Abstract Background: Intra-abdominal hypertension is known as a factor affecting cerebral haemodynamics. Sustainably elevated abdominal pressure may disturb the balance of intracranial/blood pressure ratio, eventually developing perfusion pressure to drop. Aim: The aim of this study is to investigate the influence of artificially elevated intra-abdominal pressure upon brain pial vessels condition and contractile reactivity of isolated rat arteria carotis communis and vena jugularis to norepinephrine and serotonin. Materials and methods: The abdominal pressure of rats anaesthetized with xylazine 10 mg/kg and ketamine 100 mg/kg was increased up to 25 mm Hg by insufflation of air through venflon cannula and maintained for period of 1 to 3 hours. Craniotomy of left parietal area was carried out by micro drill. Open scull and cranial window techniques were applied. Outer diameters of superficial pial vessels were measured by USB digital microcamera (magnification up to 400x). Contractile reactivity of smooth muscle preparations from arteria carotis communis and vena jugularis of euthanized abdominal-hypertensive (AH) rats was registered isometrically. Results: Increased smooth muscle reactivity of a. carotis communis from AH rats to serotonin (10−8-10−4 mol/l) but not to norepinephrine compared to controls was registered. The changes tended to be higher in long lasting (3 hours) exposure of AH rats. Increase in outer diameter of pial vessels during maintenance of abdominal hypertension in both open scull and cranial window techniques was found. Conclusions: The increased intra-abdominal pressure causes dilatation of small superficial cerebral blood vessels and increases the smooth muscle reactivity of isolated arteria carotis communis to 5-HT.

Zusammenfassung Hintergrund: Schnitt-, Stich- und Schussverletzungen der Kopf-Hals-Region stellen aufgrund der hohen Gefahr von vaskulären Läsionen ein potentiell lebensbedrohliches Krankheitsbild dar. Ein konsequentes, standardisiertes Notfallmanagement, in der Regel assoziiert mit einer operativen Exploration, ist Grundlage der effektiven Rettungskette. Methoden und Patienten: 11 Fälle selbst- oder fremdverschuldeter penetrierender Verletzungen der Kopf-Hals-Region werden hinsichtlich Alter, Geschlecht, Verletzungshergang, Art der Verletzung, beteiligter Strukturen, diagnostischen und operativen Vorgehens sowie Outcome beschrieben und der aktuellen Literatur und Leitlinien gegenübergestellt. Ergebnisse: Bei 10 von 11 Patienten wurde, im Rahmen eines interdisziplinären Schockraummanagements, nach einer klinischen Untersuchung eine CT-Angiografie durchgeführt. Bei 9 dieser Patienten wurde daraufhin eine chirurgische Exploration und Wundversorgung vorgenommen. In je 2 Fällen waren die Arteria carotis communis, die Arteria carotis externa sowie große venöse Blutgefäße des Halses verletzt. Keiner der Patienten verstarb oder litt unter bleibenden Schäden. Schlussfolgerung: Eine rasche, systematische und fächerübergreifende Versorgung in speziellen Trauma-Zentren hat einen wesentlichen Anteil daran dass Patienten mit penetrierenden Verletzungen im Kopf-Hals-Bereich seltener an ihren schwerwiegenden Verletzungen oder deren Folgen versterben.

Abstract:
Duplexultraljud av arteria carotis communis (CCA) och arteria carotis interna (ICA) utförs med ultraljud kombinerat med doppler för att bedöma kärlväggar samt flöden och hastigheter. Lumendiametern i CCA (CLD) är en markör för sjukdomar som ateroskleros, hjärtinfarkt och stroke. Syftet med studien var att undersöka om kroppsposition vid duplexultraljud av CCA och ICA påverkar lumendiameter (LD) och flödeshastighet i artärerna samt om det fanns någon korrelation till ålder och längd. I resultatet inkluderades 32 deltagare, vilka undersöktes bilateralt sittande i 45o och liggandes plant. För analys av data användes paired sample T-test. En signifikant skillnad i CLD påvisades mellan liggande och sittande på både höger (P=0,001) och vänster sida (P=0,011). Ingen signifikant skillnad påvisades i LD i ICA eller i flödeshastighet. En positiv korrelation identifierades mellan ålder och skillnad i vänster CLD (r=0,385; p=0,029). Två signifikanta korrelationer till längd identifierades, en med LD i vänster ICA (r=-0,431; p=0,014) och en med flödeshastighet i höger CCA (r=0,371; p=0,037). Skillnader i artärernas uppbyggnad och struktur kan vara anledningen till att skillnad sågs mellan liggande och sittande i CCA men inte i ICA. Då resultatet visar en signifikant minskning av CLD vid sittande bör detta kunna påverka riskbedömning, diagnostisering och behandling.

