Abstract:
Abstract Pre-malignant cells disseminate during very early and sometimes asymptomatic stages of tumor progression. Once settled in permissive environments at secondary organs, they can act as a source for metastasis (e.g. in breast, pancreatic, colon, melanoma and lung carcinomas), thus emerging as important players in future prognosis. Granted that biological barriers to tumorigenesis exist, there must also be limiting steps to early dissemination in pre-malignant cells, all of which remain largely unknown. We report that the orphan nuclear receptor NR2F1/COUP-TF1, downstream of Her2 signaling via p38alpha serves as a barrier to early dissemination in pre-malignant breast cells. Our work shows that NR2F1 expression is positively regulated by p38α-MAPK pathway and inhibited by HER2 oncogenic signaling pathway. Furthermore, NR2F1 downregulation in human and murine pre-malignant cell lines led to a partial epithelial-to-mesenchymal transition characterized by decreased E-cadherin expression and β-catenin membrane localization, disorganized laminin 5 deposition, and increased expression of CK14, TWIST1, ZEB1 and PRRX1, thus explaining an increased motility and dissemination of early cells upon loss of NR2F1. Interestingly, NR2F1 lost also maintained epithelial markers (like CK18), suggesting an hybrid luminal-mesenchymal phenotype that could favor cellular plasticity to adapt to distant sites. We also showed that NR2F1 blocked b-catenin nuclear localization via a WNT-dependent pathway. Significantly, downregulation of NR2F1 increased the in vivo dissemination potential of pre-malignant cells to lungs in a HER2-dependent manner. Lastly, Nr2f1 expression was downregulated in non-invasive DCIS biopsies when compared to benign adjacent tissue, and its expression was inversely correlated to Prrx1 levels. The inverse correlation between Nr2f1 and Prrx1 levels was lost in genetically evolved primary tumors, which suggests that a unique inhibitory mechanism of dissemination dominates at early stages of tumor progression. Overall, our data suggest the existence of an inhibitory mechanism of dissemination regulated by NR2F1 downstream of HER2 signaling in premalignant breast cells and propose that therapeutic strategies that aim to maintain NR2F1 expression could reduce dissemination of premalignant cells and delay relapses. O objetivo deste artigo é investigar os percursos éticos do cantar no discurso de denúncia social em três canções de protesto. Percebemos a emergência de reflexões a respeito do cancioneiro latino-americano (hispano-falante, em nosso caso) pelo tratamento dado à denúncia social que tem dispensado em seu discurso. Especificamente, estudamos as seguintes canções: “Al centro de la injusticia”, letra de Violeta Parra, interpretada por Isabel Parra; “Cuando tenga la tierra”, letra de Daniel Toro e Ariel Petrocelli, interpretada por Mercedes Sosa e “Si se calla el cantor”, letra de Horácio Guarany, cantada por ele e por Mercedes Sosa. No aporte teórico, revisamos a noção de ato ético, conforme discutida na reflexão bakhtiniana. O procedimento de análise é o cotejamento de textos e os resultados apontam que o ato de cantar, nas canções ora analisadas, constitui a luta traduzida em ato ético; cantar se torna colocar o corpo (e a voz) na luta de classes.

Abstract Background: The Metastatic Breast Cancer Project (MBCproject) is an ongoing research study that directly engages patients (pts) through social media and advocacy groups, and empowers them to share their biospecimens, clinical history, and experiences. The goal is to create a publicly available dataset of linked clinicogenomic and pt-reported data to enable research. From 2015-10-20 to 2020-3-31, 3,245 MBC pts who received treatment at >1,700 institutions, consented to share medical records, pt-reported data, and tumor/saliva/blood samples and to have genomic analysis performed. Here, we describe an analysis of the clinical and genomic features in this initial MBCproject cohort. Methods: We performed whole exome sequencing (WES) on 379 tumors (with matched germline) from 301 pts, 377 germline samples and RNA sequencing (RNA-seq) from 200 tumors from 141 pts. In 14 pts, we characterized 2 or more serial tumor biopsies. WES data was analyzed for mutations and copy number variants. RNA-seq data was used to call fusions, research-grade PAM50, and gene set enrichment scores. Medical records and pt-reported data were abstracted to create detailed clinical record for each pt. Results: WES of 249 metastatic tumors identified 34 cancer genes (e.g., TP53, PIK3CA, CDH1, PTEN, AKT1, NF1, ESR1) that were significantly recurrently altered. Potential clinically actionable alterations were identified in 39% of metastatic tumors. 45 tumors (22.5%) had fusions with known functional effects and 24 (12%) had in-frame fusions in key cancer genes like FANCD2 (3), FGFR3 (2), ESR1 (1), BRAF (1), and NCOR1 (1). PAM50 classification suggested depletion of Luminal A subtype in this cohort compared to TCGA breast cancer cohort (p-value <0.05). Of 29 pts with paired RNA-seq biopsies, 10 pts showed a PAM50 subtype switch [LumB to Basal = 1, Her2 to Basal =1, Basal to LumA = 1, Her2 to LumA = 2, LumA/B to Her2 = 3, LumA to LumB = 2]. Germline analysis showed that 30.2% (114/377) of pts had at least one pathogenic variant in a cancer predisposition gene, including BRCA1/BRCA2 (5%), NF1 (5.8%), ATM (1.06%) and PALB2 (1.59%). In 10.9% pts (33/301), pathogenic variants in a cancer predisposition gene was accompanied by an apparent somatic loss of function event in the same gene. There was ~ 90% concordance between abstracted features (e.g., receptor status, histology, diagnosis dates, metastatic sites) and pt-reported data, enabling the use of both data types for integrated analyses. The utility of integrated analyses is illustrated by a case study of a pt who had an initial diagnosis of ductal carcinoma in situ and developed metastatic disease 5.5 yrs later. Analysis of 3 metastatic tumors (WES and RNA-seq) and a circulating tumor DNA sample (WES only) from this pt collected over the course of therapy revealed evidence of tumor evolution and multiple mechanisms of resistance to endocrine therapies and CDK4/6 inhibitors, including 2 distinct acquired ESR1 mutations, an activating BRAF fusion, and estrogen receptor (ER) loss by immunohistochemistry with concomitant development of ESR1-PLEKHG1 in-frame fusion. RNA-seq data also showed a PAM50 luminal phenotype and a persistent ER. signature throughout despite the apparent ER loss, suggesting compensation by the ESR1 fusion. Analyses of 13 additional pts with serial biopsies will be presented. Integrated