Abstract:

Abstract:

Abstract:
Abstract:

Abstract:

Abstract:
Objective This study was performed to describe the treatment of aortic arch pathologies with a physician-modified fenestration (PMF) technique in thoracic endovascular aortic repair (TEVAR). Methods From August 2015 to August 2017, 32 patients with aortic arch pathologies underwent TEVAR with the PMF technique. All patients’ clinical data were analyzed with GraphPad Prism 7.0. Results Thirty-four aortic stent-grafts were implanted in 32 patients. The mean proximal diameter of the stent-graft was 32.4 ± 3.4 cm, and the mean length was 170.0 ± 25.2 cm. Twenty-nine PMF procedures were performed to preserve the left subclavian artery (LSA) and three to preserve both the LSA and left common carotid artery. The mean distance between the pathology and LSA was 8.4 ± 4.0 mm. The mean procedure time (from first to last digital subtraction angiography) was 22.8 ± 20.8 min. The mean follow-up time was 8.3 ± 5.3 months. During follow-up, the all-cause survival rate was 83.3% and the patency rate of the branch artery after PMF was 96.0%. Conclusion The PMF technique is a relatively safe, feasible, and time-saving method to preserve the branch artery in TEVAR for aortic arch pathologies. The short- to middle-term result of this technique is satisfactory.

Abstract:
ABSTRACT The December 21, 1985 Arco Anchorage crude oil spill affected approximately 7,000 feet of the sheltered, south-facing shore of Ediz Hook, Washington. Some of the stranded crude oil penetrated sand and gravel sediments in the intertidal zone. An aggressive beach agitation program was implemented between February and April 1986 to remove most of the oil trapped within the intertidal sediments. Post-cleanup monitoring activities between August 1986 and January 1988 determined the crude oil content of intertidal and shallow subtidal sediments at five elevations along thirteen sampling transects. Sediment samples were also obtained for laboratory grain-size testing at five of these transects. Post-cleanup monitoring of sediment chemistry indicated a consistent trend of decreasing crude oil content of the intertidal sediments between March/April 1986 (mean oil concentration 670 ppm) and July 1987 (mean oil concentration 110 ppm). A similar pattern was observed for the shallow subtidal sediments, where the mean crude oil concentration decreased from 460 ppm in August 1986 to 110 ppm in July 1987. The mean oil content of these sediments increased somewhat after July 1987, indicating probable hydrocarbon inputs from other sources. Chromatograms of the hydrocarbons in the sediments after July 1987 were dissimilar to chromatograms for the unweathered oil from the Arco Anchorage. The grain-size distribution of the sediments changed relatively little during the post-cleanup monitoring period, reflecting the relatively sheltered nature of the study area. However, the minor changes observed in sediment grain size suggest that gradual redistribution of intertidal and subtidal sediment occurred along the south shore of Ediz Hook during the monitoring program.

Abstract:
Background: Acute aortic dissection is one of the most fatal conditions, with high surgical risks and severe postoperative complications. Treatment modalities vary from center to center, and a total arch replacement sometimes is not sufficient to provide a good result. Frozen elephant trunk (FET) offers a good option for a more complete repair. Because of its complexity and expense, however, this excellent technique is not always available. In this article, we present our hybrid technique including total arch replacement and retrograde thoracic endovascular aortic repair (TEVAR) for acute aortic dissection via a ministernotomy. Objective and methods: Our study aimed to estimate the safety, early results, and some midterm outcomes of total arch replacement in combination with retrograde TEVAR for acute aortic dissection from August 2013 to May 2017. We analyzed the intraoperative variables (duration of cross-clamp, circulatory arrest, ventilation, and intensive care unit stay), 30-day mortality, and early complications as well as the midterm results of these patients. Results: From August 2013 to May 2017, 22 patients (17 male) diagnosed with acute aortic dissection were treated with total arch replacement and concomitant retrograde endovascular repair via an upper ministernotomy. Mean age (± SD) was 60.8 ± 10.7 years. Thirty-day mortality was 4.5%: 1 patient died of acute renal failure. One patient had a postoperative stroke. The average number of stents used intraoperatively was 1.83. Midterm mortality was 1 patient (4.5%). Complete thrombosis of the false lumen in the thoracic aorta was achieved in 18 patients (90%). Conclusions: Retrograde TEVAR in combination with total arch replacement via an upper ministernotomy might be safe and effective in treating acute type A aortic dissection, with fairly low mortality and perioperative complications, and a very good rate of total false lumen thrombosis in midterm follow up.

