Abstract:
OBJECTIVE The authors hypothesized that when the anatomical variant of an anconeus epitrochlearis is present, the risk of developing cubital tunnel syndrome would be reduced by replacing the normal roof of the cubital tunnel (Osborne's ligament) with a more forgiving muscular structure, the anconeus epitrochlearis. The authors further hypothesized that when the presence of an anconeus epitrochlearis contributes to ulnar neuropathy, it would be secondary to muscular hypertrophy, thereby making it more likely to occur in the dominant arm. Therefore, the goal of the present study was to evaluate these hypotheses. METHODS This retrospective cohort study was performed by reviewing the records of all adult patients who underwent operative intervention for cubital tunnel syndrome between 2005 and 2014 as the experimental group and all asymptomatic patients in the medical literature who were part of a series reporting the prevalence of an anconeus epitrochlearis as the control group. The primary outcome of interest was the presence of an anconeus epitrochlearis in asymptomatic individuals versus patients with cubital tunnel syndrome. RESULTS During the study period, 168 patients underwent decompression of the ulnar nerve for cubital tunnel syndrome, and an anconeus epitrochlearis was found at surgery in 9 (5.4%) patients. The control group consisted of 634 asymptomatic patients from the medical literature, and an anconeus epitrochlearis was present in 98 (15.5%) of these patients. An anconeus epitrochlearis was present significantly less frequently in the symptomatic patients than in asymptomatic individuals (p < 0.001). Among patients undergoing surgical decompression, an anconeus epitrochlearis was associated with symptoms in the dominant arm (p = 0.037). CONCLUSIONS The authors found that an anconeus epitrochlearis was present significantly less often in patients with cubital tunnel syndrome than in asymptomatic controls. The mechanism of protection may be that this muscle decreases the rigidity of the entrance into the cubital tunnel. When an anconeus epitrochlearis does contribute to cubital tunnel syndrome, it is significantly more likely to occur in the dominant arm, possibly due to repetitive use and hypertrophy of the anconeus epitrochlearis. The presence of an anconeus epitrochlearis may be protective against the development of cubital tunnel syndrome, although this is a preliminary finding.

The anconeus muscle is a small and minor functioning muscle located at the posterolateral elbow region. It helps forearm extension and tightening of the joint capsule. Despite its limited functions, pathologies of the anconeus muscle can mimic other abnormalities of the elbow joint. Here, we report a rare case of a traumatic anconeus muscle contusion in a 15-year-old boy due to falling during dance. MRI showed contusion in the anconeus muscle, as well as strain in the ulnar collateral ligament and edema in the coronoid process of the ulna. To our knowledge, this is the first described case of traumatic anconeus muscle contusion in the literature. The presence of long-lasting lateral elbow pain in trauma cases without fracture should alert clinicians to consider anconeus muscle abnormalities. MRI is the best modality for diagnosis in these cases.

Background: Cubital tunnel syndrome is a common compressive neuropathy of the upper extremity. The anconeus epitrochlearis muscle is an unusual but occasional contributor. We review our experience with this anomalous muscle in elbows with cubital tunnel syndrome. Methods: We retrospectively reviewed charts of 13 patients noted to have an anconeus epitrochlearis muscle associated with cubital tunnel syndrome. Results: Ten patients had unilateral ulnar neuropathy supported by nerve conduction studies. Three had bilateral cubital tunnel syndrome symptoms with 1 of those having normal nerve conduction studies for both elbows. Eight elbows were treated with myotomy of the anconeus epitrochlearis muscle and submuscular transposition of the ulnar nerve. The other 8 elbows were treated with myotomy of the anconeus epitrochlearis muscle and in situ decompression of the ulnar nerve only. All but 1 patient had either clinical resolution or improvement of symptoms at follow-up ranging from 2 weeks to 1 year after surgery. The 1 patient who had persistent symptoms had received myotomy and in situ decompression of the ulnar nerve only. Conclusions: An anomalous anconeus epitrochlearis occasionally results in compression of the ulnar nerve but is usually an incidental finding. Its contribution to compression neuropathy can be tested intraoperatively by passively ranging the elbow while observing the change in vector and tension of its muscle fibers over the ulnar nerve. Regardless of findings, we recommend myotomy of the muscle and in situ decompression of the ulnar nerve. Submuscular transposition of the ulnar nerve may be necessary if there is subluxation.

