Full textThis is a qualitative study, which through the application of a gender perspective as well a discursive approach has aimed to highlight different discourses in nurses’ talk of anorexia. Background: Anorexia Nervosa is one of the most common forms of eating disorders. Despite this, explanations regarding underlying causes are still lacking and no definite answers exist concerning the overrepresentation of women carrying the disease. Theorizing from a gender perspective offers new models of understanding but is neglected within nursing science. Purpose: This study aims to highlight and identify how anorexia is constructed within different discourses in nurses’ talk of anorexia. Method: A discourse analytic approach was applied to the material, which consisted of interviews with four registered nurses working in primary health care. Results: Three different discourses were identified and labeled The Individuality Discourse, The Societal Discourse and The Gender Discourse. This showed how the understanding of anorexia can both differ and be understood in different ways simultaneously depending on one’s reading. The first identified discourse discussed anorexia as an individual disease with individual underlying causes, the second as a condition constructed in relation to societal influence and the third as an expression of an unequal society. Conclusion: Anorexia can be understood as an individual disease as well as a condition caused by society and media pressure and also as a problem on a structural level where the real sickness is to be found within society itself.

Full textDenna uppsats är en intervjustudie där Carol Lee Bacchis ”What´s the problem”-modell fungerar som inspiration för hur materialet begripliggörs och analyseras. Syftet är att undersöka synsättet på anorexi inom den specialiserade ätstörningsvården med utgångspunkt i ett feministiskt teoretiserande kring detta. Specifikt undersöks när- och frånvaro av genusperspektiv och i vilken utsträckning de fyra intervjuade företrädarna för den specialiserade vården uppfattar problematiken som individuell kontra strukturell. Författaren har en förståelse av kön som konstruerad kategori och i analysen berörs genuskonstruktionens betydelse för förekomsten av anorexi. Bland andra Karin Johannisson har bidragit med ett historiserat synsätt på det konstruerade även i sjukdomsdiagnoser. Analysen lutar sig på socialt och strukturellt inriktade teoretiker och mycket inspiration hämtas från Susan Bordos feministiska perspektiv. Liknande tankegångar återfinns även hos bland andra Helen Malson och Nina Björk vilka bidragit till att skapa en teoretisk analysram. Vad som framkommer i analysen är en svårighet att både formulera och enas kring vad anorexi konstitueras av. Medicinska referensramar är frekvent återkommande men outvecklade och problematiken beskrivs vid en närmare analys som snarast strukturell – även om detta inte artikuleras direkt. En framträdande slutsats är förekomsten av en ”omedveten medvetenhet” hos respondenterna. Med detta avses små och partikulära genusanalyser hos respondenterna som inte direkt sätts i samband med förekomsten av anorexi. Om diskussionen kring anorexi kan komma att utvidgas till att kombinera individualpsykologiska perspektiv med en strukturell förståelsemodell, vilket författaren föreslår som en fruktbar utveckling, avgörs enligt resonemanget som förs av om motsägelserna artikuleras eller förblir skymda.

Full textThe eating disorder anorexia nervosa is a societal problem that has a long history. Despite this, there is not much research on those who have the disease or on those who work with it. The aim of this bachelor’s thesis was thus to illustrate the professionals’ knowledge and experience in the work with adolescences (13-16 years old) who have been diagnosed with anorexia nervosa. What treatment is offered to young people and what are the important factors for recovery? The results of the survey made us realize what significant role the professional can play in such complex situation as a patient. It also gave us an insight into how the social pedagogical approach can be applied in such work. The results show that family-based therapy (FBT) is the most commonly treatment, used for adolescents with anorexia nervosa, which is consistent with previous research. It also shows that the work is based on relationships, where the professional needs to make allowances for many different factors that affect the patient and that it can be difficult to suffice for those who need more comprehensive care. The survey is based on a qualitative method, with semi-structured interviews, and based on Aaron Antonovsky’s theory Sence of coherence (SOC).