analyses of several additional cohorts of interest, such as de novo MBC (91 pts), resistance to CDK4/6 inhibitors (39 pts), and pts diagnosed with breast cancer <40 yrs, will also be presented. Conclusion: This clinicogenomic dataset generated by partnering directly with pts was uniquely built with pt-reported data, medical record data, and multi-omic characterization, and serves as a powerful tool for researchers to harness to accelerate discoveries in MBC. Abstract The Metastatic Breast Cancer Project (MBCproject) is an ongoing research study that directly engages patients (pts) through social media and advocacy groups, and empowers them to share their samples, clinical information, and experiences. The goal is to create a publicly available dataset of linked genomic, clinical, and pt-reported data to enable research. In collaboration with pts, advocates, and advocacy groups, a website (MBCproject.org) was developed that allows pts with metastatic breast cancer (MBC) anywhere in the US or Canada to register. From 10/20/15-6/1/21, 6100 patients with MBC registered for the MBCproject. Registered pts are sent an online consent form that asks for permission to obtain and analyze their medical records and samples. Consented pts are sent a saliva and/or blood kit and asked to mail back a saliva sample, which is used to extract germline DNA, and/or a blood sample, which is used to extract germline DNA and cell free DNA (cfDNA). We contact participants’ medical providers to obtain medical records and a portion of their stored tumor biopsies. 3456 pts receiving care at over 1700 different institutions have consented to share medical records and tumor/saliva/blood samples and to have genomic analysis performed. Whole exome sequencing (WES) is performed on tumor DNA, germline DNA, and cfDNA; transcriptome sequencing (RNA-seq) is performed on tumor RNA. Medical records and pt-reported data are abstracted to create a detailed clinical record for each pt. Table 1 highlights clinical data collection, biospecimen acquisition, and genomic data generation to date. Examples of clinicogenomic analyses are shown in Table 2. De-identified linked genomic, clinical, and pt-reported data is shared regularly via public databases (mbcproject.org, cBioPortal, dbGaP, NCI Genomic Data Commons). To date, this data has been cited in over 40 publications. Study updates are shared with participants regularly. The MBCproject continues to enroll new patients, generate additional data, and perform integrated clinical and genomic analyses with the goal of building a dataset that is representative of patients with MBC. We have partnered with over 30 non-profit breast cancer advocacy groups. We also have several community engagement efforts underway to more directly reach patients in underrepresented communities, including partnerships with faith-based organizations and colleges/universities, as well as targeted engagement with the African American community. In addition, in partnership with Latinx patients, advocates, and researchers, a Spanish-language version of the MBCproject was launched in June 2021. Partnering directly with pts rapidly enables thousands of pts to remotely share tumors, blood, saliva, and medical records to accelerate research. The resulting publicly shared clinically annotated dataset is a resource that allows researchers to identify patients with specific phenotypes, who have often been challenging to identify with traditional approaches. AbstractThe objective of this work is analysing absolute interrogative sentences from the Spanish of San Antonio de Texas using three corpus featuring different levels of spontaneity (formal ad hoc, situational and conversational), and uttered by native speakers with superior studies. The choice of these corpus and their acoustic analyses have been performed according to the methodology established by the international project AMPER (Atlas Multimédia Prosodique de l’Espace Roman). Furthermore, we have undertaken the phonetic-phonological labelling of the boundary tones and of initial and nuclear pitch accents within the framework of the autosegmental-metrical model. This study has been carried out with the aim of contributing to the knowledge of Texan intonation, in order to make comparisons with other varieties of Spanish language as well as to include it in the international Atlas AMPER. We hypothesize that interrogatives from the informants considered follow the patterns described for Mexican interrogatives (Sosa, 1999; Ávila, 2003; Martín Butragueño, 2004, among others), taking into account the geographical location of San Antonio de Texas in relation to Mexico and the influence of Mexican immigration, which has a strong presence in San Antonio. Results provide evidence of such influence, particularly in the final ascent of the F0.

El siguiente trabajo tiene por objeto acercarse a las “mudanzas” que Antonio Gamoneda realizó en 2016 a partir de una serie de cantos del rey-poeta Nezahualcóyotl. El objetivo es poner en relación su actividad traductora con un ejercicio de reanimación del verbo. El poeta, a modo de chamán, rescata del lenguaje su energía o, dicho de otro modo, lo dota de nueva vida como si de un organismo se tratase. Afín a este proceso queda la idea de un despertar las propiedades musicales de la voz, primer paso del traductor antes de fijar su forma definitiva. El recorrido realizado será articulado por medio de aspectos o conceptos clave como lo cultual, el pharmakon, la naturaleza del cantor o la dialéctica entre dos modelos de lenguaje.

Es bien conocido el gusto de Franco por la copla y la canción española que contribuía a reforzar el sentimiento nacional español. Cada año, en la conmemoración del 18 de Julio, las estrellas del momento como Conchita Piquer, Juanita Reina, Lola Flores, Antonio Amaya, Antonio Molina etc. eran invitadas a interpretar las canciones favoritas del Generalísimo. Entre los intérpretes de la canción española, Manolo Escobar fue uno de los más populares en los años 60 y 70, a pesar del impacto de la música ye-ye y de los Beatles entre los jóvenes. Su repertorio evoluciona durante los sesenta de la canción andaluza hacia la canción española que canta a España y al pueblo español en general. Sus canciones se dirigen a un público sencillo, describiéndose a sí mismo como el cantor del pueblo. De 1963 a 1980 fue el protagonista de películas taquilleras, una cada año, y su abundante discografía muestra su creciente popularidad. ¿Cuál fue la clave de su éxito? ¿Existe algún vínculo entre su éxito y el apoyo del régimen? Finalmente, ¿de qué manera las canciones de Escobar han contribuido a crear un sentimiento de orgullo nacional?