Een losse processus anconeus, onderdeel van het elleboogdysplasiecomplex, is een frequent voorkomende orthopedische ontwikkelingsstoornis bij snelgroeiende jonge honden behorend tot grote en chondrodystrofe rassen. Proximale ulnaire osteotomie is een van de mogelijke behandelingsmethoden, op voorwaarde dat het dier nog niet volwassen is. Deze ingreep is gericht op het herstellen van de lengteverhouding tussen radius en ulna, waardoor de opwaartse kracht, die de losse processus anconeus veroorzaakt, opgeheven wordt. Door de osteotomie is tevens een kanteling van het olecranon mogelijk, waardoor de losse processus anconeus alsnog kan fusioneren en het manken verdwijnt. Wanneer een proximale ulnaire osteotomie succesvol uitgevoerd wordt, zonder bijkomende articulaire chirurgie, wordt er minder osteoartrose gevormd en is het resultaat op lange termijn gunstiger. In dit artikel wordt de behandeling door middel van ulnotomie bij een jonge mastino napoletano besproken en worden de resultaten van de behandeling toegelicht.

Ce travail étudie les transformations du système politique et économique de la côte adriatique au IVe siècle. Dans le chapitre 1,on présente le littoral et son arrière-pays à la veille des invasions gauloises. Il s'agit d'un territoire peuplé d'ethnies variées,Vénètes,Etrusques,Ombriens et Picéniens,jouissant d'une grande prospérité grâce aux relations nouées avec la Grèce. Les comptoirs,Adria,Spina et Numana s'affirment comme les pôles économiques de la région. Le chapitre 2 analyse les invasions et leurs conséquences,la rupture des relations entre littoral et arrière-pays,la mise en place d'une nouvelle géographie territoriale et la progressive intégration des nouveaux venus au monde étrusco-italique. Le chapitre 3 démontre que Denys l'Ancien n'a pas eu de visées expansionnistes en Adriatique septentrionale. Adria ne fut pas une colonie de tyran,et Ancône non plus,même si elle s'affirme comme un foyer héllénique actif. Le chapitre 4 analyse la survivance du commerce attique à partir de l'exemple du Fat-Boy à Spina etre replace dans ce contexte la tentative de fonder une colonie en 325. Le dernier chapitre montre qu'après la disparition du commerce attique,les centres côtiers renforcent leurs liens avec la péninsule et développent leurs productions. Il finit par un tableau à la veille de la conquête romaine

Abstract:
The anconeus, a small triangular muscle positioned on the posterolateral part of the elbow joint, has been the subject of considerable research without a satisfactory conclusion being reached regarding the role it plays during normal elbow kinematics. The aim of this investigation was to elucidate the function of the anconeus muscle and find the relative contribution that it makes to elbow kinematics by examining relative electrical muscle activity and elbow kinematics both before and after anconeus defunctioning carried out using a local anaesthetic (lidocaine). The study was performed through an examination of the myoelectric activity of the representative elbow flexor and extensor muscles (biceps brachii and triceps brachii) and the elbow kinematics and kinetics. Right-handed, healthy volunteers performed elbow flexion-extension and supination-pronation movements in both horizontal and sagittal planes before and after blocking of the anconeus. The kinematics and kinetics of the elbow were assessed using inertial sensors, and muscle electrical activity was recorded using surface electromyography. In the following stage of the study, the anconeus muscle was blocked through an injection of lidocaine and then the flexion-extension and pronation-supination movements were repeated. The relative electrical activity results from the anconeus before blocking clearly indicate that the activity of the muscle was higher during the extension portion of the flexion-extension cycle, suggesting that it behaves as an extensor muscle. However, from the paired sample t-test analysis, it was found that blocking of the anconeus had no effect on the kinematics and kinetics of the elbow, including the angular velocity, net torque, power and net joint work. Moreover, the angular velocity data for the elbow, before and after the blocking for all movements, showed a linear trend with slopes and Pearson's correlations close to unity, indicating no apparent difference on the elbow kinematics. In addition, the relative electrical activity of the biceps and triceps brachii muscles did not alter significantly following blocking of the anconeus. These findings suggest that the anconeus muscle is a relatively weak elbow extensor as it is likely that the small contribution that the anconeus provides during extension before blocking is compensated by the triceps brachii after the anconeus is deactivated. In order to provide additional weight and support to the findings of the experimental study, a computational model of the elbow joint was created in Abaqus CAE with the aim of investigating the contribution of the anconeus during the flexion-extension motion. In particular, the effect on the range of motion and contact area of the elbow joint was investigated both before and after anconeus blocking. The analysis was done in a range of motion of 90°, starting with the elbow extended 30° and ending flexed 120°. The elbow joint model considered cortical bone, trabecular bone, cartilage, collateral ligaments, the anconeus, biceps brachii and triceps brachii. The results of the investigation indicated that the anconeus muscle does not produce a significant change in the range of motion and contact area in the articulation, an outcome that supports the findings of the experimental investigation.