Full textBackground: during the last decades anorexia nervosa has had a rising incidence in Sweden as well as in other countries. The disorder may sometimes be difficult to understand due to its multifaceted nature of experienced pros and cons. Furthermore, reluctance toward treatment is frequent among people suffering from anorexia nervosa and treatment for eating disorders have rather high drop-out rates. Aim: The aim of this study was to highlight experiences of living with anorexia nervosa. Method: A literature based study consisting of nine qualitative research articles. Results: Three categories emerged from the analysis; the ambivalence between good and evil, varying emotions and other people are influenced and influences the person with four subcategories. Conclusion: Emphasis on achieving increased knowledge of the lived experiences of anorexia nervosa to enhance understanding for its complexity, might enable nurses’ support-providing to enlarge the patient’s motivation to recovery.

Full textBackground: Anorexia nervosa (AN) is a mental illness with eating disorder and emaciation as a result of a refuse to eat and loss of appetite. Mental health among young women is deteriorating and in recent decades, the existence of AN has increased. Aim: To describe the experience of living with AN, based on young women's perspective. Method: A qualitative study where six autobiographies have been analyzed and compiled. Results: Four themes are presented (1) not being able to identify with his diagnosis, (2) to find respite in activity and good relations, (3) to experience a chaos of existential feelings and (4) to feel the fear of it healthy. Discussion: Living with AN means a great suffering which manifests itself in various forms. Despite illness, a well-being can in some situations be experienced which is significant for the health process. Conclusion: When nurse is given knowledge of how life can be affected by AN a patient's life-world can better be understood. This allows the care and treatment to be adapted to the patient with AN. When experience of a good treatment occurs, the health process promotes.

Full textBackground Until the 80’s Anorexia Nervosa (AN) was a rare disease. With the influence from media and the growing ideal of how a human body should look like, AN is nowadays a more common disease. AN often have a negative effect on parents. Therefore, it is important that nurses and society gain an understanding of parents experiences of having a child with AN, and how it impact their lifes. Aim The aim of the study was to illuminate the experience of being a parent to a child with AN. Method This study is a literature review over qualitative scientific articles, which have been analyzed through a method of five steps according to Friberg. Results The results showed that AN has a negative impact on parents, both the personal life and family life. It also describes parents’ need for support. Parents experience many negative feelings as worrying, frustration and fear. They often had bad experiences of treatment from medical staff. Conclusion This study provides an understanding of parents’ feelings of having a child with AN. Parents have an important role for their children during their illness and on the road to recovery. If parents should be able to take the responsibility that comes with having a child with AN, they need support. Otherwise they can be a threat to their own health. This is something that the nurse should pay attention to.

Full textBackground: Anorexia means loss of appetite and is misleading for a disease that is about self-starvation. The disease is deadly, people with AN have a sixth increased risk of dying compared to the general population. Severe AN is one of psychiatry's most challenging conditions to treat. There is still a lack of studies on adults with AN, most research is on teenagers with AN. Aim: The aim with this study was to highlight which factors people with a personal history of Anorexia Nervosa considered to be positive and/or negative for their recovery. Method: Literature review based on systematic database searches in the databases Pubmed, Cinahl and PsycINFO. The result was based on 13 articles with qualitative method design based on patients' experiences. The result was presented according to identified third-level themes and second-level themes. Results: The study resulted in negative and positive factors regarding recovery divided into one third-level theme of factors related to the individual, and one third-level theme of factors related to the environment and nursing care. The literature study culminated in a finding that AN is about so much more than food and weight and shows important steps for the individual and the administered care, which together lead to recovery from AN. Discussion: Common to the two third-level themes of the result was the individual's desire for the caregivers to look behind the disease and work with the patient. Person-centered nursing such as core competence and salutogenic theory where therefore important and useful starting points in the care for recovery. Conclusion: The focus of treatment should be on the individual instead of the disease in order to attain sustainable recovery.

Full textBackground: Amongst young female adults in the western society, eating disorder is the third most common disease after asthma and obesity. About 10 % of the people who suffers from anorexia nervosa die due to the disease. Aim: This studies aim is to describe women’s experiences of being cared for Anorexia Nervosa. Method: This study was completed with a qualitative method and a narrative analysis. The gathering of data was founded on four autobiographies, written by women who has suffered from, and been cared for Anorexia Nervosa. Result: In the results, different experiences from the treatment for anorexia nervosa could be pointed out. The experience was divided into different themes and was clarified with quotations. The themes were To compete, To let go of the control, To be offended, To be seen, To get insight and To feel fear of being healthy. These themes reflected the women’s experiences from being cared for anorexia nervosa. Conclusion: This study can contribute so that health professionals receive a greater understanding about women’s experiences from being cared for Anorexia Nervosa. It could help the health professionals to promote these women’s need of care, it can also increase the women’s experience of health and well-being.