Además, siempre se ha estudiado a partir de las fuentes islámicas, obviando las cristianas. Si es cierto que para la Edad Media las fuentes cristianas no ofrecen demasiada información y están además viciadas por su carácter polémico, también lo es que para la Edad Moderna, con el surgimiento de un tipo de literatura sobre el islam que está alejado de la tradición polémica, disponemos de un buen número de importantes fuentes cristianas que merecen ser tenidas en cuenta en cualquier análisis histórico de la religiosidad islámica. Es el caso especialmente de la Topographía e Historia general de Argel (1612), cuyas descripciones y observaciones resultan de una riqueza sin precedentes.PALABRAS CLAVE: Edad Moderna, cinco pilares del islam, religiosidad, España, Argel.ABSTRACTTraditionally, the historical study of Islamic religiosity has been faced with the problem of the scant amount of sources. Moreover, it has always been approached on the basis of Islamic sources, disregarding the Christian ones. If for the Middle Ages Christian sources do not present much information about the subject and this is tainted by its polemical character, for the Early Modern Age, with the emergence of a new kind of literature about Islam that does not belong to the polemical tradition, we have at our disposal a good number of sources of information. Analizamos a descuberta do galego do Bierzo a través da reflexión propiciada polo filólogo catalán Mariano Cubí e o propio encargo de escribir nesta lingua que asume Morales con entusiasmo. Indagamos nas fontes e na procura de temáticas para o libro. Emerxen así os costumes das xentes humildes que o poeta gusta describir, tanto na aldea como na vila, ao longo dun calendario tradicional peculiar, onde detectamos unha tendencia ao contraste e a utilización dun ton satírico e crítico sutil por veces, as máis proto-realista. A paisaxe natural e humana do Bierzo que retrata con vigor e detalle, fará do noso autor un cantor referente da identidade berciana á vez dun fito relevante para definir o movemento do Rexurdimento nos lindeiros da galeguidade. Abstract Influenced by feminist perspectives on urban and architectural studies, this essay examines the relationship between premodern Muslim urbanism and gendered relations, an issue that has received little attention in Hispanic cultural studies to date. This discussion will center on two particular features of domestic architecture and city planning typical of sixteenth- and seventeenth-century Algiers: the location and nature of street-facing windows and the communicable rooftops. Focusing primarily on Christian writers such as Antonio de Sosa, Miguel de Cervantes, and Lope de Vega, Velasco explores how their accounts document the ways in which women in Algiers could find unexpected and illicit ways of turning the high windows and the communicable rooftops to their advantage. Despite the constraints presented by an urban design and domestic architecture intended to segregate the sexes and control behavior, women in early modern Algiers (many of whom were recent immigrants from Spain) found ways to utilize their built environment to observe and navigate the world around them, gain access to a public domain commonly reserved for men, and sustain networks with other women in ways that might bypass traditional spatial, social, and personal restrictions and limitations. Through female-only gatherings and other forms of camaraderie that made use of traditional Islamic urbanism, Muslim women in Algiers found ways to challenge long-standing dichotomies of male/female, public/private, interior/exterior, visible/hidden, and chaste/lustful.

Em “Águas do meu princípio”, presente em Pensatempos, livro lançado em 2005, o qual reúne textos de opinião de vária ordem, Mia Couto, ao relembrar a própria infância na Beira – sua terra natal e importante cidade portuária de Moçambique –, conduz os leitores através de uma ambiência que flerta, nostalgicamente, com uma atmosfera mítica. Nessas linhas, o escritor moçambicano convoca um importante legado cultural oriundo da Bahia: a célebre canção “É doce morrer no mar”, do cantor e compositor Dorival Caymmi, cujos versos são atribuídos a Jorge Amado e a elementos textuais que permeiam Mar Morto, romance publicado, em 1936, pelo escritor brasileiro mais reverenciado nas literaturas africanas de língua oficial portuguesa. A partir de tais reminiscências e, sobretudo, como exercício de crítica literária, poder-se-á estabelecer relações de contiguidade entre excertos de “Águas do meu princípio” e a estrutura interna que rege o núcleo de maior pulsão poética em O Último Voo do Flamingo, romance de 2000. Para tanto, as reflexões de Antonio Candido, Roland Barthes, Benjamin Abdala Junior, Pierre Bourdieu, entre outros, serão implicadas neste estudo a fim de demonstrarem os elos literários e musicais que ligam Brasil e Moçambique através dos séculos.

These neurofibromas are typically benign and called Plexiform Neurofibromas (PNFs), in some cases, they can transform into malignant peripheral nerve sheath tumors (MPNSTs). MPNSTs are highly aggressive and invasive cancers that contribute to a significant proportion of soft tissue sarcomas. The exact mechanisms behind this transformation are not fully understood, however it involves additional genetic mutations and alterations in cellular signaling pathways. With limited effective targeted therapies available, surgical removal remains the primary treatment option. However, due to tumor size and location, surgery may not always be feasible. The MEK inihibitor, selumetinib, has received approval for the treatment of PNFs, however some patients who were treated with selumetinib continued to develop MPNSTs suggesting that MEK inhibitors may have limitations in addressing MPNSTs. To target the RAS pathway and mitigate the effects of NF1 gene alterations, we aim to explore the therapeutic potential of two novel, potent and orally bioavailable small molecule inhibitors targeting SOS1 (BI 1701963) or KRAS (KRASmulti inhibitor BI 3706674), as alternative treatment options for MPNSTs. The KRASmulti inhibitor BI 3706674 binds non-covalently to the KRAS wild type allele as well as multiple KRAS mutant alleles in the GDP-bound state and thereby disrupts KRAS signaling. Therefore, we hypothesize that both inhibitors will work in NF1 models by reducing activated KRAS levels. In this study, a series of in vitro and in vivo experiments were conducted to evaluate the efficacy of BI 1701963 and BI 3706674 inhibitors, both individually and in combination. The studies were done in comparison to the MEK inhibitor selumetinib. In vitro analysis revealed a synergistic anti-proliferative effect when combining BI 1701963 and BI 3706674 inhibitors both in 2D and 3D growth conditions, which led to the downregulation of the RAS pathway, specifically