Lo studio compiuto ha come oggetto ruderi inerenti al periodo romano, medievale e moderno, attualmente senza alcuna protezione adeguata e sottoposti ad un progressivo degradamento a seguito della demolizione del fabbricato presente sopra di essi. Quest’ultimo sorgeva internamente al perimetro ellittico dei resti dell’anfiteatro di Ancona. Obiettivo di questa tesi è la conservazione di questi resti e la progettazione di un edificio al di sopra di essi che potesse proteggerli ed essere a servizio dell’area archeologica presente nei pressi del sito e del Segretariato Regionale del MiBACT per le Marche. Oltre che sul singolo volume si è svolto un lavoro di valorizzazione dell’intero comparto. Preliminarmente alle operazioni di progettazione è stato svolto un attento lavoro di ricerca d’archivio e uno scrupoloso rilievo del manufatto, il quale ha permesso di individuare le modalità d’intervento più idonee per approcciarsi a questo complesso contesto archeologico così denso di stratificazioni.

Il progetto nasce dall’esigenza di riscoprire il valore di un luogo, oggi in disuso, dotato di un notevole potenziale panoramico, paesaggistico nonché storico; l’obiettivo è quello di restituirgli un’identità persa progressivamente nel tempo e riconsegnarlo alla città. L’approccio progettuale ha visto come punto di partenza l’analisi storica e inventiva dell’area e del contesto, questo ha permesso di intervenire in maniera consapevole nella realizzazione del progetto di restauro, conservazione e valorizzazione del Complesso Birarelli. “PoDiO” (Polo Direzionale Organizzativo del MiBACT) nasce dalla collaborazione con il Segretariato Regionale delle Marche, con l’obiettivo di creare un polo culturale del MiBACT ad Ancona. All’interno del Complesso è prevista la presenza di tre enti: l’Archivio di Stato, la Soprintendenza Archivistica e Bibliografica e la Direzione Regionale dei Musei delle Marche. Nell’ottica di progettare, dal latino proiectare “gettare avanti” oltre il presente, consapevoli che: “non esiste restauro che non implichi una trasformazione” si è cercato di intervenire nell’assoluto rispetto dell’area e del manufatto, operando attraverso le logiche del restauro conservativo, considerando la fabbrica come palinsesto storico e mirando ad un mantenimento attivo nell’uso quotidiano. Un’ulteriore attenzione è stata posta al superamento delle barriere architettoniche; il tema dell’accessibilità è stato per noi una linea guida che ci ha accompagnato fin dall’inizio nel processo di progettazione. Ci siamo trovate di fronte a un’area ricca di dislivelli e salti di quota, esterni ed interni alla fabbrica, che aveva l’ambizione di diventare un luogo pubblico; abbiamo cercato di garantire una completa e fluida fruizione degli edifici e degli spazi esterni, fermandoci laddove rendere accessibile il luogo significava stravolgere la natura dell’area, cercando di trasformare le svariate situazioni di limite presenti in opportunità di scoperta per il visitatore di oggi.

Il progetto di valorizzazione dell’area del parco archeologico dell’anfiteatro romano di Ancona affronta il problema del recupero delle zone lacerate del centro antico della città, di quelle aree che in seguito ai bombardamenti della Seconda Guerra Mondiale e del terremoto del 1972 hanno perso la loro identità. Dopo aver analizzato le fasi evolutive della struttura urbana della città e il sistema di relazioni sociali che la caratterizza, il progetto ha individuato nella zona dell’ex convento di Santa Palazia, sul versante ovest del colle Guasco e attigua al parco archeologico dell’anfiteatro romano, il sito idoneo per la localizzazione di un nuovo complesso architettonico. È prevista la realizzazione di residenze, spazi per la didattica e per la cultura, attività di servizio e commerciale e la realizzazione della sede del nuovo Museo d’Arte Contemporanea della Regione Marche. Il progetto definisce un macrosistema in grado di mettere ordine e porre in relazione, attraverso spazi condivisi, edifici primari (come l’anfiteatro, la chiesa dei SS Pellegrino e Teresa e il Museo Archeologico), siti archeologici, edifici pubblici legati alla archeologia, ed edifici privati, prevalentemente ad uso residenziale. Ridisegna la città facendo della misura dell’eccellenza del passato di Ancona, la misura del nuovo progetto per recuperare l’identità perduta.

