AbstractThe clinical logics of TasP shaped PEPFAR’s latest strategic initiative to achieve ‘epidemic control’ including the organisation’s use of metrics for evaluating performance, and decisions for allocating funds to specific programs and countries. TasP was conceptualised as an ‘evidence-based’ solution for effectively treating and preventing HIV, which could be consistently measured and reported on, however its ability to produce the right kinds of evidence remained abstract and hypothetical. The effects of TasP have also been evident in the budget since PEPFAR launched the strategy to achieve ‘epidemic control’. Whereas under previous initiatives to ‘lead to the global response’ to the epidemic, PEPFAR supported a wider variety of program areas, including by strengthening health systems, under the strategy of epidemic control PEPFAR has prioritised treatment programs over and above all others. TasP also justified disproportionate spending on a subset of countries. By adopting the clinical logics of TasP, PEPFAR justified spending on a limited number of programmes in a small set of countries that could produce what it defined as the right kinds of outcomes, and laid the groundwork for the retreat of US foreign aid.

AbstractDespite efforts to scale-up biomedical HIV prevention technologies, such as oral pre-exposure prophylaxis (PrEP), many countries and regions of the world are far off–track in reaching global HIV prevention targets. Uptake of, and adherence to PrEP amongst adolescent girls and young women (AGYW) in sub-Saharan Africa has proved particularly challenging. Drawing on qualitative individual interviews and focus group discussions with thirty AGYW in east Zimbabwe, as well as interviews with healthcare providers, we investigate some of the root causes of this challenge, namely the social risks involved with accessing PrEP. We find that stigma and the worry of AGYW that privacy and confidentiality cannot be maintained in local health clinics and by local healthcare providers, presents a major barrier to the uptake of PrEP. We call for interventions that recognise the need to tackle the range of socio-cultural norms and social practices that interact and in synergy make engagement with PrEP an (im)possible and (un)desirable thing to do for AGYW.

In this context, we focus on the socio-cultural aspects of sanitation, especially the connection between socio-culture with health (C), and with materials (B), as in the Sanitation Triangle model, from the perspective of humanities and social sciences in this part. To introduce this part, we first briefly review previous studies on sanitation by humanities and social sciences, and show the socio-cultural aspects of sanitation. Then, after providing an overview of this part, we discuss the theoretical points of each chapter such as “hard work” or “dirty work,” a social configuration of technology, and Menstrual Hygiene Management (MHM) in the local context, and indicate the relevant chapters in this book.

AbstractThe generation and spread of fake news within new and online media sources is emerging as a phenomenon of high societal significance. Combating them using data-driven analytics has been attracting much recent scholarly interest. In this computational social science study, we analyze the textual coherence of fake news articles vis-a-vis legitimate ones. We develop three computational formulations of textual coherence drawing upon the state-of-the-art methods in natural language processing and data science. Two real-world datasets from widely different domains which have fake/legitimate article labellings are then analyzed with respect to textual coherence. We observe apparent differences in textual coherence across fake and legitimate news articles, with fake news articles consistently scoring lower on coherence as compared to legitimate news ones. While the relative coherence shortfall of fake news articles as compared to legitimate ones form the main observation from our study, we analyze several aspects of the differences and outline potential avenues of further inquiry.