Bakgrund: Ateroskleros är en känd bakomliggande orsak till hjärt-och kärlsjukdomar. Progressionen av ateroskleros är en långsam process som börjar redan i unga år. En accepterad subklinisk markör för utvecklingen av ateroskleros är carotis intima-media tjockleken. Syfte: Syftet med studien var att undersöka om det föreligger en statistisk signifikant skillnad på carotis intima-media tjockleken mellan unga kvinnor och män utan någon känd kardiovaskulär riskfaktor. Metod: I studien undersöktes 38 friska studiedeltagare med medelåldern 24,5 år. Carotis intima-media tjockleken undersöktes med ultraljud. Resultat: Resultatet påvisade ingen statistisk signifikant skillnad på carotis intima-media tjockleken mellan unga kvinnor och män. Diskussion: Resultatet kan påverkas av liten provstorlek och för att kunna dra mer generella slutsatser bör en större population undersökas.<br>Background: Atherosclerosis is a known underlying cause of cardiovascular disease. The progression of atherosclerosis is a slow process that begins at an early age. An accepted subclinical marker for the development of atherosclerosis is carotid intima media thickness. Purpose: The purpose of the study was to investigate whether there is a statistically significant difference in intima-media thickness in the carotid artery between young women and men without any known cardiovascular risk factor. Method: The study examined 38 healthy participants with a mean age of 24.5 years. Carotid intima-media thickness was measured by ultrasound. Results: The results showed no statistically significant difference in carotid intima-media thickness between young women and men. Discussion: The result can be affected by the small sample size and in order to be able to draw more general conclusions, a larger population should be examined.

Bakgrund: En vanlig bakomliggande orsak till kardiovaskulära sjukdomar är ateroskleros. En ökning av IMT och LD ses tidigt i den aterosklerotiska processen. För att undersöka IMT och LD används ultraljud. Det finns flera riskfaktorer för att få en ökad IMT och LD. Möjligtvis finns det en sidoskillnad av IMT och LD, däremot är inte orsaken klarlagd. Syfte: Syftet var att jämföra intima-media tjocklek (IMT) och lumendiameter (LD) mellan höger- och vänster arteria carotis communis (CCA). Syftet var även att jämföra om det fanns en sidoskillnadav IMT och LD i CCAmed utgångspunkt från riskgrupperna relaterat till respektive referensgrupp. Metod: Detta arbete var en retrospektiv studie med 41 slumpmässigt utvalda deltagare. Resultat: Det sågs ingen statistisk signifikant sidoskillnad av IMT eller LD i hela populationen. Vid jämförelse av sidoskillnad av IMT och LD mellan riskgrupp och referensgrupp kunde ingen statistisk signifikant sidoskillnad påvisas hos någon av grupperna. Diskussion: Syftet uppnåddes men begränsningar som kan ha påverkat resultatet förekom. Detta arbete har fått både liknande resultat samt avvikande resultat jämfört med tidigare studier. Slutsats: Fortsatt forskning rekommenderas för att erhålla mer kunskap inom området. Därmed kan mätning av IMT och LD utvecklas och användas för förebyggande av kardiovaskulära sjukdomar.<br>Background: A common cause to cardiovascular disease is atherosclerosis. An increased IMT and LD reflects early stages of atherosclerosis. Ultrasound is used to investigate IMT and LD. There are several risk factors for an increased IMT and LD. Possibly there is a side difference of IMT and LD, but the cause is not clear. Aim: The aim was to compare intima-media thickness (IMT) and lumen diameter (LD) between right- and left common carotid artery (CCA). The aim was also to compare if there was any side difference based on risk groups related to each corresponding reference group. Method: This thesis was a retrospective study including 41 randomly selected participants. Results: No significant side difference of IMT and LD was seen in the entire group. When comparing side difference of IMT and LD between the risk groups and reference groups, no significant side difference could be seen. Discussion: The aim was achieved, but limitations that could affect the result occurred. Compared with previous studies, this work has both similar results as well as divergent results. Conclusion: Future studies are recommended to improve more knowledge. Thus, measurement of IMT and LD can be developed and used to prevent cardiovascular diseases.