Full textForskning vad gäller självhjälpsgrupper och dess faktiska verkan är överlag begränsad och trots flera års forskning finns det fortfarande ingen behandling som ger mycket goda resultat för många patienter med anorexi. Vetenskaplig utvärdering av olika behandlingsmetoder saknas i stor utsträckning och en genomgång av den vetenskapliga litteraturen visar att det inte finns några läkemedel med dokumenterad effekt vid anorexi. Varför bildas självhjälpsgrupper? Varför räcker det inte med de professionella behandlingsmodellerna? Dessa frågor ämnar jag att finna svar på i min uppsats. Jag har forskat kring professionella behandlingsmetoder samt självhjälp. Mina kärnfrågor i uppsatsen är: vilka olika professionella behandlingsmetoder finns och vad är självhjälp? Jag har valt en kvalitativ ansats på min uppsats. Jag vill beskriva, förklara och jämföra självhjälpsgrupper för anorexi och deras sätt att se sjukdomen och hur de arbetar för att ta sig ur den. Detta i förhållande till professionella behandlingsmetoder. Mina bakgrundsstudier med förklaringsmodeller av anorexi och tillgängliga professionella behandlingsmodeller har jag gjort genom en systematisk genomgång. Min studie görs enligt kvalitativ metod och genom en systematisk beskrivning. Jag har också gjort en systematisk sökning av de tillgängliga litteraturer som finns på biblioteket respektive Internet. Självhjälpsgrupper bör ses som ett komplement till de professionella behandlingsmetoderna snarare än ett alternativ, och man bör inte utesluta en professionell behandling för att istället vara med i en självhjälpsgrupp. Däremot bör man absolut inte underskatta den kraft och styrka det kan ge, att träffa andra människor i samma situation. Att samtala om det gemensamma problem man har, stötta varandra och känna gemenskap är en stark drivkraft i kampen att bli frisk från anorexi

Syftet med denna uppsats är att undersöka om det är faktorer i själva identitetsskapandet som ligger bakom problematiken kring ätstörningar. Detta görs med hjälp av begrepp från Anthony Giddens struktureringsteori. Vi vill även försöka förstå kvinnor som haft ätstörningar och deras subjektiva upplevelser utifrån deras värld och erfarenheter. De frågeställningar vi försöker besvara är:
• Varför utvecklar en ung kvinna en ätstörning?
• Finns det faktorer i identitetsskapandeprocessen som kan orsaka en ätstörning?
Vi utgår från insamlad data från fyra respondenter i åldrarna 24-34 år. Samtliga respondenter är kvinnor. Som urvals metod har rekommendationer från ”erfarna experter” samt mailspridning använts, så kallad snöbollsurval.
För att analysera det empiriska materialet har vi använt oss av Giddens strukturerings teori.
Resultatet av analysen visar att merparten av respondenterna har upplevt ett visst mått av utanförskap under sin skolgång, individerna har känt press på sig att passa in, att anpassa sin sociala identitet. För att lyckas med detta har de känt att de måste förändra sig själva och sitt yttre. För att förändra det yttre har de valt att banta, en bantning som sedermera slog över i ett maniskt beteende som ledde till anorexi eller bulimi.

Title: a qualitative studie about identity making and eating disorders (Du är din egen lyckas smed, en kvalitativ studie om identitetsskapande och ätstörningar)

Abstract
The aim of this essay is to examine if factors in an individuals search for an identity is one of the issues that can cause an eating disorder. This with the help of terms in Anthony Giddens structural theory. We also want to understand women who had eating disorders and there subjective experiences. We have studied the following issues:
• Why does a young woman develop an eating disorder?
• Are there factors in the process of making an identity, that can cause eating disorders?
Our main sources are to our analysis are data from four women, they are all in the age-group 24-34 years. Our sampling-methods are recommendations from experts, and spreading thru mail, so called snowball sampling.
To analyse our empirical findings we used Giddens structural theory.
The result of the analysis shows that most of the interviewed has experienced a certain amount of outcast during their school years, the individuals has felt a pressure to fit, and to adjust their social identity. To succeed, they felt the need to change themselves and their appearance. To do so, they choose to go on a diet, which escalated to anorexia and bulimia.

