ZUSAMMENFASSUNGMit einer Inzidenz von weltweit mehr als 200 Millionen Menschen stellt die periphere arterielle Verschlusskrankheit (pAVK) eine ernst zu nehmende Erkrankung dar, deren Prognose oftmals unterschätzt wird. Daher ist eine frühzeitige Diagnose, verbunden mit der Einleitung optimaler therapeutischer und sekundärpräventiver Maßnahmen außerordentlich wichtig, um den Krankheitsverlauf günstig zu beeinflussen. Zum diagnostischen Screening hat sich als Standard die sog. Ankle-Brachial-Index-Messung (ABI-Messung) etabliert. Bei Nachweis einer pAVK ist eine optimale medikamentöse Therapie (Statin in allen Stadien, Thrombozytenaggregationshemmer ab Stadium IIa nach Fontaine) einzuleiten. Darüber hinaus sollte die Möglichkeit einer revaskularisierenden Therapie ab dem Stadium IIb nach Fontaine geprüft und bei kritischer Extremitätenischämie mit Ruheschmerzen und/oder akralen Ulzera und Nekrosen fachkundig durchgeführt werden. Bei der Hälfte aller pAVK-Patienten ist auch eine koronare Herzkrankheit nachweisbar, daher ist die Diagnostik und Behandlung kardiovaskulärer Co-Morbiditäten von außerordentlicher Bedeutsamkeit. Auch begleitende modifizierbare kardiovaskuläre Risikofaktoren wie Bluthochdruck, Diabetes sowie chronische Nieren- und Herzinsuffizienz sollten in regelmäßigen Abständen abgeklärt und kontrolliert werden. Hinzu kommt ein großer Anteil an Patienten mit gesundheitsbedenklichem Lebensstil, allem voran ein zumeist langjähriger Nikotinabusus, Übergewicht und körperliche Inaktivität. pAVK-Patienten benötigen daher eine breite interdisziplinäre Betreuung u. a. durch Angiologen, Kardiologen, Diabetologen, Nephrologen, häufig weiter ergänzt durch andere ärztliche und nicht-ärztliche Fachdisziplinen (z. B. Orthopädie, Chirurgie, Podologie, Ernährungsmedizin/Sportmedizin).

ZusammenfassungZiel: Entwicklung eines modifizierten Verfahrens zur lokalen rt-PA-Fibrinolyse, um Kosten, Zeit und applizierte Strahlendosis einzusparen.Methode: Bei 40 Patienten mit verschlossenen Gefäßsegmenten der BeckenBeinstrombahn im Stadium llb-lll nach Fontaine wurde eine modifizierte lokale rt-PA-Fibrinolyse durchgeführt, 24 Kurzzeitlysen (KL) und 16 Langzeitlysen (LL). Ergebnisse: Die primäre Wiedereröffnungsrate beträgt 75%, die Offenheitsrate im 6-Monate-Follow-up aller primär erfolgreich lysierten Patienten 67%. Der Arm-Knöchel-Index konnte von 0,4 ± 0,3 auf 0,8 ± 0,2 angehoben werden. Die durchschnittliche Belegungszeit des Angiographieraumes betrug 60 min, die mittlere Durchleuchtungszeit 17 min.Schlußfolgerung: Bei gleicher therapeutischer Wertigkeit bietet die Modifikation gegenüber konventionellen Fibrinolysetechniken Vorteile. Zeitaufwendige KL und LL finden überwiegend außerhalb des Angiographieraumes statt, dieser wird für andere Patienten nutzbar. Die applizierte Strahlendosis wird sowohl für Patienten als auch Personal reduziert.

Poète d'origine belge naturalisé français, vivant entre Paris et Bruxelles, André Fontainas (1865-1948) est généralement considéré comme l'un des meilleurs témoins de l'époque symboliste. L'on doit à ce disciple de Mallarmé une oeuvre littéraire abondante et diversifiée. Fontainas signe cependant tout au long de sa vie de nombreuses critiques d'art. Sa participation à l'aventure du Mercure de France, où il tint la rubrique "art moderne" de décembre 1896 à 1902, fait de lui l'un des principaux acteurs de la période. De cet engagement, moment clé de la trajectoire, résulte une mésestimée "Histoire de la peinture française au XIX e siècle "(1906), ainsi que plusieurs monographies consacrées à Courbet, Daumier, Hals, Rops, Constable ou Bourdelle. Nourrie d'archives familiales inédites, cette étude replace l'activité de critique et d'historien de l'art, d'André Fontainas dans sa vie, sa carrière et un parcours intellectuel complexe, moins inféodé au symbolisme qu'on ne l'a cru auparavant
Andre ́Fontainas (1865-1948), a poet of belgian origins but naturalized frenchman, is generally considered to be one of the principal witnesses of the symbolist era. Disciple of Mallarme, he left us a very rich and diversified literary work as well as numerous art reviews which he published throught his lifetime. His contribution to the Mercure de France adventure, for which he wrote the "Modern art" column from 1896 to 1902, qualified him for the role of one of the most important art critics of fin-de-siècle Paris. His much underestimed Histoire de la peinture de France au XIXe siècle (1906), together with his various monographs on Courbet, Daumier, Hals, Rops, Constable and Bourdelle, are the direct outcome of this key moment in his career. This study, based on Hitherto unpublished family archives, places Fontainas' criticism and history of art back into context, revealing the complexity of a thought process much less influenced by symbolism than was previously believed