Nel XIX secolo in Lombardia si completava la sistematica distruzione del patrimonio di polittici in terracotta già iniziata con la Controriforma; con la perdita dell’integrità delle ancone è venuta meno la memoria della loro stessa esistenza e della loro struttura. Analogo destino fu riservato alle ornamentazioni fittili degli edifici rinascimentali delle città lombarde, spesso riconducibili alle medesime maestranze responsabili della realizzazione dei polittici. Negli anni Sessanta dell’Ottocento, si manifesta però la volontà da parte delle costituende raccolte civiche lombarde di tutelare questo ricco patrimonio, con la creazione di fondi museali di terrecotte figurative e ornamentali. Oltre allo studio dei polittici fittili di cui esistono testimonianze materiali e documentali, l’attività di ricerca svolta nel contesto del Dottorato si è basata sull’esame di quanto conservato presso le raccolte dei Musei milanesi e cremaschi, con il recupero di lacerti di opere provenienti da edifici civili e religiosi di Milano, Crema, Cremona e Lodi. La ricerca è stata estesa ai frammenti di polittici presenti sul territorio bresciano e monzese e ai reperti archeologici riconducibili a modelli fittili per la statuaria lapidea del Duomo di Milano, risalenti al primo quarto del XVI secolo, epoca a partire dalla quale in Lombardia sembra declinare irreversibilmente l’utilizzo della terracotta sia per la decorazione architettonica che per la realizzazione delle ancone.
The methodical destruction of the heritage of terracotta altarpieces already begun with the Counter-Reformation was accomplished in Lombardy during the nineteenth century. With the loss of integrity of these altarpieces, we have also lost track of their existence and structure. A similar doom was reserved to the fictile ornamentation of Renaissance buildings in Lombardy, often created by the same workshops responsible for the polyptychs. Following the Italian unification, however, the forthcoming civic art collections decided to preserve this legacy with the creation of extensive figurative and ornamental terracotta funds. Besides the study of terracotta polyptychs for which physical or documentary evidence still exists, the PhD research focused on the analysis of the clay material retained in the civic museums of Milan and Crema, with the identification of remains mainly from civil and religious buildings in Milan, Crema, Cremona and Lodi. The research was also extended to include fictile fragments originally belonging to altarpieces in the area of Brescia and Monza, and clay models from archeological finds, used for the production of stone statues still existing today in the milanese cathedral and dating from the first quarter of the XVI century, in a period of time in which the utilization of terracotta for altarpieces and architectural decorations started irreversibly to decline in Lombardy.

Abstract:
El período de 1940 a 1980 corresponde a la configuración de Lima como una ciudad metrópoli, lo que supuso que más personas en la capital hagan uso de sus espacios públicos y, en especial, de sus playas durante el verano. Ancón es un distrito-balneario que destaca frente a los balnearios de Lima por su antigüedad, permanencia en el tiempo, y conflictos entre grupos sociales respecto a la apropiación de sus playas. En donde se presenta al grupo veraneante (propietarios de los inmuebles del balneario) que restringe a los residentes (pobladores que viven durante todo el año en Ancón) y bañistas (personas que asisten a pasar el día a las playas) del acceso a los espacios públicos. Consecuentemente, surge el interés por estudiar los modos de apropiación de los espacios públicos de Ancón y, por ello, se estableció como pregunta de investigación ¿cuáles han sido las consecuencias de las transformaciones urbanas ocurridas por la metropolización de Lima en las prácticas de apropiación del espacio público de los residentes y veraneantes de Ancón desde 1970 a la actualidad? Para abordarla se planteó los siguientes objetivos: (i) identificar y analizar los modos de apropiación en la historia que han vivido los residentes y los veraneantes en los espacios públicos de Ancón desde 1970 a la actualidad, (ii) identificar y analizar los cambios geográficos y la forma de apropiación del espacio público por parte de los residentes y los veraneantes en el distritobalneario de Ancón durante el período de 1970 hasta hoy en día, y (iii) analizar las percepciones de los residentes y los veraneantes de Ancón respecto al proceso de apropiación del espacio público que ha sucedido desde 1970 hasta la actualidad en el distrito-balneario. Respecto a los hallazgos, se presentan (i) la migración de varias familias veraneantes a los balnearios del sur, (ii) el fortalecimiento y la diversificación de las prácticas de segregación en los espacios públicos, por parte del grupo veraneante hacia los residentes y bañistas, y (iii) la ampliación de los espacios de encuentro (espacios públicos, espacios pseudo públicos y espacios privatizados).

