The echocardiogram is an extremely useful and versatile technique that enables collection of all relevant information on morphology and function of the heart in a wide range of clinical situations.Echocardiography has been pivotal in the development of modern cardiology and is an indispensable tool to effectively manage cardiological patients. The echocardiogram is a highly operator-dependent technique in which the skills of the operator in achieving optimized images and adapting the study to the possible findings is simply fundamental.1 In this chapter we will review the technical aspects invo

Infectious endocarditis (IE) is an infection of the heart’s innermost layer, the endothelium. Most cases require a predisposing injury to the endocardium to serve as a nidus for thrombus development, which in turn acts as nidus for bloodstream microorganisms. These intravascular microorganisms can result from dental and other invasive procedures, infected vascular catheters, and skin lesions. However, most episodes of IE result from transient bacteremia during menial tasks, such as chewing and brushing one’s teeth. Blood cultures and echocardiograms are critical for IE diagnosis. Echocardiography has a fundamental role in patients with adult congenital heart disease. This chapter identifies the role of echocardiography in atrial septal defects, ventricular septal defects, atrioventricular septal defects, patent ductus arteriosus, and persistent left superior vena cava. For each condition, the role of transthoracic and transoesophagael echocardiogram are shown alongside examples of main types and features and haemodynamic effect. Doppler examination is an integral part of the echocardiogram. Current systems are equipped with spectral Doppler in continuous wave mode (offering measurements of high velocities with limited spatial specificity due to integration of signal along the scan line), pulsed wave mode (high spatial specificity with maximal recordable velocity reduced by the Nyquist limit), and colour Doppler flow mapping (allowing rapid identification of flow pattern within a cross-sectional B-mode sector). The echocardiogram is an extremely useful technique that gives all relevant information on morphology and function of the heart in a wide range of clinical situations. The very first step in echo is how to achieve the images needed for the cardiac evaluation. Infective endocarditis is the fourth leading cause of life-threatening infections, accounting for 40,000 annual U.S. hospital admissions. Due to decline in rheumatic heart disease, a shift in causative organisms from viridans streptococci to S. aureus, Group D Streptococcus, and multidrug-resistant species has been observed. The spectrum of neurological complications ranges widely from cerebrovascular pathologies-including septic embolization, mycotic aneurysms, and intracerebral hemorrhages-to seizures, meningitis, cerebritis, and abscess. On October 29th 1953 in Lund, Sweden, Inge Edler, cardiologist, and Hellmuth Hertz, physicist, performed the first successful Ultrasoundcardiogram (UCG), later renamed Echocardiogram. A few weeks later, on December 16th, the neurosurgeon Lars Leksell diagnosed an intracranial bleeding in a 16-month-old boy using the same equipment, and Echoencephalography was born. The Lundensian obstetrician Bertil Sunden was in 1962 able to take the first ultrasound picture of twins in pregnancy. Pulmonary hypertension is one of the most challenging medical conditions for even the most experienced anesthesiologist to manage. The very dynamic nature of pulmonary vascular disease lends itself to potential catastrophic changes that can increase the perioperative morbidity and mortality. Given the potential for significant hemodynamic, oxygenation, and ventilation changes during perioperative care it is imperative that the pediatric anesthesiologist not only perform a history and physical exam in this high-risk patient population but also carefully evaluate the most recent cardiac studies

Echocardiography has a pivotal role in the care of heart transplant (HT) recipients. This chapter discusses the use of echocardiographic techniques for the assessment of HT patients. In the early post-transplant period, echocardiography has demonstrated its utility to assess the normal and abnormal structural and physiological changes of the transplanted heart, as well as to detect complications such as graft failure. During follow-up, development of acute/chronic graft rejection and cardiac allograft vasculopathy remains the leading causes of mortality in HT recipients and the role of convent