the MAPK and PI3K signaling cascade, and an increase in apoptosis. The in vivo efficacy of either BI 1701963 or BI 3706674 inhibitor alone or in combination was tested in four different in vivo MPNST models. We observed significant tumor growth stabilization as well as tumor regression in all tested models in vivo. In-depth pathway modulation and pharmaco-dynamic studies are ongoing. In conclusion, this study highlights the combined effects of SOS1 (BI 1701963) and KRAS (BI 3706674) inhibition in MPNSTs. The combination therapy of these novel therapeutics may hold promise as a potential treatment strategy for patients with MPNSTs. Abstract Extracellular matrix (ECM) protein expression/deposition within and stiffening of the breast cancer (BC) microenvironment, facilitates disease progression and correlates with poor patient survival. However, the mechanisms by which ECM components control tumorigenic behaviors and responses to therapeutic intervention remain poorly understood. Fibronectin (FN) is a major ECM protein controlling multiple processes related to cancer metastasis. In this regard, we previously reported that DHPS-dependent hypusination of eIF5A1/2 is necessary for fibronectin-mediated BC metastasis and epithelial to mesenchymal transition (EMT). Here, we explored the clinical significance of an interactome generated using hypusination pathway components and markers of intratumoral heterogeneity. Solute carrier 3A2 (SLC3A2 or CD98hc) stood out as an indicator of poor overall survival among patients with basal-like BCs that express elevated levels of DHPS. We subsequently discovered that blockade or knockdown (KD) of DHPS or SLC3A2 reduced BC spheroid growth. Interestingly, spheroids stimulated with exogenous fibronectin were less sensitive to inhibition of either DHPS or SLC3A2 – an effect that could be abrogated by dual DHPS/SLC3A2 blockade or KD. In this regard, we observed that inhibition of both SLC3A2 and DHPS reduced total and hypusinated eIF5A1/2 levels most effectively. We further discovered that a subset of BC cells responded to fibronectin by increasing cytoplasmic localization of eIF5A1/2. Notably, these fibronectin-induced subcellular localization phenotypes correlated with a G0/G1 cell cycle arrest. Fibronectin-treated BC cells responded to dual DHPS/SLC3A2 blockade by shifting eIF5A1/2 localization back to a nucleus-dominant state, suppressing proliferation and further arresting cells in the G2/M phase of the cell cycle. Finally, we observed that dual DHPS/SLC3A2 inhibition increased the sensitivity of both Rb-negative and -positive BC cells to the CDK4/6 inhibitor palbociclib. Taken together, these data identify an uncharacterized mechanism through which extracellular fibronectin controls cancer cell tumorigenicity by modulating subcellular eIF5A1/2 localization. These findings provide prognostic/therapeutic utility for targeting the cooperative DHPS/SLC3A2 signaling axis to improve BC treatment responses. Lelia Inés Albarracín es una destacada lingüista argentina, especializada en la lengua quichua y en educación intercultural bilingüe. Nacida en Tucumán, Argentina, es profesora en francés egresada de la Facultad de Filosofía y Letras de la Universidad Nacional de Tucumán (UNT). Ha desempeñado un papel fundamental en la enseñanza y promoción del quichua en Argentina. Durante más de una década, dictó cursos de Lingüística Quechua en la Facultad de Filosofía y Letras de la UNT. Desde 2011, es profesora de Lingüística Quichua y del Taller de Lengua Quichua en la Tecnicatura Superior Universitaria en Educación Intercultural Bilingüe con Mención en Lengua Quichua, en la Facultad de Humanidades, Ciencias Sociales y de la Salud de la Universidad Nacional de Santiago del Estero (UNSE). Esta carrera, creada a partir de un proyecto de su autoría, es la primera en Argentina enfocada en una lengua indígena con aprobación ministerial. Su labor investigativa se centra en la lingüística descriptiva, las políticas lingüísticas y el contacto entre el castellano y el quechua. Abstract Background: Evidence-based recommendations support a dietary pattern rich in whole grains, fruits, and vegetables with limited consumption of added sugars to improve cancer risk and mortality. The Optimal Living Program (OLP) is a prospective multiparametric lifestyle intervention that engages early-stage breast cancer patients at the time of diagnosis in risk-stratified, personalized lifestyle management. Here, we report baseline dietary patterns among women enrolled in the OLP. Methods: Upon enrollment, patients complete a questionnaire that incorporates the National Cancer Institute (NCI) Dietary Screener Questionnaire (DSQ), which queries the consumption frequency of 26 food items over the past month. Based on standard NCI DSQ scoring procedures, we calculated total daily serving equivalents of all food items within a dietary factor group. This included: 1) total daily cup equivalents of fruit and vegetables from fruit, fruit juice, salad, potatoes, beans, other vegetables, tomato sauce, salsa, and pizza, 2) total daily ounce equivalents per day of whole grains from cereal, whole grain bread, popcorn, and whole grain rice, and 3) total daily teaspoon (tsp) equivalents of added sugars from cookies/cake/pie, doughnuts, ice cream, candy, cereals, and sugar-sweetened beverages including soda, fruit drinks, and sugar/honey in coffee/tea. Results: There were 100 patients enrolled in the OLP for whom baseline DSQ data were available. The median age at diagnosis was 58 and median BMI was 27.4 mg/m2. 69 patients were white (69.0%), 14 were Black (14.0%), and 7 were Asian (7.0%). 62 patients had stage I disease (62.0%), 22 patients had stage II disease (22.0%), and 15 patients had ductal carcinoma in situ (15.0%). 74 patients had hormone receptor-positive disease (74.0%), 3 had HER2-positive disease (3.0%), and 7 had triple-negative disease (7.0%). Daily frequency of consumption of dietary factor groups are shown in Table 1. Only 29 patients (29.0%) met the current dietary guideline of 4-5 cup equivalents of fruits and vegetables per day and no patients (0%) met the guideline of more than 3 ounce equivalents of whole grains per day. Added sugar intake ranged from 0 to 4.8 teaspoon equivalents per day. Conclusion: Most women in this cohort with newly diagnosed early-stage breast cancer did not consume the recommended daily intake of fruits, vegetables, and whole grains per the 2020-2025 Dietary Guidelines and the American Institute for Cancer Research. Our findings identify these dietary factors as important targets of intervention with personalized dietary guidance at the time of breast cancer diagnosis. Abstract Introduction Premenopausal women diagnosed with hormone receptor-positive (HR+)/HER2-negative (HER2-) early breast cancer (EBC) face a significantly