Abstract:
Ultraljud av halsartärer är en duplexultraljud undersökning som har en förmåga attavbilda samt mäta flödeshastigheten i halskärlen. Med undersökningen kaneventuella förändringar i form av plack, stenoser samt ocklusioner utredas ihalsens artärer. Arteria carotis interna (ICA) anses vara ett väsentligt kärl belägenanatomiskt posteriort om halsen med huvuduppgift att försörja hjärnan med blod.Stenoser i ICA kan leda till ett flertal allvarliga symptom såsom transistoriskischemisk attack, övergående blindhet samt stroke. Den dominerande orsaken tilldessa allvarliga symptom samt förändringar i kärlen är arterioskleros. För korrektbehandling för den drabbade patienten betraktas bedömningen av stenosgradenvara väldigt viktigt. Syftet med denna studie är att undersöka sambandet mellanstenosgraden bedömd utifrån systolisk flödeshastighet i ICA och stenosgradberäknad med hastighetskvot mellan maximal systolisk flödeshastighet i ICA ochmaximal systolisk flödeshastighet i arteria carotis communis (CCA) samtslutdiastolisk flödeshastighet i ICA. Till studien inkluderades totalt 200 kärl(höger och vänster), av de användes 130 kärl. Parametrar som behövdes tillstudien var slutdiastoliska flödeshastighet i ICA samt maximal systoliskaflödeshastighet i ICA och CCA. Patienterna hade olika stenosgrader <40%->95%,i höger och vänster ICA. Hastighetskvoten ICA/CCA beräknades och användesför undersökning av sambandet med hjälp av statistiska analysmetoder. Resultatenav studien uppvisade att det föreligger ett signifikant samband mellanstenosgraden bedömd med maximal systolisk flödeshastighet i ICA ochstenosgraden bedömd med hastighetskvoten och slutdiastolisk flödeshastighet.Däremot så har hastighetskvoten ICA/CCA starkare överensstämmelse medstenosgraden bedömd med maximal systolisk flödeshastighet, jämfört med det förslutdiastolisk flödeshastighet. Slutsatsen är att hastighetskvoten kan användas fören säker bedömning av stenosgraden och bör med stor säkerhet återspeglastenosgraden i ICA.<br>Ultrasound of carotid arteries is a duplex ultrasonography that has the ability toimage as well as measure the flow rate in the vessels. With this analysis it ispossible to detect any changes in the shape of plaque, stenosis and occlusions incarotid arteries. Internal carotid artery (ICA) is considered to be an essentialvessel, supplies the anterior part of the brain with blood. Internal carotid stenosisleads to several severe symptoms such as transient ischemic attack, transientblindness and stroke. The main reason of these symptoms and changes in thevessels is atherosclerosis. Degree of stenosis is considered very important to givepatients the proper treatment. The aim of this study is to investigate therelationship between the degree of stenosis evaluated with maximum systolic flowrate in ICA and degree of stenosis evaluated with calculated flow rate ratio ICAand common carotid artery (CCA) and end diastolic flow rate in ICA. The studyincluded a total of 200 vessels (right and left), of which 130 vessels were used.Parameters needed for the study were vessels with stenosis degree of internalcarotid artery, end diastolic flow rate in ICA and maximum systolic flow rate inICA and CCA. The patients had different degrees of stenosis between <40% ->95% in the right and left ICA. The rate ratio ICA/CCA was calculated and used toinvestigate the relationship using statistical analysis methods. The results of thestudy showed that there is a strong and significant correlation between the degreeof stenosis assessed with maximum systolic flow rate in ICA and the degree ofstenosis assessed with the rate ratio and end diastolic flow rate. In contrast, therate ratio ICA/CCA has stronger compliance with the degree of stenosis assessedat maximum systolic flow rate, compared with that for end diastolic flow rate.This leads to the conclusion that the rate ratio can be used for a safe assessment ofthe degree of stenosis in ICA and should with great certainty reflect the degree ofstenosis in ICA.
