Abstract:
This chapter describes the bench creation of a “poor man’s scoring balloon” as an alternative to commercially available scoring balloons. This is a simple procedure whereby two wires are placed side by side through the stenosis or occlusion to be treated. A standard angioplasty balloon is advanced over the working wire adjacent to the buddy wire. A minimal twisting is performed while advancing the balloon to wrap the buddy wire modestly around the balloon. The balloon is then inflated, and the spiral buddy wire acts as a scoring element to produce a more effective angioplasty. Given that two or more wires are often utilized in a peripheral revascularization, this technique can be performed with little or no additional cost. This procedure is simple to perform and works well in calcified or fibrotic lesions.