higher risk of cancer recurrence and mortality compared to older women, particularly those diagnosed under the age of 40. The underlying causes behind this disparity remain largely unknown. Material and methods We retrospectively select tumour samples from patients (pts) diagnosed with HR+/HER2- EBC at Hospital Clinic (Barcelona, Spain) from May 2014 to March 2020 who had a PAM50/Prosigna assay (Veracyte) previously performed. This assay examines the expression of 50 genes involved in important breast cancer signaling pathways, as well as the intrinsic subtypes (IS) and the Risk of Recurrence (ROR) score. Our primary objective was to compare the distinct clinicopathological and molecular characteristics of HR+/HER2- EBC between two age groups: patients under 40 years old (Young EBC or YEBC) and those 40 years old and above (Older EBC or OEBC). Disease-free survival (DFS) was defined as the time between diagnosis and the first recurrence. Comparisons between groups were carried out using chi-square tests (categorical variables). Additionally, we performed a two-class unpaired Significance Analysis of Microarrays (SAM) with a false discovery rate (FDR) less than 5% to identify genes with significantly different expression between YEBC and OEBC. The statistical significance level was set at less than 0.05. Results A total of 441 tumor samples from 420 patients were included in the analysis, with 5% (22/441) corresponding to YEBC. With a median follow-up of 64.5 months, there were 20 breast cancer (BC) recurrences (4.8%), 12 second cancers (2.9%) and 5 BC-related deaths (1.2%), all occurring in the OEBC group. Table 1 provides an overview of the main clinico-pathological and molecular characteristics. YEBC tended to have larger tumors, with 45.5% (10/22) measuring over 2 cm compared to 36.1% (151/419) in OEBC (p=0.376). YEBC also had a higher proportion of grade 3 tumors (18.18% versus 3.1% in OEBC, p=0.418) and lower expression of estrogen receptor (ER) as determined by immunohistochemistry (mean of 72.28% vs 88.36%, p=0.004). Regarding the PAM50 IS, there was a difference observed between YEBC and OEBC (p < 0.001). Specifically, YEBC patients exhibited a tendency towards a higher proportion of Luminal B tumors (54.6% versus 38.9% in OEBC, p=0.144), as well as non-Luminal subtypes such as Basal-like (18.2% versus 0.95% in OEBC, p< 0.001) and HER2E (4.6% versus 0.7% in OEBC, p=0.186). YEBC tended to have higher ROR (p=0.167), and to receive chemotherapy in a higher proportion, especially in the intermediate-risk group (p=0.004). In YEBC tumours, SAM analysis (p < 0.05) showed an overexpression of genes mainly related with proliferation (i.e., RRM2, MELK, MIK67), cell cycle progression (i.e., KNTC2, CDC20, MYBL2) and apoptosis inhibition (i.e., BIRC5). In OEBC there were an overexpression of genes related with the oestrogen receptor-signalling pathway (i.e., ESR1, NAT1, FOXA1, PGR). Luminal B tumours in YEBC showed an upregulation of genes related to proliferation, migration, and poor prognosis (i.e., KRT17, KRT5) compared to OEBC. However, since there were no recurrences in the YEBC group, differences in terms of DFS between the groups could not be calculated. Abstract Purpose: Patients with triple negative breast cancer (TNBC) with residual disease after neoadjuvant chemotherapy (NAC) have high risk of recurrence with prior data suggesting improved outcomes with capecitabine. Targeted agents have demonstrated activity across multiple cancer types. BRE12-158 was a phase 2, multicenter trial that randomized TNBC patients with residual disease after NAC to genomically-directed therapy vs. treatment of physician choice (TPC). Patients and Methods: From March 2014 to December 2018, 197 patients were enrolled. Residual tumors were sequenced using a next generation sequencing (NGS) test. A molecular tumor board adjudicated all results. Patients were randomized to 4 cycles of genomically-directed therapy (arm A) vs. TPC (arm B). Patients without a target were assigned to arm B. Primary endpoint was 2-year disease free survival (DFS) among randomized patients. Secondary/exploratory endpoints included: distant disease free survival (DDFS), overall survival (OS), toxicity assessment, time-based evolution of therapy, and drug-specific outcomes. Results: 193 patients were randomized or were assigned to arm B. The estimated 2-year DFS was 56.6% (95%CI:0.45-0.70) for arm A vs. 62.4% (95%CI:0.52-0.75) for randomized arm B. No difference was seen in DFS, DDFS, or OS for the entire or randomized populations. There was increased uptake of capecitabine for TPC over time. Patients randomized later had less distant recurrences. ctDNA status remained a significant predictor of outcome with some patients demonstrating clearance with post-neoadjuvant therapy. Conclusion: Genomically directed therapy was not superior to TPC for patients with residual TNBC after NAC. Capecitabine should remain the standard of care; however, the activity of other agents in this setting provides rationale for testing optimal combinations to improve outcomes for this high-risk population. ctDNA should be considered a standard covariate for trials in this setting. Abstract Background: Diet is a modifiable risk factor for breast cancer risk and mortality. Current guidelines recommend a diet that provides a diverse array of nutrients, comprised predominantly of fruits/vegetables and whole grains, with limited added sugar. The Healthy Living Program (HLP) is a clinical program at Memorial Sloan Kettering Cancer Center for patients with early-stage breast cancer that offers longitudinal, personalized lifestyle management starting at the time of diagnosis. Here, we report dietary patterns among the HLP cohort and association with baseline body mass index (BMI). Methods: We included all patients enrolled in the HLP from September 2020-February 2022. At the time of enrollment, participants complete a survey containing the National Cancer Institute (NCI) Dietary Screener Questionnaire (DSQ), which consists of consumption frequency questions for 26 food items over the past month. Total daily intake equivalents are calculated for foods from every diet factor group according to standard NCI DSQ scoring as follows: 1) Total daily cup equivalents of fruits/vegetables, which includes fruit, fruit juice, salad, potatoes, beans, other vegetables, tomato sauce, salsa, and pizza; 2) Total daily ounce equivalents of whole grains, which includes cereal, whole grain bread, whole grain rice, and popcorn; 3) Total teaspoon (tsp) equivalents of added sugars from candy, doughnuts, cookies/cake/pie, cereal, ice cream, and sugar-sweetened beverages including soda, fruit drinks, and sugar/honey in coffee/tea. Adherence to recommended daily intake of fruits/vegetables, whole grains, and added sugars was assessed as per the 2020-2025 Dietary Guidelines, the American Institute for Cancer Research, and the World Health Organization guidelines. Patient and tumor characteristics were abstracted from medical records. Results: Among the 399 patients included, the median age at diagnosis was 58 and median baseline BMI was 26.1 kg/m2. 