Abstract:
Background. Clinical presentations of acute appendicitis (AA) and acute right-sided colonic diverticulitis (ARCD) are similar. However, the usual treatment for each disease differs between surgical and conservative management. The aim of this study was to identify clinical differences between AA and ARCD. Method. We performed a single-center retrospective study on adult patients, with uncomplicated AA and ARCD confirmed by computed tomography, who visited an emergency department between March 2018 and August 2019. Clinical variables including past medical history, presented symptoms and signs, and laboratory findings were compared between the two groups. A logistic regression analysis was subsequently performed to differentiate ARCD from AA based on results of univariate analyses. Results. A total of 212 (79.1%) and 56 (20.9%) patients were enrolled in AA and ARSD groups, respectively. Logistic regression analysis revealed that a past history of diverticulitis [OR: 102.679 (95% CI: 9.964–1058.055), p<0.001] was associated with ARCD, while ketonuria [OR: 2.907 (95% CI: 1.091–7.745), p=0.033], anorexia [OR: 21.544 (95% CI: 3.905–118.868), p<0.001], and neutrophilia [OR: 3.406 (95% CI: 1.243–9.336), p=0.017] were associated with AA. Conclusion. Anorexia, neutrophilia, and ketonuria were predictors of AA while a history of diverticulitis was a predictor of ARCD.

Four groups of individuals were included in this study, 230 pregnant women divided into three groups (Group 1 first trimester, Group 2 second trimester, and Group 3 third trimester) and Group 4 contained 90 nonpregnant women as control. Full automatic chemical analyzer (Cobas C311, Germany) was used to determine the biochemical parameters. The EL ×800 Absorbance Microplate Reader from BioTek (USA) instruments is used to measure the activity of superoxide dismutase (SOD), catalase (CAT), and malondialdehyde (MDA) in serum. The results of this study showed a significant increase in serum albumin, glucose, total bilirubin, direct bilirubin, alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, creatinine, urea, cholesterol, low-density lipoprotein, and MDA in pregnant women as compared to the control group, also show significant decrease in high-density lipoprotein (HDL) level, SOD activity, and CAT activity in pregnant women as compared to nonpregnant women, and nonsignificant difference in uric acid, and HDL.

Abstract:
The intervention method is a time series model which could be used to model data with extreme fluctuation whether up or down. Stock price return tend to have extreme fluctuation which is caused by internal or external factors. There are two kinds of intervention function; a step function and a pulse function. A step function is used for a long-term intervention, while a pulse function is used for a short-term intervention. Modelling a time series data needs to satisfy the homoscedasticity assumptions (variance of residual is homogeneous). In reality, stock price return has a high volatility, in other words it has a non-constant variance of residuals (heteroscedasticity). ARCH (Autoregressive Conditional Heteroscedasticity) or GARCH (Generalized Autoregressive Conditional Heteroscedasticity) can be used to model data with heteroscedasticity. The data used is stock price return from August 2008 until September 2018. From the stock price return data plot is found an extreme fluctuation in September 2017 (T=110) that is suspected as a pulse function. The best model uses the intervention pulse function is ARMA([1,4],0) (b=0, s=1, r=1). The intervention model has a non-constant variance or there is an ARCH effect. The best variance model obtained is ARMA([1,4],0)(b=0, s=1, r=1)–GARCH(1,1) with the AIC value is -205,75088. Keywords: Stock Return, Intervention, Heteroscedasticity, ARCH/GARCH

