Le malattie cardiovascolari rappresentano un’area clinica in cui maggiormente si avverte la necessità dello sviluppo di risposte assistenziali efficaci e sostenibili, nel cui ambito lo specialista deve svolgere un ruolo determinante, contribuendo a mettere in atto strategie gestionali condivise dalle diverse professionalità sanitarie e sostenute a livello istituzionale. I 200 casi clinici riportati dal progetto RIARTE sono, senza dubbio, una fotografia fedele, reale e pratica della realtà clinica in Italia per quanto attiene alle due categorie osservate: pazienti con rischio cardio e cerebrovascolare superiore al 20% secondo le tabelle del rischio SCORE e pazienti con ipertensione di difficile controllo. In entrambi i casi clinici l’adozione, nelle misure farmacologiche, della terapia a base di calcio-antagonisti diidropiridinici è apparsa efficace, sicura, di facile associazione con altre terapie e caratterizzata da un rapporto costo-efficacia particolarmente favorevole.

Abstract:
The Conclusion argues that the nation’s attachment to policy romanticism prevents its social maturation. Incorporating freely chosen, widely cherished values, policy romanticism remains antagonistic to liberal democracy, or at least to the Enlightenment hopes for social progress. Americans are not innocent of American problems. The nation’s problems are created as the effects of consensual, embedded norms. In contrast, the control of social policy making by illegitimate elites is so improbable that mass preference and mass consent would seem to be a logical default position. Mass consent should be specifically refuted with hard evidence of conspiracies of one sort or another before considering other explanations. The political passivity of Americans signaling contentment or at least acceptance is a more reasonable surmise than the success of hidden evil forces in pacifying the population through a narcosis of liquor, drugs, and the nearly infinite diversions of numbing entertainment.

Abstract:
This book focuses on the stories of eighteen transgender survivors of intimate partner violence (IPV) and how their accounts challenge conventional understandings of this form of abuse. By examining the contexts in which abuse occurs, the book anchors transgender experiences with IPV within a largely trans-antagonistic culture. The dynamics of abuse, as told by survivors, are largely informed by an existing transphobic and genderist society. The prevalent themes in the accounts describe how transphobic and genderist attacks manifested as distinct patterns of abuse. When reflecting and making sense of their reality, survivors saw many of their experiences with abuse as attempts by abusers to control their gender transition and define them on the abusers’ own terms. The book discusses a prominent dynamic of the abuse as controlling transition, in which victims felt that abusers wanted to regulate their identities. This control occurs through two generic strategies: (1) discrediting identity work, redefining the situation to focus on participant-defined insecurities, a form of altercasting; and (2) targeting sign vehicles, including regulating gender transition treatments and controlling through props. Finally, survivors described what is referred to as walking the gender tightrope in which respondents used gendered language in the processing of their victim identity. Additionally, they discussed various help-seeking strategies and how they navigated genderist boundaries and barriers to these resources. The book works toward characterizing the distinct experiences of transgender survivors of IPV while also identifying differences across the intersections of race, class, and gender identities.

Radiation nephropathy usually arises after inadvertent exposure of kidneys to radiotherapy. It may manifest as acute nephropathy as early as 6 months after exposure, or later as chronic nephropathy, hypertension, or asymptomatic proteinuria. Glomerular and peritubular endothelium and renal tubular cells are especially radiosensitive. There are no pathognomonic histological features, but renal pathology may be similar to that of haemolytic uraemic syndrome. Radiation nephropathy may be prevented by renal shielding and mitigated by radiation dose fractionation. Control of hypertension is important and angiotensin-converting enzyme inhibitors and angiotensin II receptor antagonists appear to have protective effects beyond those of blood pressure control.