45 patients had carcinoma in situ (11.3%), 296 had stage I disease (74.2%), 51 had stage II disease (12.8%), and 7 had stage III disease (1.8%). 316 had hormone-receptor positive disease (89.3%), 24 had HER2-positive disease (6.8%), and 26 had triple-negative disease (7.3%). 106 participants (27%) met the guideline recommendation of ≥4-5 cup equivalents of fruits/vegetables daily and 3 participants (0.8%) met the guideline recommendation of ≥3 ounces equivalents of whole grains daily. All patients in the cohort met the guideline recommendation of < 6 tsp equivalents of added sugars daily. Only 2 patients (0.5%) met guidelines for all three diet factors. Baseline BMI was significantly higher among patients who did not meet the recommended fruit/vegetable intake than among those who did (26.9 kg/m2 vs. 24.5 kg/m2, p=0.016). There were no significant differences in BMI between those who did and did not adhere to the other diet factor guidelines and no significant association between tumor stage or histology and dietary guideline adherence. Conclusion: Most patients with early-stage breast cancer did not meet the recommended daily intake of fruits/vegetables or whole grains. Participants who did not meet the fruit and vegetable intake guideline had significantly higher BMI at diagnosis. These findings indicate that lifestyle assessment near the time of breast cancer diagnosis identifies patients that could benefit from personalized dietary interventions to optimize prognostic factors such as BMI. <strong>ABSTRACT</strong> Introduction: Vitamin D supplementation during the critical illness for patients with vitamin D deficiency has been studied yet not fully. Daily supplementation of the recommended dietary allowance (RDA) of vitamin D slightly improve the 25(OH) D serum concentrations in those patients with vitamin D deficiency. Methodology: A prospective observational study included 56 consecutive patients who admitted to the critical care department ICU, Cairo University. Patients were analyzed for serum level of 25-hydroxy vitamin D using ELISA technique and calculating APACHE II score on admission and SOFA score on admission (SOFA 0), after 48 hour (SOFA 1) and at discharge (SOFA 2). Results: The mean age of the studied patients was 63.2&plusmn;17.4 years,42 (75%) were males and 14 (25%) were females. The mean duration of hospital stay was 13.6&plusmn;7 days with mean APACHE II score and SOFA on admission, 48 hours and 96 hours post admission. &nbsp;The mean value of vitamin D in the whole studied population was 19.01&plusmn;13.95 ng/ml. The levels showed insignificant statistical difference between male and female genders (18.9 versus 19.2 ng/ml,p value 0.9). The mortality rate was 7.14% (out of the studied 56 patients 4/56 patients. By comparing the length of hospital stay (LOS), APACHE II, SOFA scores and vitamin D level between survivors and non-survivors; there was a statistically significant higher APACHE II score, SOFA scores (SOFA 0, SOFA 1and SOFA2) and a lower vitamin D level in non-survivors. vitamin D insufficiency is commonly observed in the critically ill patients. Vitamin D levels were found to be significantly lower in the critically ill non-survived patients and correlated significantly to APACHE II score on admission and SOFA scores. &#x0D; &#x0D; &#x0D; En el mundo andino, el Qhapac Ñam, el camino principal o del Inca, refiere a una red ancestral de rutas y de estructuras de comunicación, intercambio y defensa, cuya extensión conecta más de cuatro mil kilómetros a lo largo de la cordillera andina. Este sistema vial andino, que se consolidó en el siglo XV con el incanato, y que enlaza a través de sus senderos a Ecuador, Perú, Colombia, Bolivia, Chile y Argentina, resulta también una red para intercambiar e interconectar saberes (Sosa et al., 2020). Se trata de un espacio intenso (Vilca, 2020) que involucra modos de sentir y pensar el mundo; es decir, supone prácticas artísticas, culturales, sociales, políticas y económicas localizadas geopolíticamente. Así, es posible delimitar entre las regiones culturales y literarias continentales, la región andina (Rama, 1984; Palermo y Altuna, 1996) cuyos sentidos exceden los meros criterios geográficos y se acercan, más bien, a la forma de configurar los imaginarios culturales y sus procesos de identificación de manera situada.&#x0D; Advertimos que los saberes propios de lo que, hace unas décadas, se entendían como pueblos testimonios (Ribeiro, 1969; 2017) o de lo que, en los últimos años, se aglutina bajo la denominación de pueblos del Abya Yala (Arias, 2012; Muyulema, 2019) se conectan, dialogan y tensionan con los conocimientos inherentes a la modernidad. A propósito de estos contactos, las reflexiones de Ángel Rama (1984) y Antonio Cornejo Polar (1994) son fundacionales en el campo de la crítica y la teoría literarias latinoamericanas, entre otros referentes destacados de este campo (Bueno Chávez, 2012; Lienhard, 1990; Noriega Bernuy, 2011 y 2012; Zevallos Aguilar, 2009 y 2015). Asimismo, resulta ineludible la revisión de la producción literaria y antropológica de José María Arguedas, quien sin duda marca un punto de inflexión en el vínculo entre los saberes nativos, los mestizos y los modernos, que se hallan siempre en conflicto, desplazamiento y yuxtaposición. Como sucede con los senderos del Qhapac Ñam ̧ es ineludible volver a transitar los caminos fundacionales que interconectan formas de comunicación y de conocimientos; pero también este andar, en la actualidad, significa el desafío de proponer nuevas preguntas y abordajes, revisitar el corpus canonizado, proponer la incorporación de otros textos y entablar nuevos diálogos o formas de conexión.&#x0D; &#x0D; &#x0D;