Full textThe drive to eat, like most motivated behaviors, is controlled by both intrinsic signals from the body as well as extrinsic signals from the environment. Although these factors often act in concert, in some instances environmental cues can override the body’s homeostatic signals. Prior work investigating the ability of learned cues to promote overeating in the absence of hunger identified a critical forebrain network composed of the amygdala, medial prefrontal cortex (mPFC), and lateral hypothalamus (LHA). We hypothesized that a similar forebrain network may also be critical when learned fear-cues inhibit eating despite hunger. The amygdala, mPFC and LHA are each anatomically and functionally positioned to influence feeding, and evidence suggests they could work together to support the fear-cue’s ability to inhibit feeding by overriding homeostatic hunger signals triggered by food-deprivation. Prior anatomical work identified direct pathways between these three large, heterogeneous regions; however, less is known about the organization of the underlying circuitries, especially between distinct nuclei and/or subdivisions that comprise these structures. Study 1 used a dual retrograde tract tracing design to map the topographical organization of the connections between the amygdala, mPFC, and LHA in detail, and to determine whether amygdalar pathways to the mPFC and to LHA originated from the same or different neurons. We found evidence for multiple, topographically organized, direct pathways from the amygdala to the LHA, and separate pathways from the amygdala to areas of the mPFC that send direct projections to the LHA. Importantly, nearly all amygdalar projections to the mPFC and to the LHA originated from different neurons, suggesting that amygdala and amygdala-mPFC processing influence the LHA independently. Study 2 used immediate early gene induction to map the patterns of functional activation within this amygdala-prefrontal-lateral hypothalamic network during the expression of fear-cue inhibited feeding behavior, and to assess whether these patterns were similar in males and females. We found differential activation across the network, and activation patterns related to the presentation of fear-cues, the presence of food-related cues, and the amount of food consumed were associated within distinct cell groups in the amygdala, mPFC, and LHA. Together, the studies presented in this dissertation provide anatomical and functional maps for future interrogation of the circuitry underlying fear-cue inhibited feeding

Full textIntroduction: The specialized eating disorder treatments provided in Stockholm have different treatment concepts of anorexia nervosa (AN) for adolescents. The role of the family varies as well. The purpose of this study is to compare two of the biggest eating disorder clinics in Stockholm, AB Mando and the Stockholm Centrum för Ätstörningar (SCÄ) to see whether, and if so, they differ. Questions: What views have health professionals at two different clinics on AN and the family's role in the treatment process, and how do they argue for this? How does one relate to the family and why so? Method: In this study, which has a qualitative approach, health professionals at the two eating disorder clinics participated in semi-structured interviews. The interviews were processed in a thematic analysis. Results: The two clinics differs diametrically, which is based in two completely different paradigms. Based on the theoretical background, one can see that the system theoretical approach prevails at SCA, where the family's role is crucial during treatment, only by working relationally with the whole family the patient can recover. At Mando where learning theory / biological approach prevails, the concept is the opposite; by separating the patient from the family and the home environment recovery can be made. The behavior is likely to be maintained if the patient is left in the context where the eating disorders has developed. Discussion: These differences should result in different clinical implications. The issue of equal treatment is raised, but also that it can be an advantage with a varied range of treatments based on the varied needs of the families.

