The SEVESO II and III Directives relate to approximately 12,000 EU establishments working with the hazardous substances. The majority of EU member states implement new requirements of the SEVESO III Directive to their legal environment. The third revision goes hand in hand with the enforcement of the CLP legislation, concerning the Classification, Labelling and Packaging of chemical substances and mixtures. Risk Management is appearing as one of the most important challenges nowadays to raise the prevention level in these establishments. The book chapter analyses the industrial accidents and identified consequences and impacts whose results can be implemented to the effective prevention. The benefit of this chapter is the summarisation of the legal regulations, information systems and especially the statistics of the industrial accidents in Slovakia and the EU. The risk assessment is one of the problem areas of prevention therefore, it was necessary to present the methods and techniques utilised here and to clarify the approach used in the Slovak Republic. The programme ALOHA is most frequently used for modelling the consequences and therefore we presented its possible utilisation on a particular example at the end of this article. The design of serious games based on sound learning and instructional principles is important to ensure learning is integrated in the ‘game-play’. However, the process of achieving this is not yet fully understood, and research is hampered by the lack of practical demonstrations of how effective instructional design is when used alongside game design. This chapter provides an example of a successful application of instructional design to the development process of a serious game for traffic accident investigators in the Dubai police force. We use the findings from an experiment conducted for 56 police officers to analyze how learning objects are affected by the instructional principles used. To conclude the chapter, we describe the implications of the use of serious games in the police force for policymakers, educators, and researchers.

In contrast to the millennium development goals (MDGs), the sustainable development goals (SDGs) entail a universal and equitable approach to global health by defining health problems as multidimensional issues. SDG3, which aims to “ensure healthy lives and promote well‐being for all at all ages,” targets a broad range of communicable diseases, mother and child health, noncommunicable diseases, mental health, substance abuse, traffic accidents, and health threats from hazardous environmental pollution, and to that end suggests supporting research and development of appropriate technology. This chapter will review how science, technology and innovation (STI) are conceptualized in the SDGs and how they are assumed to tackle health inequalities. Inspired by insights from science and technology studies, and using examples from the authors’ ethnographic studies on development and implementation of point-of-care diagnostics and development of cookstoves, it will discuss what the challenges are with the way in which STI is conceptualized in SDG policies. Based on these analyses, the authors propose a responsive and responsible approach to STI that is based on a thick description of the different stakeholder settings involved and of their perceptions and needs; takes the coproduction of knowledge, innovation, and society into consideration; and continuously reflects on the (un-)intended consequences.

The traditional approach to the study of human factors in the maritime field involves the analysis of accidents without considering human factor reliability analysis. The main approaches being used to analyze human errors are statistical approach and probability theory approach. Another suitable approach to the study of human factors in the maritime industry is the quasi-experimental field study where variations in performance (for example attention) can be observed as a function of natural variations in performance shaping factors. This chapter analyzes result of modelling for human error and human reliability emanating from the use of technology on board ship navigation in coastal water areas by using qualitative and quantitative tools. Accident reports from marine department are used as empirical material for quantitative analysis. The literature on safety is based on common themes of accidents, the influence of human error resulting from technology usage design, accident reports from MAIB, and interventions information are used for qualitative assessment. Human reliability assessment involves analysis of accidents in waterways emanating from human-technology factors. The chapter reports enhancement requirement of the methodological issues with previous research study, monitoring, and deduces recommendations for technology modification of the human factors necessary to improve maritime safety performance. The result presented can contribute to rule making and safety management leading to the development of guidelines and standards for human reliability risk management for ships navigating within inland and coastal waters.

As described in the GTI, ISIL's transnational tactics in combination with lone actor attacks inspired by the group drove an increase in terrorism to its highest levels ever across Europe and many OECD countries (upwards of a 650 percent increase in deaths since 2014). The attacks by ISIL in Paris, Brussels, and in Turkey's capital Ankara, were amongst the most devastating in the history of these countries and reflect a disturbing return of the transnational group-based terrorism. Actor network theory (ANT) was applied as a systems lens to open the “blackbox” of terrorism. The systems view facilitated by ANT highlighted how dynamic networked actors shape radicalization through the actor network process of translation. This chapter applies functional resonance accident model (FRAM) methodology. The FRAM method was used to analyze how radicalization activities (as described through ANT) take place and where and how intervention strategies can be designed to interfere with the radicalization process.

Several actions and/or operations might interfere with those required during car-driving and thus elicit dual task conditions. Those related to driving itself involve manipulating commands or instruments and should be automated during the learning to drive period to ensure safety. Others, independent of driving may be delayed (eating, smoking a cigarette). Finally, others like manipulating a navigation system or holding a cellphone have potential interference more or less related to driving. The authors now step back about 25 years to analyze the interference between driving and phoning and assess the risk associated with it. Epidemiology provides an overview of mobile phone use and hypotheses about accident causes. If hand-held phones obviously interfere with driving actions, the authors should explain why hands-free kits do not solve all safety concerns. Then, analyzing the operations affected by phoning and describing the objective measures revealing impaired driving performance will address this issue. The authors finally highlight the conditions for relatively safe phone use as well as those that should be banned. Deciding to phone or not will thus depend on driving safety education, during which skills of caution should have been learned.