Abstract Introduction: Paclitaxel induced peripheral neuropathy (TIPN) is one of the most important survivorship issues of cancer patients with the potential to substantially impact quality of life. TIPN can lead to dose reductions, early cessation, and has the potential to compromise treatment efficacy. We previously reported both rare (SBF2) and common germline (FCAMR) predictors for TIPN in patients of African descent through whole-exome sequencing (WES) and a genome-wide association study (GWAS), respectively, in the adjuvant, randomized phase III breast cancer trial, E5103. These predictors are now being evaluated in the prospective ECOG-ACRIN trial EAZ171 (NCT04001829). We have also previously reported our evaluation of common germline variants for patients of European descent through GWAS in E5103 with validation in another adjuvant breast cancer trial, E1199. Herein, we evaluated for rare variants that were associated with risk of TIPN in patients of European ancestry from WES in E5103. Methods: Gene-based rare variant analyses using SKAT were performed to compare select cases of severe TIPN (n=172) and controls without any evidence of TIPN (n=170) derived from E5103. Cases were defined as those with at least grade 3 TIPN and controls were optimally. matched by demographic and risk factors and had no reported evidence for TIPN of any grade. Germline WES was conducted using Ion Proton™ Sequencers from Life Technologies with an average &amp;gt;100X coverage of the exome. Rare variants with a minor allele frequency &amp;lt;3% predicted to be deleterious by protein prediction programs were retained for the analysis. 7,278 genes passed quality control and were included in the analysis, setting the threshold for significance at a p-value &amp;lt; 6.87 ×10-6 after Bonferroni correction. Results: Cytochrome P450 oxidoreductase (POR) was significantly associated with an increased risk of grade 3+ TIPN (p =1.82 ×10-6). Six variants predicted to be deleterious in POR were identified in the study population. Paclitaxel is predominately metabolized by cytochrome P450 (CYP) 2C8 and 3A4. A functional cytochrome P450 oxidoreductase is required for the catalytic activities of all 57 CYP family members to metabolize drugs, xenobiotics, and steroid hormones. Conclusion: Rare variants in cytochrome P450 oxidoreductase predicted an increased risk of severe TIPN in patients receiving paclitaxel. These rare variants in POR may modulate the catalytic activities of P450 enzymes that are responsible for paclitaxel metabolism and have a striking biological rationale as contributors to TIPN. Work is ongoing to validate these findings mechanistically and to identify strategies to nullify the impact of these deleterious variants. Abstract Breast cancer (BC) is among the most commonly diagnosed cancer in women and a leading cause of cancer death worldwide. Hispanic/Latina women represent a heterogeneous group with diverse genetic backgrounds, including varying proportions of Native American, European, African and to a lesser extent Asian ancestry. In the US, Hispanic and African American women have lower BC incidence overall yet higher incidence of triple negative breast cancer (TNBC). Furthermore, African American and Hispanic women tend to present with more aggressive disease, at younger ages and worse survival compared to women of European ancestry. These disparities have been attributed to biologic and non-biologic socioeconomic factors. Genomic analyses of Hispanic Mexican (HM) women have revealed specific genetic signatures distinct from those of non-Hispanic women. These variations include AKT kinase mutations activating PI3K/AKT/mTOR signaling pathway and PI3KCA mutations which could potentially benefit from targeted therapies. 4EBP1 phosphorylation by mTORC1 is generally considered a marker of activated mTOR signaling and high levels in tumors associate with worse outcomes in different cancers. To evaluate clinical significance of PI3KCA expression and PI3K/AKT/mTOR signaling, we performed immunohistochemical (IHC) analyses of tissue microarrays (TMA) including 272 BC patient samples from HM women. Protein expression of PI3KCA and p4EBP1 were evaluated quantitatively by receiver operator characteristics (ROC) curves based on area under the curve (AUC) as well as sensibility and specificity. p4EBP1 levels were associated with clinicopathological characteristics. Chi-squared and Fisher’s Exact tests showed higher p4EBP1 expression correlated with estrogen (P=0.001) and progesterone receptor negative BC (P=0.003), TNBC (P&amp;lt;0.001), tumor size (P&amp;lt;0.090), and advanced stage (P&amp;lt;0.001). PI3KCA expression was positively correlated with 4EBP1 expression (P=0.044) as shown by Mann Whitney U tests; but PI3KCA lacked prognostic value after screening through ROC curves. However, uni- and multi-variate Cox regression analyses did show that increased 4EBP1 expression correlates with worse OS. Overall, our study indicates that higher p4EBP1 expression is an independent prognostic factor of worse OS in BC patients. The activation of 4EBP1 may result from several upstream oncogenic alterations or mutations, representing a potential therapeutic target and biomarker. Further research is necessary to better define biological differences in Hispanic women to advance understanding and targeting of health disparities. Abstract Alterations of KRAS are found in approximately one in seven of all human cancers, making KRAS one of the main oncogenic drivers in cancer. Gain-of-function missense mutations in KRAS are found in ~90% of pancreatic cancer, ~40% of colorectal cancer and ~30% of lung adenocarcinoma. In addition, focal high-level amplification of the KRAS wild-type (wt) allele, an alternative means of activating this oncoprotein, is observed in ~7% of cancers and lacks effective targeted therapies. Thus, there is an urgent need to identify effective therapies to address KRAS-driven tumors. Recently, we have reported the identification of compounds active against a broad range of oncogenic KRAS variants (KRASmulti) that selectively inactivate downstream signaling and tumor growth (Kim et al., Nature 2023). Herein we describe BI 3706674, a novel, potent and orally available small molecule inhibitor of the KRAS oncogene. BI 3706674 binds non-covalently to multiple KRAS mutant alleles, including the KRAS wt allele, in the GDP-bound state and thereby disrupts oncogenic signaling. Using several in vitro assays, we show that BI 3706674 is a) highly selective for KRAS vs HRAS or NRAS; b) blocks the interaction between KRAS mutant/wt and its guanidine exchange factor (GEF) SOS1 and c) shows potent antiproliferative activity in isogeneic cell lines that are dependent on various KRAS mutant alleles. In two large cancer cell line panels, BI 3706674 potently and selectively inhibits proliferation of KRAS mutant cancer cell lines as well as cancer cell lines with an amplification of the KRAS wt allele. Significant pharmacodynamic (PD) biomarker modulation (including inhibition of ERK1/2 phosphorylation and DUSP6 mRNA down-regulation) was observed in cell lines with a KRAS wt relative copy number (CN) &amp;gt; 10, treated with BI 3706674. Amplification of the KRAS wt allele is most frequently observed in gastric (~5%), esophageal (~13%) and gastroesophageal junction cancers (~12%). BI 3706674 was well-tolerated and showed dose-dependent efficacy in cell line-derived and patient-derived xenograft models (CDX and PDX) of human gastric cancer with a KRAS wt CN &amp;gt; 10. A twice daily oral dose of 30 mg/kg was sufficient to induce significant tumor regression. BI 3706674 treatment induced PD biomarker