Abstract:
Nelle vicinanze del fiume Aspio, collocata su un colle circondato da una cinta di alberi, sorge la Rocca di Bolignano , una costruzione risalente al XIV-XV secolo, collocata sulla strada di collegamento tra Ancona ed Osimo. La fortificazione fu preposta alla difesa di Ancona dalla città di Osimo, a quei tempi sua rivale e nonostante fosse predisposta per la difesa del luogo da potenziali attacchi, in realtà vide un solo conflitto, la Battaglia del Porco nel 1477. Modifiche successive hanno visto la Rocca divenire un magazzino nel 1532, una residenza privata nel 1610 e l’abbandono nel 1976. Il complesso è costituito dalla Rocca di Bolignano con annessa la chiesa di Sant’Antonio Abate costruita nel XVIII secolo e da un edificio che probabilmente era l’abitazione dei contadini, aggiunto in epoca successiva. Questo indica che verosimilmente vicino al complesso era annesso del terreno vocato a coltivazione agricola.

AbstractSmart meters, one of the crucial enablers of the smart-grid concept and cornerstones in smart planning for cities, offer the opportunity for consumers to address their energy consumption effectively through timely and accurate data on their energy usage. However, previous studies have shown that smart meters may not lead to the desired energy savings unless actively used by households. To this end, the research presented in this paper investigates the penetration of smart meters at community level and explores how such a metering system can help people to understand and manage their energy use better. It examines the awareness about smart meters, looks into their presence in current accommodation and focuses on the views people have about smart meters. For this purpose, a questionnaire was prepared and distributed to a group of individuals residing in the wide area of Ancona province in Italy. Although the deployment of modern second-generation smart meters started in 2017 replacing the outdated smart meters massively installed in the 2000s, the results show low-to-moderate levels of awareness of modern smart meters among the respondents and a low presence of second-generation metering devices in their current accommodation. However, the general view expressed by the participants about smart meters is positive. The findings demonstrate that respondents are in need not only of a gauge that measures energy consumption but also of a tool that assists them to manage effectively their energy use.

The Italian military architecture of Ancona’s Citadel: construction techniques and defensive systems in the sixteenth centuryThe objective of this research is regarding the construction techniques used in the military architecture of Cittadella-Fortezza (Ancona, Marche, Italy). In this case, attention will focus primarily on historical, bibliographic and archive research, then through a comprehensive analysis of building methods used in the sixteenth century and on the strategic function that this fortification covered in the coastal strip of the Middle Adriatic. Together with Rocca Paolina (Perugia) and Fortezza da Basso (Florence), it has in fact a remarkable importance in the military architecture’s history, as it was one of the first experiments of fronte bastionato all’italiana. Built from 1532 by Antonio da Sangallo il Giovane, it rises on the top of Astagno hill in a panoramic and defensive position, overlooking the city and the port. It clearly distinguishes itself from the surrounding building fabric as it is characterized by five mighty bastions in bricks and by the central bulwark with the vaulted ground floor. The study aims to investigate the structural details of Ancona’s fortress such as the modeling of walls, the suppression of protruding volumes, the extension and rounding of the corner towers and the introduction of the central type plan. A great understanding of this research will be analyzed in the drawings and the volumes’ reliefs, which highlighted the general geometric data, the materials used for the realization of the work, the angle of the curtain walls and the technical/constructive strategies. Therefore, the methodical-metric knowledge of the parts will be made more accessible also in relation to the three-dimensional modeling of the fortress, in addition to the critical comparison based on other historical examples of military architecture in the Renaissance period.
