Abstract Abstract 5122 T-cells are the most important mediators of antileukemic reactions after allogeneic stemcelltransplantation (SCT) or donorlymphocyte-infusions (DLI) in patients (pts) with myeloid leukemias (AML,MDS), although relapses occur. Presentation of leukemic antigens (LAA) is improved by conversion of leukemic cells to leukemia derived DC (DCleu). Our hypothesis is, that myeloid blasts convert in vivo spontaneously to DCleu and prime CTL; moreover DCleu can be generated ex vivo from blasts and be used to prime T-cells enabling the generation of leukemia-specific CTL. However, inhibition of NOTCH signaling with γ-secretase inhibitors (GSIs) has shown limited antileukemic activity in clinical trials. Here we performed an expression-based virtual screening to identify highly active antileukemic drugs that synergize with NOTCH1 inhibition in T-ALL. Among these, withaferin A demonstrated the strongest cytotoxic and GSI-synergistic antileukemic effects in vitro and in vivo. Methotrexate (MTX) and mercaptopurine (MP) are widely used antileukemic agents that inhibit de novo purine synthesis (DNPS) as a mechanism of their antileukemic effects. To elucidate pharmacodynamic differences among children with acute lymphoblastic leukemia (ALL), DNPS was measured in leukemic blasts from newly diagnosed patients before and after therapy with these agents. Patients were randomized to receive low-dose MTX (LDMTX: 6 oral doses of 30 mg/m2) or high-dose MTX (HDMTX: intravenous 1 g/m2) followed by intravenous MP; or intravenous MP alone (1 g/m2), as initial therapy. Ces approches sont basées sur une amélioration de la production de cellules NK à partir de cellules souches hématopoïétiques (quantitative et qualitative en améliorant leur activité lytique), mais aussi sur une manipulation de la sensibilité des cellules leucémiques

AbstractThe cellular basis of cancer immune surveillance, already hypothesized in ancient times, was only proven with the advent of HCT. Indeed, the discovery of the nature of GVHD and its antileukemic effects (Weiden et al. 1979) was followed by the first successful attempts of adoptive immunotherapy using donor leukocytes (Kolb et al. 1990). To address the significant GVHD risk associated with allogeneic T cells, several approaches of T-cell manipulation were developed and tested (Table 60.1). Some of these strategies rely on the genetic manipulation of T cells. First, suicide gene therapy a