Abstract:
2077 Background: The classical treatment for multiple brain metastases is whole brain radiotherapy (WBRT). For patients with 1-3 brain metastases stereotactic radiosurgery (SRS) results in better response, similar survival and less short and long term toxicity such as neurocognitive dysfunction than WBRT. For patients with > 3 metastases there is little prospective data of SRS partly due to the technical and logistic difficulties of delivering SRS to multiple metastases. We present a new SRS planning program (MMM) that allows the planning and delivery of linac-based SRS to up to 10 brain metastases simultaneously in less overall time than for WBRT. Methods: Between August 2014 and August 2016, patients referred for RT with 2 or more brain metastases were offered treatment with MMM. System QA included ArcCheck diode array and film dosimetry and gel dosimetry based on a patient derived phantom. Individual QA included replanting treatment arcs on an independent system. Results: 94 patients with 2-10 (median 5) metastases and a combined volume of 0.01-8.64 cc were treated. Planning time including image fusion, target delineation and RT plan generation required on average 20 minutes minutes. SRS was delivered using 4-5 (is more 7-10 since return arcs do not address the same targets) non-coplanar arcs and a single isocenter at the center of mass of the metastases. Treatment time to deliver 18-24 Gy to all metastases was 25 mins (beam on time ~6 minutes) for a total time of 45 minutes. This is the same as the time for each individual metastasis on earlier planning systems and compares to 20 minutes planning time and 12 mins delivery time for each of 10 fractions for a conventional 2 field WBRT plan WBRT (total 140 minutes). At 6 months follow up, 88% of treated metastases had decreased in size, 10% were stable and 2% grew. Acute toxicity was mild except for one patient with intractable seizures. Conclusions: Linac based SRS for multiple brain metastases is efficient, requiring the same resources as for treatment of a single metastasis and less resources than for WBRT, with a high rate of local control. Appropriate equipment is available in most radiotherapy departments which will allow more prospective studies of SRS for multiple metastases.

Commissioned by the Three Choirs Festival and performed there for the first time on 13 August by the Philharmonia Orchestra under Martyn Brabbins, these variations on Vaughan Williams's hymn tune ‘Down Ampney’ (so named after the village of the composer's birth), were surprising individual successes. The overall shape of the work, arch-like with a slow-fast-slow-fast-slow structure, was suggested by John McCabe, celebrating his 65th birthday year and featured composer at this year's Three Choirs. McCabe, joined in this collaborative effort by Robert Saxton, James Francis Brown, David Matthews and Judith Bingham, also sensibly requested that the orchestration be limited to double woodwind, modest brass and percussion sections, harp and the usual strings.

The Intercollegiate Surgical Curriculum Programme (ISCP) is a web-based system, approved by the Postgraduate Medical Education and Training Board, designed for the modern surgical trainee. First launched in August 2007, the ISCP provides the syllabus for surgical training in the UK. The website can be utilised by the trainee to record workplace-based assessments (WBAs), operations and appraisals under the guidance of an assigned educational supervisor (AES). This evidence is then used to demonstrate that the trainee has achieved the necessary targets set by the programme director (PD). The trainee then attends an annual review of competencies progression (ARCP), in which his or her suitability to progress within the programme is evaluated using the collated data.