The endocannabinoids are part of a large family of lipid signaling molecules derived from arachidonic acid, including the prostaglandins and leukotrienes, which are important mediators of inflammation. Far less is known about the newer members of the endocannabinoid group, and it remains unclear whether they all play important functional roles. This chapter reviews the multiple members of this family and their biosynthesis and inactivation. Physiological functions, including retrograde synaptic signaling, control of energy metabolism, regulation of pain sensitivity, and cardiovascular control, are discussed. In addition, the chapter reports the synthesis of novel agonists, antagonists, and compounds inhibiting endocannabinoid inactivation as novel medicines.

This chapter discusses impulse control disorders in children 12 years of age and younger. In this age group, the available research focuses on fire setting/pyromania, trichotillomania, and pathological gambling. Less well studied are kleptomania, intermittent explosive disorder, and the impulse control disorder not otherwise specified, pathological skin picking. Clinical presentation, diagnosis, epidemiology, age of onset, risk factors, sociocultural factors, and comorbidity will be reviewed. Psychotherapeutic interventions for the age group will be explored, with particular emphasis on behavioral therapy. The available literature on psychopharmacological treatments, particularly selective serotonin reuptake inhibitors and opioid antagonists, will also be explored.

Kleptomania, a disabling impulse control disorder, is characterized by repetitive and uncontrollable theft of items that are of little if any use. Kleptomania often goes undiagnosed or is misdiagnosed as a mood disorder, obsessive-compulsive disorder, or a substance use disorder. Unlike typical shoplifters, individuals with kleptomania steal for symptomatic relief rather than personal gain. Although the etiology of kleptomania is unknown, various biological and psychosocial theories may explain why some individuals develop kleptomania. Although cognitive behavioral therapy has shown early promise in treating kleptomania in case reports, the only controlled data for treatment involve the use of the opioid antagonist naltrexone.

Hormones are highly biologically active endogenous compounds that control the growth, development, physiology, and homeostasis of numerous organ systems. Because of this, they have long been thought likely to play key roles in both normal and abnormal (malignant) growth. They are also noteworthy for being produced away from the tissues that they control, and are thus secreted into circulating blood to reach their target organs. This combination of potent, targeted agents of growth and development that can be measured in available biologic fluids has made steroidal and peptide hormonesparticularly susceptible and relevant to epidemiologic investigation. In addition, medications containing hormones and hormone antagonists have come into widespread use, providing further opportunities for epidemiologic insights into hormonal carcinogenesis. The development of increasingly more accurate assays to measure sex hormones and their metabolites has resulted in major advances in understanding the hormonal etiology of breast and gynecologic malignancies.

Hormone therapy describes the role of hormones in the growth of a variety of cancers, and the therapeutic effects of manipulation of hormone levels in these diseases. Sex hormones stimulate the growth of breast and prostate cancers, many of which respond to surgical removal of the hormone-secreting gonad. Pharmacological measures to deliver hormone therapy in these diseases include luteinising hormone releasing hormone (LHRH) agonists and antagonists, inhibitors of sex hormone synthesis, and inhibitors of hormone-receptor binding. These treatments have established benefits in both in the control of advanced disease and the adjuvant therapy of early-stage disease. The pros and cons of combination hormone therapy are discussed. Resistance to hormone therapy may be primary or acquired, and the likely mechanisms are described.

Drug abuse and incentive learning explains how abused drugs, including nicotine, ethanol, marijuana, amphetamine, cocaine, morphine, and heroin, produce conditioned place preference and are self-administered; dopamine receptor antagonists block these effects. Stimuli that become reliable predictors of drug reward produce burst firing in dopaminergic neurons, but the drug retains its ability to activate dopaminergic neurons. Thus, repeated drug users experience two activations of dopaminergic neurotransmission, one upon exposure to the conditioned stimuli signaling the drug and another upon taking the drug. This may lead to long-term neurobiological changes that contribute to withdrawal and addiction. Withdrawal can be remediated by abstinence but this does not reduce the conditioned incentive value of cues associated with drug taking; those cues can lead to relapse. Effective treatment will include detoxification and systematic exposure to drug taking-associated conditioned incentive stimuli in the absence of drug so that those stimuli lose their ability to control responses.