The discursive and critical positions of the ‘classic’ nineteenth-century novel, particularly the woman’s novel, in the field of adaptation studies have been dominated by long-standing concerns about textual fidelity and the generic processes of the text-screen transfer. The sociocultural patterns of adaptation criticism have also been largely ensconced in representations of literary women on screen. Taking a decisive twist from tradition, this thesis traces the evolution of representations of masculinity in the malleable characters of Rochester and Heathcliff in film and television adaptations of Charlotte Brontë’s Jane Eyre and Emily Brontë’s Wuthering Heights between 1939 and 2009. Concepts of masculinity have been a neglected area of enquiry in studies of the ‘classic’ novel on screen. Adaptations of the Brontës’ novels, as well as the adapted novels of other ‘classic’ women authors such as Jane Austen, George Eliot and Elizabeth Gaskell, increasingly foreground male character in traditionally female-oriented narratives or narratives whose primary protagonist is female. This thesis brings together industrial histories, textual frames and sociocultural influences that form the wider contexts of the adaptations to demonstrate how male characterisation and different representations of masculinity are reformulated and foregrounded through three different adaptive histories of the narratives of Jane Eyre and Wuthering Heights. Through the contours of the film and television industries, the application of text and context analysis, and wider sociocultural considerations of each period an understanding of how Rochester and Heathcliff have been transmuted and centralised within the adaptive history of the Brontë novel.

Cette thèse propose l'analyse des processus de l'intégration discursive des « Nouveaux Mondes » – c'est-à-dire des nouveaux lieux de l'expansion européenne des XVe et XVIe siècles – dans les imprimés français d'avant 1560. Le corpus s'y veut exhaustif, mais l'étude porte en priorité sur (1) les mentions brèves (dites sublimées), que celles-ci relèvent de la digression ou qu'elles soient intégrées, et (2) les textes actualisés, c’est-à-dire les traductions, les rééditions avec annexes, les adaptations, etc. Une division bipartite et heuristique est proposée, avec d'une part les actualisations d'écrits procédant par l'exclusion d'un ou d'élément(s) de l'hypotexte (c'est-à-dire du matériel textuel qui préexiste à l'acte de publication), et d'autre part par l'adjonction d'éléments supplémentaires. Cette division permet d'analyser les dits lieux, d'une part en fonction d'un principe de pertinence (lequel est révélé par les phénomènes d'exclusion), et d'autre part en fonction d'une recherche de l'inédit, voire de l'acte contentieux (qu'exprime éloquemment l'acte d'adjonction). La thèse démontre comment les deux principes, celui de la pertinence et celui de la recherche de l'inédit, sont liés à la culture émergente de l'imprimé, ainsi qu'au développement d'une grande variété de genres et de discours: littérature géographique, chroniques annales, pamphlets ou manuels anti-syphilitiques, lettres missionnaires, littérature eschatologique, traités didactiques, etc. Le concept d'acte de publication, qui est corrélatif à celui d'actualisation, permet d'aller au-delà d'une opposition entre l'ouvrage dit « périmé » et l'ouvrage dit « progressiste ». Pour ce faire est reconsidéré l'apport, pour la diffusion des nouveaux lieux, de textes comme le Tractatus de sphaera de Joannes de Sacrobosco (XIIIe siècle) et la lettre apocryphe du Prêtre Jean (XIIe siècle), qui aux XVe et XVIe siècles sont réactualisés suivant une perspective heuristique ou didactique, et participent ainsi à l'émergence d'une économie discursive des « Nouveaux Mondes ».

This chapter focuses on the individuals immortalized in the portrait panels in the new library of the Collège de Sorbonne. Built on a north-south axis, the long sides of the new library’s upper floor feature thirty-eight windows that not only provided necessary light but also serve as the library’s principal ornamentation. Twenty-five of the windows were embellished with portrait panels, presenting in each window an individual who had some ‘claim on the college's gratitude’.
