of syntactic operations and IS-primitives. Operations will be divided into those that are A-related (A-scrambling, passive), those that are A′-related (A′-scrambling), those that involve doubling with a pronoun or clitic, and finally those that involve extraposition, and it is assumed that IS-primitives are restricted to topic, focus, contrast, and givenness. Some discussion is offered of how the four generalizations identified here might emerge as effects of deeper properties of the language faculty or human psychology.