Abstract:
Continuous retrograde hypothermic low flow cerebral perfusion (CRCP) with deep hypothermic systemic circulatory arrest (DHSCA) during aortic arch surgery was employed in six patients, aged 21-79 years. From August 1991 to November 1992, five of these patients were operated for ascending and arch aortic dissection type I, and one patient was operated for an aneurysm extending from the ascending aorta into the arch. Cardiopulmonary bypass (CPB) technology included a centrifugal pump and low-dose aprotinin. Venous drainage was established via the superior and inferior caval veins and arterial return via the femoral artery. Prior to CPB, a bypass line connecting the arterial line with the superior vena cava cannula was implemented. Prior to DHSCA, the patients were systemically cooled to a mean nasopharyngeal temperature of 15.2°C. After induction of systemic circulatory arrest, the femoral artery cannula was clamped. Thereafter, the implemented bypass line was opened to achieve reverse flow into the superior vena cava to allow venoarterial perfusion. The perfusate was returned to the CPB circuit through drainage from the inferior caval vein and by aspiration of blood from the opened aortic arch. CRCP flow rate ranged from 250 to 450 ml/min (mean 375 ml/min) maintaining an internal jugular vein pressure between 18 and 25 mmHg. The duration of CRCP ranged from 24 to 55 minutes (mean 39 minutes). Postoperatively, one patient died of cardiac failure. The other five patients regained full consciousness without neurological deficits, as defined by the Glasgow coma score, within 48 hours after the operation. Neither did we see other major organ complications. At present four patients are alive nine to 24 months after surgery and they are in New York Heart Association (NYHA) functional classification I-II. Our experience indicates that CRCP is safe and effective, avoiding cerebral circulatory arrest. Furthermore, this technique avoids clamping of cerebral vessels, reduces the chances of embolism of particulate debris and of cerebral air intrusion into opened cerebral vessels.

Abstract:
PurposeThis paper analyses the incremental housing process developed at Villa Verde, a housing project designed by the Chilean architecture firm Elemental, whose director Alejandro Aravena received the Pritzker Prize in 2016. This project is conceived within a social housing framework and designed as an affordable “half-house” to be incrementally extended by the owners.Design/methodology/approachThis paper is based on research undertaken in August 2017 with data obtained through site surveys, trace analysis, interviews with 32 residents and photographic surveys. The researchers mapped the modifications made by all households at Villa Verde in the four years after occupation.FindingsThe strategy of designing a formal framework for informal additions has generally been successful with most houses undergoing substantial expansion to a high standard of construction. The paper raises concerns regarding the settlement's urban design, response to local climate and the quality of shared open space. We also find evidence of over-development as informal additions extend across front and rear yards that are in some cases fully enclosed.Originality/valueThis project is critiqued within the context of a long series of architectural attempts to harness the productive capacities of self-help housing. Villa Verde engages the freedom to build in a self-organised manner within a formal framework. But what will stop these additions from escalating into a “slum”?

Towards the end of August 1350, Petrarch wrote from his home at Padua to Philippe de Vitry, chastising his friend for a letter that he had sent to their mutual patron, Cardinal Guy de Boulogne, papal legate in Italy. Vitry's mind has slowed since their first acquaintance, writes Petrarch, so that he now considers even a glorious absence from France undesirable. The man who, when asked where he was from, answered that he was a citizen of the world, now thinks any departure from France an exile. The dust of France lies too heavily on his shoes; the Petit-Pont in Paris, ‘its arch not quite in the shape of a tortoise shell’, is too appealing to him, and the murmur of the Seine delights his ear too much.

Abstract:
Abstract OBJECTIVES Total aortic arch replacement is an invasive procedure with significant risks for complications. These risks are even higher in older, multimorbid patients. The current trends in demographic changes in western countries with an ageing population will aggravate this issue. In this study, we present our experience with total aortic arch replacement using the frozen elephant trunk (FET) technique in septuagenarians. We compared the results of septuagenarians with those of younger patients and analysed if there was an improvement in outcome over time. METHODS Between August 2001 and March 2020, 225 patients underwent non-urgent FET procedure at our institution. There were 75 patients aged ≥70 years (mean age 74 ± 4) who were assigned to group A, and 150 patients aged <70 years (mean age of 57 ± 11) who were assigned to group B. In groups A and B, the indications for surgery were chronic dissection (21% vs 53%), aortic aneurysm (78% vs 45%) and penetrating atherosclerotic ulcer (1% vs 2%). RESULTS The rate for temporary dialysis was significantly higher in group A than in group B (29% vs 13%, P = 0.003), although the majority recovered kidney function. Rates for re-exploration for bleeding and stroke were comparable in both groups. In-hospital mortality was significantly higher in group A than in group B (24% vs 13%, P = 0.037). Logistic regression analysis showed that age >70 years was an independent statistically significant risk factor for in-hospital mortality (odds ratio = 2.513, 95% confidence interval = 1.197–5.278, P-value = 0.015). Follow-up was complete for 100% of patients and comprised a total of 1073 patient-years with a mean follow-up time of 4.8 ± 4.5 years. The 1- and 5-year survival rates were 68% and 49% in group A, and 85% and 71% in group B, respectively (log rank, P < 0.001). Survival did not significantly improve over time. Discussion Total aortic arch replacement using the FET technique has a significantly higher risk for perioperative morbidity and mortality in septuagenarians than in younger patients. Long-term survival is significantly impaired in older patients. We recommend thorough patient selection of those who require total aortic arch replacement, and optimization of perioperative management to improve outcomes.