Full textEn av de viktigaste aspekterna i omvårdnad är betydelsen av mänsklig kontakt i interaktionen mellan patienter och vårdgivare. Anorexia Nervosa är en komplex sjukdom av psykisk och fysisk karaktär och vårdpersonal spelar en viktig roll för dessa patienters tillfrisknad. Målet med denna studie var att nå en djupare förståelse för den mänskliga kontaktens betydelse i omvårdnaden av patienter med Anorexia Nervosa samt att belysa essensen i en vårdande relation. Syftet med studien var att beskriva Anorexia Nervosa patienters erfarenheter av den mänskliga kontaktens betydelse i möten med vårdpersonalen. En kvalitativ ansats valdes och datainsamlingen grundade sig på sex självbiografier, skrivna av sex unga kvinnor som har varit sjuka i Anorexia Nervosa. En kvalitativ textanalys användes för att tolka data. Åtta teman framkom i resultatet: ”att känna förtroende” ”att få hjälp med att gå till botten med problemet”, ”att inte bli förstådd”, ”att inte våga öppna sig”, ”att inte bli sedd”, ” att få bekräftelse”, ”att känna maktlöshet” och slutligen, ”att någon talar till den friska delen”. Dessa teman speglade dessa personers erfarenheter av den mänskliga kontaktens betydelse i möten med hälso- och sjukvårdspersonal. Denna studie kan bidra med att ge en ökad förståelse för den mänskliga kontaktens betydelse i omvårdnaden av patienter som lider av Anorexia Nervosa samt att hjälpa vårdpersonal att tillmötesgå dessa patienters behov.

Full textDenna uppsats undersöker hur webbserien SKAM representerar psykisk ohälsa; på vilket sätt diskursiva konstruktioner kring psykiska sjukdomar befästs och utmanas. För att analysera detta undersöks säsong ett, två och tre. Uppsatsen bygger på Stuart Halls representationsteori, Michel Foucaults teorier kring vansinnet och Norman Faircloughs diskursteori. Den metodologi som används för studien är en kritisk diskursanalys där Norman Faircloughs tredimensionella modell är utgångspunkt. Analysen sker främst på tre olika nivåer: textuell nivå; karaktärernas dialoger och seriens visuella uttryck. Hur formatet används; hur berättelsen distribueras genom webb och sociala medier. Samt hur detta sker i en större social kontext; hur berättelsen om psykisk ohälsa ofrånkomligen också är en del av en kulturell, institutionell och samhällelig ordning. Vårt resultat visar att SKAM befäster kulturella föreställningar kring psykisk ohälsa genom att representera psykiska sjukdomar som tabubelagda och stigmatiserade samt som ett problem som (främst) ligger på individnivå. De psykiskt sjuka i serien framställs som hot mot samhällets normer där den psykiska ohälsan läggs på individnivå och inte som en produkt av samhällets konstruerade begär eller strukturella problem. Mycket begränsad hjälp till psykiskt sjuka presenteras i serien. Utlämnandet av acceptans och botemedel för de psykiskt sjuka medför att de hamnar utanför samhället och får svårare att bidra på samma sätt som en frisk, arbetande person kan.

Full textThis qualitative study contains a review of how persons with an anorexia-similiar disease and borderline personality disorder, is experiencing their lives through this two conditions. A central question is to find possible causes to explain the comorbidity between an anorexia-similiar disease and borderline personality disorder. In this study, there have been five inerviews. The basis of this study have proceed from two theories; the attachment theory and biosocial theory, which also have been the basis to analyze the comorbidity between an anorexia-similiar disease and borderline personality disorder. The results of the study, indicates that there are some possible reasons why these persons have developed these two diseases and that there is a intelligible comorbidity between this two conditions. Possible factors why these persons have developed these two diseases, have lead to three notions; attachment, vulnerability and abuse. The fourth and final concept, comorbidity, summarizes the possible factors why these persons have developed and anorexia-similiar disease and borderline personality disorder.

Full textThere are several possibilities for those with an eating disorder, who wishes to get treatment and recover to get help and support. But for those who are not ready to recover, there are not any help to receive. Pro-ana is a youth culture where individuals who want to achieve the anorectic ideal help and support each other. The purpose of this study was to describe and analyse the content and the need for the Pro-ana community on Internet, among young people with eating disorders. More specifically the aim was to find out how young Swedish people uses the Pro-ana communities on Internet. The study was an ethnography based on concealed observations of 6 different Pro-ana websites on Internet. During the observations five themes has been used: anonymity, thinspiration, life choice or disease, help or no help and identity. The analysis was based on Goffmans (2004) The presentation of self in everyday life, and the authors thesis about how humans represents them self in the accompany of others. Pro-ana websites consists of diary notes about weight, food, exercise and how to cope with an eating disorder. The Pro-ana culture online has become kind of a support group to which young people turn when society has no knowledge and no resources to meet them and social work are very limited on these sites. When not dare to tell friends and family about their situation, they turn to Pro-ana, which has been described as a sanctuary away from society’s judgements.