modulation (including inhibition of ERK1/2 phosphorylation, DUSP6 down-regulation, repression of the MAPK Pathway Activity Score (MPAS) signature and Ki67 reduction) in two KRAS wt amplified gastric cancer xenograft models. The depth and duration of PD biomarker responses following treatment correlated well with the doses used in the respective efficacy studies. Finally, these results support the clinical use of biomarkers such as the MPAS signature score and Ki67 to investigate treatment effects elicited by BI 3706674 in tumor biopsies. Disponível em: O artigo objetivou identificar a percepção de produtores rurais de Tupã/SP sobre a legislação que implantou e regulamenta a logística reversa de embalagens de agrotóxicos, bem como as informações que têm sobre sua obrigação. Buscou-se trazer elementos contidos na legislação e considerações de diversos autores, por meio de pesquisa bibliográfica, assim como foi aplicado um formulário com 19 questões, sobre o comportamento de 20 produtores rurais frente às informações relativas à devolução das embalagens vazias de agrotóxicos. As discussões revelam que cerca de 60% dos respondentes são indiferentes, conhecem muito pouco ou desconhecem sua responsabilidade como primeiro elo da cadeia da logística reversa das embalagens vazias de agrotóxicos, talvez em virtude de falhas no processo de comunicação e na fiscalização. ABSTRACTWith the purpose to identify the perception of farmers Tupa/SP on legislation that established and governs the reverse logistics of empty pesticide containers, as well as the information they have on their obligation. It sought to bring elements contained in the legislation and considerations of various authors, through literature, as well as a form with 19 questions about the behavior of 20 farmers in the face of information on the release of empty containers of pesticides were applied. The discussions show that about 60% of respondents are indifferent, know little or unaware of their responsibility as first link in the chain of reverse logistics of empty pesticide containers, perhaps because of flaws in the communication process and inspection. It is suggested that there should be greater commitment from the Government in order to step up surveillance and make more understandable the responsibilities of farmers.KEYWORDS: Reverse logistics, communications and agribusiness, empty pesticide containers environmental legislation. RESUMENCon el propósito de identificar la percepción del agricultor Tupa / SP en la legislación que estableció y administra la logística inversa de los envases vacíos de plaguicidas, así como la información que tienen sobre su obligación. Se intentó traer elementos contenidos en la legislación y las consideraciones de diversos autores, a través de la literatura, así como un formulario con 19 preguntas sobre el comportamiento de los 20 agricultores en la cara de la información sobre la liberación de los envases vacíos de plaguicidas se aplicaron. Los debates muestran que alrededor del 60% de los encuestados son indiferentes, saben poco o conscientes de su responsabilidad como primer eslabón de la cadena de la logística inversa de los envases vacíos de plaguicidas, tal vez debido a fallas en el proceso de comunicación e inspección. Este distinguido cirujano, panameño, hizo sus estudios profesionales en la Universidad de Notre Dame, Notre Dame, Indiana donde se graduó con honores siendo miembro de la Sociedad de honor AlphaEpsilonDelta. Continuó sus estudios de medicina en la George Washington University School of Medicine, Washington, DC., donde obtuvo el grado de Medicina, graduándose también con honores siendo nombrado en la Sociedad de Honor de Medicina de los Estados Unidos, AlphaOmegaAlpha. Hizo su residencia en cirugía en el New York Hospital Cornell Medical Center y en Oncología Quirúrgica en el Memorial Sloan Kettering Cancer Center de Nueva York. Ha sido Certificado y Recertificado por el American Board of Surgery. Fellow y ExGobernador del American College of Surgeons y Ex Presidente del Capítulo de Panamá del American College of Surgeons. Apart from being licensed in the Republic of Panama, it is licensed by the state of California and Washington DC in the United States. Recently, he has been worthy of a recognition, reserved for few outstanding surgeons, in the world, being recognized as Honorary Fellow of the American College of Surgeons (American College of Surgeons), after 43 years of providing his knowledge for the care and well-being of your patients. It should be noted that there are currently only 487 surgeons worldwide who have received such a distinction. The American College of Surgeons awards this honor each year to four or five candidates who have provided humanitarian services, especially in the field of medical science. He is the third Panamanian to receive this recognition; the first to be distinguished was Dr. Augusto S. Boyd, in 1923, then the neurologist Antonio González Revilla, in 1973. Introducción. La rápida transición del medio rural a la adopción de hábitos alimenticios de zonas urbanas, el sedentarismo, las dificultades de acceso a la salud, ponen a la población en mayor riesgo de desarrollo de diabetes, sin embargo, son pocos los estudios que se enfocan en este fenómeno a nivel rural. Conocer el riesgo permite identificar las áreas problemáticas y revertir la situación a través de modelos de atención para esta población. El objetivo de este estudio fue describir los factores de riesgo presentes en una comunidad rural de Guanajuato. Material y Métodos: Estudio descriptivo, observacional, transversal; muestra no probabilística de 164 personas sin diabetes, se calculó con Epidat 3.1, intervalo de confianza al 95%, precisión del 3%; se aplicó el Cuestionario de Factores de Riesgo para la Diabetes Mellitus. Para la colecta se hizo un recorrido de casa por casa hasta completar la muestra, para el análisis estadístico se utilizó la chi cuadrada y cálculo de odds ratio. Resultados: El 71%(117) de la población fueron mujeres; el promedio para la edad fue de 43 años; el 85% de los sujetos presentaron un alto riesgo de padecer diabetes; los tres principales factores de riesgo fueron: 57.9% (95) con sobrepeso y obesidad, el 77.4%(127) de la muestra con cintura superior al parámetro recomendado, 75.6% (124) de la muestra presentó antecedentes heredofamiliares; en cuestión de género las mujeres presentaron 3 veces más riesgo de diabetes (p=&lt;.05). Estaba formada por un maestro un organista y varios instrumentistas, más los mozos de coro y algún cantor. Various mandates of the diocesan visitor and the payments to the musicians account for the activity of the musis chapel of the parish of Nuestra Señora de la Asunción in El Barco de Ávila. It was made up of a chapelmaster, an organist and several instrumentalists, plus the choirs and some singer.