Abstract:
Year-to-year fluctuations of Ethiopia climate are investigated to develop statistical predictions at one-season lead time. Satellite vegetation data from NASA and rainfall from ARC2 are the basis for analysis. The “target” seasons are May–July and August–October, while “predictors” are December–February and March–May, respectively. Global fields of surface temperature, sea level air pressure, and upper and lower level zonal winds are employed in point-to-field correlations. After step-wise multivariate regression, the leading predictors are: surface temperature across Europe (cold-favourable), 850 mb zonal winds over the tropical Atlantic (easterly-favourable), and surface temperature in the tropical Indian Ocean (cold-favourable). Predictive algorithms for early and late rainfall exhibit a consistentr2fit of ~0.50, while those for vegetation reach ~0.65 in late summer, indicating that fluctuations in food resources could be forewarned.

Abstract OBJECTIVES: This study aims to analyse the impact of a simplified frozen elephant trunk (FET) technique on early outcome. METHODS: Between October 2010 and August 2018, 92 consecutive patients (mean age 64.4 ± 12.2 years) underwent FET surgery. Underlying pathologies were thoracic aneurysm in 35 patients, acute aortic dissection in 25 patients and chronic dissection in 32 patients. Thirty patients underwent a simplified FET technique with deployment of the stent graft in arch zone 2 with an extra-anatomic bypass to the distal left subclavian artery using the third branch of the Thoraflex™ Hybrid Plexus prosthesis via a supraclavicular access during reperfusion. These patients were compared to 62 patients who received the conventional FET procedure, in which a distal anastomosis is performed in arch zone 3. RESULTS: Circulatory arrest (41.7 ± 10.5 vs 76.5 ± 33.0 min; P < 0.001) and antegrade cerebral perfusion times (60.9 ± 13.5 vs 92.1 ± 33.1 min; P < 0.001) were significantly reduced in zone 2 vs zone 3 patients, respectively. The 30-day mortality rate was 3.3% (n = 1) in zone 2 patients vs 17.7% (n = 11) in zone 3 patients (P = 0.75). Stent deployment in zone 2 was associated with significantly reduced rates of postoperative stroke [zone 2: n = 0 (0.0%); zone 3: n = 11 (17.7%), P = 0.046] and recurrent nerve palsy [zone 2: n = 1 (3.3%); zone 3: n = 14 (22.6%), P = 0.020). CONCLUSIONS: Simplifying the FET procedure leads to reduced circulatory arrest and cerebral perfusion times and improves early outcome.

This study provides new insights into the recent time-varying pattern of gold volatility surrounding the Subprime Global Financial Crisis (GFC) 2008 with special emphasis given to small open economy of Malaysia. Using daily data of gold bullion Kijang Emas issued by the Bank Negara Malaysia from August 2005 till July 2018, the study modelled the gold volatility using different models of ARCH-GARCH family. Under the context of emerging economy, the study found significant enhanced gold volatility during the middle of GFC, signifying that gold price is highly sensitive to specific economic disturbance. Next, our result displayed inverted asymmetry effect where investors interpret gold as a safe-haven investment during good gold returns, hence contributed to enhanced gold volatility. Finally, bilateral exchange rate (MYR/USD) affected volatility with positive sign, indicating that gold investment could be adversely affected during the depreciation of the Malaysian Ringgit.

Abstract:
We report results of an ichthyological survey of the upper Rio Yuruá in southeastern Peru. Collections were made at low water (July-August, 2008) near the headwaters of the Brazilian Rio Juruá. This is the first of four expeditions to the Fitzcarrald Arch — an upland associated with the Miocene-Pliocene rise of the Peruvian Andes — with the goal of comparing the ichthyofauna across the headwaters of the largest tributary basins in the western Amazon (Ucayali, Juruá, Purús and Madeira). We recorded a total of 117 species in 28 families and 10 orders, with all species accompanied by tissue samples preserved in 100% ethanol for subsequent DNA analysis, and high-resolution digital images of voucher specimens with live color to facilitate accurate identification. From interviews with local fishers and comparisons with other ichthyological surveys of the region we estimate the actual diversity of fishes in the upper Juruá to exceed 200 species.

Abstract:
Abstract. An unusually extended period (5 h) of polar cap auroral display on 3 August 1986 is examined. Auroras have been investigated using ground-based data as well as measurements from the IMP-8 spacecraft in interplanetary space and simultaneous observations from the polar-orbiting satellites Viking and DE-1 in the northern and southern hemispheres, respectively. It is found that visible Sun-aligned arcs are located inside the transpolar band of the θ-aurora observed from the satellite in ultraviolet wavelengths. The transpolar band can contain several Sun-aligned arcs that move inside the band toward the morning or evening side of the auroral oval independent of the direction of the band movement. Intensifications of polar cap auroras with durations of up to about 30 min are observed. No change has been found in either IMF parameters or substorm activity that can be related to these intensifications. The θ-aurora occurred during a 2-h period when the B z-component of the IMF was negative. A tendency is noted for dawnward (duskward) displacement of the transpolar band when By>0 (By<0) in the southern hemisphere. Simultaneous observations of auroral ovals during interplanetary Bz<0, By<0 and Bx>0 in both hemispheres and convection patterns for Bz<0 and By<0 have been displayed using satellite and ground-based measurements. It was found that the transpolar band of the <theta>-aurora in the sunlit hemisphere was situated in the region of large-scale downward Birkeland currents.

Abstract:
The purpose of the study was to evaluate the efficacy of subtalar arthroeresis for II stage tibial tendon dysfunction by Johnson and Storm classification. From November 2007 to August 2009, 18 patients with that pathology were operated on. After implantation of Kalix® II endorthesis the plasty of posterior tibial tendon was performed. The results were assessed by American Orthopaedic Foot and Ankle Society (AOFAS) scores system. After treatment average estimate by AOFAS increased from 47.2 before operation to 79.0 after surgery, pain index from 16.3 to 30.0, foot function from 28.7 to 41.6, foot position from 2.6 to 8.4. According to X-ray data the longitudinal arch angle decreased by 14.3° at an average and talar-calcaneal angle by 8°. Study results showed that technique of subtalar arthrodesis was effective enough for correction of flat foot resulted from II stage posterior tibial tendon dysfunction. Combination of that technique with tendon plasty ensured the full-value reconstruction of the foot.

Abstract:
AbstractZooplankton sampling in 1997 identified the frontal zone of the Norwegian Coastal Current as a reproduction habitat for Calanus finmarchicus in June–August. This area is subject to considerable ultraviolet radiation (UVR), as calculated from satellite observations of ozone and cloudiness. While in situ experiments indicated UVR-induced mortality in reproducing C. finmarchicus, monthly UVR doses during the actual reproduction period did not appear to affect the abundance of the resulting generation of adolescent copepodites (CIV-V) that accumulated in a fjord habitat during October 1983–2000. Local UVR in the spawning grounds of Arcto-Norwegian cod at the Lofoten Islands in March–May was positively correlated with the stock's 0-group index, which resulted in the rejection of the hypothesis that local UVR leads to high mortality of cod eggs or reduces the abundance of prey for cod larvae. Rather, the result suggests an indirect positive effect of UVR on the survival of cod eggs and larvae, possibly by controlling harmful microbes.
